Hemimasticatory spasm is a rare neurologic disorder characterized by unilateral, paroxysmal involuntary contraction of the masticatory muscles. It can be confused with other unilateral facial spasms. In this report, we present two patients with hemimasticatory spasm who were initially misdiagnosed with other disorders. The first patient was a 54-year-old man with a 9-year history of right facial spasm. He underwent microvascular decompression, without benefit, at another hospital with the diagnosis of hemifacial spasm two years prior to presenting at our hospital.On examination, right masseter and temporalis muscles had irregular contractions with twitches and prolonged spasms. His brain magnetic resonance imaging was normal.He showed a good response to carbamazepine. The second case was a 60-year-old man presented with a 10-year history of jaw-closing movement. Previous treatment for temporomandibular joint disorder was ineffective. He was previously diagnosed as oromandibular dystonia at another hospital, and biperiden and diazepam treatment resulted in slight improvement. There was tonic contraction and hypertrophy of the left masseter. He improved with carbamazepine. These cases illustrate the importance of hemimasticatory spasm as differential diagnosis, and the good response to carbamazepine.

Torticollis is an usual symptom that can be the result of various disorders, such as sternocleidomastoid muscle pathology, bony abnormalities of the cervical spine, disorders of the central or peripheral nervous system, various ocular diseases, and brain tumors, especially in children. A 12-month-old male visited our hospital because of torticollis. He was normal on systemic examination, with no neurological abnormality, and his cervical spine CT was also normal. About 4 weeks later, he revisited the emergency department due to vomiting with altered mental status, and was diagnosed with a cerebellar tumor with hemorrhage. Although torticollis is known to be an important sign of a posterior fossa tumor, associated neurological or ocular symptoms are usually present. We report here a patient with posterior fossa tumor where torticollis was the only initial presenting symptom.

Multiple neurological deficits can occur in acute disseminated encephalomyelitis (ADEM) but rarely movement disorders. A 24-year-old female patient was admitted because of tremor in bothupper limbs. After admission, her mental status progressively declined into coma. Brain magnetic resonance imaging and cerebrospinal fluid analysis were compatible with ADEM. Tests for central nervous system infection and autoimmune diseases were all negative. She was given steroid and IVIG, and the patient fully improved. We report here the first case of adult ADEM, who presented with tremor as the initial neurological manifestation.

Through Epi-info 6, the data shows that the number of patients with nervous disease is admitted hospital increasing gradually (1%) from 1990 to 1996, especially cerebral vascular accident. The first rank is inflammatory diseases: 53,66%, cerebral vascular accident: 27,33%, sciatic nerve: 17,54% and epilepsy: 7,26%. Encephalitis, meningitis and epilepsy mainly occurs for people under the age of 20 (male: 59,17%; female: 40,83%). Sciatic nerve mainly occurs for people above the age of 20 and autonomic nervous system disorder (male and female) from 20 to 40 of age, cerebral vascular accident mainly occurs for the people above 40 : 88,30%.

The study on number of admission, morbidity and pathology as season, age and gender has shown that the number of patients with nervous diseases are increasingly (about 1%) every year, especially cerebrovascular accident. The encephalitis, meningitis and epilepsy mainly occurred in persons with 20 age and below. Sciatic pain mainly occurred in persons with ages of above 20. The autonomic disorder occurred in age of 20 - 40 and cerebrovascular accident occurred in age of above 40
