1.Effect of improved laparoscopic techniques on ability of control urine in patients with prostate cancer

Objective To study the effect of improved laparoscopic techniques on ability of control urine in patients with prostate cancer.Methods 96 patients with prostate cancer were randomly divided into two groups,the observation group(51 cases) and the control group(45 cases).The patients in the control group were operated through the conventional treatment,while the patients in the observation group were operated through the improved laparoscopic technique,which protected the external urethral sphincter,reserved urinary nerve and reconstructed the bladder neck.The ability to control urine for 30,60 and 90 days were compared between two groups.Results The time of operation and bleeding of operation had no significant differences between two groups (all P > 0.05).There were significant differences between two groups in indwelling catheter time and the ability to control urine for 30 ;60 and 90 days (t=10.0359,P<0.05;t=7.9746,14.0205,10.8445,all P<0.01).Conclusion Improved laparoscopic technique can improve the ability of control urine in patients with prostate cancer after operation.
2.Transureteroscopic holmium laser technology in urethral stricture

Objective To study transureteroscopic holmium laser technology in urethral stricture.Methods 68 patients with urethral stricture were randomly divided into the two groups,the observation group (n =34 cases) and the control group (n =34 cases).The patients in the observation group were operated through transureteroscopic holmium laser technology,while the patients in the control group were operated through cold knife urethrotomy technology.The operation time,blood loss,hospital stay,maximum urinary flow rate,urinary extravasation,secondary bleeding,rectal injury,recurrence and reoperation were observaed.Results The operative time was longer in the control group [(31.24 ± 9.71) min vs (22.37 ± 9.22) min] (t =3.914,P < 0.05).The blood loss,hospital stay in the observation group were less than these in the control group[(13.28 ± 5.24)mL,(4.06 ± 1.16)d vs (28.31 ± 7.19) mL,(5.92 ± 1.71) d)] (t =4.091,2.776,all P < 0.05).After treatment,patients have a significantly increased maximum urinary flow rate (t =5.163,9.772,P < 0.05,P < 0.01),and compared with the control group after treatment,the observation group was significantly more improved Qmax (t =4.291,P < 0.05).Postoperative control group of 12 patients developed complications,the incidence was 35.3%.The observation group had complications occurred in 2 patients,the rate was 5.9%.The incidence of complications in the observation group was significantly less than the control group (x2 =7.240,P < 0.05).Patients were followed up for 1 year,and the recurrence rate in the control group(16%,11%) were significantly higher than those in the observation group (4%,1%) (x2 =11.290,13.860,all P < 0.01).Conclusion Transureteroscopic holmium laser technology had a significant effect,fewer complications,a low incidence of recurrence and surgery in patients with urethral stricture.
3.Effects of minimal residue of polyacryamide hydrogel on prosthesis implanted into the breasts simultaneously

BACKGROUND:Up to date,whether prosthesis can be implanted simultaneously with the residue of polyacryamide hydrogel (PAMHG) in the breasts still not consistent. OBJECTIVE:To explore the feasibility and long-term curative effect of prosthesis implanted simultaneously with the residue of PAMHG in the breasts. DESIGN,TIME AND SETTING:The clinical cases analysis was performed at the Department of Plastic and Aesthetic Surgery,No.1 People's Hospital of Nanning were collected. PARTICIPANTS:Totally 23 cases,aged 22-42 years,who received silicone gel breast implant (SBI,100-400 mL) were collected. The time from injection to hospital visit was 0.4-4.5 years. METHOD:PAMHG was cleared as possible,followed by implanted 200-260 mL SBI was via retro-mammary space or sub-pectoral space when cases without inflammation reaction. MAIN OUTCOME MEASURES:The wound healing,regression of breast induration,as well as complication and long-term curative effect was followed up. RESULT:The 0.3-3 years follow-up showed that the whole PAMHG was cleared in 7 cases,a majority of PAMHG was cleared in 8 patients,and partially PAMHG was remained in ectopectoralis of 8 patients. All the breasts of the patients were good at exterior. No obvious nodules,infection,ulcer or gel migration occurred. All patients were satisfied with the surgical results. CONCLUSION:It is feasible and safe to implant prosthesis simultaneously with minimal residue of PAMHG if there is no inflammatory reaction.
4.Clinical Study on 180 Cases of Prophylactic Treatment with Bifid Triple Viable Enteric-coated Capsules for Preventing Children Diarrhea after Pneumonia

