1.Correlation of gut microflora and intestinal mucosal immunity

Gut microflora , an important part in maintaining the intestinal homeostasis , can participate in the development of intestinal mucosal immune system , promote the synthesis of secreted IgA ( sIgA) and interact with intestinal immune cells .Gut micro-flora also plays a significant role in the development of inflammatory bowel diseases , irritable bowel syndrome , pediatric allergic disea-ses and other disorders .This paper reviews the advances about the correlation of gut microflora and intestinal mucosal immunity .
2.Application of cyclosporine A in noninfectious uveitis

Uveitis is a kind of refractory blinding eye diseases.Noninfectious uveitis is an important part of uveitis,which is thought to be an autoimmune disease,and systemic immunosuppressive therapy is usually required to prevent visual loss.As a selective T-cell inhibitor,cyclosporine A is one of the most widely used immunosuppressants,and has obvious therapeutic effect on many kinds of uveitis.Cyclosporine A combined with glucocorticoid,other immunosuppressants or biological agents can improve the therapeutic effect and reduce the side effects.Recently,the inhibitory effect on glucocorticoid-resistant Th17 cells and topical application of cyclosporine A,show its advantage and potential in the treatment of noninfectious uveitis.
3.Role of bone marrow mobilization in the angiogenesis in ischemic hindlimbs of diabetic mice

Objective To investigate the angiogenesis effect induced by hone marrow mobilization on the ischemic hindlimbs of diabetic mice.Methods Diabetes mellitus was induced in mice by injection of streptozotocin.To observe the angiogenesis effect in ischemic limbs,unilateral femoral artery and all the side branches were excised and then was given granulocyte-colony stimulating factor (G-CSF) for bone marrow mobilization.Results The level of nitric oxide (NO) in plasma was significantly lowered 8 weeks after establishment of diabetes mellitus.After mobilization for 8 days,CD_(34)~+ cells in peripheral blood of mice were increased significantly [from (0.15?0.05)% to (2.30?0.41 )%],the vascular endothelial growth factor (VEGF) mRNA level of ischemic limb,which was examined with semi-quantitative RT-PCR,became higher in diabetic mice than that in non-diabetic mice.Two weeks following mobilization,the blood perfusion detected by LASER Doppler perfusion scan in the ischemic hindlimbs was significantly higher in the diabetic mice than in that the nondiabetic mice [(76.37?6.10) % vs (18.07?3.40) %,P<0.05],vWF immunohistochemical staining showed that the density of capillaries was significantly greater in the mobilization group than that in the non- mobilization group [ (26.6?4.8 vs 11.5?2.6 )/per field,P<0.05 ].Conclusion Diabetes mellitus reduced the modulatory role of the endothelium in mice,but therapeutic angiogenesis induced by bone marrow mobilization could be an effective treatment for ischemic limb disease in diabetic mice.
4.Expression of phosphatidylinositol-3 kinase in epithelial ovarian carcinoma

Objective To explore the expression and significance of phosphatidylinositol-3 kinase (PI3K)in ovarian cancer,and the effect of combined PI3K inhibitor(LY294002)therapy with cisplatin on epithelial ovarian carcinoma,and to explore if there is a synergistic effect between the two therapies. Methods The expression levels of PI3K p85 subunit proteins and mRNA were evaluated by western blot and RT-PCR in normal ovarian tissue(G1),ovarian benign tumor tissue(G2),ovarian borderline tumor tissue(G3)and ovarian cancer tissue(G4),and the relevant clinical pathological parameters were analyzed.SKOV3 ceils were isolated and cultured by enzymolysis method.SKOV3 cells were treated with culture medium only,LY294002(1,10,30,50,100 ?mol/L),cisplatin(0.33,1.25,2.5,5, 10 ?mol/L),LY294002(50 ?mol/L)+cisplatin(10 ?mol/L)for 2 days,respectively.The effect of LY294002 and cisplatin on the growth of SKOV3 cells was measured by methyl thiazolyl tetrazolium assay. Results There was no positive expression of PI3K p85 subunit proteins in G1 and G2,while the expression was 2/6 in G3,and 85%(33/39)in G4.PI3K p85 subunit mRNA expression levels were 0.178?0.102 in G1,0.643?0.112 in G2,0.847?0.058 in G3,1.689?0.423 in G4;there was a significant difference between G1,G2,G3 and G4(P0.05).Significant differences were noted between protein expression levels in G4(Ⅲ,Ⅳ)and G4(Ⅰ,Ⅱ;P
5.Quality assessment of Liuwei Wuling tablet by HPLC fingerprint and quantitative analysis.

