2.Prevailing Practice Versus Clinical Guideline: The In-Patient Assessment And Management Of Childhood Bronchopneumonia In A Malaysian District Hospital

Introduction: Pneumonia is the most common
diagnosis made in hospitalised children. The Malaysian
Clinical Practice Guidelines on pneumonia and
respiratory tract infections provides a comprehensive
guidance in the local context. We evaluated the
documented assessment and management of children
diagnosed with pneumonia admitted to the children’s
ward, Hospital Batu Pahat against this guideline.
Methods: We performed a retrospective analysis of
hospital case notes for children admitted from January
to May 2004.
Results: Ninety six case notes were analysed. Most
patients (84%) had at least four positive clinical features
leading to the diagnosis of pneumonia. 92% met the
guideline criteria for admission. Sp02 was performed for
58% on admission, and 58% with reading below 95%
received supplemental oxygen. Throughout hospital
stay, each patient had an average of four investigations
(range: 1 – 12). Among 23 patients who had
further investigations, justifications were only recorded
in seven patients (30.4%), and changes in
management resulted in 23%. The most common
antibiotic prescribed was intravenous Penicillin (97 %).
In 17 patients who met the guideline classification for
severe pneumonia, none received the recommended
antibiotic combination. The median time to fever
resolution was 22 hours (range 2 – 268), and median
hospital stay was 3 days (range 1 – 12).
Conclusions: Although the quality of clinical
assessment and antibiotic choices were acceptable,
there was a failure to critically evaluate patients
according to disease severity and initiate corresponding
investigations and managements. Future efforts need to
be directed at promoting further guideline adherence
and the exercise of critical judgment in patient
evaluation.
3.Efficacy of measuring visual performance of LASIK patients under photopic and mesopic conditions.

INTRODUCTIONThe aim of the study was to compare the visual performance of LASIK eyes measured using high-contrast logMAR letter charts under bright (photopic) and dim (mesopic) conditions.
MATERIALS AND METHODSA total of 46 subjects (35 +/- 8 years of age) undergoing LASIK procedures were recruited for the study. The best spectacle-corrected visual acuity (BSCVA) of each subject was measured using the high-contrast ETDRS logMAR chart under photopic and mesopic conditions at 3 visits: preoperative (Pre), 1 month postoperative (Post1) and 3 months postoperative (Post3). The differences in logMAR scores for the right eyes only were analysed for the Pre-Post1 (n = 46), Pre-Post3 (n = 18) and Post1-Post3 (n = 16) comparisons.
RESULTSThe logMAR scores of subjects were worse at the 1-month postoperative visit than preoperatively, and improvement in visual performance was seen at the 3-month postoperative visit. These changes in visual performance became more evident under mesopic conditions. The means and standard errors of the differences in logMAR scores for the Pre-Post3 (0.097 +/- 0.020) were slightly larger than those of the Pre-Post1 (-0.067 +/- 0.019) and Post1-Post3 (0.031 +/- 0.012) comparisons. Under mesopic conditions, the visual performance of the subjects was statistically significant for the 3 comparisons, but not under photopic conditions.
CONCLUSIONHigh-contrast logMAR chart performed under mesopic conditions has the potential to replace visual acuity measured under photopic conditions in providing more reliable representation of postoperative visual outcomes of LASIK eyes. Eye doctors should consider performing this vision test routinely to gauge the success of LASIK surgery.

5.A web-based educational intervention module to improve knowledge and attitudes towards thalassaemia prevention in Malaysian young adults

Background: Thalassaemia is a public health burden in
Malaysia and its prevention faces many challenges. In this
study, we aimed to assess the effectiveness of a web-based
educational module in improving knowledge and attitudes
about thalassaemia prevention amongst Malaysian young
adults.
Methods: We designed an interactive web-based
educational module in the Malay language wherein videos
were combined with text and pictorial visual cues.
Malaysians aged 18-40 years old who underwent the module
had their knowledge and attitudes assessed at baseline,
post-intervention and at 6-month follow-up using a selfadministered validated questionnaire.
Results: Sixty-five participants: 47 Malays (72.3%), 15
Chinese (23.1%), three Indians (4.6%) underwent the module.
Questionnaires were completed at baseline (n=65), postintervention (n=65) and at 6-month follow-up (n=60). Out of a
total knowledge score of 21, significant changes were
recorded across three time-points- median scores were 12 at
pre-intervention, 19 at post-intervention and 16 at 6-month
follow-up (p<0.001). Post-hoc testing comparing preintervention and 6-month follow-up scores showed
significant retention of knowledge (p<0.001). Compared to
baseline, attitudes at 6-month follow-up showed an
increased acceptance for “marriage avoidance between
carriers” (pre-intervention 20%, 6-month follow-up 48.3%,
p<0.001) and “prenatal diagnosis” (pre-intervention 73.8%,
6-month follow-up 86.2%, p=0.008). Acceptance for selective
termination however, remained low without significant
change (pre-intervention 6.2%, 6-month follow-up 16.7%,
p=0.109).
Conclusion: A web-based educational module appears
effective in improving knowledge and attitudes towards
thalassaemia prevention and its incorporation in
thalassaemia prevention programs is potentially useful in
Malaysia and countries with a high internet penetration rate.
6.Open-wedge high tibial osteotomy and unicomartmental knee arthroplasty in treating medial compartment osteoarthritis of the knee: a Meta analysis.

