Malaysian Journal of Medical Sciences 2013;20(2):62-66
Platelet flow cytometry is an emerging tool in diagnostic and therapeutic hematology. It is eminently suited to study the expression of platelet surface receptors both qualitatively as well as quantitatively. It can serve as a useful marker for the documentation of in vivo platelet activation, and thus, fore-warn the risk of thromboembolism in patients with diabetes mellitus, coronary syndromes, peripheral vascular diseases, and pre-eclampsia.
This technique can also be extended to study and compare the effect of various antiplatelet drugs on the level of activation of platelets and to establish any dose-effect relationship of these drugs. Topographical localization of platelet granules and study of platelet-platelet and platelet-leukocyte interaction is also possible by this procedure. All these parameters serve as pointers towards the presence of activated platelets in the circulation with its thromboembolic consequences.
This is a simple reliable and cost effective technique which has a wide application in the diagnosis of various inherited and acquired platelet disorders. Study of platelet cluster of differentiation (CD) markers in various inherited disorders i.e. Bernard Soulier’s disease, von Willebrand disease, Glanzman’s disease, and Grey platelet syndrome may help categories the molecular lesions in these oft under-studied disorders.
2.Availability of sugar-free medicine for children: Are we denying their needs?
Archives of Orofacial Sciences 2018;13(2):63-69
This study aimed to review the availability of sugar-free medicine available in the market using the
online Monthly Index of Medical Specialties (MIMS) Malaysia 2016. Data collection was obtained from product
information (PI) for prescription medications contained in the MIMS Malaysia 2016. Concise information for each
product was collected based on therapeutic class groupings. The therapeutic class obtained from PI was
assigned to one of the 11 major therapeutic classes. PI that stated its use in paediatrics was further reviewed for
the availability of sugar free ingredient. Each product was then allocated into the poison groups: Group B, Group
C and Non-Scheduled Poison. A total of 282 PI items from 336 companies in MIMS Malaysia 2016 that satisfied
our eligibility criteria were reviewed. Of the 282 PI items reviewed, most of the products, 169 (59.9%) were sugarbased product, while 87 (30.9%) of them belong to sugar free products. Our study found that most of the
medicines reviewed in the MIMS Malaysia 2016 were under Group C (134 of total 282). There is still a high
number of sugar-containing medicines despite the promotion of sugar-free medication. Therefore, consumers
need to be educated on the use of sugar-free medicines in children to increase the demand and availability in the
market.

Asian Spine Journal 2022;16(2):212-220
Methods:
Fifty-one discs were obtained from patients undergoing surgery for herniated disc. Their ages ranged from 19-65 years, and 31/51 were male. Pre-surgical T1 and T2 weighted lumbar-spine MRIs were analyzed to observe Pfirrmann grade, VEP defects, herniation type, Modic changes, and high-intensity zones (HIZ) at the affected level. Five-micron thick sections were stained with hematoxylin and eosin, Alcian blue periodic acid–Schiff stain; examined for histological degeneration scores (HDS; 0–15), inflammation (0 [absence]–3 [severe]), and presence of cartilaginous endplate (CEP). Three-micron thick sections were stained with protein-gene-product 9.5 and expression was counted/mm2. Data was analyzed, and p<0.05 was considered to indicate statistical significance.
Results:
VEP defects, Modic changes, and HIZ were respectively observed in 30/51, 16/51, and 6/51 of the samples. CEP was observed in 26/51 samples and in 23/51 with endplate defects. Discs with adjacent VEP defects showed increased HDS (p<0.001) and inflammation (p<0.001). Discs with adjacent Modic changes also revealed increased HDS (p=0.01). Histological sections with CEP showed increased HDS (p<0.001) and inflammation (p<0.001), and nerve density was significantly positively correlated with HDS (r=0.27, p=0.02).
Conclusions
VEP changes can modulate degeneration and inflammation of herniated discs. Presence of these changes is highly predictive of the occurrence of CEP in herniated discs, which leads to slow resorption and persistent clinical symptoms.

Malaysian Orthopaedic Journal 2021;15(No.1):72-78
Introduction: The study aimed to target the current practices
of the orthopaedic community in outpatient (OPD),
emergency (ER) and surgical services (OT) during COVID19.
Material and method: This study surveyed 303 orthopaedic
surgeons from all over Pakistan. The survey had 30 questions
targeting the setup of outpatient, emergency and operation
services in orthopaedic departments of different hospitals in
Pakistan.
Result: A total of 302 surgeons were included from 53 cities
all over Pakistan. Between 35-48% of the respondents
reported lack of availability of standard operating procedures
in OPD, ER and in OT. Majority of the respondents noted
that their OPD and surgical practice had been affected to
some degree and 69% of the surgeons were only doing
trauma surgery. This trend was higher in younger consultants
of less than 45 years of age (p<0.001). Almost two-third of
the surgeons, mostly senior (p=0.03) were using surgical
masks as the only protective measure during various
practices of OPD, ER and OT, while most of the setups were
not assessing patients even for signs and symptoms of
COVID. Almost 89% of the orthopaedic community is
facing definite to mild stress during this pandemic and this
has significantly affected the senior surgeons (p=0.01).
Conclusion: Our study highlighted that COVID-19 has
resulted in marked changes to the practices of the majority of
Pakistani orthopaedic surgeons. Despite a sharp upsurge in
the number of cases and mortality due to COVID-19,
guidelines were still lacking at most of the settings and a
substantial percentage of the orthopaedic community were
not following adequate safety measures while attending to
patients.

