1.Syncope in a 3-year-old male: A case report

Syncope is a common presenting complaint in
pediatric emergency departments. The differential
diagnosis of syncope is vast, including various
neurological and cardiac conditions. Brugada syndrome
(BrS) is a primary inherited arrhythmia which can present
with syncope but can lead to sudden cardiac deaths due
to ventricular fi brillation. BrS is a rare genetic condition,
often diagnosed in the adulthood. BrS presenting in
childhood, especially symptomatic BrS, is extremely
rare. We present a case of a previously healthy 3-year-old
male who presented to the emergency department with
syncope and was later diagnosed with BrS.
2.Effect of petroleum ether extract of Sesbania sesban (Merr.) roots in streptozotocin (STZ) induced diabetes in mice

Objective: The present study was carried out to investigate the hypoglycemic effects of the petroleum ether extract of Sesbania sesban (SS)(Merr.) roots, which are widely used in inflammation, fever, ulcers, leucoderma and diabetes in various parts of India. Methods: SS was administered orally at different doses (250, 500 and 1000mg/kg) to normal and streptozotocin (STZ) induced type- 2 diabetic mice. The fasting blood glucose (FBG), biochemical parameters in serum, change in body weight, internal organs weight, food intake, water intake and glycogen level in livers were performed for the evaluation of hypoglycemic effects.Results: All the doses of SS caused a marked decrease of FBG in STZ induced type -2 diabetic mice. SS decreased the cholesterol, triglyceride (TG), urea, creatinine level and increased the insulin, HDL cholesterol, and total protein level. Decrease in body weight and glycogen level induced by STZ was restored. Increase in water and food intake induced by STZ was decreased. Conclusions: The results suggest that SS may have hypoglycemic potential for the type 2- diabetes and support the traditional use of the roots of plant as a hypoglycemic agent.
3.Efficacy of golden rain tree against free radicals and H2O2-induced damage to pUC18/calf thymus DNA

Objective: To investigate the antioxidant and genoprotective potential of the methanol extract (METH-KP) along with its hexane fraction (HEX-KP) from the leaves of Koelreuteria paniculata (K. paniculata) Laxm. Methods: The antioxidant potential was checked using metal chelation assay, ABTS, DPPH (2,2-diphenyl-2-picryl-hydrazyl), reducing power and superoxide anion radical scavenging assay, Folin-Ciocalteu reducing capacity (FCR assay), Total flavonoid content (TFC assay) and genoprotective activity against the DNA damage induced by Fenton's reagent using pUC18/calf thymus. Results: Results showed that radical scavenging activities of the both test extract/fraction revealed a concentration-dependent antiradical activity in all the assays. In Metal chelation assay, METH-KP and HEX-KP showed 39.04% and 32.51% of scavenging at highest tested concentrations. The METH-KP exhibited IC50 of 54.54 μg/mL in ABTS, 115 μg/mL in DPPH, 110 μg/mL in reducing power and 135 μg/mL in superoxide anion radical scavenging assay while that HEX-KP was found to be very poor in radical scavenging in all the above assays. The phytochemical analysis showed good amount of phenolic and flavonoid compounds in METH-KP while the HEX-KP fraction lacks phenols. The METH-KP extract and HEX-KP fraction both showed DNA protective effect in Calf thymus/pUC18 DNA protection studies. Conclusions: The activity of METH-KP may be attributed to its polyphenolic constituents which needs further isolation of its active constituents which may lead to the development of novel drugs to combat cancer.
4.Trends in the Antibiotic Resistance Patterns of Enteric Fever Isolates – a Three Year Report from a Tertiary Care Centre

Objectives: The incidence of multidrug resistant enteric fever is increasing alarmingly. This study was planned to determine the rate of isolation of Salmonella spp. and to compare the isolates for their epidemiological parameters and antimicrobial susceptibility patterns at our center.
Methods: The study was conducted over a span of three years with a total of 8142, 8134, and 8114 blood culture samples processed for the years 2008, 2009, and 2010 respectively. The minimum inhibitory concentration (MIC) for ciprofloxacin and chloramphenicol was determined using an agar dilution method. The MIC for ciprofloxacin was also confirmed by Epsilon-test (E -test) strips.
Results: Of the total 302 Salmonella spp. isolated, 257 were Salmonella enterica serotype Typhi (85.1%) and 45 (14.9%) were S. enterica serotype Paratyphi A. The majority of the isolates recovered were from the pediatric age group (54.6%) and males (60.6%). Complete susceptibility was observed to chloramphenicol, cefotaxime, ceftriaxone, and azithromycin over the last two years (2009 and 2010), with an increase in resistance to nalidixic acid (100%) and ciprofloxacin (13.6%).
Conclusions: In our study, we found augmentation of resistance to nalidixic acid and fluoroquinolones and complete sensitivity to ceftriaxone along with reemergence of chloramphenicol sensitivity for Salmonella isolates. This report emphasises the necessity of continuous surveillance of antibiograms of enteric fever isolates in an area.

