1.Conversion from Parataxis to Hypotaxis in Chinese-English Translation for TCM Classics
Journal of Zhejiang Chinese Medical University 2014;(10):1228-1230,1235
Objective] To explore the concrete policies and approaches in Chinese-English translation for TCM classics. [Methods] Transformation from parataxis in Chinese to hypotaxis in English is expounded by converting thinking modes and grammatical structure, based on analyzing the characteristics of the two languages in semantic structure and grammar, including tenses, voices, etc. as wel as their relationship with thinking modes. Sentences in English version of Huangdi Neijing(The Inner Classic of Yel ow emperor) are taken as examples. [Results] The implicit logic in Chinese can be unfolded in an explicit fashion in English by restructuring sentences, adding subbjects and conjunctions. [Conclusions] The intrinsic and mutual relationship in or between sentences in source language should be identified before Chinese-English translation for TCM classics is done. Thereafter adjusting and reconstructing can be carried out according to the thinking modes and grammatical rules in target language, achieving semantic correspondence and faithfulness.

Objective:To find the rate of anxiety and depression in peritoneal dialysis (PD) patients, and the relationship between these emotion disorder and PD processes.Method:88 PD patients were studied, their mood state was evaluated with Hamilton Anxiety Scale and Hamilton Depression Scale. Nutritional indexes, residual renal function and dialysis adequacy, and inflammation status were also recorded as well as history of cardiac-cerebral vascular disease. These patients were followed for 2 years.Result:44.3% patients were detected as anxiety, and 56.8% depression. Higher SGA was associated with higher Hamilton anxiety/depression scale. Patients with higher anxiety and higher depression had lower serum albumin, lower protein intake, lower body index, and lower mid-arm muscle area and fat area. Anxiety and depression is related to cardiac-cerebral vascular diseases and diabetes. There was no significant relationship between Hamilton anxiety/depression score and patient's survival. Conclusion:There is a high rates of anxiety and depression in PD patients and it may be a potential risk factor of malnutrition.

Objective To compare the safety and efficiency of laparoscopic transabdominal preperitoneal (TAPP) and totally extraperitoneal (TEP) procedure for herniorrhaphy. Methods Between Jan 1997 and Jan 2005, 235 patients (274 hernias) underwent laparoscopic inguinal hernia repair with 163 TAPP in 139 patients and 111 TEP procedures in 96 patients. Postoperative patients were followed-up for 6-24 months. Results No conversion was necessary. There were no significant differences between TAPP and TEP when considering operating time, postoperative hospital stay, VAS and time to return to daily activity. The recurrence rate was 1. 8% and 0. 9% respectively in TAPP and TEP( P = 0. 524). The total incidence of postoperative complications were 15. 3% and 10. 8% respectively ( P = 0. 282 ) , with seroma, transient neurapraxia and urinary retention being 5. 5% vs. 4. 5% ( P = 0. 707 ) , 5. 5% vs. 3. 6% ( P = 0.464) and 3. 1% vs. 2.7%(P = 1.000). The inhospital cost was significantly higher in TAPP than in TEP(P=0.000). Conclusion TAPP and TEP are both safe and efficient tension-free technique. Surgeons' experience was more important than choice of approach to laparoscopic repair for groin hernias.

OBJECTIVETo evaluate the clinical significance of low ligation of inferior mesenteric artery (IMA) and preservation of left colic artery in laparoscopic radical operation for rectal cancer.
METHODSClinical data of 103 rectal cancer patients undergoing laparoscopic radical operation in Ruijin Hospital from May 2015 to January 2016 were retrospectively analyzed, including 61 cases with preservation of left colic artery (low ligation group, LL group) and 42 cases without preservation of left colic artery (high ligation group, HL group). Clinical conditions during operation and after operation were compared between the two groups.
RESULTSAll the patients underwent operation successfully without transferring to laparotomy, intra-operative or post-operative death, and severe intra-operative or anesthetic complications. Age, gender, BMI, ASA score, tumor size and tumor location were not significantly different between the two groups. Four cases(9.5%) in HL group presented ischemic changes in colonic stump during operation, receiving additional colonic resection and no such ischemic changes were found in LL group (P=0.025). The number of harvested lymph node was 16.1±6.8 in HL group and 15.5±7.2 in LL group, number of harvest lymph node in the root of IMA was 4.2±1.7 in HL group with positive rate of 9.5%(4/42) and 4.3±1.7 in LL group with positive rate of 4.9%(3/61), both were not significantly different between the two groups. Lower margin was (2.2±1.4) cm in LL group and (2.8±1.7) cm in HL group, and difference was not significant as well(all P>0.05). There were no significant differences in terms of operation time, blood loss, post-operative complication, recovery of bowel movement and hospital stay. Median follow-up time was 4.5 months(2 to 10 months) and no long-term complications and local recurrence were found.
CONCLUSIONLow ligation of IMA with preservation of left colic artery in laparoscopic radical operation for rectal cancer can provide better blood supply for proximal colon and anastomosis, and can achieve same radical clearance of lymph nodes as high ligation without prolonged operation time, which is worth clinical promotion.
