1.Diagnostic value of conventional and ultrasound-guided fine-needle aspiration biopsy for thyroid nodules:a Meta-analysis

Objective:To compare the diagnostic value of conventional fine needle aspiration biopsy(C-FNAB) and Ultrasound-guided FNAB(USFNAB) for thyroid nodules by Meta-analysis.Methods: PubMed and CNKI database were searched for English and Chinese language articles related to FNAB diagnosis for thyroid nodules.The literatures were assessed and screened according to the Standards for Reporting of Diagnostic Accuracy(STARD).Data were pooled by Metadisc software.Results: A total of 8 810 specimens were included in conventional FANA group and the pooled sensitivity,specificity,and the AUC of weighted summary receiver operating characteristic(SROC) curve were 84.9%,76.3% and 0.919 4,respectively;2 393 specimens were included in US-FNAB group and the above numbers were 84.0%,80.8% and 0.935 6,respectively.Conclusion: FNAB has high accuracy in diagnosing thyroid nodules;the accuracy of ultrasound-guided FNAB is higher than conventional FNAB.

3.Comparision of three methods in treatment of osmidrosis

Objective To compare the therapeutic effects of three operative procedures,removal of armpit skin,minimally-invasive aspiration and small transverse incision in the treatment of osmidrosis.Methods Retrospective review was conducted in the treatment of osmidrosis for near ten years.Three methods were compared in the indication,feature and effectiveness.Results A total of 459 cases (123 males and 336 females) of osmidrosis were treated with the three methods.Satisfactory rates of the three methods were 98%,96% and 97%.respectively.Recurrence was found in 7 cases.Conclusions Modified procedure of minimally invasive aspiration has advantages of quick recovery,less complications and satisfactory results.It is the first choice for patients with mild and moderate osmidrosis.Bat for male patients with severe or recurred osmidrosis,treatments are surgical removal of the skin with armpit hairs and suture method.
4.Application of transcatheter arterial embolization in the intractable epistaxis

Objective To evaluate the value of transcatheter arterial embolization(TAE) in the treatment of intractable epistaxis.Methods TAE using gelform or polyvinyl alcohol(PVA) particles of forty-one patients with intractable epistaxis were undertaken by the femoral artery approach,through selective catheterization of involved maxillary artery or the bleeding arteries for the stopage of bleeding..Results Of the forty-one patient,39 cases were cured by once TAE and the other 2 with recurrent bleeding on the next day after the TAE,to whom a second interventional treatment fullfilled the requirement.Conclusions Transcatheter arterial embolization is a simple,safe and effective treatment for the intractable epistaxis.
5.Protection of Lazaroid U-74389G pretreatment against graft warm ischemia reperfusion injury in rat liver transplantation from non-heart-beating donors

ObjectiveTo investigate the protective role of Lazaroid U 74389G pretreatment against graft warm ischemia reperfusion injury of rat liver transplantation from non heart beating donors.MethodsRat liver transplantation was carried out in 4 groups (N 45, N 60, tN 45 and tN 60), according to pretreatment with U 74389 G or not, and the non heart beating time of 45 or 60 minutes before donor liver was harvested.Survival rates, liver functions, MDA values and graft pathologies were compared between the groups.ResultsThe one week survival rates of group N 45, N 60, tN 45 and tN 60 were 25%(2/8), 0(0/8), 58%(7/12) and 33%(4/12) respectively.U 74389G pretreatment not only significantly increased survival rate of rat liver transplantation from non heart beating donors, but also improved liver functions and graft pathologies, as well as decreased MDA expression.Conclusions U 74389G pretreatment could attenuate ischemia reperfusion injury on donor livers and increase the survival rates in rat liver transplantation from non heart beating donors.

8.Curative effect observation of capecitabine combined with intensity modulated radiotherapy for posto-perative mediastinum lymphonode metastasis in esophageal carcinoma

Objective To evaluate the efficacy and toxicity of capecitabine combined with intensity modulated radiotherapy( IMRT)for postoperative mediastinal lymph node metastasis in esophageal cancer. Methods A total of 62 esophageal cancer patients with postoperative mediastinal lymph node metastases were randomly divided into the irradiation group(A group,31 cases)and the capecitabine combined with IMRT group( B group,31 cases). Both of two groups received IMRT radiotherapy with a total dose of 60-66 Gy, 30-33 times in 6-6. 5 weeks. The patients in B group were treated with capecitabine(1 250mg/ m2 ,2 f/ d, d1-d14,21 d × 2 cycle). Results Group A:there were 7 cases of complete response(CR),12 of partial response(PR),10 of stable disease(SD),and 2 of progressive disease(PD);the effective rate was 61. 3%(19 / 31). Group B:there were 10 cases of CR,16 of PR,4 of SD,and 1of PD;the effective rate was 83. 9%(26 / 31). There was a statistical significance between the effective rates of A group and B group(χ2 = 3. 971, P < 0. 05). Our experiment showed that the rates of grade Ⅱand Ⅲ myelosuppression in group A and group B were 29. 0% and 38. 7%(χ2 = 0. 648,P = 0. 421). The rates of Ⅰ and Ⅱ level radioactive pneumonia in group A and were 19. 4% and 25. 8%(χ2 = 0. 369,P = 0. 544). The different incidence of the two adverse reactions between group A and B had no statistical significance. Conclusion Compared with IMRT alone,IM-RT combined with capecitabine may have better curative efficacy without increasing toxicity to esophageal cancer patients with postoperative mediastinum lymphonode metastasis.
9.Endovascular therapeutic strategy for multiple occlusive lesions of aorta and iliac-femoral arteries

Objective To explore the endovascular therapeutic strategy for multiple occlusive lesions of aorta and iliac-femoral arteries, and to discuss the technical skill as well as the clinical significance. Methods A total of 8 patients with multiple occlusive lesions of aorta and iliac-femoral arteries were enrolled in this study. Preoperative CT angiography and MR angiography were performed in all the 8 patients. The lesions included complete occlusion of abdominal aorta below renal artery level (n = 2), distal abdominal aorta occlusion (n = 4), distal abdominal aorta stenosis (n = 1), distal abdominal aorta membranous occlusion (n = 1), and diseased iliac artery (n = 12), external iliac artery (n = 8), femoral artery (n = 1) and popliteal artery (n = 2). Endovascular interventional management, including opening channel, thrombolysis, balloon dilation, stent implantation, etc. was carried out via different routes. The results were analyzed. Results After endovascular interventional management the abdominal aorta was completely reopened in all the 8 patients. Of 12 diseased iliac arteries, 9 were successfully reopened by interventional treatment and the remaining 3 were not treated. All the diseased external iliac arteries were opened up. The involved femoral artery and popliteal arteries were not treated. The patients were followed up for 1 -12 months. During the follow-up period, ischemic symptoms of the lower limb disappeared in 5 patients and were obviously improved in 2 patients. Recurrence of thrombotic occlusion was observed in one case, which returned to normal after transcatheter thrombolysis therapy. Conclusion For the treatment of multiple occlusive lesions of aorta and iliac-femoral arteries, endovascular interventional management is safe, simple and effective with fewer complications. The ischemic symptoms of the lower limb can be significantly improved.
