1.Research advancement of humidification in patients with artificial airway during mechanical ventilation

In order to improve the quality of humidification in patients with artificial airway during the mechanical ventilation, ensure the quality of artificial airway nursing. From the way of humidification , the choice of humidification liquid, the volume of humidification liquid control, the temperature of humidification liquid and the evaluation of humidification effect to research the present situation of humidification of artificial airway during mechanical ventilation. Put forward best way of patients with artificial airway nursing during the mechanical ventilation is wetting by temperature humidity of the Breathing machine′s and at the same time to strengthen the monitoring of the airway of humidification effect.
2.Minimally invasive transverse process plasty for Bertolotti's syndrome

[Objective] To gain more insight into the presentation,diagnosis of Bertolotti's syndrome and to investigate the indications,the procedure and the early clinical results of minimally invasive transverse process plasty.[Methods]Thirteen cases(Santavirta Type I)with Bertolotti's syndrome underwent minimally invasive transverse process plasty under MED X-tube(Medtronic Sofamor)from Apr.2006 to Dec.2008.The Visual Analogue Scale(VAS),Modified Oswestry Disability Index(ODI)score and subjective sensation of patients were recorded to evaluate the function.[Results]The operation blood loss was from 120 ml to 800 ml and operative time was from 60 to 280 minutes,average 115minutes.There was no severe complication.Nine cases were followed up for 10 to18 months,average 13.6 months.The VAS decreased from(6.7?2.2)before operation to(2.2?0.7)one week after operation and(2.8?0.9)three months after operation.ODI score decreased from(27.8?3.6)before operation to(5.9?2.2)three months after operation.The results were excellent in 10 case and good in 3 cases.[Conclusion]Minimally invasive transverse process plasty is an effective method for Bertolotti's syndrome,especially for Santavirta type I.It shows less blood loss and injury and deserves to be spreaded.Disectomy is recommended in one stage if the patient is complicated with disc herniation.
3.Myostatin Gene Expression Level in the Soleus Muscle of Tail-Suspended Rats

Objective To study the effect of tail-suspending on myostatin(MSTN)expression in the soleus muscle of rats.Methods The MSTN mRNA levels in soleus muscle of tail-suspended and normal control rats were measured and compared.Results The MSTN mRNA in soleus muscle of tail-suspended rats was 2.2 times(hind limb unloading for 14 days)and 3.5 times(hind limb unloading for 30 days) higher than that in normal control rats (P<0.05).The relative wet weight of the soleus muscle in tail-suspended rats was decreased by 11%(unloading 14 days)and 1 9%(unloading 30 days),respectively,compared with that in normal control rats.Conclusion The expression of MSTN mRNA is elevated in the soleus muscle of tail-suspended rats,probably due to atrophy of the soleus muscle.
4.Evaluation of the analgesic effects of preoperative peripheral nerve block using two kinds of local anaesthetics in patients with trigeminal neuralgia undergoing radiofrequency thermocoagulation of gasserian ganglion

Objective:To evaluate the analgesic effects of two kinds of preoperative peripheral nerve block drug in patients with tri-geminal neuralgia undergoing radiofrequency thermocoagulation(RFT)of gasserian ganglion.Methods:90 patients with classic trigemi-nal neuralgia of the 3rd branch were scheduled to undergo RFT of the gasserian ganglion and were randomly divided into 3 groups(n =30).The patients in group N without nerve block were served as the controls.The patients in group A and B were treated by the nerve block of inferior alveloar with 2 ml of 2% lidocaine hydrochloride and 2 ml of compound lidocaine hydrochloride for each respectively 2 d before RFT.The pain control was studied by VAS method before and 24,48 h after nerve blocking and during pucture for RFT.Re-sults:Compared with the control group,24 h after nerve blocking,the average pain and the most pain VAS value of group A and B were significantly reduced(P <0.01);sleep quality and analgesia satisfaction were improved in group A and B(P <0.05).48 h after nerve blocking the pain relief and sleep quality improvement of group B were more than those of group A(P <0.05).In addition,during RFT puncture the pain intensity of group A and B were less than that of group N(P <0.05).The pain control in group B was more effective than that in group A.Conclusion:A single peripheral nerve block with long-term narcotic compound lidocaine hydrochloride can be an effective way to relieve preoperative and operative pain for RFT of trigeminal neuralgia.
5.The risk factor profile of pneumonia caused by pandrug-resistant Acinetobacter baumannii in intensive care unit

Objective To investigate the risk factor profile of pneumonia caused by pandrug-resistant Acinetobacter baumannii (PDRAB)in intensive care unit for better distinction between PDRAB infection and colonization.Methods The clinical data of 99 patients diagnosed as A.baumannii pneumonia between February 2009 and March 2012 in Fangcun Hospital,a branch of Guangdong Provincial Hospital of Traditional Chinese Medicine,were reviewed and analyzed retrospectively.The risk factors associated with pandrug-resistance in A.baumannii were examined with multivariate logistic regression analysis.Results ICU stay,duration of invasive ventilation,longer duration of treatment with carbapenems,beta-lactam/beta-lactamase inhibitor,or quinolone antibiotics,use of ≥3 classes of antimicrobial agents,≥3 invasive procedures (including indwelling gastric canal, catheterization,central venous catheter,arterial sheath),and fiberoptic bronchoscopic treatment were associated with PDRAB pneumonia (P <0.05).Multivariate logistic regression analysis showed that use of ≥3 classes of antibiotics,fiberoptic bron-choscopic treatment,and longer duration of treatment with beta-lactam/beta-lactamase inhibitor were independent risk factors predicting the emergence of PDRAB pneumonia.Conclusions We should pay more attention to these risk factors in clinical prac-tice for better differentiation between PDRAB infection and colonization so that appropriate control measures can be taken pro-actively.
6.Research on demand analysis and implementation of hospital consultation management information system

