1.The Effects of Annatto Tocotrienol on Body Composition and Serum Adiponectin, Leptin and Glucose Level in a Rat Model of Androgen Deficiency Induced by Buserelin

Androgen ablation therapy using gonadotropin-releasing hormone agonists is reported to be associated with metabolic abnormalities. Annatto tocotrienol (AnTT) is reported to reduce the expression of genes related to adipogenesis but the mechanism remains elusive. This study sought to determine the effects of annatto tocotrienol on body composition (lean and fat mass), serum adiponectin, leptin, and glucose levels in male rats treated with buserelin, a testosterone ablation agent. Three-month-old male Sprague Dawley rats (n=32) were randomly divided into four groups. The normal control (n=8) was given corn oil orally daily and normal saline subcutaneously daily. The remaining groups were injected with buserelin subcutaneously (75µg/kg/day). The buserelin group (n=8) was given corn oil orally, while the treatment groups were supplemented orally with AnTT at 60 or 100 mg/kg (n = 8/group). After treatment of 12 weeks, rats were euthanized. Dual-energy x-ray absorptiometry was performed to determine the lean and fat mass of the rats. Blood was collected for the evaluation of adiponectin, leptin and glucose levels. After 12 weeks, the lean mass, fat mass, adiponectin and leptin levels for all groups increased significantly compared to their respective baseline levels irrespective of their treatment (P<0.05). All groups, except rats receiving AnTT at 60 mg/kg, experienced a significant increase in glucose level after 3 months (P<0.05). Androgen ablation and AnTT do not influence body composition, adiponectin and leptin levels in male rats. However, annatto tocotrienol at 60 mg/kg may improve glucose metabolism.
2.Relationship between Appetite, Food Intake and Body Composition among Elderly Malays from an Urban Residential Area in Kuala Lumpur, Malaysia

Loss of appetite, decrease in food intake and changes in body composition appear to be inter-related factors that can influence the well-being of older individuals.
Therefore, a study was conducted to determine the level of appetite, food intake and its relation to body composition and functional status among noninstitutionalised
elderly Malays in Cheras, Kuala Lumpur. The Council on
Nutrition Appetite Questionnaire (CNAQ), Diet History Questionnaire (DHQ), Bio-impedance Analysis (BIA) and Instrumental Activity of Daily Living (IADL)
questionnaire and handgrip dynamometer were used to measure appetite, food intake, body composition and functional status respectively. A total of 112 subjects
(41.1% men and 58.9% women) participated with mean age being 66.0 ± 5.0 years for men and 66.3 ± 6.2 years for women. Prevalence of poor appetite was higher
in elderly women (72.3%) than in men (52.3%) (p<0.05). Pearson’s correlation test showed that CNAQ score correlated significantly with age (r=-0.255, p<0.01),
energy intake (r=0.272, p<0.01), IADL score (r=0.408, p<0.01) and handgrip strength (r=0.263, p<0.05). Energy intake correlated significantly with fat free mass (r=0.424, p<0.05), muscle mass (r=0.456, p<0.05) and total body water (r=0.403, p<0.05). Multiple regression analysis showed that 27.0% of poor appetite could be
explained by advanced age, low energy intake and decreased functional status. In conclusion, the study showed that poor appetite was prevalent among the
subjects, especially women and this was influenced by aging, inadequate energy intake and decreased functional status.
4.Factors Associated with Discharge Against Medical Advice from Emergency Department, Universiti Kebangsaan Malaysia Medical Centre

Patients who self-discharge against medical advice (DAMA) are susceptible to
life-threatening consequences. By understanding the factors associated with
DAMA, healthcare centres can build strategies to assist patients to receive optimal
medical care and prevent unfavourable outcome. The objective of this study was
to determine the factors associated with DAMA from the Emergency Department
(ED) of Universiti Kebangsaan Malaysia Medical Centre (UKMMC). This was a
prospective unmatched case control study conducted over a 4-month period. For
every DAMA episode, two patients who were admitted on the same day were
randomly selected as control. Following patient consent, data was collected using
a standardized questionnaire. Patients were contacted by the investigator for
information regarding hospitalization within two weeks of DAMA. Ninety three
patients were recruited; 31 DAMA patients and 62 admitted patients. Payment
method was significantly associated with DAMA (OR 3.17 95% CI 1.29-7.98; p=0.01).
The likelihood of self-paying patients to take DAMA was three times higher than
those who had a guarantor letter from their employer or insurance provider. Other
factors which influence DAMA were family obligations (OR 4.08 95% CI 1.09-
15.26; p = 0.03) and work problems (OR 3.83 95% CI 1.13-12.94; p=0.03). A total
of 19.4% of DAMA patients left following symptomatic pain relief. A total of 80.6%
DAMA patients were admitted to hospital within two weeks of the DAMA episode.
Payment method significantly influences DAMA. Payment planning, social welfare
services, non-governmental organization funds and the introduction of a national
health policy scheme may aid hospital payment, alleviate financial limitation of
patients and reduce DAMA episodes.

