2.The Modified Uvulopalatopharyngoplasty(UPPP)to Prevent the Complications and Destroy of UPPP.

Objective To improre typieal proeedure of UPPP,increase clinical effects and deerease complioations caused by the operation.Methods Twenty eight patients with obstructive sleep apnea syndrome who had been confirmed by PSG in our hospital from 2003 to 2005 were involved in this study.1)improving stripping method:the fat and tonsil were removed by electric knife.2)improving molding method:the muscle of uvula was maitained,but the fat was removed;mucous and the tissue under it were double-layer sutured.3) the data before and 6 months after UPPP were analyzed by questionnaire and polysomnography(PSG)detection.Result No fissuration occurred.The distance between soft palate and posterior wall was 3?0.6 before operation and it was 6?0.8 6 months after operation.The effect is 100% based on the questionnaire investigation and PSG detection.Conclusions 1)Hemostasis of electric knife was reliable;2)Double-layer suture of mucous and the tissue under it can avoid hemorrhage and insure molding of pharyngeal cavity;3)The results of the uvula preservation can improve the effect of the UPPP by increasing the nasopharyngeal and oropharyngeal cavity,and avoiding velopalatal insufficiency.
3.Fast-track surgery in laparoscopic gastrectomy for gastric cancer

Objective: The purpose of this study was to evaluate the safety and effectiveness of fast track surgery in laparoscopic gastrectomy for gastric cancer. Methods: All patients received elective gastric cancer resection, and were divided into three groups: group Ⅰ (open gastrecomy, n = 30) , group Ⅱ (open gastrecomy with fast track surgery, n = 30) , and group Ⅲ (laparoscopic gastrecomy with fast track surger-y ,n =30) . Clinical data and gut function were assessed in three groups. Results: There were no significant differences in postoperative complication and the number of lymph node harvest between 3 groups. Blood loss was less in group Ⅲ than group Ⅰ and group Ⅱ (P <0.01) ,but its operative time was longer and its cost was more than other two open surgery groups (P < 0. 01). Recovery of bowel function in group Ⅲ was faster,and postoperative hospital stay was shorter than group Ⅰ (P<0.01). However, recovery of bowel function and postoperative hospital stay in group Ⅲ was not different from group Ⅱ (P > 0. 05). Conclusion: Postoperative hospital stay can be shorter and recovery of bowel function can be faster in laparoscopic gastrectomy with fast track surgery plan compared to traditional care group. But when using fast track surgery plan, laparoscopic gastrectomy was not superior to open surgery.
4.Diagnostic value of serum VEGF level for advanced cervical cancer and its relationship with therapeutic efficacy of concurrent chemoradiotherapy

Objective To investigate the diagnostic value of serum vascular endothelial growth factor (VEGF) level for cervical cancer and its relationship with the therapeutic efficacy of concurrent chemoradiotherapy (CCRT).Methods Eighty patients with a pathologically confirmed diagnosis of advanced cervical squamous cell carcinoma (CSCC),20 patients with chronic cervicitis,and 20 healthy persons (as controls) were enrolled in the study.The CSCC patients received CCRT.All the subjects had their baseline serum VEGF levels measured by enzyme-linked immunosorbent assay (ELISA);also,the CSCC patients had their serum VEGF levels measured by ELISA at 3 months after CCRT.The relationship between the serum VEGF levels and therapeutic efficacy of CCRT was analyzed.Results The CSCC patients had a significantly higher baseline serum VEGF level than the chronic cervicitis patients and controls (237.36 μg/L vs.142.36 μg/L,t =4.03,P =0.000).The serum VEGF level was closely related to tumor stage,tumor differentiation,and lymph node metastasis.A serum VEGF level above 197.52 μg/L was the optimal cut point for diagnosing cervical cancer,with a sensitivity of 0.88 and a specificity of 0.72.The serum VEGF levels before and after CCRT showed a close relationship with the therapeutic efficacy.The patients achieving a complete remission had a significantly greater change in serum VEGF level than those with progression; the patients with high baseline serum VEGF levels had a significantly higher tumor progression or death rate than those with low baseline serum VEGF levels.Conclusions Serum VEGF level has poor diagnostic value for cervical cancer,though it is closely related to the tumor stage,tumor differentiation,and lymph node metastasis of cervical cancer.However,serum VEGF level has some predictive value for the therapeutic efficacy of CCRT in patients with cervical cancer.
5.Construction of tissue-engineered skin by mix-seeding.

