By comparing the differences of " Decision of the Standing Committee of the National People's Congress on the prohibition against narcotic drugs " and "Law of the People's Republic of China on Narcotics Control " and their implications for harm reduction,the article analyzed the ethical issues existing in applying harm reduction policy in HIV/AIDS prevention and control,and held that the new policy to promote the methadone use would play a major role for control of drug abuse and AIDS.
2.Effect of High Intensity Focused Ultrasound on Immunity of Patients with Advanced Primary Liver Cancer

Objective To investigate the effect of high intensity focused ultrasound(HIFU) on the immunity of patients with advanced primary liver cancer(PLC).Methods Forty cases of PLC admitted to our institution from Mar.2003 to Dec.2003 were included in this study.Patients were divided into 2 groups and received either HIFU or radio-frequency ablation(RFA) treatment randomly.CD3,CD4,CD8,CD4/CD8,NK,IL-2,TNF were chosen to assess the immune status before and after treatment.The results were compared statistically.(Results The) survival rate after HIFU was 80.0%,61.1%,42.9%,33.3% at 3 months,6 months,9 months and 1 year respectively,which was similar to that after RFA treatment.The changes of immunity parameters of CD3,CD4,CD8,CD4/CD8,NK,IL-2 and TNF were not significant after HIFU treatment.In addition,the differences of those parameters between HIFU group and RFA group were insignificant.Conclusion There are no detrimental effects on immunity in the early period after HIFU treatment.
3.Standardize and pragmatism- the key to promote the healthy development of obesity and metabolic surgery in China.

After more than half a century of development, obesity and metabolic surgery in western countries has become a complete surgical specialties, but the introduction of obesity and metabolic surgery was only 10 years old in China. The surgical treatment of obesity and related metabolic diseases, especially obesity of type 2 diabetes has been recognized and acknowledgd by the majority of surgical colleagues and patients as a new direction of development. The current trend of obesity and metabolic surgery is favorable, but there are many problems in the development of obesity and metabolic surgery in China, for example, the surgery nomenclature is not standardized, surgical indications are loosened, the surgical procedure is modified arbitrarily, the evaluation criteria is confusing, the postoperative follow-up protocol is not unified, etc. These problems require the majority of surgical colleagues to work together to make the standards scientifically a nd objectively in accordance with the actual situatioin of our country, so as to promote the healthy development of obesity and metabolic surgery in China.

4.The Distribution of Prestin on the Whole Basolateral Surface of Outer Hair Cells

Objective The prestin,a motor protein responsible for outer hair cell(OHC)electromotility,is expressed on the OHC surface.Previous experiments revealed that OHC electromotility and its associated nonlinear capacitance was mainly located at the OHC lateral wall and was absent at the apical cuticular plate and the basal nucleus pole.Immunofluorescent staining for prestin failed to demonstrate the prestin expression at the OHC basal ends in whole-mount preparation of the organ of Corti.The aim of this study is to investigate the distribution of prestin at OHC.Methods In this experiment,the localization of prestin protein in single dissociated OHCs from cochlea of normal mouse,rat and guinea pig,were examined by immunofluorescent staining and confocal microscopy.Results We found that prestin was uniformly expressed on the OHC basolateral surface,including its basal pole.No staining was observed on the cuticular plate and stereocilia.The OHC lateral wall had a trilaminate organization and was composed of the plasma membrane,cortical lattice,and subsurface cisternae.By with co-staining with a membrane marker di-8-ANEPPS,prestin-labeling was found locating at the outer layer of the OHC lateral wall.Further separating the plasma membrane from the underlying subsurface cisternae,using a hypotonic extracellular solution,prestin-labeling was shown locating at the plasma membrane instead of the subsurface cisternae.Conclusion The data revealed that prestin is expressed in the plasma membrane on the whole OHC basolateral surface.
5.The use of in vivo cryotechnique in combination with electrocardiograph to study cardiac microcirculation in living mouse

