OBJECTIVE:To establish a RP-HPLC method for simultaneous determination of 4 flavonoids in Herba Euphorbiae Humifusae.METHODS:The samples were separated on Hypersil BDS C18 column(150 mm?4.6 mm,5 ?m)with CH3OH-0.3%H3PO4 as mobile phase(gradient elution)at a flow rate of 1.0 mL?min-1.The column temperature was maintained at 30 ℃ and the detective wavelength was set at 360 nm.RESULTS:The linear ranges of 4 flavonoids including rutin,quercetin,luteolin and kaempferol were 0.127 2～2.034 4 ?g(r=0.999 9),0.089 1～1.425 6 ?g(r=0.999 8),0.076 6～1.226 4 ?g(r=0.999 7),0.113 0～1.808 0 ?g(r=0.999 8)respectively,and their average recoveries were 97.6%,99.4%,98.1% and 101.9%,respectively with RSD at 0.94%,0.73%,0.99% and 1.34%,respectively(all n=9).CONCLUSION:This method was proved to be simple,accurate,reproducible and applicable for the quality control of Herba Euphorbiae Humifusae.

OBJECTIVE:To investigate the general patterns and characteristics of the adverse drug reactions(ADR) induced by interferon and to provide guidance for its rational use in the clinic.METHODS:A total of 64 ADR cases induced by interferon reported in the domestic medical journals during the period 2000～2006 were collected and analyzed statistically. RESULTS:The ADR induced by interferon was more often seen in patients aged 30～39 years old(35.94%),with male showing higher proportion than female(68.75% vs. 31.25%).ADR more often occurred after multiple dosing and were characteriz-ed predominantly by allergic reactions of immune system in which multiple organs or systems may be involved.CONCLUSION:Interferon should be used rationally based on indications of medication and be closely monitored to prevent and reduce the occurrence of ADR.

Objective To investigate the changes of cardiac structure and function in type 2 diabetes mellitus (T2DM), analyze the related risk factors of heart failure, and improve the awareness of the effect of diabetes on the heart. Methods Randomly selected 170 cases of T2DM patients As the object of study, 170 cases of non diabetic patients as con-trol group, according to whether complicated with heart failure were divided into heart failure patients and non heart failure patients.Comparison of each group left ventricular posterior wall thickness, left ventricular end diastolic diameter, left ven-tricular end systolic diameter, left ventricular ejection fraction, E/A ratio Differences, analysis of the related risk factors of heart failure by logistic regressive.Results T2DM group left ventricular posterior wall thickness, left ventricular end diastolic diameter and left ventricular end systolic diameter increased , Left ventricular ejection fraction decreased, E/A ratio de-creased, were statistically significant difference compared with the control group (p <0.05).These differences are mainly come from non heart failure patients.Logistic regression showed that factors in patients with diabetes duration, HbA1c, com-pliance and complications of hypertension, coronary heart disease, is a risk factor for heart failure complicated with T2DM (all p<0.01).These may be the risk factor for diabetic patients with heart failure.Conclusion Diabetes can cause cardiac remodeling, systolic and diastolic dysfunction maybe some T2DM patients.They did not show symptoms of heart failure in the clinical, but their Cardiac structure and function are abnormal.We should pay attention to the cardiac function examination and early evaluation, prevention of risk factors, to avoid the occurrence of heart failure.

OBJECTIVE: To observe the correlation between traditional Chinese medicine (TCM) syndromes ("deficiency of qi and yin" and "deficiency of liver yin and kidney yin") and A267G in 5'-untranslated region within exonal of megsin gene, and to search the substantial genetic basis for micro-differentiation of TCM syndromes in primary immunoglobulin A nephropathy (IgAN). METHODS: A total of 120 IgAN cases meeting the diagnostic criteria were enrolled. The sequence of single nucleotide polymorphism (SNP) of A267G in 5'-untranslated region within exonal of megsin gene was tested. The correlation between SNP and TCM syndromes was observed. RESULTS: There were 83 cases carrying GG genotype, 34 cases carrying GA genotype and 3 cases carrying AA genotype in 120 cases of primary IgAN. There was a high proportion of "deficiency of liver yin and kidney yin" in IgAN cases with AA and GA genotypes, and a high proportion of "deficiency of qi and yin" in IgAN cases with GG genotype (P<0.01). Odds ratio in TCM syndrome distribution between GG genotype and GA plus AA genotype was 9.800, and 95% confidence interval was 3.969-24.199. The discrepancy also resided in IgAN patients with different genders and ages. CONCLUSION: A267G in 5'-untranslated region within exonal of the megsin gene may be one of the substantial genetic basis for differentiating "deficiency of liver yin and kidney yin" syndrome and "deficiency of qi and yin" syndrome in primary IgAN.