OBJECTIVE: To observe clinical efficacy of Bifid tripie viable enteric-coated capsules in the treatment of children diarrhea after pneumonia. METHODS: 360 cases of children pneumonia were randomly divided into two groups. Treatment group were treated with antibiotics and Bifid triple viable enteric-coated capsules in duration of hospital stay(n=180). Control group were treated with antibiotics only (n=180). Pancreatic neuroendoerine tumors (PNET) is a complicated and heterogeneous group of neoplasia,and the incidence of PNET is growing rapidly recently.The current study on PNET lags behind what is known of other pancreatic tumors.Therefore,understanding of the molecular pathology is important for improving the early diagnosis,treatment and prognosis.With the development of molecular biology,there are already several potential targets proved to have application role in treatment of PNET.Here,the authors present the current knowledge about the molecular alterations of PNET.
6.Prevention of fibrous capsule contracture using triamcinolone combined with vitamin E following silica gel prosthesis implantation

OBJECTIVE: To observe the effect of triamcinolone combined with vitamin E on preventing fibrous capsule contracture following the second breast augmentation. METHODS: Totally 65 patients who had 130 breasts receiving the second breast augmentation at the Department of Plastic and Aesthetic Surgery, No.1 People's Hospital of Nanning were collected and served as the treatment group, including 47 cases with capsules contracture and 18 cases with hydrogel clearing. Tri-Kort was injected after the second breasts augmentation, and vitamin E was taken orally for 3 months. Additional 65 cases with 130 breasts received the second breast augmentation were served as control group. The follow-up was performed after half month, and the curative effect was evaluated by Baker grading at 1 year after operation. RESULTS: All cases in the two groups were received 6-24 months follow-up. Only 3 cases (5 breasts) in 65 patients (130 breasts) in the treatment group had capsule contracture (Baker II-III), with 3.8% of capsule contracture, which was obviously less than 18% of capsule contracture in the control group (P < 0.01,13 cases with 23 breasts in the control group). Simultaneously, no drug adverse reaction occurred in the treatment group. CONCLUSION: Triamcinolone combined with vitamin E can obviously depress the rate of capsule contracture after the second breast implantation.
7.Impacts of Ruangan-Lidan tang on the the immune function and survival for liver cancer patients receiving TACE combined with 3D conformal radiotherapy

Objective To observe the impacts of Ruangan-Lidan Tang on immune function for liver cancer patients receiving TACE combined with 3D conformal radiotherapy, evaluation of short-term efficacy, survival, to explore the changes of cellular immune function in the treatment. Methods 86 cases of liver cancer patients were randomly divided into treatment group and control group were taken in 3D conformal radiotherapy to 50-60GY/5-6 week for fourth weeks after TACE, the treatment group taking Ruangan-Lidan Tang intervention, control group without the use of traditional Chinese medicine intervention.Before the start, after 1 weeks of TACE, within 3 days before radiotherapy,end of radiotherapy, a total of 4 times for T cell differentiation antigen were measured, and between two groups were compared with t test, 2 months after radiotherapy CT or MRI evaluation of curative effect, survival. Results 5 cases of loss of cases, the total cases were 81 patients, including 40 cases of treatment group, 41 cases in After 1 weeks of TACE, CD3+, CD4+, CD8+of the treatment group were (55.15±4.76)%, (31.88±5.50)%, (22.00±3.18)% of the control group were (53.39±5.00)%, (30.49±5.94)%, (23.39±3.41)%. There was no significant difference between two groups (P>0.05); Before radiotherapy, CD3+, CD4+, CD8+ of the treatment group were (59.05±5.91)%, (40.55±6.17)%, (18.63±3.14)%, of the control group were 55.88±4.65%,33.88±4.41%,22.00±3.78%. There was significant difference between two groups(P<0.01);At the end of radiotherapy, CD3+,CD4+,CD8+ of the treatment group were (61.03±5.54)%, (42.65±4.57)%, (18.10±3.20)%, of the control group were (55.56±5.14)%, (32.95±4.01)%, (23.32±2.69)%. There was significant difference between two groups (P<0.01). Short-term efficacy(CR+PR)of the treatment group was higher compared with the control group(82.50%, 75.61%), but no significant difference (U=1.177, P>0.05). In the treatment group, the median survival time was 22 months,the average survival time was 21.7 months, the half year survival rate was (97.5± 2.5)%(39/40);In the control group, the median survival time was 20 months, the average survival time was 19.3 months, the half year survival rate was (94.9±3.5)% (37/41). Follow-up survival analysis, treatment group than in the control group increased, but there was no statistical difference (P=0.132, P>0.05). Conclusion Ruangan-Lidan Tang can improve the immune function of liver cancer patients receiving TACE combined with radiotherapy, but in the Short-term efficacy and survival did not reflect advantage.
8.Transurethral plasma kinetic enucleation of the prostate for benign prostatic hyperplasia more than 80 mL

