2.Effect of T-lymphocyte subpopulations in peripheral blood on disease-free survival of patients with different molecular subtypes of breast cancer

Objective:To determine the distribution of T-lymphocyte subpopulations in peripheral blood and its relationship with disease-free survival (DFS) of patients with different molecular subtypes of invasive breast cancer (IBC). Methods:Data and plasma samples of 372 female patients with IBC treated at Tianjin Medical University Cancer Hospital and Institute between January 2008 and August 2009 were obtained. CD4+/CD8+ratio and the amount of regulatory T cells (Treg cells) and natural killer cells (NK cells) were measured through flow cytometry. Results:A total of 133 (35.8%), 124 (33.3%), 48 (12.9%), and 67 cases (18.0%) presented luminal type A, luminal type B, HER-2 over-expression, and triple-negative type breast cancer (TNBC), respectively. In TNBC, a longer DFS was observed when the peripheral CD4+/CD8+ratio was high or when the level of Treg cells was low (CD4+/CD8+P<0.05 and Treg P<0.05). However, CD4+/CD8+and Treg are not independent prognostic factors for the DFS of TNBC cases. In other molecular subtypes of breast cancer, CD4+/CD8+ratio and the amount of Treg cells are not correlated with DFS (P>0.05). The level of NK cells is unrelated to the DFS of breast cancer with the four molecular subtypes (P>0.05). Conclusion:CD4+/CD8+ratio and the amount of Treg cells in peripheral blood may predict DFS of TNBC but are not considered as independent prognostic factors.
3.Bone marrow MRI in adult patients with untreated acute leukemia and its clinical application value

Objective To observe bone marrow MR imaging of adult acute leukemia patients in first diagnosis and to reveal the rule of bone marrow infiltration and the role of MRI in diagnosing and predicting the prognosis of untreated acute leukemia adult patients. Methods Fifty-four adult acute leukemia patients received MRI after diagnosis relying on FAB subtype and immunophenotyping including 28 cases with AML and 26 cases with ALL. MR imaging was obtained by the short time inversion recovery and T 1W spin echo technique of pelvis and femur at one time. The examining results of morphology and blood routine were collected at the same time. 15 age-matched volunteers were selected as controls. Results MRI showed that bone marrow of all patients were infiltrated by leukemia cells. The MRI appearance was classified into five patterns based on scope of focus. MRI patters from grade 1 to grade 3 were observed in most of patients with AML and in none with ALL, however, all patients with ALL distributed in grade 4 to grade 5. 5.Endoscopic mucosal resection for rectal carcinoid tumors

Objective To evaluate the efficacy and safety of endoscopic mucosal resection (EMR) for rectal carcinoid tumors. Methods From January 2006 to January 2009, EMR was performed in 28 patients with rectal carcinoids, who were followed up to evaluate the therapeutic effect and safety. Results Tumor diameters varied from 0.4 cm to 1. 2 cm (mean 0.7± 0. 2 cm). Negative resection margin was a-chieved in 26 cases (92. 9% ), tumor margin within 0. 1 cm of resection margin in 1 (7. 1% ) , and two margins coincided in 1 patient (7. 1% ). Hemostasis was performed with metal clips in 14 patients (50% ) and argon plasma coagulation (APC) in 9 (32. 1% ). Except for rectal bleeding in 1 patient (3. 6% ) , no other complications were observed. There was no recurrence in any patients during a follow-up of 6-36 months. Conclusion EMR is a useful and safe method for treatment of small rectal carcinoid tumor which does not cross submucosal layer.

7.Nursing cooperation in using lung inflation system for video-assisted thoracoscopic surgery for spontaneous pneumothorax

Objective To explore the nursing cooperation in use of lung inflation system for video-assisted thoracoscopic surgery (VATS) for spontaneous pneumothorax. Method The key nursing points in nursing 42 patients with spontaneous pneumothorax undergoing VATS were analyzed. Results The operations were all successful. One patient contracted lower left atelectasis but it was cured after the tracheal tube was re-adjusted. On day one, there were two cases of gas leakage during thoracic closed drainage:one was of diffuse pulmonary bulla, and the gas leakage stopped on the second day; In the other cases, the gas leakage stopped 2 days after the tube was withdrawn by 3cm probably because the drainage tube was inserted too deep at the beginning. 6-30 months follow-ups showed no recurrence of pneumothorax in the operated site. Conclusions The lung inflation system is safe during VATS for spontaneous pneumothorax. Careful preoperative preparation of the equipment and skillful nursing cooperation at all procedures are critical for the success of VATS.
8.Antioxidative effect of schisanhenol on adipokine expression in 3T3-L1 adipocyte

Objective To explore the effect of schisanhenol on adipokine expression in 3T3-L1 adipocyte and its related mechanism. Methods 3T3-L1 adipocyte was cultured in vitro and induced to differentiation and maturity. Glucokinase was added to culture medium to make an oxidative model. The expression of adipocytokines were detected under the circumstance of different doses and at different time points of schisanhenol. Results The expression of adiponectin, leptin, resistin and visfatin were decreased with the increase of glucokinase concentration. Concentration-dependent inhibition effect was most obvious in leptin (25 mU/ml Glucokinase vs Blank group, t =7.29, P<0.01). With pretreatment of oxidative stress, the adipocytokines increased as the doses of schisanhenol increased (t=6.31,P<0.01 in adiponectin;t=5.92, P<0.01 in leptin; t=3.77, P<0.05 in resistin; t=3.63,P<0.05 in visfatin). With the extension of schisanhenol effect, the expression of four adipokines showed the process of first decrease-then increase'. The effects of schisanhenol on adipokines were parallel with the alteration of oxidative stress. Conclusions Schisanhenol increased adipocytokines expression in 3T3-L1 adipocyte by reducing oxidative stress, and the increase of leptin and adiponectin were most obvious, which indicated that schisanhenol could play a role in the treatment of diabetes by Chinese herb wuweizi.

10.Analysis of Liposoluble Constituents in Fruit of Cnidium Monnieri by GC-MS

Objective:To investigate the liposoluble constituents in fruit of Cnidium monnieri. Methods:The liposoluble constitu-ents in fruit of C. monnieri were extracted by Soxet extraction. The extracted liposoluble constituents were methyl esterified and then analyzed by GC-MS for the first time. Results:Thirty-nine compounds (85. 33%) were identified from the liposoluble constituents in fruit of C. monnieri. Phytol (15. 98%), hexadecanoic acid (12. 73%), 9,12,15-octadecatrienoic acid (11. 02%), 9-octadecenoic acid (6. 33%) and 9,12-octadecadienoic acid (4. 77%) were the main constituents of the liposoluble constituents in fruit of C. mon-nieri. Conclusion:Phytol, hexadecanoic acid, 9,12,15-octadecatrienoic acid and 9-octadecenoic acid are the active ingredients in fruit of C. monnieri.
