1.Effect of Tegafur,Gimeracil and Oteracil Potassium Capsules on peripheral blood TPS,sMICA,HIF-1αand Her-2 in advanced cervical cancer

Objective To investigate the effect of Tegafur, Gimeracil and Oteracil Potassium Capsules ( TS-1 ) on peripheral blood tissue polypeptide antigen (TPS), soluble MHC class I chain-related molecules A (sMICA), hypoxia-inducible factor-1 (HIF-1α) and human epidermal growth factor receptor 2 ( Her-2 ) in the treatment of patients with advanced cervical cancer.Methods 80 cases with advanced cervical cancer were selected and divided into two groups, 40 cases in the control group were treated with routine clinical therapy, 40 cases in the experimental group were treated with TS-1.Tumor markers, TPS,sMICA,HIF-1αand Her-2 were compared before and after the treatment.Results Compared with the control group, the levels of tumor antigen TA-4,carcinoembryonic antigen (CEA) and squamous cell carcinoma antigen (SCCA) in the experimental group were lower (P<0.05); the TPS, HIF-1, sMICA and Her-2 were lower (P<0.05), and the total effective rate in the treatment group was higher (P<0.05).Conclusion TS-1 has a obvious clinical efficacy in the treatment of advanced cervical cancer.It is speculated that the possible mechanism may reduce the serum tumor marker levels via reducing the TPS and SMICA, HIF-1 alpha, HER-2 levels.
2.Determination of the localization of rupture aneurysms in patients with multiple cerebral aneurysms and spontaneous subarachnoid hemorrhage

Objective To discuss the reasons of false judgments of localization of the rupture aneurysms and find the way to fix this problem in patients with multiple intracranial aneurysms. Methods The clinical data of 25 consecutive patients, who presented with their first spontaneous subarachnoid hemorrhage and had multiple intracranial aneurysms from 2003 to 2009 in our hospital, were analyzed retrospectively. The rupture aneurysms were determined according to Nehls' method that reported before, and the supposed responsible rupture aneurysms w0ere clipped within 48 hours after hemorrhage in all patients. More aneurysms that could not be accessed in the same surgical session were surgically terated later. Results The location of the rupture aneurysm was verified at the time of surgery in all 25 patients. The concordance rate of the prediction and the reality of the rupture aneurysm was 80% (20/25). Four patients ( 16% ) ,in whom the ruptured aneurysm was not correctly identified,rebled after surgery,and 2 patients died as a result of the rebleeding One patients had no clear diagnosis at the end. Conclusion In the reported cases, about 80% rupture aneurysms could be correctly diagnosed before treatment according to the CT and DSA examinations. If clear diagnosis couldn't be made,additional examinations should be considered, such as CTA or MRI. Rupture aneurysms must be confirmed during the operation and the other aneurysms should be checked to exclude additional responsible aneurysms in all cases.
3.Correlation analysis of NF-κB signaling pathway activated by IL-18 in CD4+ T cells and the pathogenesis of PBC

Objective To explore the correlation between NF-κB signaling pathways activated by IL-18 in CD4+ T cells and the pathogenesis of PBC.Methods We detected the expression of IL-18 mRNA in PBMCs,IL-18 level in plasma,receptor IL-18R on surface of CD4+ T cell,proliferation rate of CD4+T cell and its NF-κB signaling pathway protein IκBα and NF-κB p65 by qRT-PCR,ELISA,flow cytometry,MACS and Western blot on 32 cases of patients with PBC (PBC group) and 32 healthy people (control group) in Guizhou provincial people′s hospital.Results The level of IL-18 in PBC group was significantly higher than that in control group (P<0.05).The relative expression of IL-18 mRNA in PBC group was significantly higher than that in control group (P<0.05).The percentage of CD4+T cells expressing IL-18Rα in PBC group was higher than that in control group (P<0.05).The proliferation rate of CD4+T cells stimulated by IL-18 in PBC group was significantly higher than that in healthy control group (P<0.01).The relative expression levels of NF-κB p65 protein were up-regulated in IL-18,and the expression of IκBα protein in each group was significantly increased,especially in PBC group (P<0.01).Conclusion IL-18 can activate NF-κB signal pathway in CD4+ T cells and participate in the pathogenesis of primary biliary cirrhosis.
4.Predictors of recurrence and prognosis in patients with stage Ⅰ and Ⅱ endometrial carcinoma

