Objective To investigate the effect of Zhuang medicated thread moxibustion on postherpetic neuralgia model rats, and to verify the mechanism of Th1/Th2/Th17 cytokine balance regulation in central nervous system. Methods 50 SD rats were randomly divided into blank group, model group,thread-moxibustion without drugs group,Zhuang medicated thread moxibustion group and pregabalin group, with 10 rats in each group.Except for blank group, the rat model of postherpetic neuralgia was induced by a single intrapitoneal injection of resin-toxin(RTX).After successful modeling, each group was given corresponding treatment for 21 days. The Mechanical Pain Threshold(PWT)and Thermal withdrawal latency(TWL)were detected 3 days before modeling,1,4 and 7 days after modeling, and 3,7,14 and 21 days after intervention. After the intervention, extract spinal cord tissue,Flow cytometry was used to detect changes in the proportion of Th1/Th2/Th17 cells; The mRNA expression levels of IFN-γ,IL-2,IL-4,IL-10,IL-17 and IL-22 were detected by qPCR;The positive expressions of IFN-γ,IL-2,IL-4,IL-10,IL-17 and IL-22 were observed by immunohistochemistry. The protein expressions of IFN-γ,IL-2,IL-4,IL-10,IL-17 and IL-22 were detected by Western blot to elucidate the regulation effect of this therapy on the equilibrium mechanism. Results After intrapitoneal injection of resin-toxin,the PWT of rats showed a downward trend, while the TWL showed an upward trend, showing the separation of heat and pain, which was consistent with the clinical manifestations of PHN.After Zhuang medicated thread moxibustion therapy,PWT increased and TWL decreased in PHN rats, which reduced the separation trend of heat and pain.Thread-moxibustion without drugs group,Zhuang medicated thread moxibustion group and pregabalin group can promote Th1 response in the central nervous system(spinal cord),increase the proportion of CD4+IL-2+cells and the contents of IFN-γ and IL-2 in the spinal cord tissue, and up-regulate the expression of IFN-γ and IL-2 mRNA and protein.Meanwhile,Th2 and Th17 responses in the central nervous system were inhibited, and the proportions of CD4+IL-4+and CD4+IL-17A+cells and the contents of IL-4,IL-10,IL-17 and IL-22 in the spinal cord tissue were reduced. The mRNA and protein expressions of IL-4,IL-10,IL-17 and IL-22 were down-regulated.In addition,Zhuang medicated thread moxibustion group and pregabalin group had similar efficacy in correcting Th1/Th2/Th17 lymphocyte imbalance in the central nervous system, and both were better than the Thread-moxibustion without drugs group. Conclusion Zhuang medicated thread moxibustion can improve the separation trend of heat and pain in postherpetic neuralgia rats, which may be achieved by regulating Th1/Th2/Th17 cell balance and related cytokine levels.

In order to solve the problem of high incidence of work-related musculoskeletal disorders (WMSDs) in upper limb lifting workers, a wearable auxiliary device which could be used in upper limb lifting operation was designed. The auxiliary device could be used in upper limb lifting through torsion spring device to reduce the tension of the arm muscles. The back bracket could be adjusted to adapt to the wearers of different height. The 3D model of the auxiliary device was constructed by using SoildWorks software. And the relative position and constraint between the auxiliary device and the digital simulation human were adjusted by using Jack as the main simulation tool. And then the virtual model of upper limb lifting were simulated and analyzed. Through the comparison of Rapid Upper Limb Assessment (RULA) scale, Krist comfort score and simulation results of two-handed reachable domain before and after wearing the auxiliary device, which proved that the wearable auxiliary device could reduce the risk of WMSDs in upper limb lifting workers.

6.Selection of antiviral drugs for patients with hepatitis B virus-associated hepatocellular carcinoma
Journal of Clinical Hepatology 2022;38(11):2452-2456
Hepatitis B virus (HBV) infection is a key risk factor for the development and progression of hepatocellular carcinoma (HCC) and it promotes the progression of HCC by inducing liver fibrosis, genetic and epigenetic alterations, and the expression of active virally encoded proteins. Many studies have confirmed the influence of viral status on the prognosis of HCC, and effective antiviral therapy can help prevent or delay the recurrence of HBV-associated HCC, increase the overall survival time of patients undergoing radical or systemic, and improve the prognosis after multimodality therapy. Therefore, correct selection of antiviral drugs is crucial for the antiviral treatment of patients with HBV-associated HCC. This article briefly reviews the selection of antiviral drugs for patients with HBV-associated HCC, so as to provide a reference for clinical practice.

Objective To investigate the safety, feasibility and clinical application effect of upper arm totally implantable venous access port (TIVAP) inserted by the combination of oncologists and nurses in patients with tumor. Methods A total of 34 patients, who needed long-term transfusion treatment, were included in this study with upper arm TIVAP from March 2017 and December 2017.There were 20 males and 14 females. The median age was 63 (35 ~ 83) years.Upper arm TIVAP was implanted by both doctors and nurses into the patients with tumor, and the TIVAP related success rate, complications and patients satisfaction were recorded. Results 34 patients all succeeded in TIVAP implantation with the operation success rate of 100%.The average operation time was about 40 minutes (30 to 60 minutes) from the disinfecting cloth to the end of the suture. The operations of all patients were successful. After the operation two patients died of cancer progression, one patient had soft tissue infection around capsular bag,None of the patients had other complications such as blocked infusion, catheter shift, port reversal, and so on,and the incidence of complication was 2.94% (1/34).Conclusions Upper arm TIVAP has the advantages of safe puncture, shorter operation time, few intra-operative and post-operative complications and higher feasibility for operation by both oncologist and nurse, which can supplement the limitations and deficiencies of the chest wall port and PICC in a certain extent, therefore is a good choice for clinical application.

Objective:
To investigate the efficacy and drug related adverse reactions of sorafenib and sunitinib as first-line tyrosine-kinase inhibitors (TKIs) for patients with metastatic renal cell carcinoma (mRCC) and analyze the clinical prognostic factor for survival.
Methods:
The data of 271 patients with metastatic renal cell carcinoma who had complete clinicopathological data were retrospectively analyzed, including 174 cases in sorafenib group and 97 cases in sunitinib group, to access patients′ overall survival (OS) and progression-free survival (PFS). Prognostic values of all characteristics were determined by using univariate and multivariate Cox regression models.
Results:
The objective response rates (ORR) of the sorafenib and sunitinib groups were 14.9% and 19.6%, respectively, and the disease control rates (DCR) were 85.1% and 88.6%, respectively. No significant difference was found between the sorafenib and sunitinib group in ORR (