Objective The use of in vivo cryotechnique (IVCT) in combination with electrocardiograph (ECG) to study cardiac microcirculation under different hemodynamic conditions in living mouse.Methods Living mouse heart monitored by electrocardiograph was suffered from IVCT and freezing substitution under normal blood flow,myocardial ischemia or cardiac arrest conditions.Hematoxylin eosin (HE)staining,Schiff's staining and immunofluorescence staining for serum albumin,immunoglobulin were utilized on continuous paraffin sections,respectively.Confocal microscopy and statistical analyses were used.Results Comparing with normal hemodynamics,microvascular red cell volume reduction,morphology changed,myocardial cell glycogen loss,serum albumin ectopic distribution to myocardial cytoplasm,T tubular network failure and spacing width were happened in myocardial iscbemia condition; different shapes of red blood cells,myocardial cells glycogen deficiency,T tubular network failure and interval narrowing were found under cardiac arrest conditions.Conclusions Cardiac microcirculation,pathological changes of myocyte and its surrounding microenviroument in living mouse heart can be immediately captured in situ by the application of IVCT and ECG.
6.Clinical significance of monocyte chemoattractant protein-1, tissue factor in elderly patients with acute exacerbation of chronic obstructive pulmonary disease

Objective To investigate the levels and clinical significance of monocyte chemoattractant protein (MCP)-1 and tissue factor (TF) in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods Forty-nine elderly patients with AECOPD(AECOPD group) and 30 healthy controls (control group) were selected,and AECOPD group was divided into procalcitonin (PCT) increased group (PCT ≥ 0.5 μ g/L,19 cases) and PCT normal group (PCT < 0.5 μ g/L,30 cases) according to the level of serum PCT.The levels of plasma TF,serum MCP-1 and PCT were determined by enzyme linked immunosorbent assay.Results The levels of plasma TF and serum MCP-1 were (203.6 ± 92.9),(152.8 ±99.9) ng/L in AECOPD group,and (136.9 ±24.3),(87.5 ±41.5) ng/L in control group.There were significant differences in the levels of plasma TF and serum MCP-1 between AECOPD group and control group (P<0.01).The level of plasma TF was positively correlated with serum MCP-1 (r =0.673,P=0.029).The levels of plasma TF and serum MCP-1 were (215.3 ±71.2),(181.1 ±61.6) ng/L in PCT increased group,and (192.4 ±79.7),(137.3 ±74.4) ng/L in PCT normal group.There were significant differences in the levels of plasma TF and serum MCP-1 between PCT increased group and PCT normal group (P<0.05).Conclusions In patients with AECOPD,hypercoagulability state is activated,and it is more severe in the patients with increased PCT.The level of plasma TF is positively correlated with serum MCP-1 in patients with AECOPD.To monitor the levels of plasma TF and serum MCP-1 is particularly important for elderly patients of AECOPD with hypercoagulability state to prevent cardiovascular,lungs and cerebrovascular thrombotic disease.
7.The changes of plasma soluble CD40 and soluble CD40L in patients with chronic renal failure and their clinical significance

