1.Public Health Experience in the ‘Snow Country’ Niigata, Japan

In the cold winter month of January 2012, two post graduate students from the Department of Community Health, Universiti Kebangsaan Malaysia (UKM), went on a two weeks field attachment with the Division of International Health (Public Health), Niigata University Graduate School of Medical & Dental Sciences (NU). This report is an account of our first hand learning experience about the public health system and culture in Niigata, Japan. Famously known as the ‘Snow Country’, Niigata prefecture is approximately 350 kilometers north of Tokyo, in the middle of the west coast of Honshu island, facing the Sea of Japan. It borders on the east with Fukushima prefecture, which was badly affected by the great tsunami disaster in March 2011. Niigata has a population of two and a half million, of which 21.3% is above the age of 65. Niigata University is located in Niigata City, the capital of Niigata prefecture. This attachment was under the UKM-Global Student Mobility Programme (Outbound) and it was taken as an opportunity to improve the memorandum of understanding between UKM and NU. The objectives were to gain knowledge and experience in various public health issues in a developed nation like Japan. Specifically, we were interested to learn about the local public health programmes, the influenza surveillance system, public health programmes for the elderly population, the Geographical Information System (GIS) and the Japanese culture in general.

2.Intramuscular Abscess Causing Radial Nerve Palsy

Radial nerve palsy is one of the commonest upper limb compressive neuropathies encountered in orthopaedic practice. More frequently associated with fractures of the humeri, it can also be the result of other atraumatic causes such as the so-called 'Saturday Night Palsy'. Inthis article, a case is presented where a palsy of the radial nerve was caused by the expansion of an intramuscular abscess in a patient with chronic renal failure. This is a relatively rare cause of radial nerve palsy that resolved progressively after surgical drainage.
3.Association Between Psychosocial Factors With Upper Extremity Musculoskeletal Disorders: An Academic Review

Work-related upper-extremity disorders (WRUEDs) are frequently occurring among workers in general. Various researches support the empirical results of the Autogenic factors related to such problems of health and safety. Research shows the association of different aspects of work organization strengthen the possibilities of work-related stress and other psychosocial factors, which may lead towards adverse health conditions. To review the previous work done for finding the association between psychosocial factors and upper extremity disorders. Findings from different studies have been reviewed to examine the psychosocial factors association in developing WRUEDs. Published studies, which were primarily focused on WRUEDs and psychosocial factors association, were reviewed. Various studies have shown the relationship between psychosocial factors and UEDs. Due to the different nature of epidemiology of the disorder and variable differences, findings from different studies contradicts. Finally, this review presents the limitations in existing studies, and further research has been proposed for future.
4.The association of birth order and sociodemographic factors with caries experience among adolescents in Tumpat.

Sociodemographic factors have received considerable attention in the literature with regards its relationship with caries. However, the related information among children ages 12-17 is fairly limited. Birth order has been consistently found to be associated with diseases hypothesized to have an infectious aetiology but only few studies have examined its relationship with dental caries. The aims of this study were to determine the association of birth order and the sociodemographic background of subjects with caries and high caries experience among 16-year-old schoolchildren in Tumpat district, Kelantan. This was a case-control study. A total of 1087 16-year-old schoolchildren were examined intraorally for their caries status. They were categorized into three groups namely caries free (DMFS=0) mild caries (DMFS= 1-7) and high caries (DMFS ≥8) group based on DMFS score. Cases were the caries group and the controls were the caries free group. In each group, 163 subjects were selected by using simple random sampling. A questionnaire was used to obtain personal information of the subjects, birthweight and sociodemographic background. Analysis involved was ordinal logistic regression. The factor that was found to have association with caries and high caries experience determined using multiple ordinal logistic regression analysis was educational level of parents (OR=1.55, 95% CI: 1.06; 2.28). This study showed that birth order was not a significant factor associated with caries experience. Among the sociodemographic factors, only parent’s educational level was found to be associated with caries and high caries experience.

5.Locally produced bovine bone sponge as a haemostatic agent.

