1.Effects of Naloxone on Ischemia Reperfusion Injury in Rat Brain and Its Mechanism

OBJECTIVE:To study the influence of different doses of naloxone on focal cerebral ischemia reperfusion injury in rats and to explore its mechanism.METHODS:The rats were randomly divided into the sham operation group,the model group,the naloxone group(high dosage,moderate dosage,and low dosage)and the positive control group.The modified Longa method was applied to establish the models of focal cerebral ischemia reperfusion in rats.Infarct size,neurological deficit scores,superoxide dismutase(SOD)activity,malondialdehyde(MDA)content in serum of the model rats were detected.The ultrastructures of the brain tissues were observed by electron microscopy.RESULTS:Ischemia reperfusion injury in model rats resulted in increased neurological deficit scores,larger infarct size,decreased activity of SOD in serum and increased content of MDA.The pathological change and ultrastructural change were evident.The above indices were improved in all the groups administered with different doses of naloxone.CONCLUSIONS:Naloxone has protective effects on rat brain injury caused by focal cerebral ischemia reperfusion,whose mechanism might be related to inhibition of lipid peroxidation.
2.Establishment and application of a rapid PCR detection method of Brucella spp

Objective To establish a polymerase chain reaction (PCR) method for detecting Brucella spp .and to evaluate its ap‐plication .Methods The PCR primer aiming at the outer membrane protein(omp‐2) coding gene were designed and the PCR method for detecting Brucella spp .was established .By using DNA of 19 kinds of common bacteria and Brucella spp .,the specificity and sensitivity were evaluated .Three strains of suspected Brucella spp .were performed the nucleic acid detection .Then ,3 suspected strains of Brucella spp .were amplified by the PCR method with omp‐2 gene ,and the results were compare to the those by the cul‐ture method .Results The established Brucella spp .PCR detection method could only detect the Brucella spp .positive strain ,the control bacterium DNA did not appear the target band ;the sensitivity was 100 copies/response;the PCR method detected omp‐2 gene band in 3 strains of suspected Brucella spp .,which was identified as Brucella spp .and was same to the results by the isolation culture method .Conclusion The established PCR method for detecting Brucella spp .is accurate and rapid compare with the isola‐tion and culture method ,which is suitable for the rapid detection of brucellosis epidemic situation .
3.Radiotherapy Combing with Interventional Chemotherapy in Treating Advanced Esophageal Carcinoma

Objective To investigate the clinical value of radiotherapy combing with super selective esophageal intra-arterial infusion chemotherapy in advanced esophageal carcinoma. Methods 38 cases with medial or late stage of esophageal carcinoma wererandomly divided into two groups,the one group was treated with radiotherapy as the sole measure of treatment,while the other group was treated with radiotherapy combing with super selective esophageal intra-arterial infusion chemotherapy. All the patients were followed up within 3 to 24 months. Results In the most cases,the clinical symptoms and X-ray findings were improved obviously after treatment. But the rate of grade Ⅰof radiography was different obviously between two groups after different treatments.Conclusion Radiotherapy combing with interventional chemotherapy can increase drug concentration locally and therefore, has a better drug effect. On the other hand, it candecrease toxic reaction and can also decrease tumor volume effectively. So it can be a effective stratagem to those cases of locally advancedesophageal carcinoma which have poor reaction or severe radiotherapy reaction to merely radiotherapy.
4.Characteristics of high-pressure injection injuries of the hand

[Objective]To report the surgical managements and their outcomes in high pressure injection injuries of the hand(HPIIH) in 5 cases.[Method]Open wound technique and series debridement in all 5 cases were performed.The residual skin defects following debridement were repaired with cross-finger island flap in 1 case.[Result]From November 2001 to November 2005,5 cases of HPIIH were treated in our department and the wounds were healed satisfactorily.The flap was survived.Amputation of the finger was executed in 1 case.[Conclusion]The time of the first debridement after injury determines the prognosis.Aggressive wound debridement is a key point for good wound healing in HPIIH.
5.Effect of persistence and protophase motor nerve baby-sitting on nerve function recovery in nerve root re-implantation for brachial plexus avulsion injury

Objective To observe the effect of motor nerve baby-sitting and different babysitting stages on nerve regeneration in nerve root re-implantation for brachial plexus avulsion injury.Methods Twenty-four SD rats were used to induce the brachial plexus avulsion injury.C5-7 roots were avulsed and C6 anterior root was re-implanted to the spine cord via a posterior approach.Branches erupting from C6 and the branches affiliated to the musculocutaneous nerve were cut away via an anterior approach.Re-implanted C6 anterior root was connected singly with musculocutaneous nerve.After that,the end of one branch of medial pectoral nerve was sutured to the lateral musculocutaneous nerve.The animal was divided into persistence motor nerve baby-sitting group (Group A,n =12) and protophase motor nerve baby-sitting group (Group B,n =12) according to the random number table.Electrophysiological evoked potential,muscle wet weight and muscle fiber cross-sectional area were measured at postoperative 2 and 3 months.Results At postoperative 2 and 3 months,the restoration rates of motor evoked potential in Group A [(12.2 ± 3.3) % and (15.1 ± 3.5) %] revealed significant differences from these in Group B [(30.1 ±4.2)% and (54.8 ±9.6)%] (P <0.01),the restoration rates of muscle wet weight in Group A [(75.9 ± 5.3) % and (92.7 ± 8.6) %] were similar with (76.4 ± 7.3) % and (94.3 ± 4.4) % in Group B (P > 0.05),and the restoration rates of muscle fibrin cross section area in GroupA [(74.4±5.5)% and (88.5±7.5)%] were similar with (76.9±6.7)% and (92.9± 3.5) % in Group B (P > 0.05).Conclusions Motor nerve end-lateral baby-sitting can improve functional restoration of the skeletal muscle in the early stage after nerve root re-implantation,but prevent functional restoration of skeletal muscle in the late stge.On the contrary,protophase motor nerve babysitting avoid the inhibitory effect in the late stage,which gains better restoration rate of muscle motion evoked potential.
6.Apparent Diffusion Coefifcient and Diffusion Weighted Imaging Findings in Portal Vein Tumor Thrombus Caused by Hepatic Carcinoma

