1.Coronavirus Disease 2019 Transmission: Blood Viremia and Aerosol Generation from Spinal Surgery. Is There an Increased Risk to the Surgical Team?

As a respiratory pathogen, the novel coronavirus is commonly associated with aerosol-generating procedures. However, it is currently unclear whether spinal surgical procedures pose an additional risk of viral transmission to the surgical team. We reviewed the available evidence to ascertain the presence of coronavirus disease 2019 (COVID-19) blood viremia and the virus’ blood transmissibility, as well as evidence of blood-aerosol generation and operating room contamination from spinal surgical procedures. There is established evidence of COVID-19 blood viremia, a viral pathogenic cycle via angiotensin-converting enzyme 2 (ACE-2) receptors and similar blood transmission risk data from the SARS (severe acute respiratory syndrome)/MERS (Middle East respiratory syndrome) era. Spinal surgical practices demonstrate significant blood-aerosol generation from the operative wound due to the use of common surgical instruments, such as electrocautery, as well as high-speed and high-impact devices. Based on the evidence, there is an established additional risk of viral transmission faced by surgical teams from blood-aerosols generated from the operative wound of COVID-19- infected patients via the inhalation of virus-laden aerosols and the subsequent initiation of the viral pathogenic cycle through binding with pulmonary ACE-2 receptors. Recognizing this additional risk amidst the ongoing pandemic serves as a caution to front-line surgical personnel to strictly adhere to personal protective equipment usage in operating rooms, to modify surgical techniques to reduce the hazard of surgical aerosol generation and COVID-19 viral exposure, and to consider it as an integral aspect of planning and adapting to the “new normal” operating practices.
2.The effect of thermocycling on the bonding of different restorative materials to access opening through porcelain fused to metal restorations.

PURPOSE: Porcelain fused to metal (PFM) crowns provide the best treatment option for teeth that have a large or defective restoration. More than 20% of teeth with PFM crowns or bridges require non-surgical root canal treatment (NSRCT). This may be due to the effect of restorative procedures and the possible leakage of bacteria and or their by-products, which leads to the demise of the tooth pulp. Thus, this study was planned to compare the ability of the restorative materials to seal perforated PFM specimens. MATERIALS AND METHODS: The study evaluates the ability of amalgam, composite or compomer restorative materials to close perforated PFM specimen's in-vitro. Ninety PFM specimens were constructed using Ni-Cr alloys and feldspathic porcelain, and then they were divided into 3 groups: amalgam (A), composite + Exite adhesive bond (B) and compomer + Syntac adhesive bond (C). All the PFM samples were embedded in an acrylic block to provide complete sealing of the hole from the bottom side. After the aging period, each group was further divided into 3 equal subgroups according to the thermocycling period (one week for 70 cycles, one month for 300 cycles and three months for 900 cycles). Each subgroup was put into containers containing dye (Pelikan INK), one maintained at 5degrees C and the other at 55degrees C, each cycle for 30 sec time. The data obtained was analyzed by SPSS, 2006 using one way ANOVA test and student t-test and significant difference level at (P<.01). RESULTS: The depth of dye penetration was measured at the interfaces of PFM and filling materials using Co-ordinate Vernier Microscope. The lowest levels of the dye penetration for the three groups, as well as subgroups were during the first week. The values of dye leakage had significantly increased by time intervals in subgroups A and C. CONCLUSION: It was seen that amalgam showed higher leakage than composite while compomer showed the lowest level of leakage.

3.Factors Related To Low Birth Weight Babies In Baghdad City, Iraq

Great importance has been attributed to birth weight all over the world because it is considered as one of the best predictors of prenatal survival and a good indicator of quality life. The objective of this study was to determine the prevalence of low birth weight babies (LBW) and factors related to it in Baghdad city. A cross sectional study was carried out in four general hospitals in Baghdad city, Iraq. A total of 225 newborn babies, alive, singleton and without congenital malformation were selected randomly from these four general hospitals.The result of the study showed the prevalence rate of low birth weights was 21.3%. Mothers’ educational level, monthly family income, mothers with chronic hypertension, mothers with history of previous low birth weight infants and anemic mothers were significantly associated with low birth weight babies (P= 0.03, 0.01, 0.02,<0.01, 0.02) respectively. It is clearly evidenced the lack of equity in populations and disparity in socioeconomic status are common related factors for the low birth weight babies as can be seen as a public and global health problem.

Pathogens handled in a Biosafety Level 3 (BSL-3) containment laboratory pose significant risks to laboratory staff and the environment. It is therefore necessary to develop competency and proficiency among laboratory workers and to promote appropriate behavior and practices that enhance safety through biosafety training. Following the installation of our BSL-3 laboratory at the Center for Microbiology Research-Kenya Medical Research Institute in 2006, a biosafety training program was developed to provide training on BSL-3 safety practices and procedures. The training program was developed based on World Health Organization specifications, with adjustments to fit our research activities and biosafety needs. The program is composed of three phases, namely initial assessment, a training phase including theory and a practicum, and a final assessment. This article reports the content of our training program.

