1.Effect evaluation of choking risk assessment and intervention on the choking prevention of homeless inpatients with psychiatric

Objective To explore the effect evaluation of choking risk assessment and intervention on the choking prevention of homeless inpatients with psychiatricc. Methods Totally 163 homeless inpatients with psychiatric disorder were selected from July 2012 to June 2016, 56 cases from July 2012 to December 2013 were assigned to the control group, another 107 cases from January 2014 to June 2016 was set as the intervention group. The control group received routine nursing safety management, on the basis of it, the intervention group received choking risk assessment through choking risk assessment scales. To take choking risk intervention on the patients of reached choking danger threshold. The incidence of choking and choking ending were compared between the two groups and the choking risk levels and scores in the intervention group before and after intervention were also compared. Results The incidence of choking in the intervention group and the control group was 0.93%(1/107)and 10.71%(6/56)respectively. Choking ending: 2 heal, 3 aspiration pneumonia, 1 death of 6 cases in the control group and 1 case was cured, no death and aspiration pneumonia occurred in the intervention group, the statistical differences were significant (χ2=9.399,18.554, P < 0.01 or 0.05). There were 28.0% (30/107) reached level Ⅰ, Ⅱ and Ⅲ choking threshold in the intervention group and rank mean of choking risk scores were 41.29. After interventions were 8.4% (9/107) and 25.50 respectively, the statistical differences were significant (χ2=10.231, P<0.05;Z=7.511, P<0.01). Conclusions The corresponding nursing intervention on the patients of choking risk can reduce and avoid choking accidents and protect the safety of homeless inpatients with psychiatric disorder effectively and improve the quality of care.
2.Application of fingerprint recognition technology in identification and management of mental patients

Objective To investigate the effect of using fingerprint identification instrument on the identity management quality and satisfaction of patients.Methods The relevant information of mental patients was inputted by portable fingerprint machine at the time of admission.It was unneeded for providing information from patients or their family members in various nursing operation.The patients’ ID information could directly confirmed through fingerprint comparison.Mental patients compliance rate, defect rate, wrist strap material cost saving rate, nursing staff satisfaction on identity recognition were compared before and after the using of fingerprint identification.Results Before the using of fingerprint identification instrument, psychiatric patients on identity compliance rate was 82%, defect rate was 36%,wrist strap material cost saving rate was 5%, and nursing staff satisfaction on identity recognition was 86%; while there were 98%,6%,45%,99% after, and the differences were statistically significant (χ2 =7.11,13.56,21.00,46.89,respectively;P<0.05).Conclusions Fingerprint identification system can improve the compliance of psychiatic patients identity recognition, reduce the defect identification rate, save costs and improve nursing satisfaction, and improve the mental patient identity management quality.
3.Identity recognition and safety nursing effects of first-use identification wristband in "Three None"psychopaths

Objective To explore the identity recognition and safety management effects of first-use identification wristband in "Three None" hospitalized psychopaths. Methods A total of 79 "Three None"hospitalized psychopaths were carried out wristbands identification information management in our hospital from July 2013 to September 2014 and were selected as experimental group, while a total of 110 patients without wristband were selected as control group. The correct rate of identification recognition and incidence of nursing errors were compared between the two groups. Results The correct rates of identification recognition were 96. 2% and 84. 5% in the experimental group and control group respectively, while the incidences of nursing error were 3. 8% in the experimental group and 13. 6% in the control group (χ2=6. 013, 5. 165;P<0. 05). Conclusions The identification wristband on the "Three None" psychopaths can help nurse to enhance the ability of distinguishing patients′identity and medical condition, and reduce the incidence of nursing errors, so as to increase nursing safety.
4.Effects of restrictive protection according to related legal regulations among inpatients with mental diseases

Objective To investigate the effects of the implementation of Mental Health Law of the People′s Republic of China and the “review standard of the tertiary mental hospitals” on the quality of restrictive regulations among inpatients with mental diseases , in order to provide references for the further standardization and management using retrospective investigation of the clinical data .Methods The patients of control group received the normal restrictive protective nursing and the patients of intervention group was strictly carried out the restrictive protective care in accordance with the mental health law and the “review standard of the tertiary mental hospitals”.The effects were compared between the two groups including:qualified rate of proper practice following doctor′s advice , the application of nursing procedures , nursing records , basic nursing care , incidence of the adverse event , patients′restraint rate, average restraint time per person , and average restraint frequency per person.Results The differences of the qualified rate of proper practice following doctor′s advice, the application of nursing procedures , nursing records , basic nursing care , and the incidence rate of the adverse event had statistical significance between the two groups (P<0.05).The restraint rate were statistical different (χ2 =10.11, P<0.05), as well as the average restraint time and frequency per person (t=7.92, 47.88;P<0.05).Conclusions The implementation of mental health law and the “standard of review of the tertiary mental hospitals” can significantly improve the quality of clinical restrictive protective nursing management , and reduce the restraint rate as well as the average restraint time and frequency per person .
5.Nursing risk assessment and intervention methods of psychiatric inpatients

Objective To discuss the effects of applying all kinds of nursing risk assessment scale to hospitalized psychiatric patients in order to achieve safe management .Methods We chose a number of 9 292 hospitalized psychiatric patients between January 1,2013 and December 31,2014, and divide them into control group (4074 cases,admitted in 2013) and intervention group (5 218 cases,admitted in 2014)according to hospitalization time sequence .We implemented all kinds of nursing risk assessment scale to assess nursing risk and managed patients in intervention group , while the patients of control group received routine safe nursing interventions.Two groups′frequency of attack,self-injury and suicide, fall or drop bed,choking food,fleeing and constraints were compared .Results After the implementation of nursing risk management ,incidence of attacks caused others injury,self-injury and suicide, fall or drop bed,choking, constraints of intervention group were 0.21%,0.08%,0.34%,0.00%,18.91%, which were all lower than that of the control group (P<0.05);number of patients running away from hospital in the intervention group had no difference with that in the control group[8(0.15%) vs 7 (0.17%);P >0.05].Conclusions Nursing risk assessment and management for hospitalized psychiatric patients can effectively reduce the psychiatric accident and can help clinical nurses to manage in-patients directly and safely .
6.Comparison of different identification application in identity management of patients with mental disorder

Objective To explore the effects of different identity identification methods in the quality of clinical nursing management of psychosis patients. Methods By the conventional recognition, wristband identification, fingerprint identification, we compared the different identification correct rate, compliance rate, defect rate, wristband material cost saving rates, satisfaction of identification for mental patients before and after the identification. Results The rate of correct identification for patients with different identification were respectively 95. 60%, 99%, 100% with statistical significance (χ2 =62. 58,P<0. 05). After several correct rate of identification comparison, there were statistically significant differences in any two of three groups (χ2 =25. 42,44. 99, 10. 06;P<0. 01). Before and after the use of fingerprint identification instrument, the mental patients to identity compliance rates were 82% and 98% (χ2 =45. 81,P<0. 05); defect identification rates were 36% and 6% (χ2 =282. 74,P<0. 05);wrist strap material cost saving rates were 5% and 45% (χ2 =490. 08,P<0. 05);satisfaction with the way of identifying rates were 86% and 99% (χ2 =10. 07,P<0. 05). Conclusions Patients with different identification have different identity recognition correct rate. Wristband helps us improve the accuracy of identification, while the fingerprint identification device have the highest correct rate to indentify mental patients to without limitation use in Psychiatry Department by improving the psychiatric nursing compliance of both patient and medical staffs identification, reducing the defect identification rate, saving manpower and material costs and improving both nurses and patients satisfaction rate.
