1.Public Health Experience in the ‘Snow Country’ Niigata, Japan

In the cold winter month of January 2012, two post graduate students from the Department of Community Health, Universiti Kebangsaan Malaysia (UKM), went on a two weeks field attachment with the Division of International Health (Public Health), Niigata University Graduate School of Medical & Dental Sciences (NU). This report is an account of our first hand learning experience about the public health system and culture in Niigata, Japan. Famously known as the ‘Snow Country’, Niigata prefecture is approximately 350 kilometers north of Tokyo, in the middle of the west coast of Honshu island, facing the Sea of Japan. It borders on the east with Fukushima prefecture, which was badly affected by the great tsunami disaster in March 2011. Niigata has a population of two and a half million, of which 21.3% is above the age of 65. Niigata University is located in Niigata City, the capital of Niigata prefecture. This attachment was under the UKM-Global Student Mobility Programme (Outbound) and it was taken as an opportunity to improve the memorandum of understanding between UKM and NU. The objectives were to gain knowledge and experience in various public health issues in a developed nation like Japan. Specifically, we were interested to learn about the local public health programmes, the influenza surveillance system, public health programmes for the elderly population, the Geographical Information System (GIS) and the Japanese culture in general.

2.Prevalence and characteristics of e-cigarette users among Malaysian current and ex-smokers

Introduction: Electronic cigarettes (ECs) are new devices that have been accepted widely by both
smokers and non-smokers. However, the evidence on EC used in Malaysia is scarce. The objective of thisstudy was to determine the prevalence of EC use and the socio-demographic and smoking characteristicsassociated with current EC use among Malaysian current and ex-smokers.
Methods: This was a sub-analysis of data from a cross-sectional, national-population- based EC study
conducted from May to June in 2016 in Malaysia. A detailed description of the sampling methods can
be found in the National E-cigarette Survey (NECS) 2016 report. Briefly, data were obtained from 1396
individuals who had ever been smokers, i.e., 957 (68.6%) current smokers and 439 (31.4%) ex-smokers.
Results: Current EC use was found predominantly among current smokers (8.0%) as compared with
ex-smokers (4.3%). Among current smokers, the main reasons given for smoking ECs were wanting to
try it (44.7%), followed by intention to quit tobacco smoking (15.8%) and to reduce tobacco smoking
(10.5%). Using multiple logistic regression analysis, we found that among current smokers, current EC
users were more likely to be younger, i.e., 18-44 years (aOR= 4.83, 95% CI= 1.97-11.86, p=0.001),
urban residents (aOR= 1.89, 95% CI= 1.15-3.11, p=0.012), single/ divorced/ widowed (aOR= 2.11,
95% CI= 1.24-3.61, p=0.006) and students (aOR= 2.25, 95% CI= 1.01-5.01, p=0.048). Among exsmokers, only younger respondents (18-44 years old) was reported as being more likely to be current EC users (aOR= 3.81, 95% CI= 1.14-12.76, p=0.030).
Conclusion: This study showed that currently using and ever having used ECs were more prevalent
among current smokers. The reasons given for initiating EC use among current smokers were mainly
wanting to try it, followed by intention to quit and to reduce tobacco smoking. Current EC use appears
to be common among current smokers who are younger, urban residents, single/divorced/widowed and students. Therefore, EC cessation intervention strategies and policies should target these high-prevalence
groups

4.A Systematic Review of Prevalence and Risk Factors of Latent Tuberculosis Infection Among Medical and Nursing Students

Introduction: Clinical training may expose medical and nursing students to workplace hazards comparable to those
encountered by healthcare workers (HCWs). This study was designed to investigate the prevalence of latent tuberculosis infection (LTBI) and associated risk factors among medical and nursing students. Methods: Following the
Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) criteria, a systematic review was
conducted utilising four electronic databases to appraise and synthesise the literature on LTBI which used the tuberculin skin test (TST) and the blood interferon-gamma release assay (IGRA). Results: Original articles published in the
English language between 2010 and 2020 were included, yielding 14 relevant articles. The average prevalence of
LTBI in high-burden countries was 38.2% for TST and 20.6% for the IGRA test. According to TST and IGRA findings,
the average prevalence of LTBI in nations with an intermediate burden was 16.7% and 4.7 %, respectively. The average prevalence was 2.8% and 1.1% from the TST and IGRA tests for low-burden countries, respectively. A greater
risk of LTBI was shown to be related with an increase in age among postgraduate medical school students, a history
of Bacillus Calmette-Guerin (BCG) vaccination, origin from high-risk tuberculosis (TB) countries, increased hours of
hospital exposure, a history of contact with TB cases, a high body mass index, older age group students, and a lower
TB knowledge score. Conclusion: The available evidence from this review emphasised the importance of developing
and implementing efficient and cost-effective TB infection-control programmes, particularly in high-burden countries and amongst students at risk.
5.COVID-19 Pandemic Fatigue: A Scoping Review of the Literature

Pandemic fatigue encompasses exhaustion, emotional strain, and decreased motivation due to prolonged pandemic
effects. This scoping review examines pandemic fatigue issues related to COVID-19, aiming to suggest effective management strategies. We assessed 2,558 records from 1st of December 2019 until 27th of March 2022 in PubMed,
Science Direct, ProQuest, and ClinicalKey. Following Arksey and O’Malley (2005) and PRISMA-ScR principles, we
selected 31 studies including research papers employing either quantitative or qualitative methods and editorials
and other summary articles after excluding redundant and irrelevant works. The authors provide six critical answers
to understand better and manage pandemic fatigue, including the need for more research and support, the collaboration between key stakeholders and the community, and the use of information to address pandemic fatigue. The
review found that pandemic fatigue is prevalent among certain groups, such as students and healthcare workers.
Factors contributing to pandemic fatigue include age, gender, high perceived severity of COVID-19, low trust in
government or health organizations, and low social connectedness. The review also identified an instrument, the
COVID-19 Pandemic Fatigue Scale, that can be used to measure pandemic fatigue. The review concluded that
identifying pandemic fatigue as early as possible is absolutely important and people and their government should
work together to handle the fatigue. Many countries have acknowledged the presence hence manage it differently.
Nonetheless the strategy suggested by WHO has provided an excellent framework to tackle the pandemic fatigue.
