4.Seeking the common end of both doctors' and patients' value orientation
Chinese Journal of Hospital Administration 1996;0(10):-
How to handle the relationship between doctors and patients is currently a hot spot of society. The author first expounds the fundamental propositions in the theories of the science of value. Then, using the theories of the science of value, he first describes the relationship between the object and subject of value, viz. who comes first, the patients or the staff, arguing that in the mind of the hospital director there should always be an unbiased balance of value benefiting both sides. Then he describes the realistic form in which value exists, viz. which is more important, the value of utility or the value of effectiveness, arguing that the hospital director should lay equal emphasis on the staffs value of utility and the patients value of utility, thus satisfying both sides. Thirdly, the author describes value accounting and the cost of value, viz. the dual unity of the values of effects and effectiveness, arguing that creating value while paying cost indicates the dual nature of the practice of hospital administration. Hence the need to lower cost as much as possible and create greater value.
5.Optimization on Formulation of Controlled-onset Extended-release Tablets of Nicorandil by Central Composite Design-response Surface Method
Herald of Medicine 2016;35(8):867-870
Objective To optimize the formulation of controlled-onset extended-release tablets of nicorandilby central composite design-response surface method. Methods The tablets containing nicorandil were prepared bydry-compression coating technique. The influence factors included the amount of HPMC, the amount of EC, and the ratio of lactose/MCC in coating film. The evaluation parameter was Q4 h and Q5 h . The possibly optimal formulation was predicted by response surface method. Results The OD was simulated using second-order polynomial equation and the r2was 0.970 1.Optimal prescription was as follows:HPMC 89.96 mg, EC 46.21 mg, Lactose/MCC 1.87.Bias between the observed and predicted values of the OD was within 0.43%. Conclusion The controlled-onset extended-release tablets of nicorandil can release drug quickly in vitro at the predetermined time.Central composite design-response surface method can be used to optimize the formulation and the model developed in this study was proved highly predictable.
6.Emergence of novel variants of gyrA and parC genes in a group of multiple resistant Klebsiella
Chinese Journal of Clinical Infectious Diseases 2013;(2):98-103
Objective To investigate the distribution and variety of quinolone-resistance genes in a group of multiple resistant Klebsiella. Methods A total of 20 strains of multiple resistant Klebsiella were collected from inpatients in Huai' an First People' s Hospital of Nanjing Medical University during February 2010 and March 2012.Strains were identified by molecular identification, and quinolone target genes (gyrA,ParC) and quinolone-resistance genes mediated by mobile genetic elements (qnrA,qnrB,qnrS,aac (6′)-Ⅰ b-Cr,qepA) were analyzed with polymerase chain reaction (PCR).Results In 20 strains of Klebsiella,19 were Klebsiella pneumonia, and 1 was Klebsiella variicola.gyrA and parC genes were found in all 20 strains, and gyrA mutation was found in 11 strains (55.0%),andparC mutation was also found in 11strains (55.0%).aac(6′)-Ⅰ b-cr was positive in 10 strains (50.0%),qnrA was positive in 1 strain (5.0%),and qnrB was positive in 3 strains (15.0%).gyrA and parC in strain No.6 and No.10 were both novel variants (GenBank registration number:JX123016,JX123017,JX144393,JX144394).Conclusion Quinolone-resistance-determining region plays a key role in resistance to quinolones in this group of Klebsiella, and it is the first report on the emergence of novel variants of gyrA and parC in one Klebsiella pneumonia in China.
7.Differential diagnosis of lung cancer in patients with a history of breast cancer
Journal of International Oncology 2011;38(5):367-370
A differential diagnosis between primary and metastatic cancers is required in lung cancer patients with a history of breast cancer, However, the limited morphological findings are not enough to differentiate many atypical cases. Essential immunohistochemical and molecular markers are helpful for differential diagnosis. Combined markers currently specific and sensitive to lung or breast carcinomas can improve the efficacy of differential diagnosis between primary lung cancers and metastatic breast cancers.
8.aac(6')-Ⅰ b gene in drug-resistant strains of Pseudomonas aeruginosa
Chinese Journal of Clinical Infectious Diseases 2011;4(3):163-167
Objective To investigate the distribution of aminoglycoside modifying enzyme genes (AMEs) and 16S rRNA methylase genes in drug-resistant strains of Pseudomonas aeruginosa. Methods Twenty strains of drug-resistant Pseudomonas aeruginosa were isolated from sputum and wound secretion samples collected from the First People's Hospital of Huai' an in Jiangsu province. Eight AMEs [aac(3)-Ⅰ , aac(3)-Ⅱ, aac(6')-Ⅰb,aac(6')-Ⅱ, ant{2)-Ⅰ ,ant(3)-Ⅰ , ant(4')-Ⅰ , aph(3')-Ⅱb] and 6 16S rRNA methylase genes (armA, rmtA, rmtB, rmtC, rmtD, npmA) were analyzed by PCR and verified by DNA sequencing. Results Out of 20 strains of Pseudomonas aeruginosa, aac(6')-Ⅱ was positive in 8 strains (40.0% ) , ant2- Ⅰ in 8 strains (40.0% ) , aac(3)-Ⅱ in 5 strains (25.0% ) , aac(6')- Ⅰ b in 2 strains (10.0% ) and rmtB in 1 strains (5.0% ) , respectively. The rest 9 genes were not detected. Among 2 strains harboring aac(6')- Ⅰ b, DNA sequencing confirmed that 1 was aac(6')- Ⅰ b (the clssical type) and another was aac(6')- Ⅰ b-cr. Conclusion Gene aac(6')- Ⅰ b-cr exists in drug-resistant Pseudomonas aeruginosa, and it has modifying effect on both aminoglycosides and quinolones.
9.Nerve Regeneration after Spinal Cord Injury(review)
Chinese Journal of Rehabilitation Theory and Practice 2006;12(7):556-558
There was an early view that the injured neurons and axons in central neural system couldn't regenerate after injury. However, it was found by recent researches that the injured neurons and axons can recover in different degrees after spinal cord injury of adult animals. In proper environment, some injured axons of central nervous system can regenerate slightly and form functional synaptic connections with target cells. Based on that, application of neurotrophic factors and cellular transplant therapy might be developed to treat spinal cord injury in the future.