The aim of this study was to evaluate the morphological and biological properties of a locally produced "Bovine Bone Sponge" for use in dentistry. Bovine bone sponge was prepared from local calf bone. Endotoxin level and surface properties were investigated. The pore size and water uptake ability were measured and results were compared with the commercial haemostatic agent. The material was tested for its haemostatic property and its inhibition of alveolar bone resorption in a sheep model following dental extraction. Results revealed a significant difference in haemostatic effect, and a shorter bleeding time and a lower rate of alveolar bone resorption in bovine bone sponge compare to a commercial haemostatic agent.

6.Prevalence of Stress and Coping Mechanism among Staff Nurses in the Intensive Care Unit

The working environment of intensive care unit (ICU) nurses is a constant source of stress.
Researchers have described ICU as a stressful environment because of the complex
nature of patients’ health problems requiring an extensive use of very sophisticated
technology. This study aimed to identify the prevalence of stress among staff nurses
working in ICU, Hospital Universiti Kebangsaan Malaysia (HUKM), factors influencing
stress and to explore the symptoms of stress experienced. This descriptive study was
conducted on 67 staff nurses working in ICU, HUKM. Data was collected using selfadministered
questionnaires. The questionnaires included sections on socio-demographic
data, factors influencing stress and symptoms of stress experienced. Data was analysed using frequency and percentage. The Chi square test was used to examine the
relationship between socio-demographic data and factors influencing stress. Findings
indicated that stress symptoms were experienced by 100 per cent (n =67) of staff nurses.
Knowledge, working experience, critically ill patients, and environmental factors were one
of the many factors contributing to the stress experienced by the ICU nurses in HUKM.
Nurses working in ICU, HUKM are found to have a high level of stress. Recommendations
like encouraging ICU nurses to take up post basic ICU courses and increasing the nurses
quota can reduce the prevalence of stress among ICU nurses. Hospital management has
an important role to play in reducing the stress levels of nurses working in ICU as stress
has an effect on job performance and quality of nursing care.
7.Primary Mediastinal Myxoid Liposarcoma

In this study, we report an extremely rare case of liposarcoma which arises primarily in mediastinum. The
patient appeared to have progressive dyspnoea and prolonged cough for a duration of one year. Chest
radiograph and Computed Tomography (CT) of the thorax revealed a large right mediastinal mass with
fatty component. It was confi rmed to be primary liposarcoma on histopathological examination.
8.Mechanical and thermal properties of hydroxyaptite filled poly (methyl methacrylate) heat processed denture base material.

The aim of this study was to evaluate the effect of powder-to-liquid ratio on the glass transition temperature (Tg) and the tensile properties of denture base material prepared from poly (methyl methacrylate) (PMMA) and hydroxyapatite (HA) previously treated with 3-trimethoxysilylpropyl methacrylate (gamma-MPS). Specimens for mechanical testing were prepared by adding composites powder (PMMA, BPO and HA) to the monomer (MMA and EGDMA) followed by hand mixing as in dental laboratory description usage. The glass transition temperature was studied by using differential scanning calorimetry (DSC). It was observed that the tensile properties and the Tg were affected by the powder-to-liquid ratio. The mechanical characterization of the materials were performed by using single edge notch-tension (SEN-T) specimens; the fracture toughness was slightly higher in formulation which contained HA filler compared to commercial denture base material.

9.Ex vivo growth of rabbit bulbar, fornix and palpebral conjunctival epithelia in a serum-free and feeder layer-free culture system.

This study was conducted to explore the feasibility of culturing conjunctiva epithelial cells in serum-free and feeder layer-free culture system with regard to the cell morphology and immunocytochemistry of the rabbit bulbar, fornix and palpebral conjunctiva epithelia. The results showed that epithelium cells from all the three conjunctiva regions can be cultured in a serum-free and feeder layer-free environment. We obtained highest epithelial growth from fornix region with minimum invasion of fibroblast cells compared to other area. All cultured cells were stained positive for cytokeratin 19 and MUC5AC and negative for cytokeratin 3. These findings suggested that fornix was a better source of cells for the development of tissue engineered conjunctiva for future clinical application.

Neurogenic claudication resulting from focal hypertrophy of the ligamentum flavum in the lumbar spine due to ochronotic deposits has not been reported till date. The authors discuss one such case highlighting the pathogenesis, histological and radiological features. Salient features of management are also emphasized upon.
