1.Changes in voltage-dependent anion channel 3 in an animal model of sepsis-induced myocardial injury

Objective To observe changes in voltage-dependent anion channel 3(VDAC3)in a mouse model of sepsis-induced myocardial injury and to explore its potential mechanism.Methods Twenty male C57BL/6J mice were divided randomly into a Sham group and Sepsis group,respectively(n=10 mice per group).Sepsis was induced by the cecal ligation and puncture(CLP).Serum levels of interleukin(IL)-6,tumor necrosis factor(TNF)-α,creatine kinase MB(CK-MB),and cardiac troponin T(cTnT)were detected by enzyme-linked immunosorbent assay.Pathological changes in heart tissue were observed by hematoxylin and eosin staining.Structural and functional changes in the heart were evaluated by echocardiography.Changes in total glutathione,reduced glutathione(GSH),oxidized glutathione,and malondialdehyde(MDA)in heart tissue were detected by spectrophotometry.The morphological structure of mitochondria in mouse cardiomyocytes was observed by transmission electron microscopy.Expression levels of IL-6,IL-1β,VDAC3,glutathione peroxidase 4(GPX4),solute carrier family 7 member 11(SLC7A11),lipocalin-2(LCN2),and prostaglandin-endoperoxide synthase 2(PTGS2)mRNA were detected by real-time quantitative polymerase chain reaction and the localization and expression of VDAC3 and GPX4 proteins in mouse heart tissue were detected by immunofluorescence staining.The correlations between VDAC3 mRNA and GPX4,SLC7A11,PTGS2,LCN2,IL-6,and IL-1β mRNA were analyzed.Expression levels of VDAC3,GPX4,and SLC7A11 proteins were detected by Western blot.Results IL-6,TNF-α,CK-MB,and cTnT levels were significantly higher in the Sepsis group compared with the Sham group(P<0.05).In the Sepsis group,myocardial fibers were torn,the ventricular wall was thickened and edematous,the mitochondrial membrane was ruptured,and mitochondrial cristae were broken or absent.GSH levels were significantly reduced in the Sepsis group(P<0.05)and the lipid peroxide MDA was increased in the Sepsis group(P<0.05)compared with the Sham group.VDAC3,GPX4 and SLC7A11 mRNA and protein levels were all lower in the Sepsis group compared with the Sham group(P<0.05),while expression levels of IL-6,IL-1β,LCN2,and PTGS2 mRNA were increased(P<0.05).VDAC3 mRNA was positively correlated with GPX4 and SLC7A11 mRNA levels,and negatively correlated with LCN2,PTGS2,IL-6,and IL-1β.Conclusions VDAC3 expression decreases in myocardial injury,and it may participate in the occurrence of sepsis-induced myocardial injury by regulating ferroptosis.
2.Correlation analysis of urinary sodium excretion and early renal functional impairment in patients with primary hypertension

Objective:To analyze the correlation between 24-hour urinary sodium excretion and early renal function impairment in patients with primary hypertension.Methods:This cross-sectional study included patients with primary hypertension who were admitted to the Department of Cardiology of the Chinese People′s Liberation Army General Hospital between January 2021 and October 2024. Patients were divided into low-sodium, medium-sodium, and high-sodium groups based on their 24-hour urinary sodium excretion. General clinical data were collected using the electronic medical record system. Urinary sodium, protein, and microalbumin excretion were analyzed from 24-hour urine samples. Spearman correlation analysis was used to explore the relationship between 24-hour urinary sodium excretion and urinary microalbumin excretion. A multiple linear regression model was used to further assess the independent association between these variables. Subgroup analyses were conducted based on age were performed to determine whether age influenced the relationship between urinary sodium excretion and renal function impairment.Results:A total of 1 065 patients with primary hypertension were included, with a mean age of (55.26±14.06) years, including 568(53.33%) males. The low-sodium, medium-sodium, and high-sodium groups included 223, 579, and 263 patients, respectively. The 24-hour urinary microalbumin excretion in the high-sodium group was significantly higher than in the medium-sodium and low-sodium groups, and this trend remained consistent across different age groups (all P<0.05). Spearman correlation analysis showed a positive correlation between 24-hour urinary sodium excretion and urinary microalbumin excretion ( r=0.220, P<0.001), and this relationship was observed in all age groups (all P<0.05). Multiple linear regression analysis confirmed an independent association between 24-hour urinary sodium excretion and urinary microalbumin excretion (all P<0.001), which persisted across different age groups (all P<0.05). Conclusion:In patients with primary hypertension, 24-hour urinary sodium excretion is closely associated with microalbumin excretion, suggesting a potential link to early renal function impairment.
3.The implementation situations, opportunities, and challenges of China’s integrated elderly care and medical services model and comprehensive care for the elderly

