1.The difference analysis of prescription dose between ICRU report 83 and Chinese recommendation in the nasopharyngeal carcinoma when using IMRT

Objective To analyze the difference of prescription dose between ICRU report 83 and Chinese recommendation in the nasopharyngeal carcinoma (NPC) for intensity modulated radiation therapy (IMRT).Methods Eighty-four NPC were treated using IMRT technology from Jan 1,2010 to Apr 1,2012.All dose volume histogram of the 84 IMRT plan were analyzed retrospectively.The target volumes of planning gross tumor volume of nasopharynx (PGTVnx) or planning clinical target volume and high risk lymphatic drainage (PCTV1) and doses of D100,D98,D95,D50,D2 and D0 were recorded.The mean,standard error,medial,range,coefficient of variation (CV) of PGTVnx,PCTV1,and D100,D98,D95,D50,D2and D0 were calculated,respectively.The homogeneity index (HI) and deviation between D95 and D50 of PGTVnx and PCTV1 were calculated,respectively.The differentiation of grouping results were analyzed with grouped t-test method.Results The HI of PGTVnx and PCTV1 were 0.118 ± 0.045 and 0.272 ± 0.037,respectively.It is the bigger target volume,the worse HI;and the advanced T stage,the worse HI.Either PGTVnx or PCTV1,D95 were less than D50.The average deviation was-5.15% and-10.97%,and the actual difference value was (382± 180) cGy (P=0.000) and (741± 140) cGy (P=0.000).Conclusions D550,which is the recommendation prescription dose of PTV in ICRU report 83,could evaluate accurately the IMRT plan with combining D98 and D2· When D50 is used to instand of D95,the prescription dose of PGTVnx and PCTV1 should increase 5% and 11%,respectively.
2.Antitussive, Expectorant and Antiasthmatic Actions of Zhisou Lixiao Capsule

Objective: To study the Antitussive, expectorant and antiasthmatic actions of Zhisou Lixiao capsule(ZLC). Methods: The incubation period and the frequency of cough in mice induced by ammonia water, the secretion of PSP from mouse bronchus and the incubation period of asthma in guinea-pigs induced by histamine were observed. Results: ZLC 0.8~3.2g?kg-1?d-1 ig could obviously prolong the incubative period of cough and decrease the frequency, ZLC 0.8~3.2g?kg-1?d-1 ig could increase the secretion of PSP in mice, 0.5~2.0 g?kg-1?d-1 increase the bronchial secretion in rats, and 1.0~2.0 g?kg-1?d-1 could prolong the incubation period of asthma in guinea-pigs. Conclusion: ZLC exerts a good Antitussive, expectorant and antiasthmatic effect.
3.The clinical significance of IgG4 in the bronchoalveolar lavage fluid of rheumatic disease related interstitial lung disease

Objective To evaluate the clinical significance of the level of [gG4 in the bronchoalveolar lavage fluid (BALF) of patients with rheumatic disease related interstitial lung disease (RD-ILD).Methods Eighteen patients with RD-ILD,14 patients with pulmonary infection and 12 patients without ILD or pulmonary infection,were recruited consecutively from the in-patient ward of Peking University People's Hospital.Clinical features and laboratory data were extracted from the medical record database of Peking University People's Hospital.The level of IgG4 was determined by ELISA.The statistical analysis of data using t test,and the correlation between the two variables were analyzed using linear regression analysis.Results The level of IgG4 was significantly higher in BALF of patients with RD-ILD than patients' without ILD or pulmonary infection,whereas there was no difference compared to patients with pulmonary infection.The level of IgG4 in BALF of patients with RD-ILD was positively correlated with the percentage of lymphocytes (r=0.53,P=0.03),and significantly negatively correlated with the percentage of macrophages (r=-0.65,P=0.005).Conclusion The level of IgG4 is significantly elevated in BALF of patients with RD-ILD,which is probably secreted from the lymphocyte of the lung.
4.Establishment of quality control evaluation system for 3D-ultrasound-based image-guided radiotherapy

