1.The study on immunity and tumor recurrence of oral cancer patients with the combination therapy of hy-perbaric oxygen treatment and postoperative adjuvant chemotherapy

Objective To study the treatment influences on immunity and tumor recurrence of oral canc -er patients with the combination therapy of hyperbaric oxygen treatment and postoperative adjuvant chemotherapy . Methods In the period from March 2011 to March 2014 ,84 patients were clearly diagnosed with oral cancer and randomly divided into hyperbaric oxygen treatment combined postoperative adjuvant chemotherapy group ( group A,n=42)and postoperative adjuvant chemotherapy group (group B,n=42).From day 1 prior to the last day of chemotherapy ,patients in group A were treated with hyperbaric oxygen ,and patients in group B were just accepted chemotherapy.After chemotherapy,we tested the levels of CD3 +,CD4 +,CD8 +and NK cells by flow cytometry, and the expression of IL -2,IL-4,IL-10 and INF-γby ELISA essay.All patients had been followed for two years.Results The levels of CD4 +and NK cells in group A were higher than group B (P<0.05),but the levels of CD3 +and CD8 +between the two groups were obviously different (P>0.05).Compared with group B,the ratio of CD4 +and CD8 +was increased in group A(P<0.05).The levels of cytokines involved IL -2,IL-4,IL-10 and INF-γin group A were higher than the levels of group B ( P<0 .05 ) .Follow-up results showed that pa-tients treated with hyperbaric oxygen combined postoperative adjuvant chemotherapy had less recurrence in 2 years(P<0.05).Conclusion Hyperbaric oxygen combined postoperative adjuvant chemotherapy can improve the patients′immune function ,and to be effective in preventing two years′recurrence .
2.Exploration of the cause of short P-R interval syndrome

Objective To explore the causes of short P R interval syndrome.Methods 15 patients of short P R interval syndrome were tested with transesophageal atrial pacing (TEAP),and then the results and the parameters of TEAP were analyzed.Results The detection rates of dual atrioventricular nodal pathways(DAVNP),L G L syndrome and latent preexcitation syndrome were 66.67%(10/15),26.67%(4/15)and 6.67%(1/15)respectively.Conclusion The most common causes of short P R interval syndrome are DAVNP and the L G L syndrome

To observe the effect of IL-2 gene modification on the character of biology and function in dendritic cells( DC) and to investigate the immune mechanism of specific anti-tumor of IL-2 gene modification in DC. Methods: DCs were prepared from mouse bone marrow and genetically modified by IL-2 adenovirus. Then observe the changes of DC morphology by scanning electro-microscopy, analyzed molecules on DC by FACS, examined the expression of IFN-? mRNA in DC by RT-PCR.The stimulatory capacity of DC to T cells detected by MLR their capacity of antigen present were measured by 3H-TdR mix into assay. Results: After IL-2 gene modification, the morphology of DC was changed, its pseudopod was more and longer. The expression of Ia, B7-1, B7-2 and CD40 molecules was more on DC surface. The IFN-7 mRNA was expressed in the DC-EL-2 and DC-rhIL-2.DC-IL-2 could stimulate allogeneic T cells more potently and IL-2 gene-modified DC could induce more potent antigen-specific autogeneic CTL. Cooclusioo: IL-2 genetic modification can promote DC growth and up regulate their expression of membrane immune molecules that are relevant for antigen presentation of DC,and enhanced the biologic activity of the DC.

