1.Effects of hyperglycemia on the expression of corneal epithelial tight junction protein claudin-1 in type 2 diabetic rats

Objective To assess the expression of the corneal epithelial TJ protein claudin -1 in type 2 diabetic rats at different time points.Methods 80 eight-week-old male SD rats were randomly divided into the normal control and diabetic mellitus(DM) groups(n =40 each).A high-fat diet combined with STZ injection was used to induce type 2 DM.Normal and diabetic rats were sacrificed at 4, 8, 16 weeks respectively(after STZ injection) before de-bridement.Hematoxylin and eosin was used to study the morphological differences between normal and diabetic cor -nea.Immunofluorescene and Western blot were used to determine the expression of corneal epithelial TJ protein claudin-1.Results Corneal epithelial cells reduction and stroma edema were evident at 8 and 16 weeks by HE. Claudin-1 expression in the corneal epithelium of the diabetic group was lower and fainter compared to the normal group at 16 weeks(after STZ injection)(P <0.05), which were similar to the normal group at 4 and 8 weeks.Con-clusion Continual hyperglycemia has a negative effect on ocular surface tissues and the expression of corneal epi -thelial tight junction protein claudin -1 with progressing of type 2 diabetic mellitus.
2.Laser lithotripsy for postoperative intrahepatic residual stones under choledochoscopy report of 74 cases

ObjectiveExplore the clinical effect of U- 100 double- frequency laser lithotripsy in postoperative residual intrahepatic calculus under choledochoscopy.MethodsSeverty-four patients of postoperative intrahepatic residual stones with U-100 double-frequency laser lithotripsy under choledochoscopy were analyzed retrospectively.ResultsSixty-three (85.1%) of 74 cases were treated successfully in the first operation,8cases ( 10.8% ) obtained satisfactory outcome in the second operation,and 3 cases (4.1%) in the third.All lithotripsies were performed successfully.ConclusionsU-100 double-frequency laser lithotripsy in postoperative residual intrahepatic calculus under choledochoscopy provides a safe and effective method.
3.Comparison of anti-D immunoglobulin and high-dose intravenous immunoglobulin therapy in childhood acute idiopathic thrombocytopenic purpura:a systematic review and meta-analysis

Objectives To compare the effectiveness and safety of intravenous anti-D immunoglobulin (IV anti-D) with high-dose intravenous immunoglobulin (IVIG) as initial treatments of acute idiopathic thrombocytopenic purpura (ITP) in children. Methods Randomized controlled trials comparing anti-D with high-dose IVIG in the treatment of childhood acute ITP were systematically reviewed from PubMed, Embase and Cochrane Central Register of Controlled Trials and hand-searched reference lists. The number of patients with a platelet count>20 × 109/L at 72 hours after treatment initia-tion, and the decrease in hemoglobin were the primary outcomes. The meta-analysis was performed by RevMan 5.1. Results A total of 771 relevant articles were retrieved, and ifve studies were included. The RR (anti-D versus high dose IVIG) of achieving a platelet count>20×109/L at 72 hours was 0.90 (95%CI:0.82~0.98). However, subgroup analysis suggested no signiifcant difference between anti-D at a dose of 50μg/kg and high-dose IVIG (RR 0.98, 95% CI: 0.84~1.13), as well as between anti-D at a dose of 75μg/kg and high-dose IVIG (RR 0.88, 95%CI:0.75~1.03). Hemoglobin drop was greater in the anti-D group. No patients, however, required transfusions of erythrocyte suspensions. Conclusions IV anti-D may be as ef-fective as high-dose IVIG in the treatment of childhood acute ITP at 72 hours after therapy. The side effects of anti-D were tolerated and acceptable.
4.A randomized study of peri-operative chemotherapy in patients with completed resected non-small-cell lung cancer

Purpose:To evaluate the effect of cisplatin-based peri-operation chemotherapy (CT) on survival after completed resection of non small cell lung cancer (NSCLC)Methods:A prospective, randomized, multicenter study was conducted by Shanghai Lung Cancer Team since Feb 1995 to Dec 2003 for stage Ⅰ~ⅢA NSCLC with completed resection. Patients were randomly assigned to receive pre-operative CT or no pre-operative CT (pre-op CT). Post-operative CT (post-op CT) were used for majority of the patients, except for partial stage I patients. Accumulated survival, log rank, MST, Cox uni-variance and multi-variance analyses, HR were used as statistics for evaluation Results:A total of 337 patients underwent randomization, 169 cases received pre-operative CT, and 168 cases didn't receive pre-operative CT. There was statistical survival difference between the group with no pre-op CT and with pre-op CT, 5-yr survival rate were of 47.85%∶ 36.52%, MST were 56.63∶39.14(P=0.03), respectively. Stage and post-op CT were the only two meaningful parameters with statistical survival difference calculated by multi-variance analyses (P0.05). There were 121 cases received more than 3 cycles post-op CT, 216 cases received less than 3 cycles post-op CT. The patients received more than 3 cycles had better yr-survival and MST than those received less cycles (P=0.04).Post-op CT was not benefit to the survival rate of stage I. 5.Investigation of Relationship Between Helicobacter Pylori Infection and Reflux Esophagitis

