1.Research progress on the relationship between gastrointestinal microbiota and human diseases

There are a wide variety of gastrointestinal microbiota, and the total number of bacteria reached 100 trillion. The flora interacts with the human body, and has a great impact on human health. In recent years, the relationship between intestinal flora and human diseases has become a hot research at home and abroad. Many studies have shown that intestinal microbiome plays an important role in modulating risk of multiple systemic diseases, including digestive system diseases, metabolic diseases, neuropsychiatric diseases, immune-related diseases and tumors.In this paper, the relationship between intestinal flora and human diseases was reviewed and summarized. The research progress on the relationship between gastrointestinal microbiota and human diseases in recent five years was summarized, which provided new ideas for the diagnosis and treatment of human diseases.
2.Computer aided endoscopic ultrasonography in diagnosis of pancreatic cancer

Objective To process the image of endoscopic uhrasonography(EUS)by digital imaging processing(DIP)and pattem recognition,and to evaluate its efficacy in diagnosis of pancreatic adenocarcinoma.Methods Two hundreds and sixteen patients,who underwent EUS between Feb 2005 and Feb 2007,were randomly recruited to the study.The cohort jncluded 153 cases of pancreatic cancer,which were confirmed by cytological findings after fine-needle aspiration,and 63 cases of non-pancreatic cancer(normal pancreas and chronic panereatitis).The texture features of the EUS image were selected and extracted,and cases were automatically divided into cancer and non-cancer based on findings of support vector machine (SVM).Sensitivity,specificity and accuracy of the technique were calculated.Results From each region of interest(ROI),a total of69 texture features vest in 9 sets were extracted,and 25 features with most set interval were taken as initial.The images of 216 cases were divided randomly into training set(108 eases,76 cancer and 32 non cancer)and testing set(108 cases,77 cancer and 31 non cancer).After 50 times of random tests,the average accuracy,sensitivity and specificity of the diagnosis of pancreatic cancer were (97.98±1.237)%,(94.32±0.0354)%,and(99.45±0.0102)%respectively.Conclusion DIP,combined with computer aided EUS imaging,is an accurate and noninvasive technique in diagnosis of pancreatic cancer.which warrants novel and further researches.
3.Clinical study of interrupting HBV intrauterine infection by immunization of pregnant women and infant

Objective To study the efficacy of immunization combination of pregnant women and infants in preventing intrauterine HBV infection. Methods One hundred and twenty six HBsAg positive pregnant women were divided into two groups randomly.In the combination group,126 cases were injected with hepatitis B immunoglobulin(HBIG) during pregnancy since the 28 th week of gestation and HBVac 20 ?g at 1、2、7 month combining HBIG 200 IU at 0、15 day was injected to their infants, while 90 cases pregnant women in control group were not injected with HBIG. The serum HBV IgM of pregnant women and infant were tested using ELISA and was followed up for one year. Results The incidence of intrauterine HBV infection and the rate of chronic HBV infection in the infants of combination group were significantly lower than that of the control group (19.51% vs 35.56%, 13.33% vs 3.97% respectively). The detection rate of anti HBs in newborns and protective anti HBs in 1 year were significantly higher in combination group than that in the control group (80.95% vs 0,96.03% vs 86.67%). Conclusion Combining therapy of pregnant women and infant may prevent intrauterine HBV infection effictively and increase the rate of success immunoprophylaxis significantly, and decrease the rate of chronic HBsAg infection.
4.The discrimination system of pancreatic endoscopic ultrasonography image based on M-band wavelet transfom

Objective To develop and evaluate the digital discrimination system for pancreatic ultrasound endoscopy images. Methods EUS images of 153 pancreatic cancer and 63 non-cancer cases were selected. According to the multi-fractal feature vectors based on the M-band wavelet transform, we acquired the fractal features with lower dimension with the feature screening algorithm. With the optimal feature combination, cases were classified into pancreatic cancer group and non-pancreatic cancer group automatically.Then the sensitivity, specificity and accuracy of this method were calculated, and compared with those of traditional 9 dimension fractal feature vectors. Results Three kinds of multi-fractal dimensions were introduced to the framework of M-band wavelet transform according to the EUS images to form fractal vectors of 18 dimension. With the selection by sequence forward search (SFS) algorithm, 7 dimension of feature vectors were chosen and were combined with bi-order multi-fractal dimension to a better feature combination. The Bayes, support vector machine (SVM) and ModestAdaBoost classifiers were introduced to evaluate the classification efficiency, resulting in a classification accuracy of 97.98% and short running time of 0. 49 s with lower feature dimension. Conclusion These data suggest the feasibility, accuracy, noninvasiveness and efficacy of classification of EUS images to differentiate pancreatic cancer from normal tissue based on the Mband wavelet transform algorithm. It is a new and valuable research area in diagnosis of pancreatic cancer.
5.Evaluation of global and segmental systolic functions of right ventricle in end-stage renal failure patients by real-time three-dimensional echocardiography