OBJECTIVETo investigate the feasibility of construction of tissue-engineered skin in vitro.
METHODSFibroblasts were seeded on the dermal surface of acellular dermal matrix (ADM). 7 days later, epidermal cells (5 x 10(5)/cm2) were mixed with fibroblasts (0.2 x 10(5)/cm2) and then seeded on epidermal surface of ADM. The culture medium was the mixture liquor containing K-SFM in half and the culture supernatants of fibroblasts in half. In the control group, only epidermal cells (5 x 10(5)/cm2) were seeded and cultured with K-SFM. After composite skin was cultured for 1 week and 3 week, samples were harvested respectively for morphological study and to receive identification by immunohistochemistry.
RESULTSAfter 3-week culture, there were 3-4 continuous layers of cells in the epidermis in the experiment group. The epidermis was attached tightly to the dermis with trochanterellus. But in the control group, there were just 1-2 layers of cells in the epidermis which was not connected to the dermis. Strong positive dye of Laminin indicated that basement membrane was thoroughly formed in the experiment group. The same result was demonstrated by transmission electron microscope.
CONCLUSIONSThe mix-seeding of epidermal cells and few fibroblasts would promote the adhesion and proliferation of epidermal cells on ADM, and the formation of basement membrane.

6.Cutaneous permeation comparison of Kechuan acupoint patch and power.

OBJECTIVETo compare the cutaneous permeation of Kechuan acupoint patch and power, and evaluate the possibility of dosage form reform of Kechuan recipe.
METHODTake the Eugend and Ephedrine as the indexes, HPLC was employed to determine their contents, the pond with Franz diffusion were used to measured the cutaneous.
RESULTThe permeation of Patch matched with Higuchi Equation. Take Eugend as the index, the permeation rate of total of Patch is 2.319 and 1.738 times of the powder, and 1.784 and 1.215 times of the powder with the Ephedrineas as index.
CONCLUSIONThe permeation rate of Kechuan acupoint patch was more rapid than the powder. Moreover, the total quantity of permeation of patch was also more than the powder.

7.Effects of nonylphenol exposure via placenta on early nervous reflex and locomotives of offspring in rat.

OBJECTIVETo explore the influence of nonylphenol (NP) on the neural behavioral development of filial generation rats exposed via placenta.
METHODSOn the first day of the pregnancy, the SD rats were divided into four groups, and orally administered with NP at doses of 0, 50, 100 and 200 mg/kg on gestational day 9 approximately 15 respectively. The offspring rats of each groups were examined to observe the impact of NP on the early physiological, neurobehavioral development. The changes of filial generation body weight (from generation day 1 to 28) were measured. Brain tissues were stained with Hematoxylin-eosin and Congo red to observe with optical microscope.
RESULTSIn contrast to the control group, the early physiological markers (pinna detachment, hair growth, tooth growth and eye opening) and the early neurobehavioral development indices (surface righting, air righting, acoustic startle and visual placing) were significantly delayed in the groups of NP 200mg/kg dose (P < 0.05). The developing time of physiological markers decreased from (4.5 +/- 0.8, 5.2 +/- 0.8, 12.7 +/- 1.4, 16.0 +/- 1.7) d to (3.6 +/- 0.5, 3.6 +/- 0.5, 11.1 +/- 1.1, 12.7 +/- 1.3) d while neurobehavioral developing time decreased from (6.5 +/- 0.8, 11.3 +/- 0.5, 11.2 +/- 1.0, 20.2 +/- 1.0) d to (5.1 +/- 0.4, 8.3 +/- 0.5, 9.3 +/- 0.5, 9.3 +/- 0.5) d (P < 0.05 or P < 0.01). The body weights of filial generation rats were decreased obviously from 1 st day to 28th day. Histopathological examination displayed that hippocampal neurons had congestion and oedema in the group of 100, 200 mg/kg dose.
CONCLUSIONExposures to NP during gestation might impair the neurobehavioral development of F1 rats significantly.