Purpose To explore the findings and diagnostic values of apparent diffusion coefficient (ADC) and diffusion weighted image (DWI) of the portal venous tumor thrombus (PVTT) caused by hepatic carcinoma.Materials and Methods Thirty-one patients with hepatic carcinoma (43 lesions) with 63 PVTT in the main branches and trunks diagnosed by clinical and MRI were enrolled. All patients underwent conventional MRI (cMRI) imaging, DWI and ADC imaging, the features of cMRI, DWI and ADC were observed, the relevance of ADC values between the hepatic carcinoma lesion and PVTT were analyzed.Results Among the total 43 lesions, DWI image showed low signal, iso-signal and high signal in 1, 4 and 38 lesions, respectively; and ADC image showed low signal, iso-signal and high signal in 36, 5 and 2 lesions, respectively. In the total 63 PVTT, DWI showed low signal, iso-signal and high signal in 4, 7 and 52 lesions, respectively;while their ADC images showed low signal, iso-signal and high signal in 54, 6 and 3 branches, respectively. There was good consistency for the results of two observers on the findings of ADC of tumor lesions (Kappa=0.8334,P<0.05), and a moderate consistency on that of PVTT (Kappa=0.5215,P<0.05). The average ADC value of tumor lesion and PVTT was (1.127±0.268)×10-3 mm2/s and (1.021±0.363)×10-3 mm2/s, respectively; there was a correlation of the mean values of ADC between tumor lesion and PVTT (r=0.246,P<0.05). Conclusion The features such as low signal and low value on ADC image and high signal on DWI obtain a certain clinical application value for qualitative diagnoses of PVTT.
7.The Synthetic Treatment in Vertebral Hemangioma

Objective To assess the efficacy of radiotherapy and synthetic therapy for vertebral hemangiomas.Methods Clinical symptoms,imaging findings,therapeutic methods and following up after treatment in 22 cases with vertebal hemangiomas were retrospectively analyzed.Results Complete relief of symptoms(CR)were occurred in 19 of 22 patients(86.4%) and partial relief(PR)were achieved in 3 of 22 patients(13.6%).There was no obvious complication of radiotherapy and radiation-induced neoplasms.There was no relapse.Conclusion For vertebral hemangioma,the synthetic therapy which includes intenventional therapy before operation as well as radiotherapy after operation is of obvious superiority in relieving compression rapidly,reducing hemorrhage during operation and obtaining good efficacy.

Objective To evaluate the value of CT scan in patients with obstructive sleep apnea syndrome (OSAS) pre and post surgical operations.Methods 36 patients with OSAS and 50 normal adults were included in this study .The patients were divided into two typesaccording to the location of stricture site .The upper airway from roof of nasopharynx to glottis was evaluated using a Siemens Somatom Spiral Scanner and divided into upper pharyngeal and lower pharyngeal. Objective:To study the effect of OSAHS on adult nasal-cycle.Method:The nasal cycle of 20 healthy adults,18 patients of snoring and 22 patients of OSAHS were examined with acoustic rhinometry,which was performed every 30 minutes in 7 hours.Result:The ratio of nasal cycle in healthy adults was(19/20,95.0%), in snoring patients was(15/18,83.0%),in patients of OSAHS was(15/22,68.2%). The mean alteration amplitude of nasal cycle in healthy adults was significantly larger than that in patients with OSAHS (P<0.05).The distribution of the healthy adults and patients with OSAHS between the typical cycle categories was significantly different(P<0.05).Conclusion:The characteristics of nasal-cycle in of OSAHS patients were different with healthy adults,which maybe owing to change of physical function of nasal cavity.
10.Protective Specimen Brush via Fiber Ptic Bronchoscope in Severe Pneumonia:Its Application Value

OBJECTIVE To understand the application value of protective specimen brush(PSB)in severe pneumonias.METHODS Fifty seven cases diagnosed as severe pneumonia were admitted into the group.The specimens were sampled via the fiberoptic bronchoscope(FOB)and the PSB and then were done bacteria quantitative culture(QC).According to these,we adjusted antibiotics.RESULTS With the PSB and bacteria QC technologies,45 strains were detected in 57 cases,with a positive rate of 78.95%.In the progress of operating FOB-PSB examination,patients life signs didn't change significantly.FOB-PSB's sensitivity and specificity were about 97.73% and 84.62%,respectively.The therapy efficiency rate was 91.23%.CONCLUSIONS It is a good choice to use the PSB for sampling in severe pneumonia via fiberoptic bronchoscope.
