1.THE EFFECTS OF SODIUM SELENITE ON THE FUNCTION OF DENDRITIC CELLS FROM PERIPHERAL BLOOD OF CHRONIC HEPATITIS PATIENTS

Objective To explore the methods of improving the immune function of dendritic cells(DCs) from patients with chronic type B hepatitis in vitro. Methods Monocytes from type B hepatitis patients and healthy controls were incubated with GMCSF and IL-4 and IFN-? to induce dendritic cells(DCs) generation.Then the nutrien-concentration sodium selenite was added for co-culture before the maturity of DCs.The samples were separated into three groups: hepatitis-B patients (group A),hepatitis-B patients with Se(group B),healthy control(group C).The phenotypes of DCs including CD80,CD86 were characterized by flow cytometry(FCM) and the response of allogeneic lymphocytes was detected by MTT assay.The concentration of cytokines in the supernatant such as IL-12 was tested by ELISA assay. Results After the nutrien-concentration Se was added for co-culture,the proliferation ability of isotype xenoma lymphocyte stimulated by DCs was obviously streng thened compared with group A,and the excretion level of IL-12 also increased(P

Since the discovery of the calpain family,more and more studies have demonstrated their close relationship with cell apoptosis. In addition to the consideration of their unique distribution in the cardiomyocytes,we are enthralled by their implication of a novel direction for both the diagnosis and the treatment of various heart diseases.
3.Constructions and functions of C-type natriuretic peptide and its receptor

C-type natriuretic peptide (CNP) shows remarkable sequence homology to atrial natriuretic peptide (ANP) and B-type natriuretic peptide (BNP) within the 17-residue ring portion formed by a pair of cysteine residues and being the third member of the natriuretic peptide family. Sequence analysis reveals that there are at least two exons in the coding region for preproCNP. Mature forms of CNP in body are CNP22 and its N-termi-nally elongated form CNP53. As an agonist, CNP selectively bind to natriuretic peptide receptor B (NPR-B) which has a single member-spanning helical domain and exert its biological effects by activating its C-teminal guanylate cyclase to catalyse formation of cyclic guanosine monophosphate (cGMP). CNP can also bind to natriuretic peptidereceptor C (NPR-C) which has high binding affinities for all of the natriuretic peptides. The synthesis and release of CNP are regulated by many cy-tokines, and CNP is metabolized by two main pathways : internalization and degradation through the binding with NPR-C or hydrolysis by neutral endopeptidase. CNP and its receptor distribute widespreadly in various tissues including vessels, blood and central neural systems, and produce effects of vasodilation, antianxiely effect and regulations of cell proliferation and endocrine.

CD44 is a transmembrane glycoprotein,which is a family of highly specific single chain mem-brane surface glycoproteins by a single gene.CD44 is mainly involved in the specific adhesion between cells and cells,cells and matrix.CD44v6 is a member of the CD44 family,and its correlation with metastasis and prognosis of non small cell lung cancer( NSCLC) has become a hot research topic in recent years.The current research pro-gress of CD44v6 and NSCLC are reviewed in this paper.
5.Inhibition of IKKα binding to IL-1β promoter by p50 in LPS tolerant THP-1 cells

Objective To study the effect of p50 on IKKα at IL-1β promoter in LPS tolerant cells and to reveal the mechanism of the inhibition of IL-1β mRNA by pS0. Methods THP-1 human promono-cyte model of endotoxin tolerance that simulates the sepsis leukocyte phenotype was used. Chromatin immu-noprecipitation assay(CHIP) and real-time PCR were applied to quantify the binding of p50 and IKKα to IL-1βpromoter. IL-1β mRNA transcription was studied after knocking-down of p50 and/or IKKα. Results With LPS stimulation, p50 binding did not reduce but somewhat increased at IL-1β promoter in tolerant THP-1 cells. Knocking-down of p50 increased the transcription of IL-1β mRNA, which revealed the inhibi-tory effect of p50 in tolerant cells. In contrast, the accumulation of IKKα to IL-1β promoter decreased with LPS stimulation in tolerant cells; However, IKKα binding increased after p50 gene knock-down. In the meantime, IL-1β mRNA transcription increased; At last, IL-1β mRNA decreased again after double-knoc-king down of p50 and IKKα. Conclusion p50 is an inhibitory protein at IL-1β promoter in tolerant THP-1 cells. The unresponsiveness of IL-1β mRNA transcription to LPS at least partly results from the inhibition of IKKα binding to IL-1β promoter by p50.
6.Effect of laparoscopic hepatectomy on cellular immunity