8.Comparison of Therapeutic Effects Between Tightrope Loop Plate and Endobutton Plate in the Treatment of Acromioclavicular Joint Dislocation

Objective To compare the efficacy of TightRope loop plate and Endobutton plate in the treatment of acromioclavicular joint dislocation.Methods A retrospective analysis was conducted on 94 patients with acromioclavicular joint dislocation who were treated at this center from March 2021 to February 2023.They were divided into two groups based on different admission date.The Group E(n =47)received Endobutton plate treatment between March 2021 and February 2022,while the Group T(n =47)received TightRope loop plate treatment between March 2022 and February 2023.At the last follow-up,the perioperative indicators,Visual Analogue Scale(VAS),Constant-Murley shoulder joint function scores,and surgical complications were compared between the two groups.Results The surgical time,intraoperative bleeding,incision length,and VAS scores at 7 days after surgery in the Group T were shorter or lower than those in the Group E(P<0.05).There were no statistical differences in the incidence of perioperative nerve injury,internal fixation displacement,clavicle fracture,vascular injury,and infection between the two groups(P>0.05).The subjective and objective scores of Constant-Murley shoulder joint function in both groups at9 months after surgery showed significant improvement compared to preoperative scores(all P =0.000).There was no significant difference in the subjective and objective scores of Constant-Murley shoulder joint function between the two groups at 9 months after surgery(P>0.05).Conclusions The treatment of acromioclavicular joint dislocation with TightRope loop plate ot or Endobutton plate has a significant effect and can effectively improve shoulder joint function.Compared with Endobutton plate,use of TightRope loop plate has minor surgical trauma,less bleeding,and significantly reduced postoperative pain,being more conducive to early functional exercise for patients.
9.The changes of funny currents in the ventricular myocytes of neonatal rats and adult rats.

AIMTo record funny currents (If) of ventricular myocytes and to analysize hyperpolarization-activated cation channel(HCN) expression in the rats of different ages.
METHODSFresh ventricular myocytes were isolated from 3 days rats and adult rats.HCN expressions were measured by real-time quantitative polymerase chain reaction(real-time PCR). It was recorded through whole-cell patch clamp.
RESULTSHCN1, HCN2, HCN3, HCN4 mRNA represented 0.23% +/- 0.01%, 83.58% +/- 0.04%, 0.79% +/- 0.01%, 15.44% +/- 0.01% of total HCN mRNA in the neonatal rats, respectively. If was recorded and the threshold for activation was -75 mV. In the adult rat, HCN1, HCN2, HCN3, HCN4 mRNA represented 0.72% +/- 0.02%, 91.58% +/- 0.08%, 0.27% +/- 0.02%, 7.12% +/- 0.02% of total HCN mRNA. The ratio of HCN2 to HCN4 was approximately (13.06 +/- 0.21):1. The threshold for activation of If was approximately -115 mV in the adult rats.
CONCLUSIONWith the development of rats, the value of If is smaller. The threshold for activation of If is more negative. The ratio of HCN2 to HCN4 is bigger.

10.Diagnostic and application value of 64-slice spiral CT scanning in preoperative staging of esophageal cancer.

OBJECTIVETo investigate the diagnostic and application value of pre-operative 64-slice spiral CT evaluation in operation selection for esophagus cancer.
METHODSMulti-slice computed tomography (MSCT) was conducted in 50 patients with esophageal cancer before operation, using the work station for after-treatment to get CT virtual endoscopy (CTVE), multiplanar reconstruction (MRP), shaded surface display (SSD) and Raysum images, and combined with the transect images to record the preoperative MSCT staging and to predict the operation scheme, and compared with the postoperative pathological staging and the actual operational plan.
RESULTSThe diagnostic sensitivity of MSCT for preoperative T staging was 100.0% (50/50), while the N staging was 80.0% (16/20). According to the MSCT prediction, the resection rate of esophageal cancer was 96.0% (48/50). Through the preoperative MSCT evaluation, the accuracy of CT-TNM stage was 90.0%, highly consistent with the pathological TNM stage (Kappa = 0.811, P < 0.05).
CONCLUSIONSMSCT can effectively display the shape, size and position of the tumor, determine the tumor invasion range, lymph node metastasis and distant metastasis, etc., make preoperative evaluation for esophagus cancer patients and provide evidence for clinicians to predict the operation scheme for esophagus cancer.
