1.Basic principles of satisfying medical care
Chinese Journal of Hospital Administration 1996;0(12):-
The integration of natural and human sciences has resulted in the evolution of the connotation and framework of medicine, thus enriching its scientific nature. The high degree of the integration of the scientific, technological, economic, and social factors in the field of the medical sciences with human sciences reveals the significance of the human spirit in medical care. Reliability, impartiality, effectiveness and satisfaction, which embody the human spirit in medical care, signify a hospital's character and core values and are the prerequisite and guarantee for satisfying medical care. The authors point out emphatically that the development of the codes of ethics and the human ideology in medical care constitutes both the core of the cultural construction of the modern hospital and an important foundation for its economic growth.

Aim To investigate the protective effect of cinnamic aldehyde ( CA ) on hormone-induced osteo-clasts proliferation and bone resorption in vitro and its molecular mechanisms. Methods RAW264. 7 cells induced into osteoclast were treated with RANKL and M-CSF and then were divided into control group, dexa-methasone ( DEX ) group and different doses of CA (11. 6, 23. 2, 46. 4 μg·L-1 ) groups. OCs were ob-served after tartrate resistant acid phosphatase( TRAP) staining. The cell proliferation was determined by MTT assay at different time points. The expression levels of TRACP5 b in cell cultured supernatants were measured by ELISA. RT-PCR technique was applied to examine the transcriptional levels of RANK and NFATc1 . Re-sults In MTT assay, the proliferation of osteoclasts stimulated by dexamethasone was promoted seriously compared with negative control group ( P < 0. 05 ) . Meanwhile, DEX could strengthen the content of TRACP5 b and up-regulate the expressions of RANK and NFATc1 mRNA. After administration of CA, the proliferation was inhibited, while the enhanced expres-sion of TRAP5b was reversed, and the over-expressions of RANK and NFATc1 mRNA were obviously down-regulated in a time-and-dose-dependent manner ( P <0. 05 ) . Conclusion The results suggest that CA in-hibits proliferation and bone resorption of osteoclast in-duced by DEX, which may be mediated by down-regu-lation of RANK and NFATc1 mRNA.

Objective To review and summarize the experiences of transcatheter closure of left ventricular-right atrium communication, and discuss the feasibility of interventional therapy for this kind of cardiac abnormalitis. Methods 22 patients who suffered from left ventricular-right atrium communication underwent transcatheter interventional therapy with ventricular septal defect(VSD) occluder. The operating procedures were performed as like as the transcatheter closure of VSD:established the pathway from femoral artery to femoral vein through left ventricle,VSD,right atrium (or from left ventricle to right atrial through the communication) and inferior vena cava, then inserted the polysheath from femoral vein, introduced by the pathway to left ventricle, and implanted VSD occluder through the polysheath to close the shunt. Results The operation succeeded in 21 patients. The cardiac murmur was disappeared in all patients, and there was no residual shunt or aortic regurgitation that conformed by postoperative ventriculography and echocardiography in follow up phase, and the tricuspid regurgitation was lessened than preoperative. The operation abort in 1 patient because of aortic regurgitation after implanting occluder. Conclusions Transcatheter closure of left ventricular-right atrium communication is feasible as the selected occluder is accordant, and atrioventricular block, aortic regurgitation and tricuspid regurgitation can be avoided.

Objective To investigate the effect of Wnt1 on the expression of Cyclin D1 in human corneal epithelial cells and its related molecular mechanisms. Methods 12 T25 cell culture flasks were cultured after hu-man corneal epithelial cells anabiosis ,culture and continuous passage for 2 times. Culture flasks were divided into 3 groups with 4 culture flasks in each group. Twenty-five ng/mL and 50 ng/mL recombinant human Wnt1 protein were added in two of the groups, and one group without T-cell culture medium(Wnt1)was used as control. Cells cultured in T25 flask were taken from three groups at different time(6 h,24 h,48 h and 72 h). The total number of corneal epithelial cells in each group was calculated. Expression of Cyclin D1 in corneal epithelial cells was de-tected by Western blot. Results The expression of Cyclin D1 protein in the control group decreased gradually from 0 h to 48 h,and reached the lowest level at 48 h and increased at 72 h. Cyclin D1 protein expression in 25 ng/mL group at 6 h after Wnt1 was added was not detected, and Cyclin D1 protein expression in 50 ng/mL group in-creased. The expression of Cyclin D1 protein in 25 ng/mL group and 50 ng/mL group was significantly higher than that in control group at 24 h,48 h and 72 h,reaching the peak at 48 h and decreased at 72 h. Compared with the control group, the growth rate of corneal epithelial cells in 25ng/ml group and 50ng/ml group increased after Wnt1 was added. There was significant difference in 72 h,but no significant difference in 6h,24h and 48h. Conclu-sions The stimulation of Wnt1 protein can enhance the expression of Cyclin D1 in a certain time range, and has a positive correlation with Wnt1 protein. As one of the target genes of Wnt1 signaling pathway,Cyclin D1 may play an important role in the repair of corneal epithelial injury and its cell proliferation and differentiation.