Objective To compare the effects of laparoscopic hepatectomy (LH) versus conventional laparotomy hepatectomy (CLH) on cellular immunity. Methods Fifteen ASA Ⅱ-Ⅲ patients aged 34-61 yrs, weighing 48-75 kg undergoing laparoscopic hepatectomy (LH) were studied. Another 15 patients aged 33-64 yrs, weighing 46-73 kg undergoing conventional laparotomy hepatectomy (CLH) served as control. The preoperative liver function was rated as Child classification A in both groups. The patients were premedicated with phenobarbital 0.1 g and atropine 0.5 mg i.m. . Anesthesia was induced with fenlanyl 4 ?g?kg-1, propofol 1.5 mg?kg-1 and succinylcholine 2 mg?kg-1. After tracheal intubation the patients were mechanically ventilated and PETCO2 was maintained at 35-45 mm Hg. Anesthesia was maintained with inhalation of isoflurane (MAC 1.0?0.31) and 60% N2O in O2 and intermittent i.v. boluses of vecuronium. The patients received after operation patient-controlled epidural analgesia (PCEA) with 0.125% ropivacaine and morphine 0.05 ?g?kg-1?min-1. Radial artery and right internal jugular vein were cannulated for BP and CVP monitoring. Peripheral venous blood samples were taken before operation and on the 1 st and 3rd postoperative day for determination of CD3+ , CD4+ , CD8+ T cells (by flow cytometry) and IL-6, TNF-?concentrations (ELBA) . Results CD3+ , CD4+ and CD8+ counts were significantly decreased while IL-6 and TNF-?levels were significantly increased on the 1st postoperative day compared with the baseline values before operation in both groups but there was no significant difference between the two groups. On the 3rd postoperative day CD3+ , CD4+ and CD8+ counts and IL-6, TNF-?levels returned to preoperative level in group LH while in group CLH CD3+ , CD4+ , CD8+ remained low and IL-6, TNF-?levels remained high.Conclusion The results suggest that LH exerts less effects on immune function than conventional laparotomy technique.

8.Epidural versus general anesthesia for laparoscopic cholecystectomy:A randomized controlled study of 500 cases

Objective To compare the effects between epidural and general anesthesia for laparoscopic cholecystectomy. Methods A total of 500 cases scheduled for laparoscopic cholecystectomy (LC) were randomly divided into the epidural anesthesia group (Group E, n=250) and the general anesthesia group (Group G, n=250). Results (1) Hemodynamic parameters: in both groups the cardiac output (CO) decreased significantly after CO 2 pneumoperitoneum ( P 0 05), without significant differences between the two groups; the central venous pressure (CVP) was transiently elevated after pneumoperitoneum ( P 0 05), with significant differences between the two groups ( P 0 05), without significant differences between the two groups. 9.Use of endoscopy in the treatment of calculus of intrahepatic or extrahepatic duct:A clinical study

Objective To discuss the application of endoscopic techniques in the treatment of calculus of intrahepatic or extrahepatic duct. Methods Laparoscopy in conjunction with duodenoscopy or cholangioscopy was adopted in 96 cases of calculus of intrahepatic or extrahepatic duct, including 72 cases of cholecystolithiasis complicated by choledocholithiasis, 16 cases of simple choledocholithiasis and 8 cases of left intrahepatic duct calculus complicated by choledocholithiasis. The surgical procedures included laparoscopic cholecystectomy (LC) combined with EST (37 cases), LC combined with cholangioscopic exploration (54 cases), and laparoscopic left hepatic lobectomy combined with cholangioscopic exploration (5 cases). Results Postoperative re-examination revealed no residual calculus in 93 out of 96 patients. Small amounts of biliary leakage occurred in 4 patients and healed spontaneously within a mean time of 10 days. No other severe complications took place. A conversion to open surgery was required in 3 patients, 2 of which were high bile duct stricture and 1 of which were severe portal adhesion. Conclusions Endoscopy in the treatment of calculus of intrahepatic or extrahepatic duct is feasible. Proper application of multiple endoscopic techniques is a safe, effective and minimally invasive means for the treatment of cholelithiasis.
10.Effect of hormonal disorders of postmenopausal bone metabolism on bone mineral density and related biochemical indices in women

目的观察绝经前后妇女骨密度的变化与甲状旁腺激素、降钙素和1,25(OH)2D3及相关生化指标间的关系。方法采用X线骨密度仪测量骨密度,用放免法检测患者血中甲状旁腺激素、降钙素水平,利用全自动生化仪检测血钙、血磷、血碱性磷酸酶(ALP)。结果绝经后妇女各部位骨密度均下降(P<0.05);血甲状旁腺素水平升高 (P<0.05);降钙素水平有下降趋势;血钙、血磷均无明显变化(P>0.05);血中ALP水平较绝经前升高(P<0.05)。结论妇女绝经后,骨代谢激素分泌发生紊乱,骨密度下降。
