1.Sarcoma of the Heart: a Rare Intracardiac Tumor and Review of Literature

Primary sarcoma of the heart are exceptionally rare and are aggressive tumours. We report a 38-year-old Chinese lady who had an intermediate grade sarcoma with myofibroblastic differentiation. The tumour was found in the right atrium. Literature reviews of rare intracardiac sarcomas are discussed.

Solitary fibrous tumour (SFT) adalah neoplasma fibrotik, juga dikenali sebagai
hemangiopericytoma. STAT6 merupakan protein yang berfungsi sebagai
transduser isyarat dan penggerak transkripsi keluarga STAT6 dalam fungsi imun dan
pertumbuhan sel awal. Perubahan onkogen STAT6 memainkan peranan penting
sebagai pemacu tertentu untuk genetik SFT melalui gen gabungan NAB2-STAT6.
Tujuan kajian ini adalah untuk membandingkan dan menilai ekspresi STAT6 di
antara SFT dan tumor yang menyerupai SFT. Kami menilai sebanyak 30 kes blok
tisu parafin yang sebelum ini didiagnosis SFT (n=14) dan tumor yang menyerupai
SFT (n=16) selama tempoh 15 tahun. Empat belas kes SFT (100%) menunjukkan
ekspresi nuklear STAT6. Semua jenis tumor yang lain adalah negative untuk STAT6
kecuali satu kes liposarcoma dediferensiasi satu tumor stromal gastrointestinal
dan satu lipoma yang menunjukkan pewarnaan sitoplasma yang tidak spesifik
dan lemah. Ekspresi nuklear STAT6 adalah berguna dan boleh dipercayai sebagai
penanda adjunksi SFT apabila diagnosis tidak boleh dibuat melalui kaedah
konvensional.
3.Superficial Angiomyxoma of the Eyelid. A Case Report and Review of the Literature

Superficial angiomyxoma (SA) is an angiomyxoid cutaneous tumour. It has predilection for the head and
neck, torso, limbs and genital tract. Our case is a 27-year-old female, presented with painless right medial
canthal mass for two years. It was associated with tearing when the lesion grew larger. We received a
nodular brownish tissue measuring 25x20x15mm with homogenous brownish cut surface. Microscopically,
the tumour was partially circumscribed, exhibiting bland stellate to spindle cells of moderate cellularity with
pale eosinophilic cytoplasm, indistinct border, in a loose collagenous myxoid matrix with numerous blood
vessels of different calibre. The lesional cells were present at the resected margin and were nonreactive
towards CD34, SMA and S100. SA of the eyelid is sometimes mistaken clinically as dermoid cyst or lipoma.
Reports have shown increased risks of local recurrence following incomplete excision. Close association
with Carney’s complex is an important feature. Careful clinicopathologic correlation and proper investigations
are needed for optimal patient care.
4.Factors associated with malnutrition among head and neck cancer in-patients before radiotherapy in National Cancer Institute, Putrajaya

Introduction: Head and neck cancer (HNC) patients are often malnourished during diagnosis and before treatment. This study determined the prevalence of malnutrition and factors associated with malnutrition among HNC patients. Methods: A crosssectional study among HNC in-patients before radiotherapy was conducted. Malnutrition status of the patients was determined using scored Patient GeneratedSubjective Global Assessment (PG-SGA). Nutritional parameters of muscle mass, fat mass, albumin, energy and protein intakes were collected. Nutrition impact symptoms (NIS) of the patients were assessed using a validated Head and Neck Symptoms Checklist© (HNSC©). Results: Fifty HNC patients were recruited in this study and the age range of patients was 21 to 78 years old, with gender distribution of 78% males and 22% females. More than half of the patients were malnourished, with 20% severely malnourished before radiotherapy. The lack of dietitian referral before treatment was found to significantly affect nutritional status (p=0.027). There was a significant negative relationship between energy intake (r=0.342, p=0.015) and protein intake (r=0.386, p=0.006) with PG-SGA, indicating lower energy and protein intakes related with poor nutritional status. The result showed a significant positive relationship between NIS score (r=0.731, p<0.001) and PG-SGA, indicating the lower the NIS, the better the nutritional status among HNC patients. More than half of the HNC patients had difficulty chewing. Conclusion: A strong association between nutritional status and NIS showed the importance of dietary management in HNC patients. Early identification of the nutritional status of HNC patients can ensure optimal nutritional status to improve treatment outcomes.
