1.Expression of iNOS mRNA prevented cardiac allograft vasculopathy after heart transplantation

Objective To study the prevention of cardiac allograft vasculopathy (CAV) post-transplant through the expression of iNOS mRNA. Methods Rat model of heterotopic heart transplantation (Abdomen) was developed and recipients were injected with Cyclosporin A at abdomen in the meantime of receiving L-arginine (iNOS mRNA group) and inhibitor of iNOS (L-NAME group) according to grouping. Plasma content of NO_3-, the expression of iNOS mRNA in coronary ~artery of transplanted heart and the radio of tunica intima/tunica media thickness (I/M) were assayed after ~operation . Results The plasma content of NO_3-and the expression of iNOS mRNA in coronary ~artery of transplanted heart in iNOS mRNA group were obviously higher than those in control group and L-NAME group during 2 to 4 weeks after operation. The ratio of I/M in iNOS mRNA group was ~lower than that in control group and L-NAME group 4 weeks after operation. Conclusion The ~expression of iNOS can promote the synthesis of NO and prevent CAV development.
2.Survival time of heart allografts in rats can be prolonged by administration of donor apoptotic bone marrow cells via portal vein before transplantation

Objective To study the effects of administration of donor apoptotic bone marrow cells via portal vein on the survival time of heart transplantation in rats.Methods Wistar rats were selected as donors and SD rats as recipients, and divided into 4 groups at random. In groups A, B and C, the animals were injected via portal vein with RPMI 1640 0.5 ml 6 as control group, 5?107 donor bone marrow cells and 5?107 apoptotic donor bone marrow cells 6 days before transplantation, respectively , and not subjected to cyclosporin A (CsA). In group D, the animals were injected intra- peritoneally with CsA 5 mg/kg from 3 days before transplantation till 10 days post operation. Bone marrow cells apoptosis was induced by 60 Co? radiation. Heart transplantation models were established in each group. The survival time of donor heart and histological changes were observed. Serum concentrations of IL-10 and TGF-?_1 of recipients and mixed lymphocyte reaction (MLR) were measured. 3.Role of the rotating insulin weekly abdominal injection card in safe injection of insulin in patients with type 2 diabetes mellitus

Objective To investigate how to improve safety of insulin injection in type 2 diabetes patients by“insulin pen”through the rotating insulin weekly abdominal injection card. Method All the inpatients with type 2 diabetes in the Department of 3rd Internal Medicine from January to September, 2013 were enrolled in this study. Insulin Injection Card group was composed of 66 type 2 diabetes patients who were admitted from June to September, while control group was composed of 62 type 2 diabetes patients admitted from January to April. The difference between Insulin Injection Card group and control group was that the former group used the rotating insulin abdominal injection card weekly which was designed by ourselves, while the control group patients did not use the card. Every participant of both groups was evaluated at 2 weeks of treatment by percentage of patients having absence of Safety standard of insulin injection, and at the end of 3 months of treatment by percentage of patients attaining the therapeutic targets and incidence of adverse events. Result Compared with the control group, patients in Insulin Injection Card group had lower proportion of absence of Safety standard for insulin injection. The scores of rotation injection site in the two groups was 0.59±0.14 and 2.67±0.21(P<0.01). The rate of FPG target achievement of patients in Insulin Injection Card group (84.9%) was higher than that of the control group (61.3%)(P<0.01), the rate of HbA1c target achievement of patients in Insulin Injection Card group (78.8%) was higher than that of the control group (56.5%)(P<0.01). Patients in Insulin Injection Card group had lower incidence of local adverse events (1.5%) compared with the control group (12.9%)(P<0.05). Conclusion Use of the rotating insulin weekly abdominal injection card that was designed by the diabetes health management Quality Control Group in our department showed good effect in achieving therapeutic targets (including HBA1C) and decreasing safety standard deficiency of insulin injection and adverse events.
4.Experience and procedure of the model of rat heterotopic heart transplantation

ObjectiveTo summarize the method and experience of the model of rat heterotopic heart transplantation and to improve the rate of success of the experiment.Methods120 heterotopic heart transplantations were performed by Ono's way,including 60 prepared transplantations and 60 formal transplantations.ResultsRate of success of prepared transplantations was 45%,and formal transplantation was 85%.Mean time of formal operation was (74.3±17.1)min,removal time of the donor heart was (7.9±4.2)min,ischemia time was (29.6±10.3) min and the survival time of allograft was (11.1±5.7)days.5 of the recipients had a long-term survival,and the maximum weight was 740 g.ConclusionsGood surgical technique and rigorous perioperative management is very important in setting up the rat model of heterotopic heart transplantation.
6.Value of aortic root CTA assessment in aortic valve surgery

Objective To evaluate the data of preoperative aortic root CT angiography (CTA), compare it with two-dimensional transthoracic echocardiography and investigate the correlation of the two measurements with the actual intraoperative measurement data. Methods Clinical data of 53 patients with aortic valve diseases who underwent aortic valve repair in our hospital from January 2018 to August 2020 were retrospectively analyzed, including 38 males and 15 females with an average age of 42.9±18.3 years ranging from 10 to 77 years. Preoperative two-dimensional transthoracic echocardiography (TTE) and aortic root CTA measurements were collected, including aortic valve annulus (AVA), aortic sinus (Sinus) and sino-tubular junction (STJ). In comparison with the intraoperative measurements during the aortic valve repair surgery, the consistency analysis was performed. Results Both the preoperative echocardiography AVA measurements and the CT AVA measurements were positively correlated with the intraoperative AVA measurements (P<0.001). Compared with the echocardiography AVA data [correlation coefficient (ρ)=0.74, mean squared error (MSE)=12.78], the CT AVA data were more accurate and consistent with the intraoperative AVA measurements (ρ=0.95, MSE=2.72). CT AVA data had a higher correlation coefficient with the intraoperative measurements, compared to that of the echocardiography AVA data (P<0.001). Conclusion In comparison with two-dimensional transthoracic echocardiography, preoperative morphological evaluation of aortic root CTA is more consistent with the actual intraoperative measurements during aortic valve repair surgery.
