1.Malignant transformation in monostotic fibrous dysplasia:clinical and imaging features

Objective To investigate clinical and imaging features of malignant transformation in monostotic fibrous dysplasia (FD).Methods Radiography (n=12),computed tomography (CT)(n=7)and magnetic resonance imaging (MRI)(n=5)data of 12 monostotic FD cases proved by surgery and pathology were retrospectively reviewed.Results Among 12 cases,6 cases underwent surgery.Their images showed osteolytic lesions in the operative area with direct signs of aggressiveness (poorly defined margin,cor-tical destruction and soft tissue mass).For the 6 cases without surgery,radiography and CT showed poorly margin,osteolytic le-sions within or near the area of ground-glass opacity.MRI showed lesions with heterogeneous signal intensity and pronounced con-trast-enhancement.Histopathologically,9 cases were osteosarcomas,2 cases were fibrosarcomas,and 1 case was malignant fibrous histiocytoma (MFH).Conclusion The imaging information of malignant transformation of FD can provide evidence of possible ma-lignancy.
2.MR imaging diagnosis of shoulder joint injury

Objective To analyze the imaging characteristics and the diagnostic value of conventional MRI and indirect MR arthrography in shoulder joint injury.Methods MR imaging in90 patients with shoulder joint disease were retrospectively analyzed by two musculoskeletal radiologist based on the results of arthroseopic examination,including 57 cases of rotator cuff disease and 33 cases of recurrent shoulder instability.The result of the indirect MR arthrography and conventional MR was statistically compared by Fisher exact probability test.Results (1)In the group of rotator cuff disease,37 patients underwent indirect MRA,demonstrating 10 cases of partial-thickness rotator cuff tear and 1 case of missed diagnosis;demonstrating 17 cases of full-thickness rotator cuff tear and 2 cases of misdiagnosis.For partialthickness rotator cuff tear,the sensitivity,specificity and accuracy of indirect MR arthrography were 90.9%(10/11),92.3%(24/26),and 91.9%(34/37),respectively;for full-thickness rotator cuff tear,the sensitivity,specificity and accuracy of indirect MR arthrography were 89.5%(17/19),94.4%(17/18),and 91.9%(34/37),respectively.Indirect MR arthrography in the diagnosis of partial and full thickness rotator cuff was significantly better than conventional MR(P<0.05).(2)In the group of recurrent shoulder instability,labium tear was found in 26 cases,Hill-sachs lesion in 27,reverse Hill-sachs in 2,bony Bankart lesion in 7,and capsular tear in 18 cases,respectively.Twenty patients underwent indirect MRA,demonstrating 14 cases of labrum tear,1 case of missed diagnosis,and 5 cases of normal labrum.For labrum tear,the sensitivity,specificity and accuracy of indirect MR arthrography were 93.3%(14/15),100.0%(5/5),and 95.0%(19/20),respectively.Indirect MR arthrography in the diagnosis of labrum tear was significantly better than conventional MR(P<0.05).Conclusion MRI,especially indirect MR arthrography,is the most helpful method for the diagnoses of shoulder joint injury.

4.Expression autophagy-related gene pULK and PI3KC3 and their correlation with human non-small-cell carcinoma

Objective To examine expression levels of autophagy gene pULK and PI3KC3 and to explore their correla?tion with non-small cell lung cancer (NSCLC). Methods A total of 77 samples of surgical resection from NSCLC speci? mens (including 31 cases of squamous cell carcinoma, 31 cases of adenocarcinoma and 15 cases of large cell undifferentiated carcinoma) and 21 samples of same normal lung tissue were randomly selected. Expressions of pULK and PI3KC3 in lung tissues were assessed by immunohistochemistry and Western blot. All dates were analyzed using SPSS13.0 statistical pack?age. Results Immunohistochemistry indicated that pULK and PI3KC3 localized into the cytoplasm. The expression levels of pULK and PI3KC3 are significantly lower in patient with NSCLC than those in peri-tumor tissue (35.1%vs 81.0%, 40.3%vs 76.2%respectively, P<0.01) . Immunohistochemistry and Western bolt analysis confirmed that pULK and PI3KC3 ex?pressions were significantly down-regulated (P<0.01) in patients with low grade cellular differentiation, metastasis of lymph node, or stageⅢandⅣ. And expression levels of pULK and PI3KC3 in NSCLC did not differ significantly with ages, gen?der, tumor size and pathological type. Correlation analysis showed that the expression of PI3KC3 was positively correlated with pULK. Conclusion pULK and PI3KC3 expressions were lower in NSCLC than those in normal lung tissue group. The expression levels of pULK and PI3KC3 in NSCLC were correlated with patient's clinical stage, differentiation grade, lymph node metastasis but were unrelated with age, gender, histological type and size.
5.Comparative Study of MR Imaging in Diagnosis of Shoulder Instability

