Objective To evaluate the applicability of the simplified equation of MDRD to predict glomerular filtration rate(GFR)from serum creatinine in Chinese patients with chronic kidney disease(CKD),and refit the MDRD equation parameter estimates with Chinese patients data. Methods A total of 760 hospitalized CKD patients were investigated from 2002 to 2005.Patients'sex,age,height and body weight were recorded;plasma creatinine and ~ 99m Tc-GFR(standardized by body surface area)were measured in all patients;GFR was estimated with simplified equation of MDRD and Cockcroft-Gault;the accurancy of estimated GFR with MDRD and CG were compared with~ 99m Tc-GFR in different stage of CKD.Totally 670 patients were randomly selected as the training sample, the remaining 90 patients constituted the validation sample. We used stepwise multiple regression to refit the equation of MDRD and the regression models were developed by using the training sample. The refitting equation was then tested and compared with the other equation in the validation sample. Results (1)Both the MDRD and CG-GFR prediction equations seemd to systematically underestimate GFR,but the accurancy of MDRD equation was better than CG-GFR in stage Ⅰ~Ⅲ of CKD.(2)The bias of estimated GFR(MDRD or CG-GFR equation)with ~ 99m Tc-GFR was lower in stage Ⅰ~Ⅲ than in stage Ⅳ~Ⅴ of CKD.(3)Refitting of the MDRD equation parameter estimates to Chinese patients data set resulted in the following prediction equations:GFR=234.96?(Scr)~ -0.926 ?(Age)~ -0.280 ?0.828(Female).(4)Comparing with~ 99m Tc-GFR,the bias of estimated GFR calculated by the refitting MDRD equation was lower than that of estimated with MDRD or CG especially in the stage Ⅳ~Ⅴ of CKD in Chinese patients. Conclusion the simplified equation of MDRD can estimate GRF accurately, but the equation must be refitted in Chinese patients.

Objective To evaluate the determination of glomerular filtration rate (GFR) by non-ionic contrast medium iohexol plasma clearance. Methods GFR of 93 patients, 45 male and 48 female, age ranged from 12 to 80, with different renal functions was determined. The patient underwent an overnight fast and 5 ml of iohexol was injected into peripheral vein in the morning. 2 ml plasma sample was collected 4 hours after injection, and plasma concentration of iohexol was measured by high performance liquid chromatography(HPLC) . The iohexol plasma clearance was calculated by two one-compartment model; Jacobsson's and Bubeck's model. 99mTc clearance was used as the reference method. Results Iohexol-GFR obtained by Jacobsson's model vs 99mTc-GFR yielded a correlation of r = 0. 85, standard error of the estimate (SE) = 16. 06 ml/min. There was also a high correlation between iohexol-GFR obtained by Bubeck's model and 99mTc-GFR, r = 0. 82, SE = 17. 75 ml/min. In the patients in early stage of renal failure, iohexol-GFR was the most sensitive indication as compared to Scr and Ccr. Conclusion Iohexol plasma clearance calculated by Jacobsson's and Bubeck's models is a simple and accurate method of determining GFR.

The evaluation of glomerular filtration rate is important in the diagnosis, treatment, and prognosis for patients. Although there are different equations widely used and convenient, the accuracy of them on the specific populations, including diabetic patients and the elderlys, still needs further discussion. We recommend using the Chronic Kidney Disease Epidemiology Collaboration ( CKD-EPI) from creatinine and cystatin C in the elderlys. For diabetic patients, however, this equation should be recommended with cautions.

Objective To investigate the value of MR imaging methods for the quantification of fat content in a customized lipid phantom at 3.0 T.Methods Eleven homogeneous fat-water phantoms (50 ml)with fat volume percentages from 0 to 100% were constructed with reference to Bernard's methods.Fat tractions of the lipid phantom were acquired using water selective saturation (WS),fat selective saturation (FS),in-and out-of-phase imaging (IOP),iterative decomposition of water and fat with echo asymmetry and least-squares estimation (IDEAL) gradient echo imaging and IDEAL Quant imaging methods on a 3.0 T MRI system. For statistical comparisons,paired-sample t test,Pearson correlation test, and Bland-Ahman maps were applied. Results Evaluated fat fractions acquired by WS,IDEAL Gradient echo imaging and IDEAL Quant were (49.6±28.8)%,(46.0±28.4)%,(51.0±32.0)%,the result has no significant difference with the true fat contents(t values were-0.186,-2.218,2.713;P values were 0.856,0.051,0.055).Evaluated fat fractions acquired by FS,corrected algorithm and IOP were (64.2±26.7)%,(58.9±31.9)% and (45.3±32.3)%,these three kinds of methods have significant difference with the true fat contents (t values were 5.168,4.273,-6.441;P<0.01).All the chemical shift imaging methods correlated with the true phantom model fat fractions,r values were 0.977(FS),0.978 (corrected algorithm),0.982 (WS),0.99 8(IOP),0.993 (IDEAL Gradient echo imaging),0.999 (IDEAL Quant) (all P<0.01).Each method's 95% confidence interval of the mean difference acquired by Bland-Altman map was WS (-14.7% to 13.8%),FS (-3.6% to 32.0%),corrected algorithm (-4.6% to 22.5%),IOP(-9.4% to 0.0%),IDEAL gradient echo imaging (-15.9% to 7.8%),IDEAL Quant(-2.0% to 4.0%).IDEAL Quant had the best correlation and confidence with the true fat fraction. Conclusions Chemical shift imaging methods (IOP,IDEAL Gradient echo imaging,IDEAL Quant) can acquire more accurate fat quantification results than chemical saturation imaging methods (FS,Corrected algorithm,WS) in a customized lipid phantom at 3.0 T.IDEAL Quant can acquire the best fat quantification result compared with the other imaging methods.

AIM: To study the technological parameters of the purification process of total flavonoids of Crataegus leaves with HPD-600 macroreticular resin. METHODS: Using orthogonal experiments, the adsorptive characteristics and elutive parameters of the process were studied by taking the elutive effects and purification ratio of total flavonoids of Crataegus leaves. RESULTS: Two significant factors which were the concentration of crude flavonoids solvent and elutant component were observed. CONCLUSION: The microwave-assisted extraction and purification process with macroreticular resin is employed, the content of total flavonoids is 80.75%, the rutin content is (1.38)%, The macroreticular resin can be used in the purification of flavonoids of Crataegus leaves.
10. Chemical constituents of traditional Chinese materia medica for alleviating renal damage of paracetamol:research advances
Journal of International Pharmaceutical Research 2020;47(1):22-26
Paracetamol (acetaminophen, APAP)is one of the commonly used analgesic, antipyretic and anti-inflammatory drugs, but it can cause acute kidney injury(AKI)or even acute renal failure due to blind, large, combined or long-term use. The mechanism reseach on the APAP-induced nephrotoxicity is mainly focused on the formation of metabolic toxic substances, oxidative stress, inflammatory response, mitochondrial dysfunction, endoplasmic reticulum stress(ERS), and changes in glomerular hemody namics. This paper summarizes the toxicity mechanism of APAP-induced AKI(APAP-AKI), and the research progress in the protec tive effect of active ingredients of traditional Chinese materia medicas(TCMM)on APAP-AKI, including polyphenols, flavonoids, quinones, terpenoids, amino acids and other type substances, with an aim to clarify the key mechanisms and signaling pathways of APAP-AKI and to provide reference for the development of drugs for the prevention and treatment of APAP-AKI.
