1.Early deep vein thrombosis: incidence in Asian stroke patients.

INTRODUCTIONDeep venous thrombosis (DVT) is thought to be less common in Asians than in the Caucasian population. The incidence of asymptomatic DVT in high-risk groups in the Asian population has not been well studied. While DVT incidence among Caucasian stroke patients has been extensively studied and the need for prophylaxis established, the lack of data in Asian patients leaves physicians with no firm basis for adopting prophylactic protocols in the local population. Our aim was to prospectively establish the incidence of early DVT in immobilised stroke patients in a heterogenous Asian population.
MATERIALS AND METHODSWe screened 44 patients with significant hemiplegia from acute stroke. Doppler ultrasound, the currently accepted method of investigation for DVT, was used to study patients on admission and at 1 week post-stroke. While there was no standard prophylactic regime in use, none of the patients received heparin and only 2 were given compression stockings.
RESULTSThe incidence of DVT at 1 week was 2.4%. Review at 1 month detected another patient with DVT, bringing the overall incidence at 1 month to 4.8%. This is lower than in Caucasian populations, but is similar to another local study on a different group of high-risk patients.
CONCLUSIONThe low incidence of early DVT in hospitalised stroke patients of Asian ethnicity does not justify routine screening for this population. Further research to validate this should ideally include a comparison test for DVT as ultrasound may have inherently lower sensitivity in an asymptomatic population.

2.Detection of internal carotid artery stenosis with duplex velocity criteria using receiver operating characteristic analysis.

INTRODUCTIONDuplex ultrasonography is an excellent non-invasive screening tool for carotid artery stenosis. The aim of this study was to evaluate optimal ultrasonographic criteria for determination of internal carotid artery stenosis with reference to digital subtraction angiography.
MATERIALS AND METHODSFrom January 1995 to December 2003, 114 symptomatic patients underwent both duplex ultrasonography and angiography. Seven velocity criteria were compared with angiographic stenosis and receiver operating characteristic curves were used to determine the best cutoff for each criteria.
RESULTSInternal carotid artery/common carotid artery systolic velocity ratios (PSV ICA/PSV CCA) and systolic internal carotid artery/diastolic common carotid artery ratios (PSV ICA/EDV CCA) were superior to other criteria for diagnosing internal carotid artery stenosis. For 50% stenosis, the best criterion of PSV ICA/PSV CCA was 1.5 [sensitivity 100%, specificity 85%, area under the curve (AUC) 99%], and the best criterion of PSV ICA/EDV CCA was 3.5 (sensitivity 100%, specificity 58%, AUC 99%). For 60% stenosis, the best criterion of PSV ICA/PSV CCA was 2.6 (sensitivity 100%, specificity 94%, AUC 99%), and the best criterion of PSV ICA/EDV CCA was 10.3 (sensitivity 100%, specificity 96%, AUC 99%). For 70% stenosis, the best criterion of PSV ICA/PSV CCA was 3.1 (sensitivity 100%, specificity 91%, AUC 99%), and the best criterion of PSV ICA/EDV CCA was 10.3 (sensitivity 100%, specificity 91%, AUC 99%).
CONCLUSIONOur study showed that velocity ratios are superior to other criteria for detecting carotid stenosis. Each laboratory needs to validate its own results.

3.Risk factor profile and treatment patterns of patients with atherothrombosis in Singapore: insight from the REACH Registry.

INTRODUCTIONAtherothrombosis is the leading cause of cardiovascular mortality. The Reduction of Atherothrombosis for Continued Health (REACH) Registry provided information on atherosclerosis risk factors and treatment. Singapore was one of the 44 participating countries in the REACH Registry. The objective of this study was to determine the atherosclerosis risk factor profile and treatment patterns in Singapore patients enrolled in the REACH Registry.
MATERIALS AND METHODSThe REACH Registry is an international prospective observational registry of subjects with or at risk for atherothrombosis. Patients aged 45 years or older with established vascular disease [coronary artery disease (CAD), cerebrovascular disease (CVD), peripheral arterial disease (PAD)] or 3 or more atherosclerosis risk factors were recruited between 2003 and 2004.
RESULTSA total of 881 patients (64.4% male) were recruited in Singapore by 63 physicians. The mean age was 64 +/- 9.8 years (range, 45 to 95). Seven hundred and one (79.6%) patients were symptomatic (CAD 430, CVD 321, PAD 72) while 180 (20.4%) patients had > or =3 risk factors. Approximately 13% of symptomatic patients had symptomatic polyvascular disease. There was a high proportion of diabetes mellitus (57%), hypertension (80.6%) and hypercholesterolemia (80.1%). A substantial proportion of symptomatic patients were current smokers (14.1%). Approximately half of the patients were either overweight or obese [abdominal obesity, 54.3%; body mass index (BMI) 23-27.5, 45.9%; BMI > or =27.5, 23.3%]. Patients were undertreated with antiplatelet agents (71.9% overall; range, 23.9% for > or =3 risk factors to 84.7% for PAD) and statins (76.2% overall; range, 73.6% for PAD to 82.1% for CAD). Risk factors remained suboptimally controlled with a significant proportion of patients with elevated blood pressure (59.4% for > or =3 risk factors and 48.6% for symptomatic patients), elevated cholesterol (40% for > or =3 risk factors and 24.4% for symptomatic patients) and elevated blood glucose (45% for > or =3 risk factors and 19.8% for symptomatic patients).
CONCLUSIONEstablished atherosclerosis risk factors are common in Singapore patients in the REACH Registry; and obesity is a major problem. Most of these risk factors remained suboptimally controlled.

