4.Clinical observation of of postoperative analgesia in continuous interscalene brachial plexus block by different strength ropivacaine mesylate combined with sufentanil

Objective To evaluate the efficacy and safety of postoperative analgesia in continuous Interscalene brachial plexus block by different strength ropivacaine mesylate combined with sufentanil.Methods Ninety cases of upper limb surgery in continuous peripheral nerve block (CPNB) were selected.The patients after the operation were divided randomly into three groups (A,B,and C):A group was treated with 0.158% ropivacaine mesylate + 0.5 μg/ml sufentanil,B group with 0.238% ropivacaine mesylate + 0.5 μg/ml sufentanil,and C group with 0.238% ropivacaine mesylate.20 minutes before the end of surgery,controlled analgesia device was connected with infusion rate 0.05 ml/(kg · h),PCA 1 ml,and lock time 15 min.The pain visual analog score (VAS),Ramesay sedation score,and postoperative motor nerve block recovery ratings (modified Bromage classification) were recorded at the 4,8,16,32,48 h time point postoperatively.The times of using PCA and the frequency of the adverse reaction were also recorded.Results Three groups all got good analgesic effect at 4,8,16,32,48 h time point postoperatively (VAS 1.58 ± 9 ; 1.90 ± 0.8 ; 1.62 ± 0.6 ; 1.59 ± 0.7 ; 1.53 ± 0.7) (P > 0.05) ; There were no significant difference among the three groups in Ramesay score,modified Bromage score,limb discomfort,nausea,vomiting,lethargy,pruritus and respiratory depression (P > 0.05),but the time of using PCA in A group were significantly more than that in B or C group (P < 0.05).Conclusions Sufentanil can enhance the analgesic effect significantly when the ropivacaine mesylate is in low concentrations (0.158% ~0.238%),but when the ropivacaine mesylate concentration get up 0.238%,there will be similar analgesic efficacy and safety between ropivacaine mesylate only and ropivacaine mesylate combined with sufentanil.
5.The performance evaluation of G test on patients with invasive pulmonary fungal infections complicated with chronic obstructive pulmonary disease

Objective To evaluate the value of (1 ,3)‐β‐D‐glucan(G test) in the diagnosis of invasive pulmonary fungal infection (IPFI)in patients with chronic obstructive pulmonary disease (COPD) .Methods 96 COPD patients with high risk of IPFI were en‐rolled in the study ,and were divided into IPFI group and non‐IPFI group .The G test were performed on those people while the tra‐ditional methods sputum smear and fungal culture were also performed .The (1 ,3)‐β‐D‐glucan concentrations of IPFI group and non‐IPFI group were compared ,and then the areas under receiver operating characteristic curve (ROC) were calculated .The sensi‐tivity ,specificity ,positive predictive value ,negative predictive value ,and the area under ROC of the two methods (G test and tradi‐tional methods) were compared .Results The sensitivity ,specificity ,positive predictive value ,negative predictive value of G test were 89 .5% ,89 .6% ,68 .0% ,97 .2% ,respectively ,when 20 pg/mL was the critical value .Areas under curve were 0 .942 for G test ,and 0 .790 for traditional method .Conclusion The detection of (1 ,3)‐β‐D‐glucan might be faster and with higher positive rate than traditional method ,and could be used for the early diagnosis of IPFI ,provide reference for the treatment .
6.A comparative study of application effect of interactive debridement in patients with abdominal surgical incision infection and tissue necrosis

Objective To study the interactive debridement methods in treatment and nursing of the wound in patients with abdominal surgical incision infection.Methods In May 2010 to December 2013 in West China hospital of Sichuan University,54 patients with postoperative incision infection accompanied with tissue necrosis after gastrointestinal surgery were divided into two groups,23 cases who accepted interactive debridement treatment were set as the observation group,31 cases with traditional dressing hypertonic saline gauze debridement were named as the control group.The wound debridement completion time,wound healing velocity,wound healing time,dressing change frequency and cost were compared between two groups of patients.Results Compared with those of the control group,in the observation group,the wound debridement completion time was shorter,wound healing velocity was faster,dressing change frequency was longer.Although single dressing change cost was higher in the observation group,but the total dressing change cost was lower compared with the control group.Conclusions The interactive debridement methods used in treatment and nursing of patients with abdominal surgical incision infection showed good effect and safety,which is worthy of wide clinical application.

8.Reasonable Application of Multi-enzyme Abluent in Process of Flexible Endoscope Cleaning and Disinfecting

OBJECTIVE To discuss the reasonable application of multi-enzyme abluent in the process of endoscopic cleaning and disinfecting.METHODS The multi-enzyme abluent was reasonably used to clean the endoscope in the pre-cleaning and disinfection process at thebedside,to enable it to achieve the best cleaning effect.RESULTS The reasonable application of multi-enzyme abluent could enhance the effect of endoscope cleaning to ensure the success of endoscopic disinfection.CONCLUSIONS Multi-enzyme abluent can protect the successful endoscopic disinfection and prevent the iatrogenic infection.
9.Reduction mammaplasty for patients with breast ptosis and hypertrophy

Objective To introduce the methods and cosmetic effect of reduction mammaplasty in patients with benign diseases of breast. Methods From December 1990 to November 2001, reduction mammaplasty was performed in 15 patients with breast hypertrophy, ptosis and benign diseases, including hamartoma (1 case), adenofibroma (1 case), lobular hyperplasia with adenofibroma (2 cases), mild papillomatosis (2 cases), ductal ectasia with secretion depot (2 cases), and lobular and ductal epithelial hyperplasia (7 cases). Nine patients received reduction mammaplasty using an inferior pyramidal pedicle technique, and six patients received Mckissock vertical bipedicle technique. Results The successful rate was high (89.6 %), except that three mammary glands developed necrosis of whole areola in Mckissock procedure group. Complications caused by breast hypertrophy and benign diseases were improved apparently. Conclusion Reduction mammaplasty is the best option for the treatment of breast hypertrophy and ptosis with benign diseases.
10.Establishment of porcine combined liver/small bowel transplantation model

ObjectiveTo set up the porcine combined liver/small bowel orthotopic allotransplantation model.MethodThe donors including liver, about 2 to 3?m proximal jejunum, duodenum and a sma ll part of pancreas head were transplanted to the hybridized long-white recipie nt pigs after the resection of their livers and the majority of small bowels.ResultWithout using immunosuppressive drugs, the median survival time of the animals w as 92?h with the longest survival time being 196?h, and there was about 75?% of the animals lived for more than 24?h after operation.ConclusionThe successful establishment of the combined liver/small bowel transplantation m odel will do great favor to our future clinical practice and further study of th e combined liver/small bowel transplantation.
