1.Perinatal care services and neonatal mortality in Quang Ninh province

Background: In Vietnam, although there were interventions in perinatal and neonatal care services, the neonatal mortality reduced slowly, accounting for >50% of under-5-year-old mortality rate and >70% of under-1-year-old mortality rate. \r\n', u'Objectives: Discover the status of prenatal, perinatal, and postnatal care services, and analyze the relationships between healthcare services and neonatal mortality in Quang Ninh province. Subjects and method: Information on births and neonatal deaths between January and December 2005 in Quang Ninh province was collected. The perinatal care services including antenatal, delivery and post partum cares were described. The relationship between neonatal deaths and places of delivery was analyzed in order to uncover the risk of death in newborns delivered at home. Results: 76% of pregnant women received 3 or more antenatal care visits. More than 90% of deliveries took place at health facilities, of them, 16% occurred in community health stations. Only 49% of mothers received post partum care visits, with 82% of them received the visits in the first week after delivery. There were 17,519 births and 284 neonatal deaths in 2005. The neonatal mortality rate was lowest in cases born in community health stations (7.5/1.000) and highest in home birth cases (39/1.000). Conclusion: Further improvement in safe motherhood and neonatal survival can be achieved by increasing the accessibility of women to perinatal cares during pregnancy, delivery. \r\n', u'

2.Research on adaptation and cloning of rgh5n1 vaccine reference strain to vero cells

Background: As avian influenza A/H5N1 epidemic spreads rapidly, many corporations, vaccine manufacturers in cooperation with laboratories around the world has conducted research and developed H5N1 vaccine for both humans and poultry. Objective: Evaluate the adaptation and cloning of rgh5n1 vaccine reference strain to vero cells in order to produce master seed virus and working seed virus. Subject and Method: The reverse genetics derived A/H5N1 virus strain (rgH5N1) was studied for adaptation to Vero cells by serial passage. It has been shown that the rgH5N1 strain can be propagated in Vero cells and cause CPE with the highest virus titer 1010,9 PFU/ml at Vero passage 15. The rgH5N1 strain was cloned by using plaque purification method and passaged to obtain a high stable virus titer. Conclusion: The reverse genetics derived A/H5N1 virus strain was propagated in Vero cells. Master seed virus and working seed virus were obtained at passage 6.

3.Studying the therapeutic effects of the traditional remedy of "tu than hoan" in the treatment of irritable bowel syndrome of diarrhea state

Background: Irritable bowel syndrome is a diagnosis of exclusion. It is a functional bowel disorder characterized by chronic abdominal pain, discomfort, bloating, and alteration of bowel habits in the absence of any detectable organic cause."Tu than hoan" is a traditional remedy applied for the treatment of syndromes of traditional medicine which have the similar characteristics to irritable bowel syndrome of diarrhea state of modern medicine. Objectives: The study had two purposes: (1)To evaluate the therapeutic effects of the remedy on the clinical symptoms of patients with irritable bowel syndrome of diarrhea state comparing with Duspatalin; (2)To evaluate the side effects of the remedy. Subjects and method:162 patients diagnosed with irritable bowel syndrome of diarrhea state were treated at Bach Mai hospital from March to August 2005. They divided into 2 groups, the control group included 77 patients and the study group included 85 patients. : clinical test, comparing with controls. Results: 80% of patients recovered from diarrhea; 82.4% of patients with defecation returned to normal; 93.6% stopped mucous feces; 76.5% stopped bellyache. Good therapeutic effect was 61.2% (p < 0.05). Conclusions: Tu than hoan had good therapeutic effects in the treatment of irritable bowel syndrome of diarrhea state. None of patients had to discontinue the medicine due to side effects. \r\n', u' \r\n', u'

4.Study on epidemiological characteristics of the Japanese Encephalitis in Ha Nam during 2001 - 2006

