1.Mechanisms of mesenchymal stem cells' effect on the graft-versus-host disease after hematopoietic stem cell transplantation

Allogeneic hematopoietie stem cell transplantation(alloHSCT) is a potentially curative therapy for patients with hematological malignant disease, autoimmune disease or hereditary disease.However, graftversus-host disease (GVHD), which is one of the most serious complications, influences the prognosis of alloHSCT. Mechanisms of GVHD is so complicated that there is still no perfect way to treat the patients at present. Recently, mesenchymal stem cells(MSC) become new hotspots because of their immunomodulatory properties and potential to suppress alloreactive T cell responses.Animal experiments showed that MSC were effective during the prevention and treatment of GVHD. This paper is to summarize the acting mechanism of MSC' effect on the prevention and treatment of GVHD after alloHSCT.
2.Study of optimization nutrition support in critically ill patients

Objective To assess whether the delivery of 100% of the nutritional target from days 4 to 8 in the ICU with enteral nutrition plus supplemental parenteral nutrition(SPN) could optimize clinical outcome .Methods A total of 300 critically ill patients who had received less than 60% of their energy target from enteral nutrition(EN) were randomized to treatment group(n=150) and control group (n=150) .After using the stochastic indicator method after day 3 of admission to the ICU ,one received EN and the other received EN plus SPN during days 4 to 8 ,then we calculated energy targets with indirect calorimetry repeatedly .Results The mean energy delivery between day 4 and 8 was 28 kcal? kg -1 ? d-1 for the EN plus SPN group ,compared with 20 kcal?kg -1 ?d-1 for the EN group .Between days 9 and 28 ,the EN plus SPN group had a lower incidence of nosocomial infection com‐pared with EN group (P<0 .05) ,the mechanical ventilation time of the patients without nosocomial infection in EN plus SPN group was significantly lower than that of the EN group (P<0 .05) .The in-hospital mortality and the in-ICU mortality were similar in the two groups(P>0 .05) .Conclusion Individually optimized nutritional supplementation with enteral nutrition plus supplemen‐tal parenteral nutrition starting 4 days after ICU admission could reduce nosocomial infections and should be considered as a strate‐gy to improve clinical outcome in patients in the ICU for whom EN is insufficient .

Infantile spasms is a type of refractory epilepsy syndrome.This epilepsy syndrome is characterized by special tonic spasms,a peculiar set of electroencephalographic findings termed hypsarrhythmia,and arrest of psychomotor development in most patients.The etiology is not clearly understood.Recently,mutations of the arista less related homeobox gene(ARX),cyclin-dependent kinase-like 5(CDKL5)/se-rine/threonine kinase 9 gene(STK9),membrane associated guanylate kinase 2 gene(MAGI2),et al,and abnormal chromosome had been found to be responsible for infantile spasms.In this review,progress of infantile spasms in molecular genetics are discussed.
4.Application of the plastic principles in surgical treatment for diabetic foot ulcer

ObjectiveTo explore the application of the plastic principles in surgical treatment for diabetic foot ulcer and analyze the therapeutic efficacy.Methods51 cases of diabetic foot ulcer repaired with surgical treatment were analyzed.Results26 cases repaired with free skin graft,1 1 cases repaired with skin flap graft,3 cases repaired with amputation,the percentage of first stage healing rate was 91.4%.ConclusionApplication of the plastic principles in surgical treatment for diabetic foot ulcer,could improve the healing percentage and reduce the rate of amputation.

8.Comparative observation of silicone oil and ointment applied to the lacrimal laser angioplasty

To compare the adjuvant therapeutic effect of silicone oil and ointment applied to the lacrimal laser angioplasty.METHODS: Totally, 134 eyes of 102 cases who were diagnosed as lacrimal duct obstruction ( except chronic dacryocystitis) were divided into two groups. Sixty eyes were performed laser plastic operation and silicone oil filling. Seventy - four eyes were adopted laser plastic operation with cloth enzyme dexamethasone eye ointment filling. All patients were followed up for 3 ~ 15mo. The clinical effect and the number of irrigation of two groups were compared 3mo after treatment, and to assess the merits of the method.RESULTS: The efficiency of the silicone oil group was 87% and that of the eye ointment group was 84%. There was no statistically significant difference (P>0. 05). The number of lacrimal duct flushing of the silicone oil group was 7. 20±1. 22 times among postoperative 3mo, and that of the eye ointment group was 7. 96 ± 1. 79 times. The number of lacrimal duct exploration through of the silicone oil group was 0. 62 ± 0. 87 times, that of the eye ointment group was 1. 43 ± 1. 48 times. There was statistically significant differences compared between the two groups (both P<0. 05).CONCLUSlON:The effect of laser plastic operation and silicone oil filling treating obstructive diseases of lacrimal duct is similar to laser plastic operation with cloth enzyme dexamethasone eye ointment, but the postoperative care of the former is more simple. Patients can significantly reduce the frequency of lacrimal duct flushing and lacrimal duct exploration through.

10.Effects of electroacupuncture at Tianshu (ST25) on pro- and anti-inflammatory cytokines in rats with severe acute pancreatitis.

Objective: To observe the effects of electroacupuncture (EA) at Tianshu (ST25) on pro- and anti-inflammatory cytokines in sodium taurocholate-induced severe acute pancreatitis (SAP) in rats. Methods: Sixty-six male Sprague-Dawley rats were randomly divided into three groups: sham-operation group, SAP group and EA group. An SAP model was established by injecting 3.5% sodium taurocholate at a dose of 1 mL/kg through puncturing the pancreatic duct. In the EA group, EA at ST25 was performed with sparse-dense wave (2/100 Hz, 2 mA for 30 min) immediately after sodium taurocholate injection and 30 min before sacrifice. Rats were sacrificed at 3 (n=7), 6 (n=7) and 12 h (n=8) after operation. Levels of tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6) and IL-10 in serum were estimated by enzyme-linked immunosorbent assay. Pathological changes of pancreatic tissues were examined by hematoxylin and eosin staining and observed under a microscope and also scored. Results: The levels of serum TNF-α and IL-6 of SAP rats were significantly higher than those of sham-operation rats at 3, 6 and 12 h. The level of IL-10 in serum of SAP rats was significantly higher than that of sham-operation rats at 3 and 6 h (P<0.05). EA at ST25 down-regulated the levels of TNF-α and IL-6 in serum and up-regulated the level of IL-10 and the ratio of IL-10/TNF-α, also attenuated the morphological damages at 6 and 12 h (P<0.05) in rats with SAP. The pathological scores of pancreatic tissues in the EA group were significantly less than those in the SAP group at 6 and 12 h (P<0.05). Conclusion: EA at ST25 may have a therapeutic effect on rats with SAP by down-regulating the levels of TNF-α and IL-6 and up-regulating the level of IL-10 to re-establish the balance of pro- and anti-inflammatory cytokines, and to ameliorate the inflammatory response of SAP.