China’s integrated elderly care and medical services model is mainly reflected in the close cooperation between medical institutions and pension institutions, providing comprehensive care services for the elderly, and has been promoted nationwide. However, this model still faces challenges, such as uneven resource distribution, inconsistent service standards, and a lack of talent. As the aging process of the population deepens continuously, the demand for integrated elderly care and medical services has increased, and relevant policies issued by the government and social concerns have brought development opportunities for this model. Scientific and technological advancements will also promote the development of integrated elderly care and medical services towards intelligence and personalization. It is expected that continuous investment in policies, capital, technology, and other aspects will be made to promote the standardization, specialization, and popularization of services to meet the needs of the elderly and improve their quality of life. The joint efforts of all sectors of society are also needed to continuously innovate service models and improve service levels, to effectively address the challenges brought by an aging society.
4.Meta analysis on the prevalence and influencing factors of screen exposure among preschool children in China

Objective:
To systematically evaluate the prevalence and influencing factors of screen exposure among preschool children in China, so as to provide evidence for formulating scientific intervention strategies.
Methods:
Retrieve relevant studies on screen exposure among preschool children from PubMed, Web of Science, Embase, Cochrane Library, China Biomedical Literature Database, CNKI, VIP, and Wanfang databases from the time of estaldishment to June 29, 2025. Meta analysis was performed using Stata 16.0 software.
Results:
A total of 43 studies were included. Meta analysis showed that the prevalence of screen exposure among preschool children in China was 46.0% (95% CI = 38.9 %-53.1%, P <0.01). Girls, non only child, father s age<35 years, both parents having an educational level of high school or below, being cared for by grandparents, rural residence, parents having no exercise habit, parental support for the use of screen devices, and parental screen time>1 h/d were influencing factors for screen exposure among preschool children [ OR (95% CI ) were 0.85(0.78-0.92), 1.09(1.04-1.15), 1.45(1.22-1.71), 1.38(1.24- 1.54 ), 1.78(1.32-2.40), 1.39(1.16-1.65), 1.38(1.13-1.69), 1.67(1.40-1.98), 1.70(1.38-2.10), 1.59(1.04-2.43), all P <0.05].
Conclusion
The prevalence of screen exposure among preschool children in China is relatively high, and relevant child health promotion strategies should be formulated to reduce its occurrence.
5.Path of healthcare talent team construction in public hospitals based on grounded theory

Objective This study explores issues in health talent team construction in public hospitals,using a city as a case study,and constructs a theoretical framework.aiming to provide theoretical references for optimizing talent team construc-tion.Methods Through literature review and in-depth interviews,the status of health talent team construction in public hospi-tals was analyzed.Utilizing the grounded theory,the interview data underwent three-level coding and visual analysis.Results A total of 112 initial categories,12 main categories,and 4 core categories were extracted.Issues related to talent management mechanisms were identified as the most critical concern in the construction of health talent teams in the city's public hospitals.Conclusion Focusing on the theoretical framework for health talent team construction in public hospitals,strengthening talent management,recruitment,training,and service support are the main pathways to optimize healthcare talent team development.

