AIM:To explore the effects of panax notoginsenosides(PNS)on bone histomorphometry in rats with hyperlipidemia-induced osteoporosis.METHODS:Thirty 3-month-old Sprague-Dawley rats were randomly divided into three groups with 10 per group as follows.Control group was intragastrically given distilled water at the dose of 5 mL?kg-1?d-1 for 20 weeks;Model group was intragastrically given lipid emulsion at the dose of 5 mL?kg-1?d-1 for 20 weeks;PNS preventing group was intragastrically given lipid emulsion at the dose of 5 mL?kg-1?d-1 in the morning and PNS at the dose of 5 mL?kg-1?d-1 at afternoon for 20 weeks.At the end of the experiment,the right longitudinal proximal tibial metaphyseal parameters of rats were performed undecalcifiedly and used for bone histomorphometry analysis.And the blood serum contents of FFA,TG,TC,HDL-C and LDL-C were measured by biochemical analysis.RESULTS:Compared with model group,the blood serum contents of FFA,TC and LDL-C were decreased and HDL-C was increased significantly(P
2.Study on the relationship of insulin resistance and early renal injury with genetic factor in the healthy offspring with a family history of essential hypertension

Objective To investigate the insulin resistance and microalbuminuria in the healthy offspring with a family history of essential hypertension,in order to explore the relationship between the genetic factor and early renal injury.Methods 69 healthy offsprings with the family history of essential hypertension were randomly recruited as the observation group,35 healthy offsprings without the family history of essential hypertension were considered as the control group.The consecutive 12-hour urine was collected to detect the urinary albumin,and blood biochemical examination was conducted.Results Plasma levels of triglyceride (TG),total cholesterol (TC),glucose,fasting insulin,and urinary albumin excretion rate were higher in observation group than in the control group [(1.07±0.65) mmol/L vs.(0.79±0.52) mmol/L,(4.83±0.78) mmol/L vs.(4.48±0.63) mmol/L,(5.27±0.73) mmol/L vs.(4.94±0.85) mmol/L,(10.63±3.21) mIU/L vs.(7.72±3.35) mIU/L,(45.6±29.5) μg/min vs.(18.6±11.7) μg/min,t=2.259、2.348、2.093、4.381、5.337,all P<0.05].Conclusions Insulin resistance and microalbuminuria occur before hypertension in healthy offsprings with the family history of essential hypertension,and the patients are prone to getting early renal damage,which indicates that heredity is a risk factor for chronic kidney disease.

Objective To investigate the influence of nasal continuous positive airway pressure (nCPAP) therapy on tumor necrosis factor-ot (TNF-α) and interleukin-6 (IL-6) in patients with chronic heart failure (CHF) and severe obstructive sleep apnea syndrome (OSAS).Methods Seventy-two cases with CHF and severe OSAS were divided into treatment group (36 cases) and control group (36 cases) by random digits table.Treatment group was treated by nCPAP and routine drug while control group only by routine drug treatment for 12 weeks.The changes of TNF-α and IL-6 before and after therapy were observed.Results The serum TNF-αt and IL-6 before treatment between two groups was no significant difference (P> 0.05).The serum TNF-αt and IL-6 after 12 weeks treatment in both two groups was significantly lower than that before treatment [treatment group:(29.94 ± 8.27) ng/L vs.(13.17 ± 3.61) ng/L,(92.08 ± 11.88) ng/L vs.(46.17 ± 20.01) ng/L;control group:(28.01 ± 7.21) ng/L vs.(22.13 ± 5.43) ng/L,(96.01 ± 12.19) ng/L vs.(68.66 ± 11.10) ng/L] (P < 0.05),the treatment group decreased more significantly (P < 0.05).Conclusion nCPAP therapy could effectively decrease the levels of TNF-α and IL-6 and reduce inflammation response in patients with CHF and severe OSAS.
4.Effect of nasal continuous positive airway pressure on patients with chronic heart failure due to hypertensive heart disease complicated with severe obstructive sleep apnea hypopnea syndrome