Objective To evaluate the global and segmental (inflow,body and outflow) systolic functions of right ventricle in end-stage renal failure (ESRF) patients by real-time three-dimensional echocardiography(RT-3DE) and four-dimensional right ventricle quantitative analysis (4D-RVQ) and to reveal the long-term impact of renal insufficiency on right ventricular systolic function.Methods RT-3DE was performed in 48 ESRF patients and 26 healthy individuals (control group).Images were acquired and analysed off-line in TomTec 4D-RVQ station.The parameters of RV global and segmental systolic functions,including RV global and segmental end-diastolic volume (EDV),end-systolic volume(ESV),stroke volume(SV),ejection fraction(EF),the time to minimum systolic volume(T-msv) were obtained.All the variables were standardized by body surface area (BSA) or heart rate (HR).Results ① In the ESRF group,EDV,SV and EF of body compartment were significantly higher than those of the controls[EDV:(8.5±3.4)ml/m2 vs (6.4± 1.7)ml/m2,P =0.002;SV:(4.5 ± 1.7)ml/m2 vs (2.7±0.9)ml/m2,P =0.0037;EF:(45.4 ± 11.5)% vs (41.1 ± 7.1)%,P =0.0489],and so were EDV,SV of inflow compartment [EDV:(19.6± 1.0)ml/m2 vs (16.5±0.9)ml/m2,P =0.0438;SV:(12.3±4.6)ml/m2 vs (10.1±3.2) ml/m2,P =0.0433].②Compared with the control group,the global and segmental T-msv were all delayed obviously in the ESRF group (P < 0.001).③The percentage of SV of body compartment accounting for the global SV significantly increased in the ESRF group [(19.5 ± 7.3)% vs (15.5 ± 5.5)%,P =0.0222],while that of outflow compartment reduced significantly [(20.3 ± 7.0)% vs (26.8 ± 8.2)%,P =0.0005].Conclusions The global and segmental systolic functions of right ventricle were impaired in ESRF patients,especially in the body compartment.The RT-3DE and 4D-RVQ are helpful technologies in the evaluation and follow-up of right ventricular systolic function in patients with renal failure.
6.Effects of enhanced recovery after surgery on postoperative recovery, psychological state of patients with laparoscopic gastrointestinal surgery

