1.Eyeball structure changes in high myopic patients and their significance for forensic assessment.

There are irreversible eyeball structural changes in high myopic patients. These changes include axial length, corneal radius, anterior chamber depth, fundus degeneration, macula thickness, etc. There is a close relationship between the damage degree of visual function and these changes. The incidence of complications, such as vitreous opacity, posterior vitreous detachment, cataract, glaucoma, posterior staphyloma and retina detachment, is also highly related to the myopia diopter. More and more researches have indicated that the myopia diopter and the level of visual function are affected by multiple factors. It is promising to detect all of these changes by different kinds of methods, and to assess visual function through these changes. By clarifying these changes, it is also useful to distinguish traumatic damage from disease to provide evidence for forensic assessment of eye injuries.

2.Effect of laser in situ keratomileusis on accommodation.

The accommodative function before and after laser in situ keratomileusis (LASIK) was observed, and the effect of LASIk on accommodation was investigated. In a prospective clinical trial, 48 myopic patients (96 eyes) subject to bilateral LASIK in Refractive Surgery Center, Tongji Hospital, Tongji Medical College, Huazhong University of Science and Technology (China) from March 2006 to June 2006 were selected and studied. Refractions, accommodative range, amplitude of accommodative response and high frequency component (HFC) of accommodative microfluctuations were measured with NEDIK-730A before and one week and 30 days after operation. Dominant and non-dominant eyes were determined by hole-in-card method. It was found that all of the operative eyes showed an uncorrected visual acuity of 0.8 or better one week postoperatively, and 1.0 or better 30 days postoperatively. Compared with those preoperatively, accommodative range and HFC had no significant difference at first week and 30th day after operation in both dominant eyes and non-dominant eyes (P>0.05), but there was a significant difference in the amplitude of accommodative response/accommodative stimulus ratio (A/S) after operation (P<0.01), and no significant difference was found in accommodation between one week and 30 days postoperation. No ocular dominance's change was noted. There was no significant difference in accommodative function between dominant eyes and non-dominant eyes. It was suggested that LASIK produced no significant effect on accommodation.

3.Clinical observation on therapeutic effect of myopia in children treated with abdominal acupuncture.

OBJECTIVETo observe the clinical effects of myopia in children treated with abdominal acupuncture.
METHODSNinety children with myopia were randomly divided into an abdominal acupuncture group (45 cases with 90 ill eyes) and an auricular point group(45 cases with 90 ill eyes). In the abdominal acupuncture group, Zhongwan (CV 12), Shangqu(KI 17), Xiawan(CV 10), Tianshu(ST 25), Qihai(CV 6), Guanyuan(CV 4) were treated with tapping method. The needles were not retained and down to the lower in accordance with the order from Zhongwan (CV 12) to Guanyuan(CV 4) for 5 min,and the erubescence skin without blood was proper. In the auricular point group, vaccaria seeds were pasted at gan(CO12), shen(CO10),pi(CO13), wei(CO4), yan(LO5), pingjianqian(TG2i), pingjianhou(ATli) and zhen(AT3); one ear was chosen every time and the other ear the second time with pressing of patient's own for 5 min every day. The treatment was given twice a week and 10 treatments were considered as one course in the two groups. The changes of the visual levels in visual chart before treatment, 20 min after the first treatment, after 5 weeks' and 3 months' treatment were compared between the two groups,and the clinicall efficacy was compared between the two groups as well.
RESULTS(1) The visual levels in visual chart were improved in the abdominal acupuncture group and auricular point group, and the effects 20 min after the first treatment, after 5 weeks' and 3 months' treatment in the abdominal acupuncture group were superior to those in the auricular point group(4. 78±0. 16 vs 4. 69±0. 22, P<0. 01; 4. 85±0. 16 vs 4. 79±0. 21, P<0. 05; 4. 89±0. 13 vs 4. 71±0. 25, P<0. 01). (2)The effective rates 20 min after the first treatment, after 5 weeks' and 3 months' treatment in the abdominal acupuncture group were better than those in the auricular point group[31. 4% (27/86) vs 0% (0/82). 81.4%(70/86) vs 60. 0%(49/82); 88. 4%(76/86) vs 51. 2%(42/82), all P<0. 01].
CONCLUSIONAbdominal acupuncture for improving the visual levels of myopia in children is superior to auricular point sticking.