7.Analysis of Micronuclei Induced by Nickel Chloride in Peripheral Blood Reticulocytes in Mice Using Flow Cytometry

Objective To detect micronuclei (MN) induced by nickel chloride (NiCl2) using a flow cytometry-based method, so as to study the mutagenecity of this chemical. Methods The micronucleated reticulocytes (MN-RET) in peripheral blood of NIH mice exposed to 0.75 mg/kg colchicines (COL), 40.0 mg/kg of cyclophosphamide (CP) and 5.0, 10.0 and 20.0 mg/kg of NiCl2 were detected by a flow cytometry-based method, in which anti-CD71-FITC and PI were used to stain cells and malaria-infected erythrocytes were employed as biological standards of micronuclei to optimize cytometry. Results Although COL or CP induced significant increase in the frequency of MN-RET in peripheral blood, MN formation in peripheral reticulocytes was not significantly affected by exposing to different concentration of NiCl2. Conclusion The results of flow cytometric analysis in the present paper do not support that NiCl2 exposure may induce significant MN-RET increase in peripheral blood in NIH mice.
8.Application of combined hemihepatic infow and hepatic veins occlusion in major hepatectomy

Objective To evaluate the effect of application of combined hemihepatic inflow and hepatic veins occlusion in major liver resection.Methods The clinical data of 46 cases of large hepatic carcinoma who underwent liver resection were surveyed retrospectively.The hepatic pedicle of affected side and hepatic veins were dissected and controlled initially,then hepatectomy was performed under the condition of vascular exclusion of the affected side of liver.Results All the 46 cases suffered from hepatocellular carcinoma.The average size of the tumors was 8.3 cm(6-15 cm) in diameter.One main hepatic vein was invaded by tumor in 20 cases and 2 main hepatic veins were invaded by tumor in 14 cases.Among the 46 hepatectomies,right hemihepatectomy was performed in 16,right posterior lobe hepatectomy in 14,and left hemihepatectomy in 16 cases.The mean occlusion time of the hemihepatic pedicle was 30 min(10-45 min),and occlusion time of the hepatic veins was 20 min(10-30 min).The average blood loss was 540 ml(300-1 500 mL).Postoperative complications occurred in 14 cases,and all recovered after treatment.There was no mortality in this series.Conclusions Combined hemihepatic inflow and hepatic veins occlusion in major hepatectomy is a safe,effective and practical vascular exclusion method which can effectively reduce the blood loss and the incidence of the liver function failure.
9.Local excision in the treatment of the duodenal ampullary tumor

Objective To evalute the diagnosis and the availability of local excision for the duodenal papilla tumor(DPT).Methods The clinical data of 12 patients with DPT treated by local excision in recent 9 years were retrospectively analysed.In this series,jaundice presented in 100% of patients and always accompanied with abdminal pain.Results There was no operation death in this seties.The preoperative diagnostic rate by biopsy was 75.0%. Ten patients(83.3%)were followed up for 1-5 years,5 patients with benign tumor were survival well.Four patients with early stage T 1 DPT were still survival for 62,31,84and 36months, respectively.Conclusions The indications of local excision for DPT include benign papillary tumor smaller
10.Microanatomical study of the horizontal segment of facial nerve in temporal bone

Objective To explore the microanatomy of the horizontal segment of the facial nerve (FN)in temporal bone and its adjacent structures,in order to provide microanatomical datag for the clinical works.Methods In 20 adult wet skull specimens(40 temporal bones),horizontal segment of FN and their adjacent structures in temporal bones were observed according to operational layers by simulating transmagtoid approach.Results The lenglh of the horizontal segment was[(8.85±1.01)(7.10~11.25)]mm;The diameter of the horizontal segment wag[(1.88±0.65(1.55~1.90)]mm;The angle opening towards anterior direction between horizontal segment and vertical segment wag[(115.5±6.89)(109.5~128.6)]..The ansh towards posterior direction between horizontal segment of FN and tympanic tegmen wag [(28.5±3.66)(25.8~31.5)°;The vertical distance from the cochlear form process to the horizontal segment of the FN wag[(1.89±0.58)(0.90~3.05)]mm;The vertical distance from the head of the stapes to the horizontal segment of the FN wag[(2.30±0.85)(1.97~3.11)]mm;The distance between the summit of pyramidal segment of FN and the apex of shor limb of incus wag 2.55±0.21(2.10-2.90)mm;The distance between the summit of pyramidal segment of FN and the eminence of the lateral semicircuiar canal wag[(2.86±0.31)(2.23~3.56)]mm;No branching or dislocated nerve of the horizontal segment of the FN wag found in all 40 cases.Five percent(2/40)distal part of the horizontal segment of FN tracks forward lateral to the eminence of the literal semicircular canal.Conclusion Geniculate ganglion,cochlearform process,stapes,tympanic tegmen,lateral semicircular canal,incus are important landmarks by which horizontal segment of FN might be located in manipulation of mastoid;The relations of horizontal segment of FN and its adjacent structures are complicated,compact.It will be good that the manipuliter absorb a great deal of anatomical knowledge about landmarks that can be used in locating horizontal segment of FN.