Reversed phase high performance liquid chromatography with diode array detector was employed for simultaneous determination of six components and specific chromatograms analysis in Liuwei Wuling tablets with gradient elution of acetonitrile and water containing 0.1% phosphoric acid as mobile phase. The results showed that six components containing specnuezhenide, phillyrin, schisandrin, schisantherin A, schizandrin A and schizandrin B were separated well under the analytical condition. The average recoveries ranged from 98.96% to 100.5% with RSD less than 2. 0%. Twenty-five common peaks were selected as the specific chromatograms of Liuwei Wuling tablets with schisantherin A as the reference peak. Similarities calculated by cosine of angle, correlation coefficient and peak area ratio similarity (PAR) were all above 0. 95, indicating a good similarity between the reference and twenty batches of samples. Grubbs test and cluster analysis indicated that the established HPLC fingerprints and HPLC quantitative analysis can be used efficiently in the quality control of Liuwei Wuling tablets.

8.Effects of routine early amniotomy on labor and health status of foetus and neonate: a meta-analysis

Objective To analyze the effects of routine early amniotomy on labor and health status of foetus and neonate. Methods The results of “Early amniotomy increases the frequency of fetal heart rate abnormalities, A randomized controlled trial of early amniotomy, Effect of early amniotomy on the risk of dystocia in nulliparous women, The influence of elective amniotomy on fetal heart rate patterns and the course of labor in term patients: A randomized study, Randomised trial comparing a policy of early with selective amniotomy in uncomplicated labour at term and A randomized controlled trial and meta-analysis of active management of labour were analyzed using Mantel-Haenszel method (fixed effect model) in meta-analysis. Results (1) The routine early amniotomy can shorten 94.90 min of the first stage of labor, 95% CI (-119.17,-70.52). (2) The OR for cesarean section was 1.25,95%CI (0.99-1.57),for instrumental vaginal delivery was 1.05, 95% CI (0.90-1.24). (3) There was no difference in abnormal fetal heart rate at first stage (OR=0.95, 95% CI: 0.75-1.21), but there was increased abnormal fetal heart rate at second stage(OR=1.28, 95% CI: 1.02-1.61). (4) The frequency of stained amniotic fluid and abnormal Apgar scores at 1 minute was not different significantly [ OR=1.17,95% CI (0.78-1.73);OR=0.71,95%CI: (0.49-1.03)]. Conclusions Routine early amniotomy appears to be associated with both benefits and risks. Beneficial effects include reductions in labor duration and possible decrease in frequency of abnormal Apgar score at one minute. Risks include increase in abnormal fetal heart rate at second stage and possible rise in cesarean section rate.

10.Renal impairment in Sjogren's syndrome (SS)

To further study the renal damage of SS with biopsy and recent determination. Methods 31 patients of SS with renal impairment from 1988 to 1995 were analyzed by routine, immunoassay,tubular function and biopsy examination. Results 25/31 cases presented dRTA, 3/25 with diabetes insipitus and 2/25 with hypokalemic paralysis. Glomerulopathy was occured in 8 of 31 (NS 5, GN 3) and mild renal failure( RF) in 4. Hypergamma-globulinemia was observed in 23 of 31 (74%). About 65% and 45% patients revealed anti ss-A and anti ss-B positive respectively. In 13 renal biopsies, 9 specimens showed CIN with large number of lymphoplasma cells infiltration and tubular atrophy and other 2 specimens revealed LN (type Ⅲ and Ⅳ ) and 2 MPGN. By IF, no positive findings were seen in 7 cases and deposition of IgA, IgM or 63 in some other patients. IgG deposits in the interstitial lymphoplasma cells in 1 patient. 19 patients were treated with prednisone and 3 combined with CTX. 14 cases remitted during the follow-up. In 4 patients with RF, Scr level returned to normal after treatment. Conclusion The renal impairment of SS may be the presenting or predominant feature. Their major clinical manifestations are dRTA and GN. The treatment with prednisone may decrease the infiltration of lymphoplasma cells in the interstitium, improve the renal function and correct RTA.