Malaysian Journal of Microbiology 2024;20(no.1):58-66
Aims:
The study was aimed to explore the antimicrobial potential of ethanolic leaf extracts of Eucalyptus globulus, Moringa oliefera, Syzygium cumini and Citrus limon against antibiotic-resistant Clostridium perfringens type D (n=5).
Methodology and results:
Antibiotic resistance pattern of C. perfringens type D isolates against tetracycline, gentamicin, ceftriaxone, amoxicillin and streptomycin was evaluated by disc diffusion method. Well diffusion and micro broth dilution methods were used to determine the anti-bacterial activity, sub-inhibitory concentrations and antibiotic resistance modulating effects of the plant extracts. Ethanolic extract of E. globules was selected to evaluate its modulatory impact and subjected to GC-MS analysis to separate and identify the phytochemicals. The results showed that the isolates were resistant to gentamicin (0 ± 0.00 mm), streptomycin (0 ± 0.00 mm), tetracycline (13.2 ± 2.28 mm) and ceftriaxone (0 ± 0.00 mm) while sensitive to amoxicillin (23.8 ± 1.30 mm) and tetracycline (13.2 ± 2.28 mm). Eucalyptus globulus exhibited the maximum anti-bacterial activity with a zone of inhibition (ZOI) of 14.6 ± 0.54 mm and
minimum inhibitory concentration (MIC) (1500 ± 947.85 µg/mL). Other plant extracts (M. oliefera, S. cumini and C. limon) also showed anti-bacterial activity but couldn’t modulate the resistance. The activity of ceftriaxone associated with E. globulus extract was improved with 20.2 ± 0.20 mm ZOI at 78.125 µg/mL sub-inhibitory concentration.
Conclusion, significance and impact of study:
The study results indicate the possible use of the ethanolic extract of E. globulus alone or in combination with common antibiotics for the treatment of C. perfringens infections in small ruminants.

Objective:
The objective of this study was to assess factors related to renal function in crane operators at a port container terminal in Indonesia.
Methods:
A cross-sectional study on crane operators in Medan Container Terminal, Indonesia, was conducted. Renal function was measured through Glomerular Filtration Rate (GFR). Individual characteristics such as age, nutritional status, fluid consumption, hydration status, and physical activity level were collected. Fluid consumption was obtained from a 24-h food recall questionnaire. Physical activity was measured based on the Bouchard questionnaire. Association was determined by calculating the odds ratio (OR) and its 95% confidence interval (CI).
Results:
A total of 87 subjects were involved. All were male with the median age of 30.1 ± 2.4 years old. The prevalence of decreased renal function was 49.4%. Inadequate fluid consumption (OR = 6.1, 95% CI 2.1 -17.7; p<0.001) significantly increase the risk of renal function decreased, while being overweight/obese (OR 3.3, 95% CI 0.7 -11.7;p<0.125) and dehydration (OR 2.3, 95% CI 0.8-26.9; p=0.111) also increase the risk although not significantly.
Conclusion
Inadequate fluid consumption increases the risk of decline in the renal function of crane operators in the container terminal. A company should regularly monitor and evaluate the kidney function of this group of workers through annual medical checkups and provide easier access to drinking water and convenient toilets to improve fluid consumption of the workers.

INTRODUCTION:
Chronic Fatigue Syndrome (CFS) is a collection of symptoms from a substantial reduction in the ability to engage in preillness levels of occupational, educational, social, or personal activities that persists for more than six months and is accompanied by fatigue, post-exertional malaise, and unrefreshing sleep. One of the effects of heavy metal exposure is the occurrence of CFS among workers. Artisanal and Small-scale Gold Mining (ASGM) workers used mercury in their work, and this leads to a higher risk of chronic mercury poisoning. One of the health problems dueto mercury exposure is the occurrence of CFS and this has not been studied among ASGM workers.
OBJECTIVE:
This study intended to discover the prevalence of CFS among ASGM workers and the factors associated with it.
METHOD:
This study used a cross-sectional design to find the relationship between independent variables such as age, sex, working period as a miner, type of work activities in ASGM, and cumulative urinary mercury levels with CFS in ASGM workers in West Nusa Tenggara and Banten province. CFS was measured using standardized questionnaire on effect on mercury released by WHO-UNEP, and creatinine-corrected urinary mercury levels.
RESULTS:
The prevalence of CFS in ASGM workers in West Nusa Tenggara and Banten provinces was 17.9%. Based on the results, the factors of age, sex, work period, type of work, province of origin and cumulative urinary mercury levels did not have a statistically significant relationship with CFS (p> 0.05).
CONCLUSION
There was no significant relationship between age, sex, work period, type of work, urinary mercury level and cumulative urinary mercury levels with CFS in ASGM workers.