Objective To analyze the stability and accuracy of the equipment for three-dimensional ultrasound-based image-guided radiation therapy (3DUS-IGRT) in daily practice, and to provide a basis for clinical application of radiotherapy for soft tissue tumors.Methods A specific calibration phantom was used for continuous calibration and quality control of the 3DUS-IGRT equipment in a year.The method for daily quality control of ultrasound-guided equipment was explored, and its stability and accuracy were monitored.Results The phantom position errors in both Sim and Guide stations of the 3DUS-IGRT equipment were within 1 mm.Conclusions The 3DUS-IGRT equipment has a stable performance with the support of a complete set of stringent and accurate calibration and quality control, which provides a new image-guided method for precise radiotherapy for soft tissue tumors.
5.Significance of visual electrophysiological techniques in the early diagnosis of glaucoma

Glaucoma is a group of diseases which can threaten and damage the optic nerve and its visual pathway, leading to visual impairment as a result. Glaucomatous optic neuropathy is a chronic disease accompanied by apoptosis of retinal ganglion cells ( RGCs ) , visual field defect and cupping of optic nerve head. The gold standard in functional glaucoma evaluation is standard automated perimetry ( SAP) , but it is often limited to the subjective feelings of the patients. Still, visual electrophysiological techniques cannot replace the conventional inspection, but with its rapid development, it has provided a new strategy for the early diagnosis of glaucoma as a supplement because it can show changes in amplitude and latency before visual field defect. Here we review three special electroretinograms and multifocal focal visually evoked potentials in the early diagnosis of glaucoma.
6.Up-regulated release of vascular endothelial growth factor in tissue engineered bone with implanted vascular bundles: a model of femoral defect in rabbits

Objective To investigate whether tissue engineered bone with implanted vascular bun-dles can up-regulate release of vascular endothelial growth factor (VEGF) in models of femoral defect in rabbits.Methods Thirty-two rabbits were randomized into 2 even groups.In both groups, a segmental bone defect of 15 mm in length was made at the left femur before a tissue engineered bone was inserted into the defect.In the experimental group, a femoral vascular bundle was implanted into the tissue engineered bone.In the control group, there was no vascular implantation.At 2, 4, 8, and 12 weeks after implantation, samples were taken to determine new bone formation by histology and expression level of VEGF by immuno-histochemistry.Results The new bone formation was significantly higher in the experimental group at the end of 4, 8, and 12 weeks(P < 0.05) .The expression level of VEGF in the experimental group was also significantly higher than in the control group at all time points after operation, and the expression of VEGF peaked at 4 weeks.Conclusion Tissue engineered bone with vascular bundle implanted can up-regulate VEGF release in models of femoral defect in rabbits.
7.Distance between brain metastases of non-small cell lung cancer and the hippocampus and its correlation with PTV low-dose regions in prophylactic cranial irradiation with hippocampal avoidance: an analysis of 56 patients

Objective To investigate the feasibility of prophylactic cranial irradiation with hippocampal avoidance (HA-PCI) in non-small cell lung cancer (NSCLC).Methods The clinical data of 56 patients with brain metastases of NSCLC who were treated from 2011 to 2014 were collected.Brain metastases and the hippocampus were delineated on T1 W1 contrast-enhanced MRI,and the distance between brain metastases and the hippocampus was analyzed;an HA-PCI regimen was also developed,and the distribution of the metastases in planning target volume (PTV) low-dose regions around the hippocampus was analyzed.Results None of the 139 metastases involved the hippocampus.There were 6(4.3%) and 18 (12.9%) metastases within 5 mm and 10 mm,respectively,outside the hippocampus.In the HA-PCI regimen,the D50% and D2% of PTV were 25.6 Gy and 27.1 Gy,respectively.Dmean and D2% for the hippocampus were 7.4 Gy and 9.9 Gy,respectively;D50% within 0-5.0 mm,5.1-10.0 mm,and 10.1-15.0 mm outside the hippocampus was 10.3 Gy,15.1 Gy,and 20.5 Gy,respectively.Conclusions HA-PCI may be feasible theoretically,but this needs to be confirmed by the intracranial failure pattern in patients with long-term survival.
8.Expression of tumor stem cell markers in the tumor sphere from esophageal squamous cells carcinoma Eca109