5.A rare case of an appendiceal mass masquerading as a pelvic tumour and causing bilateral hydronephrosis

Diagnosing acute appendicitis in children can be difficult due to atypical presenting symptoms. While there are reported cases of acute appendicitis or appendiceal masses causing unilateral hydronephrosis, bilateral hydronephrosis as a complication of appendiceal mass is very rare. We report a case of a child who presented with cardinal symptomatology associated with the urogenital tract. Ultrasound (US) investigation showed a pelvic mass causing bilateral hydronephrosis. An initial diagnosis of a pelvic teratoma was made based on the US and computed tomography (CT) scan findings. The final diagnosis of an appendiceal mass causing bilateral hydronephrosis was established intraoperatively.

7.Bilateral Proximal Femur Fractures in a Patient with Renal Tubular Acidosis: A Case Report

The diagnosis of pathological fractures is on the rise. Themorbidity involved does not only burden the patient and theirfamilies but it has a great toll on the healthcare system aswell. Early identification of the patient at risk is aninvaluable tool to cut cost and improve the patient’s qualityof life. Multiple renal pathologies have been highlighted inrelation to the risk of pathological fractures; however,complications in renal tubular acidosis have been rarelydocumented. Nevertheless, prompt action with adequate andrelevant patient education ultimately can reduce theassociated morbidity. We present a case of poor control ofthe disease and its debilitating pathological fracturecomplications.
8.PAEDIATRIC OCULAR TRAUMA IN KUCHING, SARAWAK, MALAYSIA

Purpose: To evaluate the demography, aetiology, type and outcome of paediatric ocular trauma in tertiary centre in Malaysia.
Method: We retrospectively studied 118 eyes from 117 patients over a period of 36 months (January 2006 to December 2008).
All ocular injuries in patients aged 12 and below seen in the Ophthalmology Department for the first time were included in this
study.
Results: Mean age of patients was 6.1±3.0 years. 68 cases (58.2%) occurred in pre-school children, whereas 49 (41.9%) in
school-aged children. Boys accounted for 65.8% of cases. There was no predilection for either right or the left eye. 47% of cases
(56 patients) occurred in Malay. Most of the injuries took place at home when the children were alone (p<0.05). Sharp objects
were the commonest cause (45 cases, 38.1%). The majority of cases (103, 87.3%) were considered preventable. The
frequency of open and closed globe injury was similar. Hyphema was more common in closed globe injury compared to open
globe injury (p<0.05). Other associated injuries such as cataract, vitreous hemorrhage and retinal hemorrhage are similar
between the two groups. Visual outcome is generally poor with only 34 eyes (28.8%) had no visual impairment.
Conclusion: Ocular trauma in children is an important cause of visual loss. Most cases occurred at home and were preventable.
Prevention through education is the best approach.
9.Correct Seatbelt Positioning For Pregnant Car Occupants

Correct seatbelt wearing is a prerequisite for the protection of a pregnant woman and her baby in the event of a crash. However, because of discomfort due to large abdomen and wrong belief, pregnant women may avoid using the safety belt or may wear it not according to the correct position as recommended. This research aims to assess the overall prevalence of seatbelt wearing and the proportion of correct seatbelt wearing among pregnant car occupants. A face-to-face interview survey was conducted on 503 pregnant car occupants in Klang Valley who are visiting pregnancy clinics for their monthly check-up. Seatbelt wearing rate among pregnant car occupants was recorded high for front occupant, 90% for driver and 85% for front passenger. However, rear passenger seatbelt compliance was low, only 24% reported always wearing seatbelt when they occupy the rear seats. Despite the high compliance rate of seatbelt usage among pregnant occupants, the correct positioning of the seatbelt was only 29% of overall respondents. The findings of the study suggest low percentage of correct seatbelt usage among pregnant occupants could increase the risk of injury in event of a crash. Thus, awareness and educations needed to advocate pregnant lady on the correct adjustment of seatbelt.

The aim of this work was to study radiation and the effects of temperature on conductivity properties of polyvinyl alcohol (PVA)-based potassium hydroxide (KOH) and propylene carbonate (PC), where the ionic conduction preferentially occurs in the amorphous phase by free radicals ions through gamma-irradiation. Alkaline composite polymer electrolyte (ACPE) consisting of PVA, KOH and PC of different concentration ratios were prepared by solvent-casting technique. The ACPE were irradiated with different doses from 5 kGy up to 200 kGy. The conductivity properties of the electrolyte films were measured at different frequencies in the range 20 Hz to 1 MHz using LCR meter. The results showed that the conductivity properties were dependent on the radiation dose, temperature and the concentration of the polymer blends.
