Atrial fibrillation (AF) is an independent risk factors for stroke.The stroke outcome in patients with AF is poor.The mortality and morbidity were also higher.As for the higher risk of stroke in patients with AF,the application of anticoagulant drugs is needed.Apixaban is an emerging oral direct Ⅹ a factor inhibitor in recent years.Compared to the traditional anticoagulants,such as warfarin,apixaban shows certain advantages in the prevention of stroke and systemic embolism in patients with AF.
4.Analysis of secondary displacement and femoral head necrosis after conservative treatment in undisplaced femoral neck fractures

[Objective]Through the clinical investigation of the conversative treatment of undisplaced femoral necks,to analyze the occurrence of secondary displacement and femoral head necrosis.[Method]The medical records of 216 patients with undisplaced femoral neck fractures who were primarily treated conservatively from January,2000 to January,2004 were analyzed.Among that,186 cases is in accordance with the selected standard,which were consisted of 82 men and 104 women.(the average age 67.5).One hundred and forty-one fractures were given to bed rest in rotation-proof shoes,and walking on the crutches was followed after four weeks.Forty-five patients began to get out of bed with double crutches on the third day.When displacement happened,the operation was carried out.Once X-rays showed the fracture line unclearly,it is considered as bone healing.The early stage of femoral head necrosis was defined by sclerotic region or cyst in the femoral head.[Result]Sixty-eight fractures(36.6%) united and secondary considered displacement occurred in 118 cases(63.4%); the average time was 16 days (2~68 d) after injury when the displacement is found.In all primary healing patients,femoral head necrosis rate was 25%.The discovery time of necrosis was between 4 and 18 months,the most was 36 cases in the interval of 6 to 12 months.As for patients of secondary displacement,92 for closed reduction and hollow screws fixation,and the head necrosis happened in 55 patients(59.8%).The discovery time was from 4 to 18 months,the most in 6 to 12 months was 36 cases.The operation of open reduction and internal fixation with hollow screws was in 26 cases,73% of necrosis rate for it.The time was range from 3 to 12 months,11 case was the most in the period of 3 to 6 months.[Conclusion]Undispalced femoral neck fractures treated conservatively bear higher risk of secondary displacement and femoral head necrosis.In addition with more mortality and general complications,we therefore recommend primary operative stabilization of undisplaced femoral neck fractures.

Objective To discuss the effect of abnormal biliary dynamics on the formation of cholelithiasis.Methods A total of 35 cases of gallstone(Gallstone Group),25 cases of gallbladder polypoid lesions(Polypus Group),and 30 normal cases(Control Group) were included in this study.The fasting gallbladder volume was measured with B-ultrasonography,the serum concentration of vasoactive intestinal poly-peptide(VIP) was determined with radioimmunoassay,and the expression of cholecystokinin-A(CCK-A) receptor in the gallbladder mucous membrane was detected with reverse transcriptase polymerase chain reactions(rt-PCR). Results ①The fasting gallbladder volume was significantly greater in the Gallstone Group than in the other two groups(F=3.45,P=0.039).②The gallbladder contraction rate was significantly lower in the Gallstone Group than in the other two groups(F=5.747,P=0.005).③The postprandial increases of VIP of the three groups were not of statistical significance(F=0.768,P=0.47).④Compared with the Polypus Group,the expression of CCK-A receptor in the Gallstone Group was significantly decreased(t?=4.390,P=0.022).Conclusions ① The increase of the fasting gallbladder volume in the Gallstone Group is associated with the formation of gallstones.②In the Gallstone Group,depressed gallbladder contraction results in the formation of gallstones.③The decreased expression of CCK-A receptor leads to depressed gallbladder contraction,which facilitates the formation of gallstones.④Serum VIP is not related to the formation of gallstones.

7.Mechanism of ursolic acid-induced apoptosis in human leukemia cells

Objective To investigate the mechanism of ursolic acid-induced apoptosis in human leukemia cells.Methods U937 cells were exposed to ursolic acid(UA) at various concentrations for 6 h and 12 h,or at 20 ?mol/L for different time intervals.Cells were stained with Annexin V/PI,and their apoptosis was determined through flow cytometry.Total protein extracts were prepared and subjected to Western blot assay using antibodies against PARP,C-Caspase-3,C-Caspase-9,Mcl-1,Bcl-2,Bcl-xL,Bax,and Bad.Parallel studies were performed in human leukemia cell lines Jurkat and HL-60.Results Exposure of U937 cells to UA resulted in increase in apoptosis in dose and time-dependent manners.We found that UA-induced lethality was associated with Caspase activation and PARP cleavage,and that UA-induced apoptosis was proceeded by the downregulation of Mcl-1.Jurkat and HL-60 leukemia cells in parallel studies exhibited apoptosis after UA exposure similar to those observed in U937 cells,although HL-60 cells were less sensitive than U937 cells in UA-induced apoptosis,PARP degradation as well as Caspases activation.Conclusion UA induces apoptosis in human leukemia cells through a process that involves Mcl-1 downregulation,followed by Caspase activation and PARP degradation.

10.Filling effect of autologous skin fibroblasts: a study of two-photon fluorescence microscopy

Objective To investigate the survival profile of the intradermally injected mouse autologous skin fibroblasts and the changes of the collagen fibers by using green fluorescent protein labeling and two-photon fluorescence microscopy. Methods The cultured cells were transfected by EGFP lentivirus, and then the cells were injected into the corresponding mouse skin. Biopsy was taken from the animals after 1 and 2 months. The specimens made serial frozen sections, the survival profile of the injected cells and the changes of the collagen fibers were observed by two-photon fluorescence microscopy. The collagenic area and dermal thickness were measured with image analysis software, and statistical analysis was also carried out. Results Two-photon fluorescence microscopy showed clear images of the injected cells and collagen fibers. Both the area of collagen fibers and the dermal thickness were significantly increased in injected cells after 2 months (P<0.05), however, there were no difference between injected cells and control after 1 mouth (P>0.05). Conclusions Autologous cultured fibroblasts could survive in a long time after transplantating into the skin, and collagen could be newly produced, the depth of dermis increases, which provides a possibility to treat mini-defects of the tissue.