5.Nanodiamonds with powerful ability for drug delivery and biomedical applications: Recent updates on in vivo study and patents

Nanodiamonds are novel nanosized carbon building blocks possessing varied fascinating mechanical, chemical, optical and biological properties, making them significant active moiety carriers for biomedical application. These are known as the most'captivating' crystals attributed to their chemical inertness and unique properties posing them useful for variety of applications in biomedical era. Alongside, it becomes increasingly important to find, ascertain and circumvent the negative aspects associated with nano-diamonds. Surface modification or functionalization with biological molecules plays a significant role in managing the toxic behavior since nanodiamonds have tailorable surface chemistry. To take advantage of nanodiamond potential in drug delivery, focus has to be laid on its purity, surface chemistry and other considerations which may directly or indirectly affect drug adsorption on nanodiamond and drug release in biological environment. This review emphasizes on the basic properties, synthesis techniques, surface modification techniques, toxicity issues and biomedical applications of nanodiamonds. For the devel-opment of nanodiamonds as an effective dosage form, researchers are still engaged in the in-depth study of nanodiamonds and their effect on life interfaces.
6.Beneficial Effect of Vitamin E in Rotenone Induced Model of PD: Behavioural, Neurochemical and Biochemical Study.

Parkinson's disease (PD) a neurodegenerative disorder for which no preventive or long-term effective treatment strategies are available. Epidemiologic studies have failed to identify specific environmental, dietary or lifestyle risk factors for PD. However, oxidative stress in the SN is the most broadly accepted hypothesis for the etiopathology of PD. The Symptoms do not appear until there is a decline of striatal dopamine levels by 80% making it difficult to have early therapeutic interventions. Thus, the present experiment was designed to track down the sequential changes starting from the initiation of motor dysfunction and associated biochemical abnormality in rotenone based PD model. The study also evaluated the neuroprotective efficacy of vitamin E. Rats were treated with rotenone 2 mg/kg b.wt (s.c.) for 35 days. The level of dopamine decreased by 70~80% which was in turn reflected by marked deterioration in motor function such as (Total locomotor activity and catalepsy). Along with these the level of GSH and SOD declined significantly which was associated with elevated lipid peroxidation levels as much as by 60%.Vitamin E co-administration at a dose of 100 I.U/kg b.wt (i.m.) ameliorated rotenone induced changes in motor functions i.e Total locomotor activity and Catalepsy at the end of 5th week. Further, vitamin E supplementation significantly decreased lipid peroxidation and improved associated biochemical parameters i.e SOD and GSH level. Most interestingly the changes appeared as early as 3rd week suggesting that supplementation of vitamin E right at the beginning should be neuroprotective in PD.

8.Unusual bilateral sensory innervation of the dorsum of hand by lateral antebrachial cutaneous nerve: a case report.

A 68-year-old male cadaver showed bilateral variation in the sensory innervation of the dorsum of hand. On the dorsum of right hand, first digit and lateral half of second digit were supplied by lateral antebrachial cutaneous nerve (LABCN); medial side of second digit and lateral side of third digit were supplied by superficial branch of radial nerve (SBRN) and medial side of third digit, the fourth and fifth digits were supplied by dorsal cutaneous branch of ulnar nerve (DBUN). On the dorsum of the left hand, lateral side of first digit was supplied by LABCN, medial side of first digit, the second and third digits as well as the lateral side of fourth digit were supplied by SBRN; medial side of fourth digit and fifth digit were supplied by DBUN. These variations would be helpful in understanding peripheral neuropathy, in interpretation of conduction velocity studies and in reconstructive surgery of hand.

10.Reestablishing Lacrimal Drainage by Canaliculorhinostomy after Dacryocystectomy: A Viable Option in Symptomatic Patients

Purpose:
To evaluate the outcome and efficacy of a modified technique of canaliculo-dacryocystorhinostomy (canaliculo-DCR) in the complete absence of lacrimal sac, as a means to reestablish lacrimal drainage in postdacryocystectomy (post-DCT) patients.
Methods:
A retrospective, nonrandomized interventional study including 15 proven and established post-DCT patients with intact canaliculi of at least 7 to 8 mm. The patients presented with bothersome tearing to the oculoplastic clinic from January 2017 to January 2018. Patients were operated by a single surgeon (ST). Procedure involved creating a bony ostium of optimum size, internal membranectomy and the nasal mucosal flap anchored appropriately to create a passage in line with the common canaliculus. Adjunctively bicanalicular intubation and mitomycin-C were used.
Results:
Of the 15 patients who were operated (10 female and five male patients), 14 (93.33%) had functionally and anatomically patent lacrimal passage after the modified canaliculo-DCR, one (6.66%) was symptomatically better with partial regurgitation of clear fluid. Three (20%) had tube prolapse after 1st month postoperatively, which although significant, was not related to the technique. They were repositioned as an office procedure and retained thereafter till removal, no other tube related or mitomycin-C related sequelae was seen. No intraoperative complication was encountered.
Conclusions
With a success rate of 93.33% and a resultant patent lacrimal tear drainage passage and trivial complication such as tube prolapse, we can conclude, this technique of modified canaliculo-DCR in post-DCT patients is a safe and effective procedure with promising results, thereby avoiding cumbersome methods and maintenance of Jones tube while at the same time providing symptomatic relief to the patients.