Objective To explore the effects of enhanced recovery after surgery (ERAS) on postoperative recovery, psychological state and complications of patients with laparoscopic gastrointestinal surgery. Methods Ninety-eight patients with laparoscopic gastrointestinal surgery admitted to Huzhou Central Hospital from January 2016 to December 2017 were enrolled and they were divided into two groups (49 cases in each group) according to difference in nursing. During peri-operative period, the routine nursing group was given routine nursing; while the ERAS nursing group received the nursing of ERAS. The postoperative recovery, inflammatory response and nutritional status, complications, psychological state and nursing satisfaction were compared between the two groups. Results Compared with the routine nursing group, the gastrointestinal tract exhaust time, common food intake time, incision healing time, ambulation time and hospital stay were significantly shorter in ERAS nursing group [gastrointestinal tract exhaust time (days): 2.43±1.02 vs. 3.46±1.15, common food intake time (days): 4.24±1.36 vs. 6.23±1.52, incision healing time (days): 7.62±1.54 vs. 9.63±1.63, ambulation time (days): 8.80±2.32 vs. 11.24±2.02, hospital stay (days):10.23±2.12 vs.14.56±2.37, all P < 0.05]. After operation, the high-sensitivity C-reactive protein (hs-CRP) in the two groups was increased first and then decreased, and the levels of transferrin (TRF) and prealbumin (PA) were decreased first and then increased, and the differences were statistically significant at different time points within-groups (P < 0.05), and the changes of indexes in the 5 days after operation in the ERAS nursing group were more significant than those in the routine nursing group [hs-CRP (mg/L): 27.4±6.2 vs. 35.6±9.1, TRF (g/L): 1.89±0.05 vs. 1.81±0.06, PA (mg/L):340±20 vs. 280±20, all P < 0.05]. The postoperative incidences of nausea and vomiting and sore throat in ERAS nursing group were significantly lower than those in routine nursing group [nausea and vomiting: 32.65% (16/49) vs. 67.35% (33/49), sore throat: 12.24% (6/49) vs. 51.02% (25/49), both P < 0.05], and there were no significant differences in postoperative incidences of abdominal distension, incision infection, pulmonary infection and anastomotic leakage between the two groups [abdominal distension: 4.08% (2/49) vs. 6.12% (3/49), incision infection: 4.08% (2/49) vs. 6.12% (3/49), pulmonary infection: 2.04% (1/49) vs. 6.12% (3/49), anastomotic leakage: 2.04% (1/49) vs. 4.08% (2/49), all P >0.05]. The scores of self-rating anxiety scale (SAS) at discharge in the two groups were lower than those before operation, and the degree of decrease in ERAS nursing group was greater than that in routine nursing group (39.21±4.64 vs. 46.56±4.53, P < 0.05). The nursing satisfaction rate in ERAS nursing group was significantly higher than that in routine nursing group [95.92% (47/49) vs. 77.55% (38/49), P < 0.05]. Conclusion ERAS for laparoscopic gastrointestinal surgery not only can promote the recovery of gastrointestinal function, improve the nutritional status and reduce inflammatory response and postoperative complications, but also can relieve anxiety, shorten hospital stay and elevate nursing satisfaction, therefore it is worthy to promote and apply in clinical nursing.
7.Effect of Occupational Therapy on Motor Function of Upper Limbs and Activities of Daily Living of Patients with Hemiplegia

Objective To investigate the effect of occupational therapy on motor function of upper limbs and activities of daily living(ADL)of patients with hemiplegia.Methods 51 patients with hemiplegia were randomly divided into the treatment group(n=26)and control group(n=25).Both two groups were received routine training,and besides,patients in the treatment group were added with occupational therapy.The motor function of upper limbs was evaluated by Fugl-Meyer scale before and after treatment,and ADL and hand function were also evaluated at same time.Results After treatment,the scores of Fugl-Meyer scale,ADL and hand function of all patients in two groups improved(P<0.05),but the therapeutic effect of the treatment group was superior to the control group(P<0.05).Conclusion The occupational therapy can strengthen the upper limbs and hands function,and effectively improve fine motors.
8.Challenges and countermeasures of new media dissemination in Chinese medicine culture

At present, there are many problems in the dissemination of new media of Chinese medicine culture, such as "bad communicators", "unsound dissemination of content, serious commercialization" and "less targeted communication". The reasons underlying may be related to the beginning of the new media dissemination of Chinese medicine culture, the imperfect auditing and supervision mechanism and inadequate related research. Therefore, it is necessary to strengthen the access management of communicators, strengthen the management of communication content audit, build TCM knowledge base, strengthen industry regulation, and strengthen audience research and analysis, so as to promote its sustainable development.
9.The opportunities and challenges of public traditional Chinese medicine hospitals under the comprehensive reform of medical separation in Beijing

This paper analyzed the situation separation of medicine and drug, the opportunity and challenge to put forward reasonable countermeasure and suggestion for the public hospital of traditional Chinese medicine.

The combination of field surveys and field interviews were conducted in Beijing Shunyi district Traditional Chinese medicine (TCM) hospital to investigate the work situation and find the effectiveness and the existing problems of the hospital after accepting the unified management, technical support, talent support, teaching and scientific research support from a 3As hospital. After trusteeship, the business volume of Shunyi District TCM hospital has increased,the level of scientific research and education have improved,and the service capacity of traditional Chinese medicine has been enhanced. But it is necessary to coordinate trusteeship funds, clear responsibility and power, strengthen the construction of hospital informatization, unified the use of hospital preparations and to improve the hospital trusteeship model.
