1.Effect of Chinese herbal medicine combined with acupuncture and massage on knee osteoarthritis

Objective To investigate the effect of Chinese herbal medicine combined with acupuncture and massage on the treatment of knee osteoarthritis. Methods A total of 92 patients with osteoarthritis of the knee were enrolled in our hospital from September 2015 to March 2017, and were randomly divided into study group and control group (46 cases) by single and double numbers (admission order).The control group was treated with acupuncture and massage, and the study group was treated with acupuncture and massage on the basis of external use of Chinese medicine. The two groups of patients with knee osteoarthritis were treated with continuous treatment for 6 courses, the pain degree and the change of osteoarthritis index were recorded before and after the treatment. Results There was no significant difference in NRS and WOMAC scores between the two groups before treatment. The scores of NRS and WOMAC in patients with knee osteoarthritis after treatment were significantly lower than those in the control group, the difference was statistically significant(P<0.05). Conclusion The application of Chinese medicine, acupuncture and massage in the treatment of proliferative osteoarthritis of the knee can achieve more satisfactory clinical efficacy, and is conducive to protect the quality of life.

3.Changes of regulatory T cells and their associated cytokines in patients with chronic myeloid leukemia

Objective To investigate the changes in the proportion of regulatory T (Treg) cells and in the levels of cytokines secreted by these cells in the peripheral blood in the patients with chronic myeloid leukemia (CML). Methods The enrolled subjects consisted of 30 CML patients who were newly diagnosed , 20 CML patients who were under the effective treatment of tyrosine kinase inhibitors (BCR-ABL 210 transcript ratio is below 10%) and 20 healthy donors whose age and sex were matched .Flow cytometry was used to detect CD4+CD25 high CD127 low /-Treg cells and CD4+ T cells.The enzyme linked immunosorbent assay was used to determine the plasma concentra -tions of interleukin-10(IL-10), transforming growth factor-β1(TGF-β1) and IL-35.Results The proportions of Treg cells in CD4+ T cells were similar among the three groups .As concerns the three kinds of Treg-associated cy-tokines, there were no significant differences in the plasma concentrations of IL -10 among the three groups.Howev-er, compared with the treatment group and the control group , the plasma concentrations of TGF -β1 and IL-35 in the newly diagnosed patients significantly increased (P <0.001), with no significant difference between the treat -ment group and the control group.Conclusion Though the proportion of Treg cells did not significantly change in the newly diagnosed patients, the plasma concentrations of TGF-β1 and IL-35 indeed significantly enhanced , sug-gesting the dysfunction of Treg cells in the newly diagnosed patients might be associated with the progression of dis -ease.Effective treatment of tyrosine kinase inhibitors could down -regulate the plasma levels of these cytokines to baseline, suggesting that monitoring these cytokines might evaluate the efficacy of therapy .
4.Factors limiting microsurgical total resection of meningiomas on tentorium and confluence of sinuses

Objective To study the factors that limited the microsurgical total resection on meningiomas of tentorium and confluence of sinuses Methods Twenty three cases of meningiomas on tentorium and confluence of sinuses,which were removed by application of microsurgery from 1983 to 2001,were analysed retrospectly and reviewed. Results In all 23 cases,tumors in 17 cases were removed totally,and 6 cases were removed subtotally In the 6 cases whose tumors were resected subtotally,there were 4 cases related with major sinus and its venous flow uncompleted compensation, 2 cases related with adhering compactly to the brain stem invaded by tumors,major vessels or cranial nerves Conclusions Heading factors restricting microsurgical total resection of menigiomas on tentorium and confluence of sinuses may be related with major sinus invaded by tumor and its venous flow uncompleted compensation, tumors adhering compactly to the brain stem,major vessels or cranial nerves
5.Microsurgical separation of the protruded cerebral tissue in the case of complex craniopagus twins using the neuronavigation system

Objective To report the microsurgical separation of the protruded cerebral tissue in the case of complex craniopagus twins and evaluate the role of the neuronavigation system in this operation Methods After the multiple bone flap were made,register again and then separate the cerebral tissue along the arachnoid plane under the guiding of the neuronavigation system(SNN) until the straight sinus was exposed Results The protruded cerebral hemisphere and cerebellar hemisphere of the twins were successfully separated Unfortunately,part of the occipital lobe of twin B(right) had to be removed because of the brain swelling Conclusion In the case of complicated craniopagus,the neuronavigation system is valuable in positioning the deep common sinus To separate the cerebral tissue along the arachnoid plane under surgical microscope can reduce the cerebral injury
6.Resection of pituitary adenoma invading the cavernous sinus by transsphenoidal microsurgery

Objective To study the criteria and technique of transsphenoidal microsurgery on pituitary adenoma invaded cavernous sinus Methods 42 patients with pituitary adenoma underwent 45 transsphenoidal operations Sellar base was opened as large as possible under C arm X ray fluoroscopy or neuronavigation monition Dura matter was cut open under microscope Tumors were removed through the direction and tunnel they in invade Bromocriptine was administered to those with little remains Partial remains and those have no response to bromocriptine were given radiotherapy Results No modality 21 tumors were nearly totally removed, 18 tumors were subtotally resected, 3 were partially resected MRI scanned in 2~3 months after the operation showed that 19 tumors disappeared, 20 tumors had little residul, 3 tumors partially remained Conclusion Pituitary adenomas invading the cavernous sinus with enlarged sella turcica and shown soft in MRI can be submitted to transsphenoidal operation Microsurgical technique can ease tumor removal and protection of the sellar and internal carotid artery
7.Endoscope-assisted transsphenoidal approach for middle and lower clival extension of pituitary adenomas

