1.Efficacy of lateral collateral ligament reconstruction surgery combined with arthroscopy in treatment of chro-nic lateral ankle instability

Objective To study the clinical efficacy of lateral collateral ligament reconstruction surgery com-bined with arthroscopy in treatment of chronic lateral ankle instability.Methods 60 cases with chronic lateral ankle instability were selected,the patients were divided into the study group and the control group according to a random number table,which has 30 cases in each group.The control group was received conservative treatment, the study group was underwent exploratory arthroscopic ankle cleanup combined with peroneus brevis tendon lateral ligament reconstruction surgery.Scores before and after treatment of the two groups were detected in American bare foot and ankle surgery-hindfoot score system( AOFAS) .And excellent rate,the recurrence rate and complication rate between the two groups were compared.Results AOFAS score before treatment of the two groups had no significant difference (P>0.05),AOFAS score after treatment of two groups were significantly higher,and that of the study group was sig-nificantly higher than the control group,the difference was statistically significant (t=9.837,10.873,8.789,all P<0.05);Excellent rate of the study group was 96.7% (29/30) significantly higher than the 73.3% (22/30) in the control group,recurrence rate of the study group was 0.0% (0/30),significantly lower than the 13.3% (4/30) in the control group,the complication rate of the study group was 3.3%(1/30),significantly lower than the 20.0%(6/30)in the control group,the difference was statistically significant (χ2 =13.982,15.037,13.298,all P<0.05). Conclusion Lateral collateral ligament reconstruction surgery combined with arthroscopy in treatment of chronic lat-eral ankle instability has better clinical efficacy,the recurrence rate is low,and has less complications.
2.Surgical Treatment of Congenital Heart Diseases with Unilateral Absence of Pulmonary Artery

Unilateral absence of a pulmonary artery in congenital heart disease may be considered as a risk factor for the surgical treatment of the basic anomaly especialy in cyanotic type of patients. This paper presents 9 such cases: tetralogy of Fallot in 7, dextrocardia and PDA in 1 and truncus arterio-sus with persistent left superior vena cave in one. Absence of left pulmonary artery was present in 7 cases and of right in two. Corrective surgical treatment were performed on 7 patients with tetralogy of Fallot. Two of them died in the early postoperative period. It was recommended that valved extracardiac conduit or woven dacron patch with pericardial monocusp be used for reconstruction of the outflow tract of right ventricle to alleviate the pulmonary regurgitation which was aggravated by higher pulmonary vascular resistance. Appropriate and active management of postoperative low cardiac output is also vital important.
3.Evaluation of Renal Peripheral Inflow Perfusion in Patients with Nephrotic Syndrome by Region of Interest Quantification

Purpose To investigate renal cortical blood perfusion in patients with nephrotic syndrome (NS) by region of interest (ROI) quantification.Materials and Methods Twenty-one patients with NS treated at Wuhan Union Hospital from September 2014 to January 2015 (NS group) and twenty-nine matched healthy subjects (control group) were enrolled in our study.High frequency blood flow dynamic images of the left kidney for all the subjects were acquired by color Doppler (Philips iU22).Resistance index (RI) of interlobar artery,arcuate artery and interlobular artery were measured.Vascularization index (VI),flow index (FI),vascularization flow index (VFI) were further analyzed by ROI quantification at Q-LAB work station.Results ① The signals of renal cortical blood flow in NS patients were sparse and slim;while the signals in the control group were continuous,most of which reached the edge of renal cortical.② No statistical differences of RI values of interlobar artery and interlobular artery were found between the two groups (P>0.05);③ The renal cortical blood perfusion in the NS patients decreased significantly compared with that in control group,and the renal cortical VI,FI and VFI values in NS group were significantly lower (P<0.01).④ Bland-Altman analysis showed that there was good repeatability in the measurement of renal cortical VI,FI and VFI.Conclusion Color Doppler flow imaging combined with RO1 quantitative technique can assess renal cortical blood perfusion accurately,which provides diagnostic information for early detection of renal damage.
4.Assessment of atrial septum morphology by live three-dimensional echocardiography.

To evaluate the morphology of atrial septum by the live three-dimensional echocardiography (L3DE) and its value of clinical application, L3DE was performed in 62 subjects to observe the morphological characteristics and dynamic change of the overall anatomic structure of atrial septum. The study examined 49 patients with atrial septal defect (ASD), including 3 patients with atrial septal aneurysm, and 10 healthy subjects. ASD in the 35 patients was surgically confirmed. The maximal diameters of ASD were measured and the percentages of area change were calculated. The parameters derived from L3DE were compared with intraoperative measurements. The results showed that L3DE could directly and clearly display the morphological features of overall anatomic structure of normal atrial septum, repaired and artificially-occluded atrial septum, atrial septal aneurysm. The defect area in ASD patients changed significantly during cardiac cycle, which reached a maximum at end-systole and a minimum at end-diastole, with a mean change percentage of 46.6%, ranging from 14.8% to 73.4%. The sizes obtained from L3DE bore an excellent correlation with intraoperative findings (r=0.90). It is concluded that L3DE can clearly display the overall morphological features and dynamic change of atrial septum and measure the size of ASD area accurately, which is important in the decision to choose therapeutic protocols.

5.Assessment of the effect of cardiomyocyte transplantation on left ventricular remodeling and function in post-infarction Wister rats by using high-frequency ultrasound.