Objective To investigate the changes of plasma soluble CD40 (sCD40) and soluble CD40L (sCD40L) in patients with chronic renal failure( CRF) and their potiential mechanisms in renal injury and immunodeficiency. Method From September to December 2006, 30 CRF patients without hemodialysis (CRF group) in department of Nephrology and 30 uremic patients receiving maintenance hemodialysis for more than 3 months ( HD group) in dialysis center in Beijing TongRen Hospital of Capital Medical University, were enrolled in this study. The normal control group consisted of 20 healthy volunteers who matched with study subjects for age and gender. The levels of plasma sCD40 and sCD40L were measured by the method of enzyme linked immunosorbent assay (ELISA). The relationship between sCD40, sCD40L and related factors were analyzed, and the influence of hemodialysis on sCD40 and sCD40L were observed. ANOVA and Pearson correlation analysis were used in statistical analysis. Results (1) The sCD40 levels in CRF group and HD group were significantly higher than those in control group(P < 0.01). The sCD40 levels in HD group were also higher than those in CRF group(P < 0.01). (2) The sCD40L levels in CRF group and HD group were significantly higher than those in control group (P < 0.01). The sCD40L levels in HD group were slightly higher than those in CRF group, but without significant difference (P > 0.05). (3) In CRF group, the sCD40 levels correlated positively with serum creatinine (Scr) and C-reactive protein(CRP) (r = 0.637,P < 0.01,r = 0.551,P < 0.05). The correlations between sCD40L and Scr and CRT were also seen (r = 0.553, P < 0.05, r = 0.686,P < 0.01), whereas the correlation with blood pressure,hemoglobin (Hb), platelet(PLT), white blood cell(WBC), serum albumin(Alb), glucose (Glu),blood urea nitrogen(Bun)and serum lipid was no seen (P > 0.05). In HI) group, there were no correlations between sCD40,sCD40L and above parameters (P > 0.05), there were also no correlations between sCD40, sCD40L and duration of the treatment by dialysis and dialysis adequacy(Kt/V) (P > 0.05). (4)After one HD session, the post-HD levels of sCD40 were slightly higher than pre-HD levels, but without statistical difference ( P > 0.05). The post-HD levels of sCD40L were obviously lower than pre-HD levels, with significant difference (P < 0.01). Conclusions The levels of sCD40 and sCLMOL in CRF patients,particularly in HD patients, were significantly higher than those of controls, which may participate in pathogenesis of renal injury and immunodeficiency.

As urine flow dynamic test is widely used in clinical,merge detrusor dysfunction of benign prostatic hyperplasia treatment research also more and more.For normal detrusor contraction force and slight damage (Pdet > 20 cmH2O) of the operative efficacy of BPH patients have reached a consensus of scholars both at home and abroad.For merger severely impaired detrusor contraction force (Pdet 0-20 cmH2O) at odds with the treatment of benign prostatic hyperplasia patients,in combination with related literature,now just urethral catheterization or colostomy,the transurethral resection of the prostate in the same period the bladder again after colostomy,the comprehensive treatment evaluation IⅡ transurethral resection of the prostate surgery and review these main treatment.
9.Study of association between HLA class Ⅱ haplotypes and pemphigus vulgaris in Han nation of northeast China

Objective:To investigate the relationship between pemphigus vulgaris(PV) and HLA-DR,DQ haplotypes in Han nations of northeast China.Methods:Polymerase chain reaction-sequence specific primers(PCR-SSP) method was used to detect the HLA-DRB1 and DQB1 alleles of 27 PV patients of Han nation of northeast China, analysed haplotyes and compared with 99 healthy controls.Results:Compared with control group, the frequencies of the haplotypes of HLA-DRB1*140x-DQB1*0503,DRB1*140x-DQB1*0201,DRB1*120x-DQB1*0503 and DRB1*140x-DQB1*0302 increased significantly in PV group. After statistical test, the difference between the two groups was significant.Conclusion:The special haplotypes may contribute to genetic susceptibility to PV in northeast Chinese.
10.The Clinical Observation the Effects of the Re-Therapeutic on the Later Stage of Sudden Deafness

Objective To observe the effects of the re-therapeutic on later stage of sudden deafness.Methods Patients who met the criteria for sudden deafness and showed poor response tO conventional therapy over two months were recruited and assigned randomly for re-therapy.The pure tone audiometry was conducted before and after re-treatment in 103 patients(112 ears).Sodium bicarbonate and dexamethasone were iniected by intravenous drip for two days and batroxobin 5 BU for 6 days.Statistics were made in the effecency of the re-treatment.Results The effective rate of re-treatment group was 46.43% and the difference was significant before and after retreatment(P<0.01).Conclusion Combined use of sodium bicarbonate dexamethasone and batroxobin is effective in re-treating later sudden deafness.