Objective To investigate the effect of mild hypothermia on the expression of hypoxia-inducible factor-1α (HIF-1α) and glucose transporter-1 (GLUT-1) in a rat model of chronic cerebral ischemia-reperfusion.Methods Thirty-six female SD rats weighing 170-210 g were randomly divided into 3 groups (n = 12 each):sham operation group (group S), normal body temperature group (group NT ) and mild hypothermia group (group MH). Arterio-venous fistula was established by end-to-end anastomosis between the right common carotid artery and right external jugular vein for 6 weeks followed by reperfusion. In group MH, mild hypothermia was induced at the initiation of reperfusion and the rectal temperature was reduced to 31.5-32.5 ℃. In group S and NT, the rectal temperature was maintained at 37-38 ℃. Six rats in each group were sacrificed at 3 and 48 h of reperfusion. The brains were immediately removed for determination of the expression of HIF-1α, GLUT-1, HIF-1α mRNA and GLUT-1 mRNA and microscopic examination. Results Compared with group S, the expression of HIF-1α and HIF-1α mRNA at 3 and 48 h of reperfusion and GLUT-1 mRNA at 3 h of reperfusion was up-regulated, while the expression of GLUT-1 and GLUT-1 mRNA at 48 h of reperfusion was down-regulated in group NT (P ＜ 0.05).Compared with group NT, the expression of HIF-1α and HIF-1α mRNA at 48 h of reperfusion and HIF-1α mRNA and GLUT-1 mRNA at 3 h of reperfusion was down-regulated, while the expression of GLUT-1 and GLUT-1 mRNA at 48 h of reperfusion was up-regulated in group MH (P ＜ 0.05).Microscopic examination showed that the injury to the ultrastructure of blood-brain barrier was significantly attenuated in group MH compared with group NT. Conclusion Mild hypothermia can attenuate chronic ischemia-reperfusion injury by down-regulating the expression of HIF-1α and up-regulating the expression of GLUT-1.

Migraine is one of the common and frequently encountered diseases. The study proves that 5-hydroxytryptamine (5-HT) receptor, plays an important role in the occurrence of migraine. Rat striatum was used for preparation of the cell membrane stationary phase (CMSP) in our experiments. The cell membrane chromatography (CMC)-offline-HPLC system was applied to specifically recognize the components from the drug pair of Chuanxiong Rhizoma and Angelicae Dahuricae Radix, which interact with the receptors on CMSP. The dissociation equilibrium constant (KD) was measured in a rat striatum/CMC system, performed by continuously pumping sumatriptan, a 5-HT1D agonist, ranging from 2.42 x 10(-8) to 4.84 x 10(-7) mol · L(-1) through a CMC column, and the capacity factors (k') were recorded. The KD value obtained from the model was (4.59 ± 0.33) x 10(-6) mol · L(-1) for imperatorin, and the rat model of migraine induced by nitroglycerin was applied to validate the pharmacological effects of the drug pair. The results indicated that the CMC method could be a quick and efficient way for characterizing the drug-receptor interactions in vitro.

