2.Treatment of periamullary diverticula complicated with biliopancreatic disease

Objective To evaluate the curative methods of periampullary diverticula(PAD) complicated with biliopancreatic disease.Methods The clinical data of 32 cases of PAD complicated with biliopancreatic disease treated by surgical or endoscopic measures were retrospectively analyzed.Results Of the 32 cases,16 cases were treated with gastroectomy and Roux-en-Y gastrojejunostomy,and 8 with gastroectomy and Billroth Ⅱ gastrojejunostomy,8 with endoscopic sphincterotomy (EST).Postoperative complication occured in 4 cases,but no operative death.Twenty-seven cases were followed up for 5 months-8 years, and the results were excellent in 20 cases, good in 7 cases.Conclusions Gastroectomy and Roux-en-Y gastrojejunostomy is a better surgical method for PAD complicated with biliopancreatic disease.EST has become a new effective measure to deal with the diseases;It will be safe to use the first method for patients with intradiverticular papilla if it treated by EST.
4.Problem-based Learning Based on Web for Rehabilitation Therapy Teaching

Objective To apply the problem-based learning (PBL) based on the web in the rehabilitation therapy teaching. Methods The 64 students of rehabilitation therapy in grade 2009 and 2010 were taught with selected 3 chapters of physiology, using traditional teaching and PBL based on the web respectively. They were investigated through questionnaire after learning. Results Compared with he traditional teaching, PBL based on the web could improve the interesting of learning, self-study ability, and scope of knowledge. Conclusion PBL based on the web could improve the quality of physiology teaching for students of rehabilitation therapy.
5.Post-marketing clinical safety assessment of Shenmai injection based on active monitoring and passive monitoring in large data background.

This paper adopted a series of related analysis methods to comprehensively analyze post-marketing clinical safety data of Shenmai injection from 4,220 cases of SRS and 32,358 cases of multicenter, prospective, registered hospital centralized monitoring in large data background, calculated ADR incidence rate was 0.93 per 1,000, main symptoms of ADR includes chest pain, chills, skin itching, palpitations, fever, nausea, dizziness, vomiting, flushing, numbness, allergic reaction, cyanosis, rash, low back pain, and "breath", "anaphylactoid reaction" and "flush" were the safety warning signals of Shenmai injection. Primary disease for chronic pulmonary heart disease, thyroid disease, and combined with cerebral vascular disease, prior to the injection and continuous use of alprostadil, cyclic adenosine monophosphate, combined with quinolones, penicillins were suspicious influence factors of ADR of Shenmai injection, these promot the clinical safety.

7.The study of clonidine on the patients in PACU

OBJECTIVE To study the influenceof clonidine on the hemadynamics and plasma catecholamine of the postoperative patients in PACU. METHODS Twenty eight patients suffered from severe OSAHS who underwent modified UPPP and monitored in the PACU after operation were divided randomly into two groups:clonidine group and control group. Both group received the same protocol of sedation and analgesia. Patients in clonidine group received clonidine 3ug/kg injected slowly when entered the PACU,and patients in control group received same volume of NS. Esmolol and Nitroglycerin were used to adjust the hemadynamics. The changing of hemadynamics was recorded,blood samples were taken before operation,2 hours after entering PACU,before discharge from PACU to determine the changing of plasma catecholamine. RESULTS The hemadynamics was more stable in clonidine group than that in control group and less vasoactive agents were needed. The plasma catecholamine was much lower in clonidine group 2 hours after entering PACU and before discharge from PACU than that in control group. CONCLUSION Clonidine could be administered to stabilize the hemadinamics,to lower the level of stress hormone and could improve the recovery of the patients.
8.Peroxisome proliferator activated receptor ?2 gene P12A polymorphism and type 2 diabetic nephropathy in Han population in Shanghai

0.05).Frequencies of P12A12 genotype and A12 allele in DN group were significantly decreased respectively,when compared with DN-0 group(for P12A12 genotype,9.1% vs 18.1%,P=0.034,OR=0.453;for A12 allele,4.5% vs 9.0%,P=0.041,OR=0.479). Conclusion The observations suggest that P12A polymorphism of PPAR?2 gene is associated with Chinese type 2 diabetic nephropathy,and A12 allele may protect the development of diabetic nephropathy in type 2 diabetic patients of Chinese.
9.Preliminary evaluation of color power doppler ultrasonography in diagnosis of sacroiliitis in patients with ankylosing spondylitis

Objective To preliminarily evaluate the value of color power doppler ultrasonography in the diagnosis of sacroiliitis in ankylosing spondylitis(AS). Methods Fifty-seven sacroiliac joints in 31 patients with active AS and 40 sacroiliac joints in 20 volunteers were detected by color power doppler ultrasonography.The color flow signs inside the sacroiliac joints were observed,and the resistance index(RI) was measured. Results In active AS,color flow signs were seen in 55 joints,and the mean RI value was 0.53?0.08 in 45 joints,while the other 10 color flow signs represented reversed phase in diastolic phase on pulse doppler ultrasonography.In the volunteers,color flow signs were seen in 16 joints,and the mean RI value was 0.97?0.01 in 6 joints,while the other 10 color flow signs represented reversed phase in diastolic phase on pulse doppler ultrasonography. Conclusion The abnormal flow signs at the sacroiliac joints can be detected by color power doppler ultrasonography.Low RI values provide diagnosis evidence for active AS.
10.Relationship between SLC12A3 gene Arg913Gln polymorphism and type 2 diabetic nephropathy in Han population of Shanghai

Objective To explore the relationship between Arg913Gln(G→A) polymorphism of solute carrier family 12 member 3 (SLC12A3) gene and diabetic nephropathy (DN) in type 2 diabetes mellitus (T2DM) in Han population of Shanghai. Methods Two hundred and fifty-eight Han ethnic people in Shanghai with T2DM (T2DM group) were divided into non-DN group (DN0 group, n=95) and DN group (n=163) according to 24 h urine albumin excretion rate (AER), and those in DN group were subdivided into microalbuminuria group (DN1 group, n=95) and macroalbuminuria group (DN2 group, n=68). Besides, 82 people with normal results of oral glucose tolerance test (OGTT), without diabetes mellitus and nephropathy were served as controls. PCR-sequencing was used to detect the genotypes of Arg913Gln polymorphism of SLC12A3 gene. Genotypic and allelic frequencies and clinical characteristics were compared among groups. Results Three genotypes (GG, GA and AA) were detected. The frequencies of GA+AA genotype and A allele in T2DM group were higher than those in control group, while there was no significant difference between groups (P>0.05). There was no significant difference in genotypic or allelic frequencies among subgroups of T2DM group (P>0.05). The level of triglyeeride (TG), AER, level of fasting insulin (FINS) and HOMA-IR in patients with GA+AA genotype were significantly higher than those in patients with GG genotype in T2DM group (P<0.05). Conclusion Arg913Gln(G→A) polymorphism of SLC12A3 gene is not significantly associated with T2DM and DN in Han population of Shanghai. The AER of people with GA+AA genotype is significantly higher than that with GG genotype. Arg913Gln (G→A) polymorphism of SLC12A3 gene may predict the risk of increase of albuminuria in patients with T2DM in Han population of Shanghai.
