OBJECTIVE:To investigate influential factors of standardization of clinical research on therapeutic efficacy of Yao-tongning capsules,and to provide direction for reasonable and standard development of clinical research. METHODS:Retrieved from CJFD,VIP and Wanfang database,RCTs about Yaotongning capsules in the treatment of related disease were collected. After data extraction and quality evaluation,Meta-analysis was conducted by using Rev Man 5.3 statistical software. The effects of the di-alectical situation,treatment course and route of administration on the evaluation of therapeutic efficacy of Yaotongning capsules were compared;based on this,the concepts of index evidence-based value which reflected clinical standardization were put for-ward,and its effects on Yaotongning capsules were investigated. RESULTS:A total of 71 RCTs were included,involving 11009 patients. Results of Meta-analysis showed therapeutic efficacy of Yaotongning capsules was better than that of other drugs in dialecti-cal case(P=0.02),while therapeutic efficacy of Yaotongning capsules was inferior to that of other drugs in non-dialectical case(P<0.01);therapeutic efficacy of Yaotongning capsules was inferior to that of other drugs when treatment course was shorter than 30 days(P<0.05),while treatment course was longer than 30 days,there was no difference in therapeutic efficacy between Yaotongn-ing capsules and other drugs(P=0.99). When did not take medicine according to the instruction,therapeutic efficacy of Yaotongn-ing capsule was inferior to that of other drugs(P<0.05);when taking medicine according to the instruction,there was no differ-ence in therapeutic efficacy between Yaotongning capsules and other drugs(P=0.94). When evidence-based value was ≥5,thera-peutic efficacy of Yaotongning capsules was better than that of other drugs(P=0.03);when evidence-based value was equal to 4, there was no difference in therapeutic efficacy between Yaotongning capsules and other drugs(P=0.56);when evidence-based val-ue was ≤3,therapeutic efficacy of Yaotongning capsules was inferior to that of other drugs(P<0.05),with statistical significance. CONCLUSIONS:It is an important guarantee of playing therapeutic efficacy of Yaotongning capsules to regularly use drugs strictly according to the principle of dialectical treatment,and take drugs according to treatment course and instructions. Clinical trials should be standardized strictly in accordance with the instructions,so as to provide objective and fair clinical findings.

Objective To discuss the effects of the health card-based services on community chronic disease management and the utility of community healthcare resources. Methods Follow-up investigation on utilization and satisfaction of card-based health services and coverage of chronic illness management was conducted among cardholder living at Dongcheng District of Beijing during July 1,2006 and December 31,2008. Results The rates of attendance and health education increased from 47.20% or 1.16% to 90.90% and 46.38% ,respectively. The coverage of high blood pressure management and standard hypertension management increased from 47.16% or 45.66% to 80.39% and 78.27%, respectively. The satisfaction of cardholder residents with community healthcare services was improved year by year. Conclusions The health card service pattern plays a significant role in improving chronic disease management,which results in more satisfaction with and utilization of healthcare resources in community.

Objective To investgate the expression and the clinical significance of survivin in the primary hepatocellular carcinoma (HCC). Methods S-P immunohistochemical staining was performed to detect the expression of survivin in 50 samples of HCC tissue and 20 of normal liver tissue. Results The positive rate of survivin expression in HCC was 66. 0% (33/50) , which was significantly higher than that in normol liver tissues (negative expression) (P<0. 001). The expression of survivin was correlated with intrahepatic dissemination and multiple masses (P = 0. 019, P=0. 030). The high survivin expression was associated with the survival time less than three years(P = 0. 018). Conclusion The survivin may play certain roles in the oncogenesis and progression of HCC. Detection of the survivin can supply certain evidence on prognosis of primary hepatocellular carcinoma.

Objective To evaluate the effects of baijin capsule on behavioral changes and monoamine neurotransmitters concentration in chronic unpredictable mild stress ( CUMS ) depression rat model.Methods The depression rat model was induced by11-week chronic unpredictable mild stress combining with solitary.After the model were established, rats were given the decoction of baijin capsule ( 12.6 g/kg, 4.2 g/kg, 1.4 g/kg ) or fluoxetine hydrochloride (3.5 mg/kg) by intragastricfor 4 weeks.During the experiment period, sucrose consumption and open-field experiment were conducted to monitor the behavior of rats, such as sucrose consumption percentage, horizontal motion, and vertical motion.At the end of the experiment, the levels of the monoamine neurotransmitters in the cerebral cortex and hippocampus were analyzed by method of high performance liquid chromatography-electrochemistry.Results Compared with the normal group, the weight, horizontal displacement distance, vertical movement times, and sucrose consumption percentage of rats in model group decreased significantly after stimulated with CUMS and solitary for 7 weeks (P<0.05,P<0.01).Compared with model group, consecutively administrated for 4 weeks, horizontal displacement distance, vertical movement times, and the percentage in sugar water consumption significantly increased with the treatment of baijin capsule (P<0.05, P<0.01).Meanwhile, the content of norepinephrine (NE), dopamine (DA), and 5-hydroxytryptamine (5-HT) in the cortex were significantly increased in rats of the baijin capsule ( P <0.05 ) .Conclusions The results indicated that baijin capsule improved the behavioral disturbances in depression rat model, which were related to enhancement of the concentration of monoamine neurotransmitters in the cortex.