Objective To investigate the prognostic factors of low-and undifferentiated gastric cancer with negative metastasis of lymph nodes.Methods The clinicopathological data of 270 patients with low-and undifferentiated gastric cancer and negative metastasis of lymph nodes who were admitted to the Tianjin Cancer Hospital from January 2002 to December 2007 were retrospectively analyzed.All the patients received radical gastrectomy.One hundred and sixty-one patients had the number of lymph nodes dissected under 15,53 patients had 15-20 lymph nodes dissected,33 had 21-30 lymph nodes dissected and 23 patients with the number of lymph nodes dissected more than 30.Patients were followed up via out-patient examination,phone call and mail till October 2013.The prognosis analysis was done by COX step-wise regression.The survival curve was drawn by KaplanMeier method,and the survival was analyzed using the Log-rank test.Results All the patients were followed up for a median time of 63 months (range,2-103 months).The median survival time was 63 months (range,2-103 months),and the 1-,3-,5-year overall survival rates were 93.0％,69.5％ and 58.5％,respectively.The median survival time of the 161 patients with the number of lymph nodes dissected under 15 was 58 months (range,2-103 months),and the 1-,3-,5-year survival rates were 91.4％,59.3％ and 48.8％,respectively.The median survival time of the 53 patients with the number of lymph nodes dissected between 15 and 20 was 68 months (range,4-95 months),and the 1-,3-,5-year survival rates were 94.3％,84.9％ and 71.7％,respectively.The median survival time of the 33 patients with the number of lymph nodes dissected between 21 and 30 was 68 months (range,34-94 months),and the 1-,3-,5-year survival rates were 100.0％,97.0％ and 87.9％,respectively.The median survival time of the 23 patients with the number of lymph nodes dissected more than 30 was 60 months (range,2-84 months),and the 1-,3-,5-year survival rates were 87.5％,62.5％ and 54.2％,respectively.There was significant difference in the prognosis between the 4 groups (x2 =25.077,P ＜ 0.05).There was significant difference in the prognosis between patients who had 21-30 lymph nodes resected and those had 15-20 lymph nodes resected (x2=3.924,P ＜0.05).Significant difference was also observed in the prognosis between patients who had 15-20 lymph nodes resected and those had more than 30 lymph nodes resected (x2=4.454,P ＜ 0.05),while no significant difference was observed in the prognosis between patients who had lymph nodes resected more than 30 and those had lymph nodes resected less than 15 (x2=0.450,P ＞0.05).The results of univariate analysis showed that gender,age,location and diameter of the tumor,serosal invasion,Borrmann classification,range of gastric resection and the number of lymph nodes dissected were factors influencing the prognosis of patients with low-and undifferentiated gastric cancer and with negative metastasis of lymph nodes (x2 =4.143,12.607,23.665,11.549,26.350,8.059,5.222,25.077,P ＜0.05).The results of multivariate analysis showed that the tumor diameter longer than 5 cm and serosal invasion were the independent factors influencing the prognosis of patients with low-and undifferentiated gastric cancer and with negative metastasis of lymph nodes (HR =1.842,3.084,95％ confidence interval:1.286-2.638,1.956-4.861,P ＜ 0.05).Lymph nodes dissected for 21-30 was the protective factor of prognosis (HR =1.533,95％ confidence interval:1.229-2.248,P ＜ 0.05).Conclusions Lymph nodes dissected for 21-30 during radical gastrectomy may improve the long-term survival of patients.The tumor diameter longer than 5 cm,serosal invasion and the number of lymph nodes dissected (less than 15,15-20 and more than 30) were the independent factors influencing the prognosis of patients with low-and undifferentiated gastric cancer and with negative metastasis of lymph nodes.