5.Study on Acupoint Selection Principles of Facial Paralysis Treatment for Acupuncture and Moxibustion Based on Ancient Literature

Objective To investigate principles of acupoint selection for the ancient acupuncture and moxibustion treatment of facial paralysis.Methods The ancient literature about acupuncture and moxibustion was collected and reviewed from the pre-Qin period to the late Qing Dynasty. According to the data selection criteria, acupuncture and moxibustion prescriptions of facial paralysis were collected and input into an ancient acupuncture and moxibustion prescription database. By use of frequency analysis and factor analysis, the principles of acupoint selection were obtained.ResultsTotally 257 prescriptions were collected which involve 74 acupoints. Through frequency analysis, 19 acupoints were obtained whose frequencies were equal or greater than 9, such as Dicang (ST 4), Jiache (ST 6), Tinghui (GB 2), Shuigou (GV 26), Hegu (LI 4), etc. Through factor analysis of acupoint variables, 6 common factors were obtained.Conclusion Based on analysis of the common factors, the principles of acupoint selection are summarized which were according to meridian, part and etiology for the ancient acupuncture treatment of facial paralysis. The results could provide a reference for clinical acupuncture and moxibustion treatment. It is feasible that factor analysis was applied to study principles of acupoint selection for acupuncture and moxibustion prescriptions.
6.Study on application of hospital -community -family -individual 4 -level intervention model on schizophrenia patients

Objective To observe the influence of hospital -community -family -individual 4 -level intervention model on schizophrenia patients in compliance,recurrence rate,psychotic symptom,social function,quali-ty of life and provide guidance for disease management.Methods 198 schizophrenia patients were divided into 4 -level intervention model group and conventional intervention group.After 6 moths follow -up,the effect was evaluated by compliance rate,recurrence rate,BPRS,WHO -DASII,SQLS.Results After the intervention for 6 months,the compliance,recurrence rate,the difference of severity of psychotic symptom,the difference of social function of the 4 -level intervention model group were 96.74%,13.04%,(7.57 ±2.83)points,[understanding and communication (6.32 ±3.64)points,physical exercise (5.16 ±3.28)points,self care (2.87 ±1.38)points,get on well with others (9.05 ±3.51)points,life activity (5.42 ±2.49)points,social participation (7.85 ±3.28)points],which of conven-tional intervention group were 74.74%,28.42%,(4.86 ±1.24)points,[understanding and communication (3.89 ± 1.86)points,physical exercise (2.80 ±1.238)points,self care (2.19 ±1.16)points,get on well with others (5.83 ± 3.04)points,life activity (3.41 ±1.96)points,social participation (4.27 ±2.17)points],the differences between the two groups were significant (χ2 =9.020,P =0.003;χ2 =6.932,P =0.009;t =8.527,P =0.000;t =5.775,P =0.000;t =6.554,P =0.000;t =3.652,P =0.000;t =6.712,P =0.000;t =6.144,P =0.000;t =8.829,P =0.000).The differences of before intervention and after intervention in the 4 -level intervention model group were psychological society (16.03 ±6.75)points,power and energy (15.79 ±6.41 )points,symptoms and side effects (19.37 ±7.28 )points,which in the conventional intervention group were psychological society (12.65 ± 5.46)points,power and energy (11.81 ±4.71 )points,symptoms and side effects (15.18 ±6.29 )points,the differences were statistically significant between the two groups (t =3.770,P =0.000;t =4.849,P =0.000;t =4.216,P =0.000).Conclusion Hospital -community -family -individual 4 -level intervention model has a positive effect on schizophrenia patients,which is worthy of clinical promotion.
7.Growth differentiation factor 5 induces adipose-derived stem cells differentiating into chondrocytes

BACKGROUND:Growth differentiation factor 5 (GDF5) has been shown to play a crucial role in the development of chondrocytes via different regulatory mechanisms.
OBJECTIVE:To explore the effect of GDF5 on the chondrogenic differentiation of adipose-derived stem cel s (ADSCs).
METHODS:ADSCs in passage 4 isolated from Japanese White rabbits were cultured in the medium containing 0 (blank control group), 10, 50, 100, 150 and 200μg/L, respectively. The morphological changes were observed, and the optimal concentration of GDF-5 was screened, and its round coverslips at 1, 2 and 3 weeks of culture were selected to undergo immunocytochemistry, toluidine blue staining and immunofluorescent staining.
RESULTS AND CONCLUSION:The growth of ADSCs was stable in the medium containing 10 and 50μg/L GDF5, while apoptotic cel s appeared after induction with 200μg/L GDF5. In the medium containing 100 and 150μg/L GDF5, some spindle-shaped cel s changed into irregular shape, and became round or oval after 7-10 days of culture. The optimal concentration of GDF5 was 100μg/L. The cel s transfected by 100μg/L GDF5 were positive for type II col agen obviously and with blue-stained nucleus. The transfected cel s were positive for toluidine blue, and metachromatic granules were visible in the cytoplasm. The proteoglycan mRNA expression of the transfected cel s was significantly increased, and reached the highest at 3 weeks. These results suggest that GDF5 promotes the chondrocytic differentiation of ADSCs.
8.Health education in community health-care service in Shanghai

