1.Treatment of fractures of thoracolumbar vertebral body with paraplegia by posterior decompression and internal fixation

Objective To study the management of thoracolumbar vertebral fracture by posterior route decompression and internal fixation. Methods 27 cases of thoracolumbar vertebral fracture with spinal injury were treated by posterior route decompression and Dick or RF internal fixation. Results All cases were followed up for an average of 11 months (from 6 months to 20 months). 27 cases had a good anatomic reduction and maintained thoracic lumbar lordosis. 19 cases gained D Frankel grades of neurological recovery and 8 cases gained E Frankel grades of neurological recovery. Conclusion Posterior route decompression and internal fixation with Dick or RF is less traumatic, simple in manipulation, and cheap in treating thoracolumbar vertebral body fracture.
2.Clinical observation of three dimensional conformal radiotherapy for na-sopharyngeal carcinoma

Objective To investigate the effects of three dimensional conformal radiation therapy for clinical efficacy and safety in patients with nasopharyngeal carcinoma . Methods Selected 50 cases of nasopharyngeal carcinoma patients from January 2007 to October 2013 in our hospital, they were randomly divided into the treatment group and the control group by digital method with 25 cases in each group,the control group used conventional radiotherapy and+FP chemotherapy,the observation group was carried out in three dimensional conformal radiotherapy with convention-al radiotherapy, and chemotherapy of FP regimen,the clinical curative effect and the parotid gland toxicity were com-pared between the two groups. Results The near future curative effect comparison,the total effective rate in treatment group was 88.0%(22/25),was significantly higher than that in control group 68.0%(17/25),the difference was statisti-cally significant(P<0.05);from the parotid gland toxicity reaction of two groups,the incidence rate of gradeⅢtoxicity in the treatment group was 8.0%(2/25),and 12.0%(3/25)in the control group without significant difference(P>0.05). Conclusion Three dimensional conformal radiation therapy for nasopharyngeal carcinoma patients with short-term effi-cacy, there is no difference of toxicity between the conventional radiotherapy and three climensional conformal radia-tion therapy,security is good,worthy of clinical popularization and application.
3.Research progress on relationships of circadian rhythm with thyroid function and diseases

Circadian rhythm is a phenomenon of diurnal changes in life activities formed by a transcription-translation feedback loop of biological clock genes affected by external environmental conditions. The circadian rhythm system controls almost all physiological processes in the organism, and these processes will change as the external environment changes. Previous studies have shown that the hypothalamic-pituitary-thyroid axis in mammals is regulated by the central diurnal pacemaker of the suprachiasmatic nucleus of the hypothalamus, so part of the thyroid function is controlled by the biological clock, and the secretion of thyroid hormones in blood can present a circadian rhythm. However, the molecular mechanism of the biological clock's regulatory effect on thyroid is still unclear. Whether circadian rhythm interference is related to the disorder of thyroid function or the occurrence of thyroid diseases is worthy of attention. This paper focused on the research progress of biological clock, circadian rhythm, and thyroid function, specifically the characteristics of circadian rhythm of thyroid physiological function and the effects of sleep deprivation, light at night, and night shift work on thyroid function, elaborated the relationships of circadian rhythm disorder with thyroid function and thyroid diseases represented by thyroid malignant tumors. The review summarized that circadian rhythm disorder may disrupt the rhythmic secretion of thyroid hormones, but no clear conclusion is reached yet on any effect on thyroid diseases, especially thyroid malignant tumors, so it is necessary to further strengthen the relevant epidemiological and molecular mechanism research.