Objective To evaluate the diagnostic value of indirect MR arthrography(MRA) and conventional MR for shoulder instability.Methods 24 cases of shoulder instability were retrospectively analyzed.13 patients underwent conventional MR,11 patients underwent indirect MRA.Based on the results of arthroscopic examination,the efficacy of indirect MRA images and conventional MR images in diagnosis of the labrum,the capsule, and the bone structure injury.Results Indirect MRA in diagnosis of labrum tear was signifiantly better than conventional MR(P0.05).Conclusion Indirect MRA is more helpful to diagnose the labrum tear than conventional MR.Further study should be taken for capsule and ligament injury.
6.Dilution validation of high level D-dimer samples in Sysmex CS5100 instrument

Objective To investigate the necessity of dilution in samples with high concentrations of D-Dimer and optimum dilution multiple.Methods Quality control products and calibration were detected by using Sysmex CS5100 for precision evaluation,including within-batch and between-run precision.Calibration were detected for validation of linear range and clinical reportable.Samples with D-Dimer<5 mg/L and fibrinogen degradation products(FDP)<20 μg/mL were serially diluted and detected to calculate recovery rate.Samples with D-Dimer>5 mg/L and FDP>20 μg/mL were also serially diluted and detected to calculated recovery rate.Results Within-batch and between-run coefficients of variation were both less than 3%.Within the scope of 0.207-5.170 mg/L,the linear distribution was fine.The clinical reportable range was 0.207-165.440 mg/L.For samples with D-Dimer<5 mg/L and FDP<20 μg/mL,no antigen excess phenomenon was found,and gradient dilution was not necessary.For samples with D-Dimer>5 mg/L and FDP>20 μg/mL,there was obvious antigen excess phenomenon,and gradient dilution was required.Conclusion For samples with D-Dimer>5 mg/L and FDP>20 μg/mL,dilution should be performed to ensure the accuracy of detected results.
7.Clinical study on the treatment of rosacea with excessive heat in lung and stomach with self-made Liangxue-Qingfei Decoction combined with western medicine

Objective:To evaluate the clinical efficacy of self-made Liangxue-Qingfei Decoction combined with conventional western medicine in the treatment of rosacea with excessive heat in lung and stomach. Methods:312 patients with rosacea in Longfu hospital from May 2017 to May 2019 were selected and randomly divided into two groups, 156 cases in each group. The control group was treated with Doxycyclane Hyclate Tablets combined with Metronidazole Gel The study group was additionally given self-made Liangxue-Qingfei Decoction on the basis of the control group. Both groups were treated for 4 weeks. Dermatology Life Quality Index (DLQI) was evaluated . The erythema index (EI) of lesion area was measured by multi-functional skin tester. The adverse reactions during the treatment were recorded and the clinical efficacy was evaluated. Results:The total effective rate was 80.8% (126/156) in the study group and 66.7% (104/156) in the control group, with statistically significance ( χ2=8.012, P=0.005). After the treatment, the scores of physiology, psychology, family, social activities, interpersonal and occupational restrictionsin, and treatment in the study group were lower than those in the control group ( t=17.942, 15.794, 19.465, 21.586, 31.375, 27.472, P<0.001); EI (619.69 ± 71.53 vs. 822.97 ± 89.47, t=32.348) was lower than that of the control group ( P<0.01). During the treatment, the incidence of adverse reactions was 4.5% (7/156) in the control group and 5.8% (9/156) in the study group, and there was no significant difference between the two groups ( χ2=0.439, P=0.514). Conclusion:The self-made Liangxue-Qingfei Decoction combined with routine western medicine therapy can effectively improve the clinical symptoms and quality of life of rosacea patients with lung and stomach excessive heat, improve the clinical efficacy with safety.
8.Porhyromonas endodontalis liopolysaccharide regulates interleukin-23 expression in mouse osteoblasts