5.Accommodative stimulus response curve of emmetropes and myopes.

INTRODUCTIONMyopes are suspected to be poorer at responding to accommodative stimuli than emmetropes, and this may worsen the degree of their myopia. The study aims to compare the abilities of young adult emmetropes and myopes in responding to accommodative stimuli, as indicated by their Accommodation Stimulus Response Curves (ASRCs) in a predominantly Chinese population.
MATERIALS AND METHODSSeventeen emmetropes and 33 myopes aged between 16 and 23 years (mean, 18.6 +/- 1.2) were recruited, of whom 11 were progressing and 22 were non-progressing myopes. The ASRC gradients of subjects were measured using the methods of decreasing distance series (DDS), positive (PLS) and negative lens series (NLS).
RESULTSThe ASRC is method dependent. The gradients of the curves are significantly different among 3 methods of measurement using single-factor ANOVA (F3.057 = 44.815, P <0.01). The slopes of the accommodative errors of all subjects were steeper using the NLS method, and the lags of accommodation increased with elevated demands. No significant differences in ASRC gradients were found between emmetropes, non-progressing myopes and progressing myopes for the range of accommodative demands for each method. Progressing myopes showed the highest error towards the higher demand compared with the emmetropes and non-progressing myopes.
CONCLUSIONAccommodative responses of myopes were more sluggish though there were no statistical differences in ASRC gradients between emmetropes and myopes. It is not certain if the poorer accommodative responses were a cause, or a consequence, of myopia.

6.The Changes of Posterior Corneal Surface and High-Order Aberrations after Refractive Surgery in Moderate Myopia.

PURPOSE: To compare forward shift of posterior corneal surface and higher-order aberration (HOA) changes after LASIK, LASEK, and wavefront-guided LASEK surgery in moderate myopia METHODS: One hundred eighty four eyes undergoing LASIK, LASEK and wavefront-guided LASEK with VISX STAR S4 were included in this study. The posterior corneal elevation was measured with Orbscan before, 2 and 4 months after surgery. Changes of the elevation were assessed using the difference map generated from preoperative and postoperative elevation maps. The values of higher-order aberrations were evaluated preoperatively and 2 months postoperatively with Wavefront aberrometer. RESULTS: The posterior corneal surface displayed forward shift of 27.2+/-11.45 micrometer, 24.3+/-9.76 micrometer in LASIK group, 23.4+/-10.5 micrometer, 23.6+/-10.55 micrometer in LASEK group, 24.0+/-14.95 micrometer, 28.4+/-14.72 micrometer in wavefront-guided LASEK group at 2 months and 4 months, respectively. There were no statistically significant differences among those three groups, and between 2 and 4 months. The root mean score (RMS) of HOA was increased after LASIK and LASEK (p=0.000, p=0.000, respectively). The mean change of HOA-RMS was significantly smaller in wavefront-guided LASEK than LASIK or LASEK (p=0.000, p=0.000, respectively, Bonferroni-corrected). CONCLUSIONS: The changes of posterior corneal surface forward shift showed no difference among LASIK, LASEK and wavefront-guided LASEK in moderate myopia. HOAs were significantly increased after LASIK and LASEK. The changes of HOAs were significant smaller in wavefront-guided LASEK than LASIK or LASEK.

7.Biological effect of electrical plum-blossom needle for treatment of juvenile myopia.