Background: Viet Nam is located in the epidemiological region of Japanese Encephalitis (JE). JE vaccine manufactured in Viet Nam has been used in the national vaccination program since 2003, but there has been no scientific study on the effectiveness of this program in Ha Nam province. Objectives: To investigate the epidemiology of JE and effectiveness of vaccinations in Ha Nam province from 2001-2006. Subjects and method: All residents in Ha Nam province, with the key focus group being JE cases. The retrospective and prospective epidemiological descriptive method was used. Results: The records of 148 cases of JE were collected. Among them, 42.5% (63 cases) were confirmed as JE by MAC-ELISA. The incidence and mortality rate were 1.29/100,000 and 0.12/100,000, respectively. Confirmed JE cases were found annually in all districts of Ha Nam province. However, it has been in decline, especially since the vaccination program was implemented for 1-5 year old children. The disease occurred in the summer with the peak in May and June (80.9% of total cases). Most of the cases were children under 15 years old (98.4%). The highest incidence rate was 6.4/100,000; 5.5/100,000 and 5.3/100,000 in the children aged 5-9, 1-4 and under 1 year old, respectively. Conclusion: The vaccination program appeared effective in reducing the mortality and morbidity of JE among children in Ha Nam province.

5.Platelet aggregation induced by ADP and collagen in dyslipidemia patients

Background: Dyslipidemia is an important risk factor of atherosclerosis. The endothelia can be injured by plaque that advantages for platelet adheadsion, aggregation and produces thrombosis. Objectives: (1) To evaluate the changes of platelet aggregation induced by ADP and collagen in hyperlipidemic patients; (2) To identify the relationship between platelet aggregation and various types of dyslipidemia. Subjects and methods: A descriptive cross sectional non-randomised control trial method was employed in this study. A total of 128 hyperlipidemic patients (in which 40 patients were type IIa, 36 were type lib, and 52 were type IV), and a control group of 88 people were measured the aggregation in platelet - rich plasma (PRP) induced by ADP and collagen. Results: There was no significant difference about the platelet counts in both intervention and control groups (p>0.05). ADP and collagen induced platelet aggregation increased significantly (p<0.01) in intervention group when compared to control group. A significant correlation was found between the plasma triglyceride concentration and platelet aggregation induced by ADP and collagen with r = 0,335 and 0,332 respectively, while that of cholesterol and ADP was 0,304. Conclusion: Platelet aggregation increased in hyperlipidemic patients and had a significant correlation with plasma triglyceride concentration. \r\n', u'\r\n', u'

6.Effects of simvastatin on coagulation parameters in dyslipidemia patients

Background: Statin and its derivate (simvastatin, autorvastatin, etc...) are used for dyslipidemia treatment and preventing thrombose. However, the mechanism of the antithrombotic action is still being studied. Objectives: (1) To study coagulation parameters in dyslipidemia. (2) To evaluate the effects of simvastatin on coagulation parameters in dyslipidemia patients. Subject and Method: A prospective study was carried out in a sample of 22 patients with primary hypercholesterolemia (type IIa), who were treated with simvastatin 20mg/d for 1 month. The lipid parameters (cholesterol, triglycerid, HDL, LDL) and coagulation parameters (PT, APTT, fibrinogen, factor II, V, VII, X, VIII, IX, XI) were compared between pre and post therapy, and to the control group (59 healthy people). Results: Most of coagulation parameter values (except factor VIII and X) of the pre treatment group were significantly change towards hypercoagulation (p<0.05%) when compared to the control group. After treatment, PT rate, APTT, APTT rate, fibrinogen, factor VII and IX were significantly changed towards coagulation when compared to pre treatment (p<0.05%). The plasma coagulation and lipid parameters of more than 50% of the hypercholesterolemia patients returned to normal values after treatment. Conclusions: Simvastatin therapy on dyslipidemia patients can reduce not only the level of serum lipid, but also coagulation, and proved its effectiveness in the prevention of thrombosis.