OBJECTIVE:
To systematic evaluate the outcome of open-wedge high tibial osteotomy(OWHTO) and unicomartmental knee arthroplasty (UKA) in treating medial compartment osteoarthritis of the knee.
METHODS:
According to the retrieval strategy made by the Cochrane collaboration, a computer-base research of Medline, Pubmed, EMbase, Cochrane Library, CBM, CNKI, and Wanfang databases was performed and search deadline was March 2018. Related Chinese and English orthopedic journals and conference papers were manually searched. Controlled studies of OWHTO and UKA in the treatment of medial knee osteoarthritis were included. The quality of included researches was evaluated, and the data of postoperative knee function, complications, total knee arthroplasty(TKA) revision rates, and postoperative pain were extracted. Meta analysis was performed using the RevMan 5.0 software.
RESULTS:
A total of 8 articles that met the criteria were included containing a total of 675 patients. Meta-analysis showed that there was no significant difference in postoperative HSS score, knee score, functional score, and Lysholm score between the OWHTO and UKA groups(=0.32, =0.87, =0.22, =0.53). The range of joint motion in the OWHTO group was better than that in the UKA group, and the difference was statistically significant(=0.009). There was no significant difference in postoperative complications and the rates of revision to TKA between the two groups(=0.81, =0.23). There was no difference in postoperative knee pain between the two groups.
CONCLUSIONS
In the treatment of medial compartmental osteoarthritis of the knee that meets the surgical indications, OWHTO had better postoperative joint mobility. The results were similar in postoperative knee score, postoperative complications, and postoperative TKA revision rates between OWHTO and UKA groups.

7.Danhong Injection Up-regulates miR-125b in Endothelial Exosomes and Attenuates Apoptosis in Post-Infarction Myocardium.

OBJECTIVE:
To investigate the involvement of endothelial cells (ECs)-derived exosomes in the anti-apoptotic effect of Danhong Injection (DHI) and the mechanism of DHI-induced exosomal protection against postinfarction myocardial apoptosis.
METHODS:
A mouse permanent myocardial infarction (MI) model was established, followed by a 14-day daily treatment with DHI, DHI plus GW4869 (an exosomal inhibitor), or saline. Phosphate-buffered saline (PBS)-induced ECs-derived exosomes were isolated, analyzed by miRNA microarray and validated by droplet digital polymerase chain reaction (ddPCR). The exosomes induced by DHI (DHI-exo), PBS (PBS-exo), or DHI+GW4869 (GW-exo) were isolated and injected into the peri-infarct zone following MI. The protective effects of DHI and DHI-exo on MI hearts were measured by echocardiography, Masson's trichrome staining, and TUNEL apoptosis assay. The Western blotting and quantitative reverse transcription PCR (qRT-PCR) were used to evaluate the expression levels of miR-125b/p53-mediated pathway components, including miR-125b, p53, Bak, Bax, and caspase-3 activities.
RESULTS:
DHI significantly improved cardiac function and reduced infarct size in MI mice (P<0.01), which was abolished by the GW4869 intervention. DHI promoted the exosomal secretion in ECs (P<0.01). According to the results of exosomal miRNA microarray assay, 30 differentially expressed miRNAs in the DHI-exo were identified (28 up-regulated miRNAs and 2 down-regulated miRNAs). Among them, DHI significantly elevated miR-125b level in DHI-exo and DHI-treated ECs, a recognized apoptotic inhibitor impeding p53 signaling (P<0.05). Remarkably, treatment with DHI and DHI-exo attenuated apoptosis, elevated miR-125b expression level, inhibited capsase-3 activity, and down-regulated the expression levels of proapoptotic effectors (p53, Bak, and Bax) in post-MI hearts, whereas these effects were blocked by GW4869 (P<0.05 or P<0.01).
CONCLUSION
DHI and DHI-induced exosomes inhibited apoptosis, promoted the miR-125b expression level, and regulated the p53 apoptotic pathway in post-infarction myocardium.