Background and Objective: There is a paucity of studies looking into the frequency of complications
after stroke among Asians. We sought to determine the frequency and rate of complications among
Asians after acute stroke. Methods: Consecutive patients with acute stroke among 10 participating Asian
countries were included in the study. The frequency and timing of pre-determined complications, and
their relation to area of admission were noted. Results: Of the 1,153 patients included in the study, 423
(41.9%) developed complications within the first 2 weeks of stroke. Recurrent stroke, chest infections
and urinary tract infections were most commonly encountered, and were most frequent within the
first week of stroke onset. A lower rate of complications was noted among patients admitted at an
organized stroke unit.
Conclusion: There is a similar rate of frequency and timing of complications after acute stroke among
Asians as compared with other populations.
5.Medication compliance in Singaporean patients with Alzheimer's disease.

INTRODUCTION:
Singapore has a rapidly ageing population and an increasing prevalence of Alzheimer's disease (AD). Compliance to AD medications is associated with treatment effectiveness. We investigated compliance to acetylcholinesterase inhibitors (AChEIs) and N-methyl-D-aspartate (NMDA) receptor antagonist and treatment persistence among patients seen at the General Memory Clinic of National University Hospital, Singapore. We also identified the reasons for non-compliance.
METHODS:
Patients seen at the General Memory Clinic between 1 January 2013 and 31 December 2014, who were prescribed AChEIs and NMDA receptor antagonist, were included in this retrospective cohort study. Non-compliance to medications was indirectly measured by failure to renew prescription within 60 days of the last day of medication supplied by the previous prescription. The reasons for non-compliance were identified.
RESULTS:
A total of 144 patients were included. At one year, 107 patients were compliant to AD medications, while 37 patients were non-compliant. Around 60% of the non-compliant patients discontinued the use of AD medications within the first six months, and the mean persistent treatment period among this group of patients was 10.3 ± 3.5 months. The main reason for non-compliance was patients' and caregivers' perception that memory loss was of lower priority than other coexisting illnesses. Other reasons for non-compliance included side effects of medications (18.9%), perceived ineffectiveness of treatment (16.2%), inability to attend clinic (5.4%) and high cost of medications (2.7%).
CONCLUSION
Our findings suggest that the reasons for medication non-compliance can be identified early. Better compliance may be achieved through a multidisciplinary approach to patient education.

Background:
and Purpose Recent studies suggested an increased incidence of cerebral venous thrombosis (CVT) during the coronavirus disease 2019 (COVID-19) pandemic. We evaluated the volume of CVT hospitalization and in-hospital mortality during the 1st year of the COVID-19 pandemic compared to the preceding year.
Methods:
We conducted a cross-sectional retrospective study of 171 stroke centers from 49 countries. We recorded COVID-19 admission volumes, CVT hospitalization, and CVT in-hospital mortality from January 1, 2019, to May 31, 2021. CVT diagnoses were identified by International Classification of Disease-10 (ICD-10) codes or stroke databases. We additionally sought to compare the same metrics in the first 5 months of 2021 compared to the corresponding months in 2019 and 2020 (ClinicalTrials.gov Identifier: NCT04934020).
Results:
There were 2,313 CVT admissions across the 1-year pre-pandemic (2019) and pandemic year (2020); no differences in CVT volume or CVT mortality were observed. During the first 5 months of 2021, there was an increase in CVT volumes compared to 2019 (27.5%; 95% confidence interval [CI], 24.2 to 32.0; P<0.0001) and 2020 (41.4%; 95% CI, 37.0 to 46.0; P<0.0001). A COVID-19 diagnosis was present in 7.6% (132/1,738) of CVT hospitalizations. CVT was present in 0.04% (103/292,080) of COVID-19 hospitalizations. During the first pandemic year, CVT mortality was higher in patients who were COVID positive compared to COVID negative patients (8/53 [15.0%] vs. 41/910 [4.5%], P=0.004). There was an increase in CVT mortality during the first 5 months of pandemic years 2020 and 2021 compared to the first 5 months of the pre-pandemic year 2019 (2019 vs. 2020: 2.26% vs. 4.74%, P=0.05; 2019 vs. 2021: 2.26% vs. 4.99%, P=0.03). In the first 5 months of 2021, there were 26 cases of vaccine-induced immune thrombotic thrombocytopenia (VITT), resulting in six deaths.
Conclusions
During the 1st year of the COVID-19 pandemic, CVT hospitalization volume and CVT in-hospital mortality did not change compared to the prior year. COVID-19 diagnosis was associated with higher CVT in-hospital mortality. During the first 5 months of 2021, there was an increase in CVT hospitalization volume and increase in CVT-related mortality, partially attributable to VITT.