7.Detection of IgM anti Japanese Encephalitis (JE) virus by antigen genotype 1 & genotype 3

Background: Recently Japanese Encephalitis (JE) virus type 1 has surfaced and is co-circulated with JE virus type 3 in the northern areas of Viet Nam, so a sensitivity of JE viral antigen genotype 3 to detect IgM is required. Objectives: To compare the sensitivity of JE viral antigen genotype 1 and 3 to detect IgM against the JE virus. Materials and method: 783 cerebrospinal fluid and serum samples from viral encephalitis cases from 1999-2005 were collected and examined by MAC-ELISA for JE viral antigen genotype 1 and 3. Results: The agreement on the diagnosis of these kinds of antigen was 99.7% and the sensitivity of JE viral antigen genotype 3 was higher than that of genotype 1. Thus, JE viral antigen genotype 3 could be considered as the selected antigen for JE diagnosis in Viet Nam. IgM titer determined by JE viral antigen genotype 1 was higher than that of genotype 3 in 2003 and 2005 and lower in 1999, 2000, 2001, 2002 and 2004. Conclusion: The dominant phenomenon of JE viral genotypes differing over the years might be due to the interaction of the virus and its vectors. Further study is required to clarify this observation.

8.Detection of Japanese encephalitis genotype 1 in central, southern and highland of Viet Nam

Background: In recent year, Japanese Encephalitis Virus (JEV) genotype 1 has been detected among isolates from mosquitoes and pig\u2019s blood samples in northern Viet Nam, but there has been no information on the presence of this genotype in the Central, Southern and Highland regions. Objectives: This study aims to detect the Japanese encephalitis genotype 1 in various different geographic regions of Viet Nam. Material and method: Sequence analysis\u2019s of whole E gene of 18 strains isolated from human, mosquitoes and pig\u2019s blood during 2001-2007. Results: 7 strains isolated from pig\u2019s blood and mosquito samples in the Northern, Central, Southern and Highland fell into genotype 1, but 11 others isolated from humans in the Northern and Central regions belonged to genotype 3. Conclusion: This is the first time that JEV genotype 1 was detected in the central, northern, highland Viet Nam and further studies on genotype 1 causing human diseases needs to be carried out.\r\n', u'\r\n', u'

9.Detection of Japanese encephalitis frequency in the pig population in Ha Nam province by GAC-ELISA.

Background: Mosquitoes and pigs play important roles in maintaining and increasing the Japanese Encephalitis (JE) virus in nature and which is then transmitted to humans. Thus, surveillance of the JE infection frequency in the pig population may predict the human JE cases. \r\n', u'Objectives: The study aimed to determine IgG antibody against the JE virus in the pig population in Hanam province \r\n', u'Subjects and methods: The study included 1791 pig serum samples collected from 3 districts of Hanam province from Apr 2006 to Mar 2007. GAC-ELISA technique was used to determine the JE virus infection in the swine population.\r\n', u'Results: The average positive rate in pig population was 34.9 % (626/1791); with the highest frequency occurring in the summer (37.7%- 84.0 %), co-incident with the JE season in Northern Vietnam. 10.Evaluation of the quality of the IgM antibody capture ELISA diagnostic kit for the Japanese encephalitis virus produced by National Institute of Hygiene and Epidemiology

Background: IgM antibody capture ELISA (MAC-ELISA) technique has been widely applied for Japanese Encephalitis Virus (JEV) diagnosis. So far rare internationally commercial kits are available. Thus, the international evaluation of the kit is required as per the recommendation of the WHO. Objectives: To evaluate the quality of the IgM antibody capture ELISA diagnostic kit for JEV produced by the Vietnam National Institute of Hygiene and Epidemiology (NIHE). Subjects and method: In this study, NIID kit was used as control to check the kit from NIHE. Both NIHE and NIID kits were used to detect JEV IgM among 38 serum and 6 CFS samples, which belongs to 5 sample groups (JE patients group, dengue patients group, other viral encephalitis patients group, Tick Born Encephalitis (TBE) patient group and healthy JE vaccinated donors group). Results: The detection of JEV IgM by NIHE kit was concurrent with the NIID kit. There is no positive with the JE in the groups of Dengue patients, TBE, other virus encephalitis patients and JE vaccinated donors. Conclusion: MAC-ELISA kit of NIHE can be used for different diagnosis of JEV and Dengue virus (both viruses are in Flavivirus genus), as well as other viruses caused by encephalitis.