Objective To investigate the clinical efficiency of nasal continuous positive airway pressure (nCPAP) on patients with chronic heart failure(CHF) due to hypertensive heart disease complicated with severe obstructive sleep apnea hypopnea syndrome (OSAHS).Methods Fifty cases with CHF due to hypertensive heart disease complicated with severe OSAHS were divided into treatment group and control group with 25 cases each by the method of random digits table.nCPAP and routine drug treatment were administered in treatment group,while only routine drug treatment was given in control group.Cardiac functional grading,blood pressure,left ventricular end diastolic dimension (LVDd),left ventricular ejection fraction (LVEF),B-type natriuretic peptide (BNP),high-sensitivity C-reactive protein (hs-CRP) were compared before and after 3-month therapy.Results The effective rate in treatment group was significantly higher than that in control group [76.00%(19/25) vs.40.00%(10/25)],and there was significant difference (P <0.05).There was significantly decreased in blood pressure in two groups at the end of the therapy [treatment group:(127.52 ± 10.38)/(72.44 ±4.92) mmHg(1 mmHg =0.133 kPa) vs.(176.80 ± 12.90)/ (100.44 ±5.55) mmHg;control group:(150.12 ± 18.96)/ (81.64 ±9.42) mmHg vs.(176.00 ± 11.69)/ (96.08 ± 18.59) mmHg] (P < 0.01),but the blood pressure was significantly lower in treatment group than that in control group (P <0.05).LVDd,BNP and hs-CRP in treatment group was declined significantly [(49.12 ± 3.18) mm vs.(57.44 ± 3.62) mm,525.52 ng/L vs.785.25 ng/L,(7.76 ± 1.69) mg/L vs.(18.04 ± 2.58) mg/L](P< 0.01);LVEF was significantly increased [(46.12 ± 2.52)% vs.(38.68 ± 4.65)%](P< 0.01).There was no significant difference in LVDd and LVEF in control group before and after therapy (P > 0.05).BNP and hs-CRP also were declined significantly [668.66 ng/L vs.850.96 ng/L,(12.88 ± 2.02) mg/L vs.(19.40 ± 2.30) mg/L] (P < 0.05 or < 0.01),but the above two were significantly lower in treatment group than those in control group(P < 0.05).Conclusion nCPAP therapy can effectively improve the symptoms of heart failure,decrease blood pressure,improve cardiac function,reduce the inflammation response to patients with CHF due to hypertensive heart disease complicated with severe OSAHS.
5.Clinical and pathological features of limb-girdle muscular dystrophy type 2B

Objective To investigate the clinical and pathological features of limb-girdle muscular dystrophy type 2B(LGMD2B).Methods The clinical data and the pathological result of skeletal muscle in five patients with LGMD2B were analyzed retrospectively.Results Five patients presented muscle atrophy and weakness(four limbs in three patients and both lower limbs in two patients),and the chronic onset,and the progressive deterioration.The pathological examination showed that the muscle fibers degenerating,necrotic and regenerating in different extents were observed,and the infiltration of inflammatory cells were appeared on all of cases.Immunohistochemical stains showed that the expression of anti-dysferlin monoclonal antibody was negative,anti-Dystrophy,Sarcoglycan and dystroglycan monoclonal antibodies were positive normaly,anti-CD8+T cell monoclonal antibody was negative,and anti-MHC-1 monoclonal antibody was up-regulated in five patients.Conclusions The clinical characters of LGMD2B are chronic onset,progressive muscle atrophy and weakness in four limbs or both lower limbs.The pathological characters of LGMD2B are similar to the changes of polymyositis.
6.Two Medical Cases of SHI Da-zhuo Using Integrated Traditional Chinese and Western Medicine for Left Ventricular Remodeling and Left Atrial Expansion after Acute Myocardial Infarction

This article summarized two medical cases of Professor SHI Da-zhuo treating acute myocardial infarction by Yiqi Huoxue Tuodu Shengji TCM therapy on the basis of conventional Western medicine therapy. Comparison of echocardiography before and after treatment showed that Yiqi Huoxue Tuodu Shengji TCM combined with conventional Western medicine therapy could inhibit the expansion of atrium and ventricle, improve the cardiac ejection fraction and improve the cardiac function after acute myocardial infarction, which could provide references for integrated traditional Chinese and Western medicine therapy for acute myocardial infarction.
7.Clinical Application of Endometrial Scratching in Patients with Repeated Implantation Failure