Objective To explore the microsurgical skills and clinical value of endoscope-assisted transsphenoidal approach for middle and lower clival extension of pituitary adenomas. Methods The sublabial transseptal,transsphenoidal approach assisted by endoscope were applied in 16 cases which invasive middle and lower clivus.CT or/and MRI and pituitary hormones were detected after microsurgery. Results The tumor was achieved approximately total removal in 13 cases and subtotal removal in 3 cases.There were 8 functioning pituitary adenomas and the pituitary hormones decreased to normal level in 3 cases.PRL level decreased more than 80% in 4 cases and less than 80% in 1 cases. Conclusion Endoscope-assisted transsphenoidal approach for middle and lower clival extension of pituitary adenomas can achieve satisfactory effect.

Objective To investigate the local-regional recurrence of nasopharyngeal carcinoma (NPC) 5 years after conventional radiotherapy and its prognosis.Methods From August 1989 to Decem ber 1999,1384 patients with newly histo-pathologicatly diagnosed NPC were treated with conventional radia tion.350 out of 1277 followed-up patients were diagnosed as local-regional failure.The intervals between completion of radiation and tumor relapse ranged from 6 months to 171 months.There were 62 patients with local-regional recurrence 5 years after the radiotherapy,including 41 in nasopharynx,19 in neck and 2 in na sopharynx and neck simultaneously.Thirty-seven patients with late local-regional recurrence received a sec ond course conventional radiotherapy.Results The median survival time was 44 months(95% CI = 30.4 -57.6) of patients with re-irradiation comparing with 14 months (95% CI = 7.1-20.8) of those without. The 5 year survival rate after re-irradiation was 42%.Conclusions Local-regional recurrence of nasopha ryngeal carcinoma can oeoure 5 years after radiotherapy.Second course converntional radiotherapy possesses good results.
10.Clinical features and long-term follow-ups of juxtapapillary retinal capillary hemangioma

Objective:To evaluate the clinical features and follow-up effect of treatment for retinal capillary hemangioma (RCH) of myopic disc.Methods:Retrospective case series studies. A total of 14 patients (15 eyes) diagnosed with juxtapapillary RCH (JRCH) at Peking Union Medical College Hospital and followed up for more than 12 months from January 2007 to December 2019 were included. Among these patients, there were 7 males (7 eyes) and 7 females (8 eyes) with the average age of 29.8±12.7 years. There were 6 isolated JRCH cases, all of which were monocular patients, including 1 case with a single peripheral RCH; 8 cases with von Hippel-Lindau disease, including 5 cases of binocular RCH. Out of 15 eyes, 6 eyes were merged with peripheral RCH. Three eyes were simply observed, and 12 eyes were treated with photodynamic therapy (PDT) alone or PDT combined with other treatments. The average follow-up time was 55.2 months. Its clinical characteristics and long-term treatment effects were analyzed.Results:At the first visit, among the 15 eyes, 3 eyes had visual acuity <0.1, 5 eyes were 0.1-0.4, and 7 eyes were> 0.5. The endogenous and fixed exogenous JRCH were 12 (80%, 12/15) and 3 (20%, 3/15) eyes, respectively. The size of the tumor was 1/4 to 4 optic disc diameters (DD); the combined macular edema, epiretinal membrane, and macular hole were 11, 5, and 1 eyes, respectively. There were 3 eyes with tumor diameter less than 1 DD. The visual acuity at the first diagnosis was 0.6-1.0, and no treatment was given. The follow-up time was 12 months in 2 eyes and 120 months in 1 eye. At the last follow-up, his vision remained stable and the tumor did not enlarge. Six eyes with tumor diameter ranging from 1.0 to 2.5 DD, visual acuity was 0.06 to 0.6 at first diagnosis, accompanied by retinal exudation. 5 eyes were treated with PDT alone, and 1 eye was treated with PDT combined with anti-vascular endothelial growth factor drugs. The follow-up time was 12 to 84 months. At the last follow-up, the visual acuity improved or stabilized in 5 eyes and decreased in 1 eye. There were 6 eyes with peripheral RCH, including 4 eyes with retinal detachment, 2 eyes with epiretinal membrane, and 1 eye with vitreous hemorrhage. The visual acuity at the first visit was 0.02-0.6. All patients were treated with PDT combined with transpupillary thermotherapy, extrascleral drainage, and vitrectomy. The follow-up time was 12 to 132 months. At the last follow-up, all eyesight decreased.Conclusions:Visual decrease cause by JRCH often occurs at young adults. Bilateral JRCH are more common in patients with von Hippel-Lindau's disease. The tumor can cause retinal exudation, macular edema, epiretinal membrane and macular hole, resulting in gradual vision loss. PDT and/or combined anti-vascular endothelial growth factor drugs for medium sized JRCH with retinal exudation can maintain long-term vision stability. Patients with large JRCH or severe complications such as retinal detachment tend to have poor prognosis.