Objective To investigate the relationship between helicobacter pylori (H. pylori) infection and reflux esophagitis (RE). Methods H.Pylori infection rates of RE in patients with simple chronic gastritis(CG),which was confirmed by gastrospcope combined with pathological diagnosis were researched.The relationship between HP infection and RE classifications using gastroscope and pathology was explored.Results ⑴H.pylori infection was found in 29(27.1%) in 107 cases of RE and 43(40.2%) in 107 cases of simple CG patients respectively,HP infection positive in RE patients less than that of simple CG patients.RE of class Ⅰ,Ⅱand Ⅲ using gastroscopy classification were 62.1%, 10.3% and 27.6% in H. Pylori-positive cases respectively, while they were 56.4%, 6.4% and 37.2% respectively in H. Pylori-negative ones. However, mild, moderate and severe RE identified by pathohistology were 72.4%, 13.8% and 13.8% in H.Pylori-positive cases respectively, and they were 57.7%, 17.9% and 24.4% respectively in H.Pylori-negative ones.Conclusions All above results suggested that H.Pylori possibly has prevention role in some extent to pathogenesis of RE. RE more commonly was seen in H.Pylori-negative cases.RE could inhibit the H.Pylori survive,So that,the RE was occurred frequently in H.Pylori-negative patients.The inflammatory extent of RE is not serious in H.Pylori-positive patients.

Objective:To analyze the craniofacial morphology characeristics of patients with peg-shaped upper lateral incisors. Methods:1 13 patients,aged 12 ~28 years,with peg-shaped upper lateral incisors were included,lateral cephalograms were taken and analyzed cephalometrically.Data were statistically analysed.Results:The sagittal facial patterns were skeletal Class Ⅰ(40.71%),skeletal Class Ⅱ(9.73%)and skeletal Class Ⅲ(49.56%).The vertical skeletal patterns were average vertical type (41.59%),high vertical type(18.58%)and low vertical type(39.82%).SNA,ANB,Ptm-A(mm),U1-SN,L1-NB,UL-EP were smaller(P<0.05)and SNB,PP-GoGn,SN-MP,Y axis,U1-L1,U1-NA,L1-MP and Z angles were increased(P<0.05).Conclu-sion:Patients with peg-shaped upper lateral incisors show Class Ⅲ skeletal pattern with maxillary retrognathia and mandibular prog-nathism.
9.A CBCT study of alveolar bone structure around incisors of the patients with verical facial type of skeletal Class Ⅱ

Objective:To investigate the structure of alveolar bone around incisors in patients with verical facial type of skeletal ClassⅡ by CBCT.Methods:60 skeletal Class Ⅱ patients were divided into low angle,average angle and high angle cases(n =20)by FMA.CBCT scanning was made and the thickness of the labial and lingual alveolar bone around the incisors in each subject was meas-ured.The measurements were analyzed using ANOVA.Results:The total width of alveolar bone at the root apical level of maxillary and mandibular incisors,the lingual thickness of alveolar bone at the root midpoint of maxillary incisors and labial thickness of alveolar bone at the root midpoint of mandibular incisors in the high angle cases were thinner than that in the average and low angle cases(P <0.05),but there was no significant difference in labial thickness of alveolar bone at the crest among 3 groups.Conclusion:The alveo-lar bone thickness around incisors of high angle patients is thinner,more attention should be paid for the alveolar bone absorpation and tooth root exposure in these patients.
10.Common gram-negative bacilli lower respiratory tract infection in PICU: analysis of pathogens and drug resistant pattern from 2004 to 2009

Objective To study common Gram negative bacterial pathogens and their drug resistant pattern in sputum isolates from patients in pediatric intensive care unit (PICU) of Chengdu children' s hospital. Methods Comparing to the period of 2004-2006 with 2007-2009, we retrospectively reviewed eligible sputum isolates of common Gram negative bacilli and their drug resistance pattern. All specimens were processed according to standard microbiological methods for both quantitative and qualitative cultures. Pathogen was the single predominant growth after two consecutive cultures. Strict quality control was used in all test agents and paper disk for drug resistance analysis. Results Total of 598 Gram negative bacterial isolates were identified in patients over 6 years. The first 5 bacteria respectively were Escherichia coli 167 samples(27.93%), Klebsiella pneumoniae 109 samples ( 18.23% ), Enterobacter cloacae 93 samples ( 15.55% ),Acinetobacter baumannii 64 samples ( 10. 7% ), Pseudomonas aeroginosa 43 samples (7. 19% ). They were mostly sensitive to Imipenam, Amikacin, Piperacillin/Tazobactam. Most drug resistance rate was found in Amoxicillin, Ampicillin, Cephalosporin. Apart from Pseudomonas aeroginosa and Acinetobacter Baumannii,no Gram negative bacteria was found to be resistant to Imipenam. Klebsiella pneumoniae and ESBL secreting Escherichia coli isolates were obviously increasing when compared to 3 years ago. Conclusion Comparing the period of 2004-2006, the Gram negative bacterial isolates were increasing in our PICU patients during 2007 to 2009. Close monitoring of drug resistance pattern changes in PICU would guide rational use of antibiotics in these critically ill children.