In this study, the time-dependent changes on dynamic computed tomograph (CT) of radiation-induced liver injury in gastric cancer patients was examined. The CT images of 52 gastric cancer patients who had received chemoradiotherapies were reviewed on the PACS system. Dynamic CT scan was performed in all the subjects. Our results showed that 18 patients were found to have radiation-induced liver injury. The CT findings of radiation-induced liver injury in gastric cancer patients tend to show up one month after radiation treatment. The damaged area was of low density on all three phases, and then it was enhanced on portal vein phase or delay phase. The focal radiation reaction of liver without basic disease vanished 9-11 months later after treatment. We are led to conclude that dynamic CT is of help in the diagnosis of CRT-induced liver injury, and it may be the method of choice for following up the whole course of the CRT-induced liver injury, i.e., form hepatic damage to healing. The classification of CT findings we recommend can avoid the influence of technological factors, and thereby serve as a better guide for treatment of CRT-induced liver injury.

Background:Acute-on-chronic liver failure( ACLF)is a commonly seen liver failure in China, and lacking an animal model that can effectively simulate the dynamic change of immune status of ACLF. Aims:To establish an animal model that can simulate dynamic change of immune status of ACLF by repeated administration of concanavalin A(ConA). Methods:Mice were randomly divided into normal control group and ConA repeated administration group. Mice in ConA repeated administration group were injected with ConA 15 mg/ kg through retrobulbar angular vein every 48 hours for 5 times, and mice in control group were injected with same volume of 0. 9% NaCl solution. Serum levels of IL-6,IL-10,IL- 12,TNF-α,IFN-γ,MCP-1 in peripheral blood were assessed by CBA assay, and the ratio of IL-10/ TNF-α was calculated. The expression of HLA-DR,number and proportion of CD4+ T cells and the expression of PD-1 of monocytes in peripheral blood were detected by flow cytometry. Results:Peripheral blood cytokines changed from predominated proinflammatory cytokines into predominated anti-inflammatory cytokines with the increasing in time of administration in ConA repeated administration group. Compared with control group,HLA-DR expression of monocytes in peripheral blood was significantly decreased(P <0. 05),number and proportion of CD4+ T cells were significantly decreased(P <0. 05), and PD-1 expression was significantly increased( P < 0. 05)in ConA repeated administration group. Conclusions:An animal model of ACLF immune status from systemic inflammatory response syndrome( SIRS) to compensatory antiinflammatory response syndrome(CARS)induced by repeated ConA stimulation is successfully established.

Chinese Journal of School Health 2019;40(3):396-398
Objective:
To explore the effectiveness of family therapy on adolescent school refusal.
Methods:
One hundred outpatient adolescents with school refusal were selected. All the patients were divided into two groups through random number table method. The intervention group (50 patients) received family therapy combined with cognitive therapy, and the control group (50 patients) received no intervention. Cognitive therapy was evaluated by the Self-Rating Anxiety Scale (SAS), Severity Impression Scale (CGI-S), Family Dynamics Self-Assessment Scale, and information on going back to school was collected.
Results:
After intervention, score in SAS, CGI-S, CBCL (father-reported), CBCL (mother-reported), family atmosphere, logic family, disease awareness, and personality significantly decreased (P<0.05). No significant differences were found before intervention. However, the intervention group showed lower scores in all the above indicators and higher rates of going back to school (92% vs 70%)(χ2=7.86, P<0.05).
Conclusion
Family therapy can effectively improve emotions such as anxiety and depressive symptoms and help going back to school in adolescents with school refusal.