BACKGROUND: Connective tissue growth factor (CTGF) has very strong regulative effect on the differentiation and proliferation of renal cells as well as the synthesis and degradation of extracellular matrix. Its abnormal expression plays a key role in the process of fibrous degeneration of various renal diseases including diabetic nephropathy.OBJECTIVE: To observe the protective effect ofjiedu tongluo baoshen gelatin capsule on renal disease in rats with diabetes mellitus and its effect on the expression of renal CTGF mRNA.DESIGN: A randomized controlled experiment taking animals as subjects.SETTING: Institute of Traditional Chinese Medicine, Changchun College of Traditional Chinese Medicine; Department of Pathology, Institute of Regenerated Medical Sciences, Jilin University MATERIALS: Totally 46 Wistar rats, of clean degree, male and female in half, weighing 150 to 200 g, were used in the experiment. Jiedu tongluo baoshen gelatin capsule was provided by Preparation Room, Traditional Chinese Medicine Hospital of Jilin Province (Batch No. 2002101, composed of Renshen, Huangqi, Shengdi, Gouqizi, Zhimuv Digupiv Huanglian,Zhenhua, Jinyinhua, Dahuang, Danshen, Yimucao and so on, 0.25 g/pill)and benazepril (Provided by Beijing Nuohua Pharmaceutical Company,Batch No. 01046) were used in the experiment.METHODS: This experiment was carried out in the Department of Pathology, Institute of Regenerated Medical Sciences, Jilin University from May 2002 to January 2003. ① Totally 46 Wistar rats used in the experiment were randomly divided into 4 groups: normal control group (n=10), model group (n=12), benazepril-treated group (n=12) and traditional Chinese medicine-treated group (n=12).Rats of normal control group were intraperitoneally injected of natrium citricum buffer solution of same volume; Rats of other groups were intraperitoneally injected of 50 mg/kg streptozotocin which was dissolved by 0.1 mol/L citric acid buffer solution before use(pH 4.2). Seventy-two hours after modeling, venous blood was collected from the tails to measure the levels of fasting blood glucose and urine glucose.Blood glucose ≥ 16.7 mmol/L and urine glucose over 卅 suggested diabetic models were successful. Rats of benazepril-treated group were performed intragastric administration of benazepril suspension of 15 mg/(kg·d);Rats of traditional Chinese medicine-treated grou p were performed intragastric administration ofjiedu tongluo baoshen gelatin capsule suspension of 5 g/(kg ·d); Rats of normal control group and model group were performed intragastric administration of the same volume of running water on each morning, once a day, 12 weeks in total. ② Levels of blood glucose,urea nitrogen, serum creatinine and urinary albumin excretion rate were measured 12 weeks after administration; Level of angiotensin Ⅱ of renal tissue was measured with radioimmunoassay; Levels of renal fibronectin and type Ⅳ collagen protein expression were measured with immunohistochemical method; Connective tissue growth factor gene expression of renal cortex was measured with reverse transcription-polymerase chain reaction and renal histological observation was also performed. ③ Analysis of variance was used in the intergroup comparison.MAIN OUTCOME MEASURES: Biochemical index of blood and urine,level of angiotensin Ⅱ of renal tissue, pathological morphology, expression of fibronectin and type Ⅳ rabbit anti-rat collagen protein and expression of connective tissue growth factor mRNARESULTS: Five, four and three rats died in the process of experiment respectively in model group, benazepril-treated group and traditional Chinese medicine-treated group, finally, 10,7,8 and 9 rats entered the stage of result analysis respectively in normal control group, model group, benazepril-treated group and traditional Chinese medicine-treated group. ① The ratio between body mass and renal mass was significantly higher in the model group than in the normal control group (P ＜ 0.05),but was significantly lower in the benazepril-treated group and traditional Chinese medicine-treated group than in the model group (P ＜ 0.01). ② Fasting blood glucose was significantly higher in the model group than in the normal control group (P ＜ 0.01),but was significantly lower in the traditional Chinese medicine-treated group than in the model group and benazepril-treated group (P ＜ 0.01). ③ Levels of urea nitrogen, serum creatinine, creatinine clearance, urinary albumin excretion rate and angiotensin Ⅱ of renal tissue were significantly higher in the model group than in the normal control group (P ＜ 0.01), but significantly lower in the benazepril-treated group and traditional Chinese medicine-treated group than in the model group (P ＜ 0.05-0.01 ). ④Pathomorphological change of renal tissue was very severe in the model group,but very mild in the benazepril-treated group and traditional Chinese medicine-treated group. ⑤Fibronectin and type Ⅳ rabbit anti-rat collagen protein expression were significantly higher in the model group than in the normal control group (P ＜ 0.01), those were significantly lower in the benazepril-treated group and traditional Chinese medicine-treated group than in the model group, but higher than normal control group (P ＜ 0.01). ⑥Expression of CTGF mRNA of renal cortex of model group was 2.65 times that of normal control group. Expression of CTGF mRNA of benazepriltreated group and traditional Chinese ine-treated group was decreased by 23.68% and 26.32%, respectively (P ＜ 0.05 or 0.01).CONCLUSION: Jiedu tongluo baoshen gelatin capsule has protective effect on diabetic nephropathy, which might be implemented by reducing the level of angiotensin Ⅱ of renal tissue and down-regulating the expression of CTGF mRNA of renal cortex of rats with diabetes mellitus, so as to reduce the deposition of extracellular matrix.

Objective To investigate the survival rate of patients with chronic severe hepatitis B (CSH) after lamivudine treatment. Method A matched retrospective cohort study using data on patients derived from a prospectively collected chronic severe hepatitis B database was conducted. The database included patients who received or did not receive lamivudine treatment from October, 1999 to December, 2003. The match was conducted according with 3 variables: sex, age and duration of disease. The match ratio was 1∶1. Cases were patients who received lamivudine treatment (n=103).Controls were patients who did not receive lamivudine treatment (n=103). All the patients were followed up and their median survival time and survival rate were compared and evaluated. Results The median survival time in lamivudine treatment group and control group were 85 and 35 d respectively.
