2."Ethical Consideration on the Researches Using Human Biological Specimen-Revision Suggestion and Enlightenment of ""Common Rule"" for Subject Protection in the United States"
Chinese Medical Ethics 2016;29(5):849-851
The use of human biological specimen has great significance to the development of biomedicine. Meanwhile, the powerful electronic data set with sophisticated analytic techniques creates challenge to the protec-tion of private information. As for the research with human biological specimen, how to facilitate the research con-duct on the basis of ethical principles is one of the key considerations when the US Department of Health and Hu-man Service initiated the revision of Code of Federal Regulations ( CFR) -theCommon Rule for subject protec-tion. The Notice of Proposed Rulemaking ( NPRM) proposed:Respect for autonomy will be enforced by broad con-sent to enforce and waiver of consent intend to be rare;If the research risks only involve privacy protection, review process will be simplified under the premise of ensuring the implementation of privacy laws and other protective measures. The paper introduced the revisions related to the researches using human biological specimen in NPRM, analyzed the terms of broad consent, exemption and exclusion, and explored the elicitation to ethical review prac-tice in China.

Objective: To compare the clinical outcome between ministernotomy and standard median sternotomy in aortic root replacement. Methods: A total of 60 patients underwent aortic root replacement (ARR) between July 1999 and October 2001 in Fu Wai Hospital. 15 patients who underwent ARR by ministernotomy were defined as the MINI group. The comparable group was 45 patients who underwent ARR by standard median sternotomy (STD group). Results: There was no post-operative death in each group. Early postoperative complications occurred in 3 (20.0%) of the MINI group and in 5 (11.1%) of the STD group (P= 0.661). In the MINI group, both the mean opera ting time and the cardiopulmonary bypass time were significantly longer than that in STD group (P=0.027, P=0.015). There was no significantly difference in ao rtic cross-clamping time between two groups. In the MINI group, the mean intubation time, postoperative blood transfusion amount, the mediastinal drainage and duration of hospital stay were less than that of the STD group. But, they all showed no statistical significant differences. Conclusion: Both approaches for patients undergoing AAR resulted in acceptable morbidity. AAR by ministernotomy provides a potential benefit of less trauma and reduction of blood transfusion.

Objective To establish an experimental autoimmune encephalomyelitis (EAE) model in Wistar rats with myelin basic protein fragment (MBP69-85) and observe its pathological changes. Methods MBP69-85 dissolved in normal saline was mixed with complete Freund's adjuvant (CFA) (including 6 mg bacille Calmette Guerin) to prepare the encephalitogenic emulsion. Ten out of 70 Wistar rats were chosen as a control group, the others were divided equally into group A,B,C according to the difference of the encephalitogenic emulsion. Rats in group A were immunized with 50 μg MBP69-85 +CFA (including 6 mg BCG). Rats in group B were immunized with 25 μg MBP69-85+CFA (including 6 mg BCG). Rats in group C were immunized with 25 μg MBP69-85+CFA (including 12 mg BCG). The pathological changes of brain and spinal cord tissues were examined by light microscopy after HE staining and immunohistochemistry of MBP and NF. Results Some of the Wistar rats immunized with 50 μg MBP69-85 showed disorder at 12~16 days after immunization. The clinical symptoms included tail acratia or paralysis of tail and limbs, head tilt, etc. and the mean score was 2.38±1.89. There were infiltration of inflammatory cells inside nervous tissue and perivascular cuffings in HE stained sections. The immunohistochemistry of MBP and NF showed demyelination in the white matter and axon injury. Conclusion To some extent, the establishment of EAE depends on the dose of the immunizing antigen. The BCG in CFA was not the major cause of morbility of the rats. The EAE model induced with MBP69-85 in Wistar rats, showing typical clinical symptoms and pathological changes of multiple sclerosis, is a reliable animal model for the study of pathogenesis and treatment of multiple sclerosis.