Objective To explore the high-risk clinicopathological features for the recurrence and prognosis of endometrial carcinoma diagnosed as International Federation of Gynecology and Obstetrics (FIGO) stages Ⅰ and Ⅱ.Methods Three hundreds ninety-eight consecutive patients with clinical stage I,Ⅱ endometrial adenocarcinoma underwent primary surgical therapy between Oct.1990 to Oct.2010 were studied retrospectively to analyze the correlation between clinicopathological risk factors and the disease recurrence or prognosis.Results Thirty-six patients (9.0%,36/398) developed recurrence,21 of them (58.3%,21/36) relapsed within 5 years of treatment.The results shown that the disease recurrence were significantly associated with clinicopathological factors including:age [≤50 years versus > 50 years,1.9% (2/103) versus 11.5% (34/295) ; P =0.004] ; histologic types [adenocarcinoma versus serous papillary adenocarcinoma,8.0% (27/338) versus 6/14; P =0.000] ; tumor grade [grade 1,2 (7.4%,25/338) versus grade 3 (17.9%,10/56) ;P =0.022] ; depth of myometrial invasion [none (3.4%,2/59) versus < 1/2 (7.5 %,18/240) versus ≥ 1/2 (16.2%,16/99) ; P =0.011] ; tumor size [≤ 2 cm (5.9%,17/287) versus > 2 cm (17.1%,19/111) ; P =0.000].peritoneal cytology [negative (6.9%,22/317) versus positive (23.8%,5/21) ;P =0.019] ;ER status[negative (20.8%,11/53) versus positive (5.5%,16/293) ; P =0.000] ; PR status [negative (30.0%,15/50) versus positive (4.1%,12/295) ; P =0.031].But lympho vascular space invasion (LVSI),p53 or PTEN status were not significant effect on recurrence and prognosis.Univariate analysis of survival indicated that age,histology,tumor grade,depth of myometrial invasion,tumor size,peritoneal cytology and PR status were correlated with overall survival(OS) and disease free survival (DFS ; all P < 0.05),while ER status only impact on DFS (P < 0.05).Conclusions The results showed that age,histologic type of carcinoma,myometrial invasion,tumor grade,peritoneal cytology,tumor size,ER and PR status are significant predictors for recurrence and prognosis of FIGO Ⅰ,Ⅱ stage endometrial carcinoma.Regular follow-up after treatment is essential for patients within 5 years.
5.Effect of intensive insulin treatment on healthcare-associated infection rate in acute stroke:A Meta-analysis

Objective To evaluate effect of intensive insulin treatment(IIT)on healthcare-associated infection (HAI)rate in patients with acute stroke and stress hyperglycemia.Methods Databases,including PubMed,Em-base,Cochrane Library,WanFang,and China National Knowledge Infrastructure(CNKI)Data,were electronically searched,relevant journals and references of the included literatures were also searched manually,literatures were selected according to the uniform inclusion and exclusion criteria,incidence of HAI and mean blood glucose in patients who received IIT for acute stroke were assessed systematically.Results A total of 13 randomized controlled trials (RCT)involving 1 032 patients were included in this systematic review.Meta-analysis results showed that 10 studies involving 832 patients were finally enrolled for comparing HAI rate, HAI rates in IIT group and conventional insulin treatment group were 28.3% and 56.1 %,respectively(Z =4.50 ),difference between two groups was statistically significant (RR=0.53 [95 %CI :0.40 to 0.70],P <0.001 );A total of 328 patients in 5 studies were finally included in the comparison of blood glucose,difference in mean blood glucose between two groups was statistically significant(MD =-2.52 [95% CI :-4.30 to -0.74],P =0.006).Funnel plot of HAI rate revealed that there was publication bias.Conclusion IIT is used for the regulation of stress hyperglycemia in acute stroke,it can reduce the incidence of HAI and blood glucose in patients.
6.Study on immunogenicity and arthritogenicity of a synthetic cyclic citrullina-ted peptide

Objective:To establish a synthetic cyclic citrullinated peptide induced arthritis model in mice, explore immunogenicity and arthritogenicity of this peptide. Methods: 36 DBA/1 mice were randomly divided into three groups, which were injected the type Ⅱ collagen ( CⅡ, CIA ) emulsion, cyclic citrullinated vimentin peptide ( CCit-Vim, CCV-IA ) emulsion, cyclic citrullinated vimentin peptide conjugated KLH ( CCit-Vim+KLH,CCV+K-IA) emulsion on day 0 and 21,respectively. Using arthritis index( AI) ,paw swelling to evaluate the incidence of arthritis;ELISA tested serum anti-CCit-Vim antibody,anti-CⅡantibody,anti-CCP antibody and TNF-α, IIF detected AKA;Histopathology of the ankle joint was obsearved. Results: There were three mice appeared arthritis in CCV+K-IA,the incidence rate of 25%,but arthritis occurs later time,short duration,and the incidence and extent of arthritis were lower than the CIA. CCV-IA no arthritis performance. CCV+K-IA produce anti-CCit-Vim antibody were significantly higher than those in CIA (1. 32±0. 59 vs 0. 78±0. 27,P=0. 031). While Anti-CCP antibody of CCV+K-IA were significantly lower than CIA (54. 73±7. 33 vs 64. 37±9. 91,P=0. 007). The anti-CⅡ antibody in CCV+K-IA and CCV-IA were lower than the CIA(15. 73±2. 10, 16. 71±3. 03 vs 19. 50±2. 36,P<0. 05). The TNF-α produced by CCV+K-IA and CIA were both significantly higher than the CCV-IA (645. 61±35. 26,618. 98±53. 32 vs 533. 63±79. 49,P<0. 05). The AKA positive rate of CCV+K-IA is 50% (6/12),significantly higher than CCV-IA 25% ( 3/12 ) and CIA 16. 67% ( 2/12 ) . Histopathology of the ankle showed that the CCV-IA and CCV+K-IA have a mild synovial hyperplasia,no obvious synovial pannus formation and inflammatory cell infiltration. Conclusion:The cyclic citrul-linated peptide conjugated KLH not only has stronger immunogenicity but also has arthritogenicity. It induced a higher positive rate of AKA than CⅡ.
7.Eliminat Cervical Cancer, Focusing on Primary Prevention