The effects of cardiomyocyte grafting on left ventricular (LV) remodeling and function in rats with chronic myocardial infarction were evaluated using high-frequency ultrasound. Chronic myocardial infarction was induced in 50 Wister rats by ligating the left anterior descending artery. They were randomized into two groups: a trial group that received neonatal rat cardiomyocyte transplantation (n=25) and a control group which were given intramyocardial injection of culture medium (n=25). The left ventricular (LV) geometry and function were evaluated by high-frequency ultrasound before and 4 weeks after the cell transplantation. After the final evaluation, all rats were sacrificed for histological study. The results showed that 4 weeks after the cell transplantation, as compared with the control group, the LV end-systolic dimension, end-diastolic dimension, end-systolic volume and end-diastolic volume were significantly decreased and the LV anterior wall end-diastolic thickness, LV ejection fraction and fractional shortening were significantly increased in the trial group (P<0.01). Histological study showed that transplanted neonatal rat cardiomyocytes were found in all host hearts and identified by Brdu staining. It was suggested that transplantation of neonatal rat cardiomyocytes can reverse cardiac remodeling and improve heart function in chronic myocardial infarction rats. High-frequency ultrasound can be used as a reliable technique for the non-invasive evaluation of the effect of cardiomyocyte transplantation.

6.Chinese patent of anti-infective urinary catheter

BACKGROUND:Catheter associated urinary tract infection is a difficult problem for clinical practice management, and its key pathogenesis is the bacterial biofilm formation on the surface of the catheter material. Therefore, developing a new anti-infective urinary catheter has become an area of interest in the current studies of anti-infective biological materials. OBJECTIVE:To review the research literatures on anti-infective urinary catheter, and provide a direction for further study and clinical application. METHODS:Al related Chinese patent papers of anti-infective urinary catheters were retrieved by Google’s proprietary search platform (http://www.google.com/advanced_patent_search) until the deadline of March 26, 2014, with the search strategy of‘Return the patents with the fol owing proprietary name:urinary catheter’. RESULTS AND CONCLUSION:According to the predefined search strategy, 949 potential y relevant patent papers were screened out for further identification, and 23 papers referred to anti-infective catheters that were obviously eligible were included. The analyses showed that:(1) The antibacterial coating agents of the majority of papers were antibacterial agents of nano-inorganic metal cations, only four papers used antibiotic coated. (2) The drug-eluting catheters were mainly composite-coated. (3) The drug release modes from coating were mainly extended-release but release mechanism was not clarified. (4) The preparation process was chemical bond or ionic bond in one paper, blending methods in one paper, repeated electroplating in one paper, electrospinning technology in one paper, and physical impregnation methods in 12 papers (52.17%). (5) The antimicrobial mode was ultrasonic-antibacterial method in two patent papers, sterile sleeve in one paper, hydrophilic coating in one paper, catheter made by blending polymer material and anti-infective agents in one paper, drug coated films made by coating with antimicrobial drug liquid and drying process in 20 papers (82.61%). In conclusion, there have been no translational and applied clinical researches about the anti-infective urinary catheter, and the relevant researches were only at the laboratory level. The research methods of Chinese patent for anti-infective urinary catheter were limited, and need to be further improved.
7.Study on hemorrheological changes in 7 cases with portal hypertension after distal splenocaval shunt

The hemorrheological changes were observed in 7 cases with portal hypertension after they were operated on with distal splenocaval shunt (DSCS). Similar cases with splenecto-my served as controls. It was found that blood hyperviscosity was slightly decreased after DSCS and the deformability of erythrocytes was improved from 7 to 14 days after DSCS. But blood hyperviscosity was significantly increased in the cases after splenectomy. Blood hyperviscosity was lower in the cases with DSCS than in those with splenectomy. Blood hyperviscosity , expressed mainly by the aggregation rate and deformability of erythrocytes, platelet count, etc, is dependent on the spleen. Since the spleen is preserved in DSCS so blood hyperviscosity is slightly decreased and complications due to blood hyperviscosity may be less in number after DSCS.

10.Study on the Plasma Concentration Determination and Pharmacokinetics of TanshinoneⅡA in Beagle Dogs

OBJECTIVE:To establish a method for determining plasma concentrations of tanshinone ⅡA and use it for pharma-cokinetics study of tanshinone ⅡA in Beagle dogs. METHODS:HPLC was performed on the column of Phenomenex Luna C18, with the mobile phase of water(5 mmol/L of ammonium acetate+0.05% H3PO4)-acetonitrile at flow rate of 1.0 ml/min;the detec-tion wavelength was 270 nm,column temperature was 40 ℃ and the volume was 20 μl. 9 Beagle dogs were randomly divided into tanshinone ⅡA low,medium and high dose groups(2,4 and 8 mg/kg),blood was respectively taken from forelimb venous blood to determine the plasma concentration before and after 2,5,10,15,20,30,45,60,75,90 and 120 min of iv administration. DAS 2.0 software was used to calculate the pharmacokinetic parameters. RESULTS:The linear range of tanshinoneⅡA was 0.097 5-12.50 μg/ml (r=0.999 8);the RSDs of precision and stability tests were all less than 10%;the method recovery was 100.4%-107.2%,and extraction recovery was 90.2%-92.3%. The t1/2αin tanshinoneⅡA low,medium and high dose groups were respective-ly(1.01±0.27),(1.03±0.46)and(1.51±0.65)min;t1/2β were respectively(16.25±4.78),(22.42±9.32)and(24.45±12.02) min;AUC0-120 min were respectively(150.88±45.25),(305.44±92.55)and(643.67±178.27)μg·min/ml;and CL were respective-ly(12.01±4.36),(12.78±5.06)and(12.17±5.41)ml/(min·kg). CONCLUSIONS:The precision,stability and recovery of the method are all in line with the determination requirements of biological samples,and tanshinone ⅡA showed a good linear relation-ship with dose in Beagle dogs in AUC0-120 min.