Objective:To investigate the effects of Porhyromonas endodontalis(P.e)liopolysaccharide(LPS)on the expression of IL-23mRNA and protein in mouse osteoblasts MC3T3-E1 and the role of phosphatidylinositol 3-Kinases (PI3K)signaling pathway in this process.Methods:MC3T3-E1 cells were treated with different concentrations of P.e LPS for different hours,or pretreated with LY294002,a special PI3K inhibitor.The IL-23 mRNA and protein expression levels were detected by real-time PCR and Western blot respectively.Results:The level of IL-23 mRNA increased in MC3T3-E1 cells with the increase of concentration and the treatment time of P.e LPS(P <0.05).The IL-23 protein expression was increased by P.e LPS in a time dependent manner(P <0.05).The mRNA and protein of IL-23 decreased(P <0.05)after pretreatment with LY294002.Conclusion:P.e LPS can induce the expression of IL-23 mRNA and protein in MC3T3-E1 cells,and the PI3K signaling pathway may play a part in this process.
9.Malnutritional risk and nutritional support in geriatric inpatients

Objective To investigate the prevalence of malnutrition and nutritional support and interventions in geriatric inpatients.Methods The elderly patients (aged ≥ 65 years)from the geriatric demonstration ward were consecutively enrolled from July 2010 to January 2012.MiniNutritional Assessment-short form (MNA-SF) was performed after admission,and data of nutritional support were collected.Results A total of 179 patients were enrolled in this study.According to MNA-SF,42 cases (23.5%)were rated as malnutrition,and 55 cases (30.7%) were rated as at risk of malnutrition.Totally,45 patients received nutritional support.50.0% (21/42) patients with malnutrition,and 29.1% (16/55) patients at risk of malnutrition received nutritional support.As to the route of nutrition therapy,the ratio of the enteral to parenteral to combination of enteral and parental nutrition was 4.4 ∶ 1.0 ∶ 1.0.Conclusions The incidence of malnutrition is high in the geriatric inpatients,and routine nutritional risk screening and assessment are essential for the elderly patients.Nutritional support and other comprehensive treatment are in great need,and the enteral nutrition is appropriate and preferred.
10.Determination of ethyl pyruvate in chitosan nanoparticles by reversed-phase high-performance liquid chromatography

BACKGROUND:Research on ethyl pyruvate detection methods is reported rarely, and moreover, literature about reversed-phase high-performance liquid chromatography (RP-HPLC) for detection of ethyl pyruvate is less.
OBJECTIVE:To establish an RP-HPLC method for determination of ethyl pyruvate in ethyl pyruvate-chitosan nanoparticles.
METHODS: The chromatographic analysis was performed on a ZORBAX Eclipse XDB-C18 column (4.6 mm× 150 mm, 5μm) at 25℃, with the mixture of acetonitrile and water (40:60, V/V) as the mobile phase at the flow rate of 1 mL/min. The determination wavelength wasset at 210 nm and the injection volume was 20 μL.
RESULTS AND CONCLUSION: The peak of ethyl pyruvate and the peaks of auxiliary materials and solvent were separated wel. The linear rang of ethyl pyruvate was 1-100 mg/L (r=0.999 6). The relative standard deviation of both the intra-and inter-day precision was less than 3% for low-, moderate-, and high-concentration ethyl pyruvate. The relative standard deviation of reproducibility test and stability test was 1.25% and 1.3%, respectively. Sample average recovery rates were (91.5±1.0)%, (3.5±0.2)%, (94.4±0.4)%, respectively. Encapsulation efficiency of samples were (87.2±0.22)%, (90.5±0.15)%, (91.1±0.17)%, respectively. The relative standard deviation of different sample content were 0.9%, 0.5%, 0.3%, respectively. The RP-HPLC method for determination of ethyl pyruvate is sensitive, accurate and highly specific with wide linear range and high sample average recovery.