Introduction: Prostate cancer is one of the most common causes of cancer deaths among men worldwide. In Malaysia however, it is the fifth leading cause of cancer among men. The increases of prostate cancer among men in Malaysia due to its close association with lack of awareness, poor knowledge and attitude. Therefore, this study is to obtain information on the prostate in terms of the level of awareness, perception, and the practice of prevention of prostate cancer among Malaysians, particularly residents of PPR Lembah Subang 1. Methods: A cross-sectional survey was collected among 200 respondents aged 18 years and above in PPR Lembah Subang 1 consisting of 37 questions comprised of socio-demographic data, source of information, risk factors, knowledge on prostate cancers, attitude on prostate cancer and practice of prevention. Likert scale scoring system used in this research. Results: Men in PPR Lembah Subang 1, show a significant association between knowledge on prostate cancer with age group, level of education, and family history showing (p value: <0.001), (p value: 0.01), (p value: 0.03), respectively. There is an association between attitude towards prostate cancer and age group and income level both with (p value: <0.001). The findings showed a significant association between the practice of prevention towards prostate cancer with level income and education showing (p value: <0.001) and (p value: 0.01) respectively. Conclusion: In PPR Lembah Subang, knowledge, attitude , and practice of prevention of Prostate Cancer found to be associated with socio-demographic risk factors.
8.A Community Based Study on Tropical Phagedenic Ulcers in Shah Alam, Malaysia: Knowledge, Attitude and Practice

Introduction: Tropical phagedenic ulcer is a skin disease caused by a cocktail of bacteria. This painful ulcer forms over the lower limbs. It is also associated with necrotic slough and foul-smelling discharge that eventually lead to amputation and permanent disabilities. Tropical phagedenic ulcer in Malaysia has not been given much attention. In light of this situation, this research was conducted to assess the knowledge, attitude and practice regarding tropical phagedenic ulcer among the public in Shah Alam, Selangor. Methods: The total respondents were 384, consisting of 164 males and 220 females who were randomly selected. Data were obtained qualitatively through structured questionnaires and analysed using the chi-square test to study the association between the dependent variables and demographic factors. Results: The collected data showed that the respondents (67.2%) had poor knowledge of tropical phagedenic ulcer; merely 65.4% considered it to be a health problem, whereas 29.7% believed it is contagious. Also, the data revealed an association between age (χ2=13.587, p =0.004), marital status (χ2=15.435, p=0.001), time spent in community (χ2=6.438, p=0.04) and knowledge of the local name of tropical phagedenic ulcer. About 74.7% of the respondents did not know the cause of tropical phagedenic ulcer. Only 22.1% of the respondents had encountered tropical phagedenic ulcer patients and an association was found between the variable with gender (χ2=4.672, p =0.031), age (χ2=24.134, p =0.000) and marital status (χ2=17.143, p =0.001). Conclusion: This study reveals misconceptions about the aetiology and transmission of tropical phagedenic ulcer which greatly influence the attitude of community members towards tropical phagedenic ulcer patients.
9.Role of locking plates in treatment of difficult ununited fractures: a clinical study.

OBJECTIVETo present our experience in treatment of difficult ununited long bone fractures with locking plate.
METHODSRetrospective evaluation of locking plate fixation in 10 difficult nonunions of long bone fractures was done. Fixation was done with locking plate for femoral shaft fracture (3 patients), supracondylar fracture of femur (gap nonunion), fracture of clavicle, fracture of both forearm bones (radius and ulna) fracture of ulna, fracture of shaft of humerus, fracture of tibial diaphysis and supracondylar fracture of humerus (one patient each). Five fractures had more than one previous failed internal fixation. One patient had infected nonunion which was managed by debridement with cast immobilization followed by fixation with locking plate at six weeks. Seven fractures were atrophic, two were oligotrophic, and one was hypertrophic. Fibular autograft was used in 2 cases and iliac crest cancellous bone graft used in all the patients.
RESULTSMinimum follow-up was 6 months (range, 6 months to 2.5 years). Average time for union was 3.4 months (range 2.5 to 6 months). None of the patients had plate-related complications or postoperative wound infections.
CONCLUSIONAlong with achieving stability with locking compression plate, meticulous soft tissue dissection, acceptable reduction, good fixation technique and bone grafting can help achieve union in difficult nonunion cases. Though locking plate does not by itself ensure bony union, we have found it to be another useful addition to our armamentarium for treating difficult fracture nonunions.

Ranking as the most communicable disease killer worldwide, tuberculosis, has accounted
with a total of 9.6 million new tuberculosis cases with 1.5 million tuberculosis-related deaths
reported globally in 2014. Tuberculosis has remain as an occupational hazard for healthcare
workers since 1920s and due to several tuberculosis outbreaks in healthcare settings in the early
1990s, the concern about the transmission to both patients and healthcare workers has been
raised. Healthcare workers have two to three folds greater the risk of active tuberculosis than the
general population. Several studies on knowledge, attitude and practices on tuberculosis among
healthcare workers worldwide have revealed that majority of the participated healthcare workers
had good knowledge on tuberculosis. Most of the healthcare workers from South India and South
Africa also reported to have positive attitude whereas a study in Thailand reported that most of
the healthcare providers have negative attitude towards tuberculosis patients. Nevertheless,
majority of the healthcare workers have low level of practice on tuberculosis prevention. An
improved communication between healthcare workers and the patients as well as their families
is the key to better therapeutic outcomes with good knowledge, attitude and preventive practice
towards tuberculosis.