Objective To study current status of health education and explore its pattern in community health-care service(CHS)in Shanghai.Methods A total of 200 health-care workers(HCWs)from five CHS centers in Shanghai were investigated by questionnaire of health education knowledge.Results Among 200 HCWs investigated,96.5%(193/200)of them have recognized importance of health education,98.0%(196/200)could undertake health education in their clinical work,only 60.5%(121/200)had receired regular training on it,and 85.5%(171/200)thought they should be further trained regularly.Correct awareness of knowledge was 94.O%.47.0%and 33.6%for health education perception,healthy life-style and health education knowledge for chronic disease.respectively in them.There was very significant difference in correct awareness of knowledge between HCWs with varied ages and districts(χ2=17.663 and 82.376,P=0.001 and 0.000,respectively),but no significant difierence was found between men and women and those with varied professional titles(χ2=0.015 and 2.406,P=0.903 and 0.300.respectively).Short of time(48.5%),lack of relevant knowledge(15.0%)in HCWs and poor compliance of patients(33.0%)were main factors influencing their implementation of health education.Conclusions HCWs in CHS have already recognized importance of health education.but have not known enough its theoretical knowledge and skills.It is necessary to set guidelines of health education intervention for HCWs in CHS,and perfect monitoring and evaluation.as well as mechanism for rewards and penalties to promote development of health education in CHS.
9.Application of early total care in treatment of unstable pelvic fractures

Objective To evaluate the clinical effect of early total care (ETC) for treatment of unstable pelvic fractures.Methods This retrospective case series study included eighteen patients with unstable pelvic fractures treated surgically from May 2013 to May 2015.There were 11 male and 7 female patients,aged (45.1 ± 13.1)years.Eleven patients were injured due to road traffic accidents and 7 due to fall from height.Tile B type pelvic fractures were seen in 8 patients and Tile C type in 10 patients.All patients were treated with ETC method and received open reduction and internal fixation within 24 to 48 hours after the primary treatment.Data were collected,such as operation time,intraoperative blood loss,hospital length of stay and perioperative complications.Majeed score and EuroQol 5 dimension (EQ-5D) were used for evaluation of clinical outcome and quality of life respectively.Excellent and good rate of operation was assessed using the Matta clinical evaluation standard.Results Three patients died and fifteen patients survived.Operation time was (120.1 ± 22.2)minutes,amount of intraoperative blood loss was (355.9 ± 56.4) ml,and hospitalization was (8.5 ± 1.6) days.Incidence of perioperative complications was 56%,including 3 patients with acute respiratory distress syndrome,1 pneumonia,2 acute lung injury,1 incisional wound infection,2 multiple organ failure and 1 diffuse intravascular coagulation.All patients were followed up for (15.1 ±2.4)months (range,12-21 months).Majeed score was decreased from preoperative (93.7 ± 6.1) points to final follow-up of (74.1 ± 9.2) points,and the EQ-5D index was decreased from preoperative 0.96 ± 0.04 to final follow-up of 0.74 ± 0.19 (both P < 0.05).Excellent and good rate of operation was 80%.Conclusions ECT provides satisfactory short-term outcome for treatment of unstable pelvic fractures,but the incidence of complications is high.Doctors should choose reasonable treatment plan according to the surgical indications.