Osteosarcoma (OS) is the most prevalent primary malignant bone tumor affecting children and adolescents. Despite ongoing research efforts, the 5-year survival rate has remained stagnant for many years, highlighting the critical need for novel drug development to enhance current treatment protocols. Ziyuglycoside II (ZYG II), a triterpenoid saponin extracted from S. officinalis, has recently demonstrated antitumor properties. This study evaluates the antitumor effect of ZYG II on osteosarcoma and elucidates its mechanism of action through the co-regulation of p53 and estrogen-related receptor gamma (ESRRG), which inhibits disease progression. The research employs in vitro experiments using multiple established osteosarcoma cell lines, as well as in vivo studies utilizing a nude mouse model of orthotopic xenograft osteosarcoma. Additionally, ESRRG shRNA was used to construct stable ESRRG-reducing OS cell lines to investigate the molecular mechanism by which ZYG II exerts its anti-osteosarcoma effects through the co-regulation of ESRRG and p53. Results indicate that ZYG II administration led to decreased OS cell viability and reduced tumor volumes. Furthermore, cell cycles were arrested at the G0/G1 phase, while the proportion of apoptotic cells increased. Expression of p53, ESRRG, p21, Bax, Cleaved Caspase-9, and Cleaved Caspase-3 proteins increased, while expression of CDK4, Cyclin D1, and Bcl-2 proteins decreased. Multiple ZYG II and ESRRG docking patterns were simulated through molecular docking. Comparing the pharmacodynamic response of ZYG II to OS cell lines with reduced ESRRG and normal expression demonstrated that ZYG II inhibits osteosarcoma progression, induces cell cycle arrest, and promotes cell apoptosis through the coordination of p53 and ESRRG. In conclusion, ZYG II inhibits osteosarcoma progression, leads to cell cycle arrest, and promotes cell apoptosis through synergistic regulation of p53 and ESRRG.

10.Cost-utility analysis of multiple sequential treatment regimens for HER-2 positive advanced gastric cancer

OBJECTIVE To evaluate the cost-effectiveness of different sequential treatment regimens for human epidermal growth factor receptor 2(HER-2)-positive advanced gastric cancer from the perspective of the Chinese health system.METHODS Survival data were obtained from the ToGA,WJOG 4007 and RAINBOW-Asia trials,and adjusted using network meta-analysis.A four-state Markov model was constructed to evaluate the cost-effectiveness of six treatment sequences,with a lifetime simulation horizon and a cycle period of 4 weeks(28 d),and the main output parameters of the model included total costs,quality-adjusted life year(QALY),and incremental cost-effectiveness ratio(ICER).Sensitivity analysis was conducted to test the robustness of the basic analysis results,and a price reduction scenario analysis for trastuzumab was performed.RESULTS Compared with chemotherapy sequentially treated with paclitaxel,the ICER of trastuzumab combined with chemotherapy sequentially treated with paclitaxel,or irinotecan,or ramucirumab plus paclitaxel were 349 845.25,772 410.64,and 2 510 470.39 yuan/QALY,respectively,all exceeding three times China's 2023 per capita gross domestic product(GDP)(268 074 yuan/QALY)as the willingness-to-pay(WTP)threshold.This indicated that chemotherapy sequential paclitaxel was the optimal treatment regimen.The sensitivity analysis confirmed the robustness of the basic analysis.The scenario analysis showed that when trastuzumab was reduced by more than 20%,trastuzumab combined with chemotherapy sequentially treated with paclitaxel became cost-effective under this study's WTP threshold.CONCLUSIONS When using three times China's 2023 per capita GDP as the WTP threshold,chemotherapy sequentially treated with paclitaxel is the optimal regimen for HER-2 positive advanced gastric cancer,with trastuzumab combined with chemotherapy sequentially treated with paclitaxel as the second best option.With enhanced medical insurance optimization,trastuzumab combined with chemotherapy sequentially treated with paclitaxel is expected to become the most cost-effective treatment regimen.
