In recent years,studies committed to the myeloproliferative neoplasms (MPN) pathogenesis suggest that potential somatic genetic alterations,and epigenetic events might contribute to MPN development,influence the MPN disease phenotype,progression,and promote transformation.The most relevant mutations identified so far can be broadly classified into two main groups.Mutations associated with dysregulated JAK-STATsignal transduction pathway,including mutations in JAK2,CALR,MPL,CBL,LNK,SOCS.Mutations in several epigenetic modifiers,including mutations in TET2,ASXL1,IDH1/2,DNMT3A,EZH2 and so on.In this paper,the abnormal clones and clonal evolution of MPN were reviewed.

[Objective]To investigate the methord of nursing care of patients with cervical fracture-dislocation and spinal cord injuries during the perioperative period.[Method]Fifty-six patients with cervical fracture-dislocation and spinal cord injuries experienced preoperative training,psychology nursing and posture exercise,and postoperative position nursing and founctional exercising.The improvement of spinal cord founction was observed.[Result]All patients were followed up from 6 to 31 months,with an average of 17.6 months.According to JOA scores,the preoperative scores was from 0 to 13,with an average of 7.76 points.The postoperative score was 6-17 with an average of 10.3.The relief rate was 68.5%.Satisfaction rate was 74.1%.[Conclusion]The perioperative nursing is important for preventing and lessening complications,achieving good operative effect and promoting rehabilitation.

Objective Study the efficacy and safety of epristeride, a new uncompetitive 5?-reductase inhibitor, in the treatment of benign prostatic hyperplasia(BPH). Methods A multicentral opened clinical trial was conducted. 2 006 BPH patients were enrolled in the trial, in which 5mg epristeride was orally administered twice a day. BACKGROUND:Recent studies have found that femoral neck angleαin patient with femoral neck hernial fossa increased significantly. However, whether the femoral neck hernia fossa can be used as imaging diagnostic indicator for femoroacetabular impingement syndrome remains controversial. OBJECTIVE:To evaluate the association between alpha angle and herniation pit in asymptomatic hip joints, influencing factors of the above indexes, and to provide evidence for the diagnosis of hip impingement syndrome. METHODS:Adults with asymptomatic hip joints from September 2013 to December 2015 were retrospectively studied. Alpha angles were measured on oblique axial MRI images. The incidence and size of herniation pit were determined by MRI. Size measures, prevalence, and statistical analyses were conducted regarding its association with age, gender, laterality (right or left hip). Intra-and inter-observer consistency was determined by intra-class correlation coefficient. RESULTS AND CONCLUSION:(1) 105 patients with asymptomatic hips were included, and 185 sides of hip MRI images were obtained. The range of age was 18-80 years old. The prevalence of herniation pit in asymptomatic hips was 21.6%. (2) The range of alpha angle was 27.6°–65.0°. Alpha angles of hip joints of 17 cases were ≥ 55°. (3) There was no association between alpha angle≥55° and the size of herniation pit (P>0.05). The prevalence of herniation pit was not correlated with age, gender and affected side (P=0.160, 0.157, 0.110). Inter-observer consistency of alpha angle was 0.523 between first measurements of first vs. second observer, respectively. Intra-observer consistency of alpha angle was 0.654, respectively. (4) There is no association between alpha angle ≥ 55 degrees and presence of herniation pit or demographic variables (age, gender and affected side). The presence of herniation pit and the increased alpha angle need to be interpreted with caution. Clinical symptoms and other examinations should be used in the diagnosis of hip impingement syndrome.

BACKGROUND: About one fourth of diabetic patients suffer from diabetic retinopathy(DR) and has greatly impacted their quality of life due to the decreased vision. Social support plays important role on rehabilitation of patients.OBJECTIVE: To analyze the relationship between social support and quality of life(QOL) in patients with diabetic retinopathy in order to direct clinical practice and improve the patients' QOL.DESIGN: Cross-sectional study based on patients with diabetic retinopathy.SETTING: Department of Ophthalmology, General Hospital of Nanjing Military Area Command of Chinese PLA.PARTICIPANTS: Totally 81 patients admitted into Department of Ophthalmology, General Hospital of Nanjing Military Area Command of Chinese PLA with diabetic retinopathy during January 2002 to May 2003 were selected. Inclusion criteria: suffer from diabetic retinopathy and willing to attend this study. Exclusion criteria: people with other complications (hypertension, kidney disease) . The patients were divided into two groups according to vision: 28 cases in the group with binocular vision between 0.06 -0. 1; 53 cases in the group with vision less than 0. 05.METHODS: Investigation was conducted to the two groups from three dimensions including objective support, subjective support and usage in order to assess the social support to patients with diabetic retinopathy of different vision. And modified quality of life index (MQLI) was used to assess the QOL in patients with diabetic retinopathy that covered 5 dimensions: somatic health, mental health, social function, self-image and material base.MAIN OUTCOME MEASURES: The total score of social support and QOL of patients with diabetic retinopathy, score of somatic health, mental health,social functions and material base as well as the correlations among them.RESULTS: The total score of social support, score of objective support factor, subjective support factor and usage factor in roup with vision between 0.06 and 0. 1 were much higher than those in group with vision less than 0.05 ( P < 0.01 ). For patients with diabetic retinopathy, the social support had strong correlation with total score of QOL and factor score of other dimensions except of self-image( P < 0.05 or P < 0.01).CONCLUSION: Social support is an important factor that impacts QOL to patients with diabetic retinopathy. It will improve their QOL by reinforcing the social support.

Vasohibin-1 (VASH1),which is induced in response to angiogenic stimuli such as vascular endothelial growth factor (VEGF) and fibroblast growth factor (FGF)-2,has recently been isolated as a novel negative feedback inhibitor of angiogenesis.Several studies have demonstrated that VASH1 plays important roles in the development of various tumors and it would potentially be a biomarker and a candidate for molecular targeted therapy for patients with cancer in the future.