Objective To evaluate the clinical effect and safety of transurethral enucleation of prostate with the bipolar plasma kinetic technique(PKEP)in the treatment of benign prostate hyperplasia (BPH)more than 80 mL.Methods The data from the 116 patients who underwent the PKEP were analyzed retrospectively.The clinical parameters include operation time,blood loss, postoperative catheter retention time,postoperative complication rates,the differences of the clinical parameters pre-and postopera-tively were compared,include maximum urine flow rate(Qmax),residual urine(RU),international prostate symptoms score(IPSS), and quality of life(QOL).Results Mean operation time was (87.46±25.01)min,Mean blood loss was (129.15±44.35)mL.Mean resected tissue weight was (77.67±19.56)g.No patient had the transurethral resection syndrome(TURS),All cases were followed up for 3 to 6 months,the clinical parameters pre-and postoperatively was respectively:Qmax(6.04±2.37)mL/s vs.(17.85±2.55) mL/s;RU(116.25±53.18)mL vs.(8.85±7.66)mL;IPSS(25.06±4.23)vs.(5.90± 1.91);QOL(4.85 ±0.65)vs.(1.71± 0.54).Conclusion The transurethral enucleation of prostate with the bipolar PKEP resects the proliferated prostate cleanly,had lesser bleeding and complication rates,had advantages of high safety,and satisfactory efficacy for the treatment of BPH more than 80 mL.

ObjectiveTo summarize and investigate the incidence,reason,diagnosis and treatment of postoperative early cardiac arrhythmia after orthotopic liver transplantation(OLT).Method A retrospective study was made for the incidence,dignosis and treatment of cardiac arrhythmia (two weeks after OLT) following OLT from June 2004 to January 2012 in the Beijing You-An Hospital.ResultsIn 500 patients who received OLT,Cardiac arrhythmia developed in 82 patients (16.4%).Among these cardiac arrhythmia,35(7.0 %) were sick sinus syndrome (including severe sinus bradycardia ),18 (3.6 % ) were paroxysmal supraventricular tachycardia,21(4.2 %) were atrial fibrillation,8(1.6 %,including 2 patients with torsades de pointes) were ventricular tachycardia and 4 (0.8 % )were cardiac arrest.Mortality rate after OLT relate to cardiac arrhythmia was 0.4% (2 patients).Cardiac arrhythmia was mainly correlated with four factors:(1)whether patient had heart disease before OLT or not(x2 =15.82,P<0.01),(2)Prolonged QT interval in patients with end-stage liver disease before OLT(x2 =11.00,P<0.01).ConclusionsCardiac arrhythmia was common complication after OLT,and it can lead to death of recipients.Careful evaluation to recipients before OLT,controlling fluid load after OLT,keeping the balance of the electrolyte,acidity and alkalescence,giving intensive monitor to patients with heart disease before OLT and prolonged QT interval are the key factor to reduce incidence and mortality of cardiac arrhythmia.Application of medication and cardiac pacemaker can prevent cardiovascular accident after OLT.
10.CT guided embedding of 125I by puncturing tissue through the cutis in patients with metastatic lung tumors

Objective To explore the clinical application and safety of treatment of percutaneous radioactive 125I seed implantation treatment in lung metastases under CT guidance.Methods Twenty-seven lung metastatic malignancy cases (67 nodules) were studied.Eighteen cases (46 nodules) were hepatic cancer,4 cases (9 nodules) were prostate cancer,and 5 cases (12 nodules) were breast carcinoma.Diameters of lung nodules ranged from 0.5 cm to 4.0 cm with an average diameter of 2.1 cm.125I seeds were embedded under CT guidance,using 2 to 33 particles/nodule with an activity of 18.5 to 29.6 MBq/grain for each particle.Tumor matched peripheral dose was 90-120 Gy.Postoperative validation and quality evaluation followed.Results Four months later,24 nodules showed CR,30 showed PR,5 showed NC and 8 showed PD.The total effective rate was 80.6% (54/67).In the course of treatment,11 patients had pneumothorax,3 had heavy lung compression and 4 had hemoptysis.All conditions were improved by pleural puncture or under close follow-up observation.Conclusion 125I seed implantation is an effective and safe technique in treatment of metastatic lung tumors under CT guidance.
