1.MULTIPLE ORGANINJURY BY HIGH CONCENTRATED PLATELET-LEUCOCYTE AGGREGATIONS IN RED BLOOD CELL CONCENTRATION AND ITS COUNTERMEASURE

To study the formation and change of platelet leucocyte aggregation (PLA) in red blood cell concentrations(CRCs) stored at 4℃ and the preventive effectiveness of filtration, 8 units of CRCs were each divided into two equal parts in two integrative bags and two paired groups were formed: control group and filtration group. The following items of control group and filtration group were detected weekly. Platelet and leukocyte counts, concentration and percentage of platelet lympocyte (P lym), platelet monocyte (P mon), platelet neutrophil (P neu) aggregatoins, and concentration of total PLA. The results showed that rate and concentration of PLA in the control group were significanly higher than those in the circulating blood. The rates of P lym and P mon in CRCs stored at 4 ℃ were two to five times of those before storage, and the rate of P neu in CRCs stored at 4 ℃ was ten to twenty times of that before storage. PLA in the CRCs separated from plasma rose to a high level at the first day of preparation. The concentration of PLA in CRCs reached the highest level at one week after stored at 4 ℃ and then decreased, but a high concentration was retained all the while. P neu is the major part (about 80%~90%) of whole PLA in CRCs. PLA was not detectable by flow cytometry during the whole storage course. The results suggested that high concentrated PLA was detected in CRCs stored at 4 ℃, which could be prevented by leucocyte depleting filtration.
2.Fast-track surgery in laparoscopic gastrectomy for gastric cancer

Objective: The purpose of this study was to evaluate the safety and effectiveness of fast track surgery in laparoscopic gastrectomy for gastric cancer. Methods: All patients received elective gastric cancer resection, and were divided into three groups: group Ⅰ (open gastrecomy, n = 30) , group Ⅱ (open gastrecomy with fast track surgery, n = 30) , and group Ⅲ (laparoscopic gastrecomy with fast track surger-y ,n =30) . Clinical data and gut function were assessed in three groups. Results: There were no significant differences in postoperative complication and the number of lymph node harvest between 3 groups. Blood loss was less in group Ⅲ than group Ⅰ and group Ⅱ (P <0.01) ,but its operative time was longer and its cost was more than other two open surgery groups (P < 0. 01). Recovery of bowel function in group Ⅲ was faster,and postoperative hospital stay was shorter than group Ⅰ (P<0.01). However, recovery of bowel function and postoperative hospital stay in group Ⅲ was not different from group Ⅱ (P > 0. 05). Conclusion: Postoperative hospital stay can be shorter and recovery of bowel function can be faster in laparoscopic gastrectomy with fast track surgery plan compared to traditional care group. But when using fast track surgery plan, laparoscopic gastrectomy was not superior to open surgery.
3.Relationship between levels of IL-2,TNF-? and nitric oxide in aqueous humor after intraocular lens implantation

AIM: To investigate the relationship between the level of interleukin-2 (IL-2), tumour necrosis factor-?(TNF-?) and nitric oxide (NO) in aqueous humor after intraocular lens implantation. METHODS: New Zealand rabbits were divided randomly into three groups: (1)control group; (2)extracapsular cataract extraction group (ECCE); (3)extracapsular cataract extraction and posterior chamber intraocular lens implantation group (ECCE+IOL). The inflammation in all experimental rabbit eyes was observed via zoom-photo slit-lamp microscope on 1, 3, 7, 14 d and 30 d postoperation. Meanwhile, aqueous humor was drawn for white blood cell(WBC) counting and classifying , and for determining IL-2, TNF-? and NO 2 -/NO 3 - contents.RESULTS: (1)The level of IL-2 and TNF-? and NO 2 - / NO 3 - in aqueous humor of ECCE+IOL group were higher than that in ECCE and control at 1 to 14 days postoperation, respectively, it increased to peak value at 3 to 7 days postoperation and decreased gradually two weeks postoperation; (2) The changes in IL-2, TNF-? and NO 2 -/NO 3 - in each group were basically similer; (3) The changes of IL-2 and TNF-? level were closely related with NO content in aqueous humor( r=0.69, P
5.Clinical study on acupoint-injection combined with epidural analgesia for labour

Objective To discuss whether the separating block of sensory nerve with acupoint-injection and epidural analgesia by ropivacaine during labour.Methods Ninty full term primigravidae with ASAⅠ~Ⅱwere randomly divided into 3 groups:group E(n = 30),group C(n = 30)and group A(n = 30).When the external cervi- cal os was dilated to 3cm,group A:acupiont-injection to Ci Liao;group E:epidural analgesia with 0.075% ropiva- caine and fentanyel 2?g/ml;group C:combining group A and group E except fentanyel 2?g/ml.Level of pain(VAS scores),degree of sensory and motor block,the mode of delivery,the progress of labour and side effects were recorded and compared.Results The level of pain(VAS scores)in group A was higher than that of group C and group E,and there were no significant differences in group C and group E,Bur there were some side effects discovered in group E. There was no significant differences of the mode of delivery in the groups.Conclusion The method that acupoint- rejection combined with epidural analgesia by 0.075% ropivacaine is effective and has no side effects for labour.