Objective To observe the expression of tumor stem cell markers P 75NTR,Oct-4,Sox-2,Lin28 and Nanog in the tumor sphere from esophageal squamous cells carcinoma Eca 109 and identify the esophageal squamous cell cancer stem cell marker .Methods The serum-free culture method was used for generating tumor spheres: proliferation was observed in enrichment culture tumor spheres .Small tumor spheres were obtained after 5 days culture and big and round tumor spheres appeared after 14 days culture which were collected for experiments and passaged .The expression and location of P75NTR,Oct-4,Sox-2,Lin28, and Nanog were detected by immunofluorescence cytochemistry .Results The expressions of P75NTR,Oct-4 and Lin28 were positive in the center of tumor spheres and some on cytoplasm and other in nuclei of Eca109 monolayer cells.However, Oct-4 fluorescence intensity was weaker than P75NTR.The expressions of Sox-2 and Nanog were positive in cytoplasm of tumor spheres and Eca 109 monolayer cells .Conclusion The cells expressing P75NTR, Oct-4, and Lin28 in the center of the tumor sphere may be esophageal cancer stem cells .
9.Tissue-engineered bone with vascular bundles or sensory nerve tracts for repairing large bone defects

BACKGROUND: Neuropeptides, a kind of endogenous active substance in nerve tissues, can modulate physiological functions of multiple body systems.OBJECTIVE: To observe the effects of vascular bundles or sensory nerve tracts implanted into tissue-engineered bone for rabbit large bone defects on the expression levels of calcitonin gene-related peptide (CGRP) and neuropeptide-Y.METHODS: Fifty-four New Zealand rabbits were enrolled to make model of large bone defects, and then, the animal models were randomly divided into three groups, including sensory nerve tract, vascular bundle, and control groups (n=18 per group), followed by implanted with sensory nerve tracts, vascular bundle, and tissue-engineered bone without sensory tracts or vascular bundle, respectively. The defected bone received gross and Masson staining at 4, 8 and 12 weeks after modeling, to compare the expression levels of CGRP and neuropeptide-Y in each group.RESULTS AND CONCLUSION: mRNA expression levels of CGRP and neuropeptide-Y in the sensory nerve tract and vascular bundle groups were significantly higher than those in the control group at different time points after modeling (P < 0.05). mRNA expression levels of CGRP and neuropeptide-Y in the tissue-engineered bone began to be increased and peaked at the 8th week, and then decreased (P < 0.05), which were the lowest at the 4th week (P < 0.05).Immunohistochemical staining results showed that CGRP was mainly found in the bridge, periosteum of newly born bones and around blood vessels; while neuropeptide-Y mainly localized in the medullary cavity and around blood vessels. These results indicate that the implantation of vascular bundle and sensory nerve tracts for bone defects can upregulate the expression levels of CGRP and neuropeptide-Y, and promote bone repair. However, sensory tract implantation may cause sensory impairment; thereafter, vascular bundle implantation is more suitable for ideal tissue-engineered construction to meet physical requirements.
10.Relationship among bone mineral density, collagen composition, and biomechanical properties of callus in the healing of osteoporotic fracture.

OBJECTIVETo study the change and relationship among bone mineral density (BMD), collagen composition and biomechanical properties of the callus in the healing process of osteoporotic fracture.
METHODSThe osteoporotic rat model and fracture model were established through bilateral ovariectomy (OVX) and osteotomy of the middle shaft of the right hind tibiae, respectively. Ninety female SD rats were randomly divided into OVX group and sham group. With the samples of blood and callus, roentgenographic and histological observation were performed for the assessment of the healing progress of the fracture, and the serum concentration of TRAP-5b, proportion of type I collagen, BMD and biomechanical properties of the callus were measured.
RESULTSThe OVX group experienced a significant delay of fracture healing. The mean serum concentration of TRAP-5b of rats in the OVX group was much higher than that in the sham group after the operation (P less than 0.05), but the difference at the same time point after fracture was smaller than that before fracture (P less than 0.05). The BMD of the callus in both groups reached the peak value at the 6 th week after fracture while the proportion of the type I collagen and the biomechanical strength reached the peak at the 8th week.
CONCLUSIONSThe deficiency of estrogen after the ovariectomy could induce the up-regulation of the osteoclasts activities, whereas the potency of further activation after fracture was depressed. Although the synthesis of collagen together with its mineralization determines the biomechanical properties of new bone, the accumulation of collagen could be assessed as an index in the prediction of biomechanical strength of bones independent of the bone mineral deposition.