As one of the most severe malignant tumors, cervical cancer poses a significant threat to women. In 2020, the World Health Organization (WHO) introduced the "Global Strategy to Accelerate the Elimination of Cervical Cancer" in response to well-established tertiary prevention measures. Primary prevention measures prioritize health education and the administration of prophylactic human papillomavirus (HPV) vaccines. China currently offers five HPV vaccines supported by extensive research data specific to the Chinese population. This paper discusses the application of HPV vaccines in China and related issues that need to be paid attention to.
8.Clinical application of computer-aided design of composite materials with epoxide acrylate maleic and hydroxyapatite in cranioplasty

Objective To evaluate the effect of computer-aided design of composite materials with epoxide acrylate maleic (E) and hydroxyapatite (H) in cranioplasty. Methods A total of 45 patients with cranium defects were treated with cranioplasty by using skull bone flaps made of composite materials including epoxide acrylate maleic (E) and hydroxyapatite (H) ,which was designed with computer aid according to individual requirements. The patients were followed up for 6-36 months. Results After cranioplasty with composite EH, there occurred subcutaneous fluid in one patient and mild bone collapse in one. The composite EH showed good histocompatibility, with no infection or rejection. Conclusion During cranioplasty, use of computer-aided design of composite EH takes advantages of good accuracy, short operation time, good biocompatibility and good clinical efficiency.
9.Correlation between histone H3-K9 methylation, DNA methylation and expression of gene MGMT in Hep-2 cell line.

OBJECTIVE:
To explore the correlation between histone H3-K9 methylation, DNA methylation and expression of carcinoma suppressor gene MGMT in laryngeal carcinoma Hep-2 cell line.
METHOD:
5-Aza-dC was used to deal with Hep-2 cell cultured in vitro. ChIP, MSP and Realtime-PCR were used to detect H3-K9 methylation, DNA methylation, of MGMT gene promoter region and MGMT gene expression before and after treatment with drugs.
RESULT:
(1) In Hep-2 cell line, gene MGMT was characterized by DNA methylation and histone H3-K9 hypermethylation. (2) 5-Aza-dC was able to reduce H3-K9 methylation of MGMT gene histone in Hep-2 cell line, 5-Aza-dC was able to reverse DNA methylation of MGMT gene histone in Hep-2 cell line, 5-Aza-dC was able to upregulate the down-regulated gene expression of tumor suppressor genes MGMT.
CONCLUSION
Promoter methylation of cancer suppressor gene MGMT may induce the gene inactivity. DNA methylation may increase H3-K9 methylation. 5-Aza-dC can reduce H3-K9 methylation of tumor suppressor gene MGMT histone by reversing DNA methylation of tumor suppressor gene MGMT, and then the expression of tumor suppressor genes is increased and tumor development is inhibited.

10.Screening for cervical cancer and diversion of abnormal screening results

Cervical cancer is still a serious threat to the health of women in China. The current strategy is a three-level prevention strategy, among which the diversion of screening and screening abnormalities in the secondary prevention is an important link in preventing cervical cancer. For more than 20 years, China has implemented diversified screening methods such as cytological examination, high-risk human papillomavirus (HPV) testing, and naked eye screening. With the discovery that high-risk HPV infection is closely related to the occurrence of cervical cancer, the screening method for cervical cancer has shifted from cytological examination to HPV testing as the preferred screening method. This article introduces the advantages and disadvantages of high-risk HPV testing and cytological examination as screening methods, and proposes the issues that need to be paid attention to in screening; The principle of diverting screening abnormalities was proposed, and it was proposed that in the process of diverting, individualized and refined management principles should be implemented for screening abnormality projects based on the patient′s age and fertility requirements.