Objective To explore the clinical application of endometrial scratching in patients with repeated implantation failure(RIF).Methods A total of 189 women with RIF who planned to undergo in vitro fertilization and embryo transfer(IVF‐ET)once again at the Reproductive Medicine Center of Tongji Hospital ,Huazhong University of Science and Technology ,be‐tween October 2013 and August 2014 ,were prospectively examined.These women were infertile due to tubal factors ,with their male partners having normal semen measurements.According to the case numbers ,the 189 patients were randomly divided into experimental group(n=96)and control group(n=93).In the experimental group ,endometrial scratching was conducted within 6 h of one menstrual cycle one month before oocyte retrieval ,while in control group ,no endometrial scratching was performed at that time.The outcomes of pregnancy were compared between the two groups after embryo transplantation.Results There was no statistical difference between the two groups in the patient age ,infertility duration ,body mass index(BMI) ,basal levels of follicle‐stimulating hormone(FSH) ,luteinizing hormone(LH) ,and progesterone(P) ,gonadotropin‐releasing hormone(Gn‐RH) injection days and doses ,the number of retrieved oocytes ,the number of transferred embryos ,the endometrial thickness and lev‐els of estradiol(E2 )and P on the day of HCG injection(P>0.05).The embryo implantation rate ,the clinical pregnancy rate and the ongoing pregnancy rate in the experimental group were much higher than those in the control group ,with statistically signif‐icant difference noted(P<0.01).Conclusion The endometrial scratching can improve the endometrial receptivity and enhance the clinical pregnancy rate of RIF patients.
8.The application of MSCT 3D reconstruction technology without contrast on pulmonary arteriovenous malformation

Objective To evaluate the application of MSCT 3D reconstruction technology without contrast on pulmonary arterio-venous malformation(PAVM).Methods The clinical and imaging data of 12 cases with PAVM confirmed by DSA were analyzed ret-rospectively.The plain scan with 16 slice spiral CT scanner performed pulmonary 3D reconstruction at post-processing work station by methods of MPR,MIP and VR.Results All cases could be diagnosed accurately,and images could clearly show the spatial struc-tures and its relations of the feeding arteries,draining vein and aneurysmal sac.Conclusion MSCT post-processing technique of PAVM without contrast is able to make a precise diagnosis,and to show the configuration of PAVM,which does not need contrast medium and acquirelow radiation dose.

10.Effects of Prinsepia utilis Royle oil on the synthesis of ceramide and expression of ceramidase

Objective To evaluate the effects of Prinsepia utilis Royle oil (PURO) on the synthesis of ceramide and expression of acid ceramidase N-acylsphingosine amidohydrolase 1 (ASH1),and to explore the mechanisms underlying its moisturizing and skin barrier-repairing effects.Methods Keratinocytes from human foreskin tissue were classified into 2 groups to be cultured in keratinocyte-serum free medium (K-SFM) with or without the presence of PURO.Enzyme linked immunosorbent assay (ELISA) was performed to measure the level of ceramide in the culture supernatant of keratinocytes at 0,3,8,24 and 48 hours.The back of nude mice was divided into 4 areas,i.e.,test area,matrix area,blank control area and negative control area.Acetone and ether were used to destroy the epidermal barrier in the test,matrix,and blank control areas,then,the former 2 areas were topically treated with emulsions containing 1% PURO and matrix,respectively,and the blank control area remained untreated.The epidermal barrier remained intact and untreated in the negative control area.Noninvasive methods were used to determine transepidermal water loss (TEWL),epidermal moisture content and skin lipid content in these areas on day 0,1,3,and 7.Skin tissue was obtained from these areas on day 0 and 7 followed by an immunohistochemical study for the quantification of ASH1 expression.Results The level of supernatant ceramide increased with time in the PURO-treated keratinocytes,which was significantly higher at 24 hours and 48 hours than at 0 hour (1.3817 ± 0.100 and 1.3737 ± 0.047 vs.0.7630 ± 0.143,both P < 0.05).The supernatant ceramide was also elevated in the PURO-treated keratinocytes compared with untreated keratinocytes at 24 and 48 hours (both P < 0.05).Noninvasive skin tests showed a gradual decrease in the TEWL,but an increase in the epidermal moisture content and skin lipid content with time in the 3 epidermal barrier-destroyed areas.As far as the test area was concerned,TEWL value was significantly lower on day 3 and 7 than on day 0 (10.85 ± 0.64 and 8.01 ± 0.58 vs.12.65 ± 0.71,both P < 0.05),while a significant increment was observed in the skin lipid content on day 3 and 7 compared with day 0 (29.14 ± 0.40 and 31.30 ± 0.88 vs.27.02 ± 0.65,both P < 0.05),as well as in the epidermal moisture content on day 1,3 and 7 compared with day 0 (13.98 ± 0.28,15.00 ± 0.38 and 15.86 ± 0.18 vs.11.74 ± 0.62,all P< 0.05).On day 7,there was a statistical decline in TEWL value,but an elevation in epidermal moisture content,skin lipid content and ASH1 expression in the test area compared with the matrix area and blank control area (all P < 0.05).Also,the expression of ASH1 was upregulated on day 7 compared with day 0 in the 3 barrier-destroyed areas (all P < 0.05).Conclusion PURO may exert skin-moisturizing and barrier-repairing effects by enhancing the synthesis of ceramide and expression of acid ceramidase ASH1.