OBJECTIVETo observe therapeutic effect of electrical plum-blossom needle for treatment of juvenile myopia and biological effect on ocular tissue and structures.
METHODSOne hundred and sixty cases with mild myopia (diopter < 3.00D) were randomly divided into two groups, a treatment group and a control group. The treatment group were treated with electrical plum-blossom needle therapy, tapping Jingming (BL 1), Chengqi (ST 1), Taiyang (EX-HN 5) and Neiguan (PC 6), and the control group with Tropicamide eye drops. The changes of vision, diopter, corneal refractive power, ocular-axial length, lens and ciliaris thickness were observed before and after treatment.
RESULTSThe total effective rate was 80.0% in the treatment group and 58.1% in the control group, with a very significant difference between the two groups (P < 0.01); the two therapies could decrease the thickness of lens and ciliaris, and eliminate spasm of ciliary muscle, however the electrical plum-blossom needle therapy had more obvious action (P < 0.05); the two therapies had no effect on the corneal refractive power and ocular-axial length.
CONCLUSIONThe electrical plum-blossom needle therapy is an effective method for increasing vision, correcting ametropia and delaying development of myopia.

8.Spontaneous reattachment of retinal detachment with macular hole in nonmyopic patients.

The occurrence of a retinal detachment in a patient with a macular hole is quite rare unless the patient is a high myope or has experienced ocular trauma. The incidence of spontaneous reattachment of retinal detachment with a macular hole is not uncommon in the highly myopic patients. However, data on nonmyopic cases are lacking. I herein describe two nonmyopic cases of retinal detachment with a macular hole, of which retinas reattached spontaneously.

9.One Sister and Brother with Mirror Image Myopic Anisometropia.

We report a case of one sister and brother with mirror image myopic anisometropia. One sister and brother complained visual disturbance. The sister was 10 years 11 months old, and brother was 8 years 4 months old. Full ophthalmic examinations were performed, including slit lamp examination, intraocular pressure, keratometry, anterior chamber depth, axial length, fundus examination and the cycloplegic refraction. The cycloplegic refractive power was -15.50 dpt cyl.+4.50 dpt Ax 85degrees (right eye), -1.00 dpt cyl.+0.50 dpt Ax 90degrees (left eye) in the sister; -1.75 dpt cyl.+2.25 dpt Ax 90degrees (right eye), -9.50 dpt cyl.+4.00 dpt Ax 80degrees (left eye) in the brother. The co-occurrence of severe myopic anisometropia in a sister and brother is extremely rare. The present case suggests that severe myopic anisometropia may be related by genetic inheritance.

10.Effect of high myopia on 24-hour intraocular pressure in patients with primary open-angle glaucoma.

BACKGROUNDAs intraocular pressure (IOP) and IOP fluctuation are known risk factors for glaucoma, it is important to understand the effects of high myopia on these ocular parameters. The aim of this study was to investigate if primary open-angle glaucoma (POAG) patients with high myopia exhibit higher IOP and greater IOP fluctuations at resting conditions over 24 hours.
METHODSWe designed a prospective control clinical study. Eighty-two eyes of 82 high-tension Chinese POAG patients only on prostaglandin analogue locally were divided into 3 groups according to various myopic grades (< -6.0 D, n = 27 and between -0.76 and -5.99 D, n = 33) or without myopia (-0.75 to 0.75 D, n = 22). Single time IOP at 10 am, mean corrected 24-hour IOP, mean corrected night IOP, 24-hour IOP fluctuation and IOPs of 10 am, 2 pm, 6 pm, 10 pm, 2 am, 6 am and 8 am were measured.
RESULTSThe IOP was higher in POAG patients with high myopia over those POAG alone in three ways: the elevated IOP value was 0.65 mmHg measured in single time IOP at 10 am, 0.84 mmHg in mean corrected 24-hour IOP, 0.97 mmHg in mean corrected night IOP. The 24-hour IOP fluctuation was lower in the two myopia groups than in non-myopia group. Further, using repeated measurement analysis of variance, there was no statistical significance among groups regarding the IOPs at the seven time points (P = 0.77) and there was no interaction between groups and time points (P = 0.71), but the difference of IOPs at the seven time points in same group was statistically significant (P = 0.01).
CONCLUSIONHigh-tension POAG patients with high myopia, even on pharmacological glaucoma therapy, still have higher IOP, but 24-hour IOP fluctuation at resting conditions was lower in these patients.