Personnel training marks a key assurance for the healthy and sustainable development of healthcare sector and healthcare institutions in China, rendering it critical importance to include personnel training into performance appraisal. The authors analyzed the connotation and orientation of personnel training index assessment in the performance appraisal at tertiary public hospitals in the country, sorted out the assessment details to be optimized. On such basis, the authors recommended on optimal assessment methodology, combining the gradual improvement of medical personnel training assessment indexes and interval range assessment. These efforts aim at providing references for further improving the scientificity, standardization, homogeneity, as well as vertical and horizontal comparability and operability of personnel training index assessment.

?AlM: To investigate theprotective effect of melatonin against hydrogen peroxide ( H2 O2 )-induced oxidative damage to human lens epithelial cells.
?METHODS: Sub-culture human lens epithelial cells preprocessed with different concentrations of melatonin for 12h and then 100 μmol/L H2 O2 for 24h. The impact of melatonin on H2 O2-induced lens epithelial cell viability was detected by MTT assay, rate of apoptosis was detected by flow cytometry instrument and activity of apoptosis-related factors, Caspase-3 and Caspase-9, were detected by colorimetric method.
?RESULTS: MTT assay showed that melatonin had no effect on the activity of lens epithelial cells, and the drug can inhibit the decrease of H2 O2-induced cell activity, as well as flow cytometry showed that melatonin can inhibit H2 O2-induced apoptosis. ln addition, melatonin can also reduce H2 O2-induced Caspase-3 and Caspase-9 activity in lens epithelial cells, and their activity decreased with effect of melatonin along with extending time.
?CONCLUSlON: Melatonin can obviously inhibit H2 O2 -induced apoptosis of human lens epithelial cells, which provide reliable experimental basis for drug on treatment of cataract.

Background Recent studies indicated that rat and mouse Müller cells can be induced and differentiated into photoreceptor-like cells in vitro, but it is not known whether this also happens to adult pig Müller cells nowadays.Objective This study was to test whether adult pig Müller cells can be differentiated to the retinal photoreceptors (the primary transmission neurons of the retina) in vitro.Methods Müller cells were isolated from the neural retina of adult pig eyes and cultured and passaged.The 3rd and 4th generation of cells were themonolayerly cultured,and the cells forced to form spheres in suspension in altra-low adherent dishes for 2-3 days first and then reseeded in normal adherent plates,and both of them were cultured in a specifically formulated medium to induce the differentiation of retinal photoreceptor.The cells was verified by immunocytochemistry.Cell morphology and immunofluorescence staining were utilized to measure the efficacy of the differentiation.Results The 2nd,3rd and 4th generation of Müller cells expressed glutamate synthetase (GS) , a specific maker of Müller cells.Inaddition, the 3rd generation of cells also expressed glial fibrillary acidic protein (GFAP) and another specific maker of Müller cells.Three visual fields under fluorescence-microscope were randomly chosen to calculate the average positive ratio of rhodopsin,a specific marker of mature photoreceptors.The photoreceptor differentiation ratios of the 2nd generation of cells for monolayer culture only and with additional sphere suspension culture were (27.99±6.53 (％ and (16.54±3.40) ％ , respectively.With passages, the number of rhodopsin positive cells gradually decreased, and the intensity of rhodopsin expression gradually weakened.The directed rhodopsin positive ratios of the 2nd,3rd and 4th generation of cells from sphere formation were (56.23±7.32)％ , (36.26 ±8.55)％ and (12.68 ±3.18)％ , respectively.Although the rhodopsin expression was weakened over passages,the differentiated cells were more slender and elongated.There was no statistic ally significant difference between different groups (F =2.618, P =0.099).Conclusions Adult pig Müller cells can be differentiated into retinal photoreceptors in vitro.The morphology of the differentiated cells appears moreslender and elongates if the sphere-induced differentiation method is used and/or the directed differentiation time is further extended.
