1.Comparative study of PTN,Cyfra21-l and CEA test in diagnosis of lung cancer

Objective: Serum pleiotrophin(PTN) is closely related to tumor,particularly to lung cancer.Many studies focused on PTN recently.This study is to evaluate the diagnostic value of PTN,Cyfra21-l and CEA tests independently or in a combined style in lung cancer.Methods: Sixty-one patients with lung cancer and 17 patients with benign lung lesion disease all received PTN,Cyfra21-l and CEA tests before the treatment.They were compared with pathological results,and then we calculated the diagnostic value of each method and the combined styles.Results: The sensitivity of PTN together with Cyfra21-l and CEA tests is 95.08% and it is the highest among the methods.Conclusion: PTN together with Cyfra21-l and CEA tests has a important diagnostic value through mutual complementation in detecting lung cancer.
2.Cognition is correlated with auditory event-related potential P300 after traumatic brain injury

Objective To investigate any correlation between cognition and auditory event-related potential (ERP) after traumatic brain injury (TBI),and to explore the diagnostic value of the P300 ERP for TBI patients.Methods Thirty TBI patients and 28 healthy subjects were recruited.Their P300 auditory event-related potentials were measured and the Montreal cognitive assessment (MoCA) was administered.Results The TBI patients' total MoCA scores and their scores on most of the sub-item components were significantly lower than those of the control subjects,though their naming and speech showed no significant difference.The TBI patients had abnormal P300 responses.Their N1,P2,N2 and P3 latencies were significantly longer and their P2 and P3 amplitudes were significantly lower than those of the control group.Stepwise regression and multivariate analysis showed that the P300 latency was significantly associated with delayed memory,impaired visual spatial executive functioning and total MoCA score.Conclusion P300 can be used as a quantitative electrophysiological index for detecting cognitive impairment in patients with TBI.
3.Experimental research of interstitially implanted ~(125)I seed on the transplanted bladder cancer in T_(739) mouse

Objective: In T_(739) inbred line mice,bladder cancer model was established via tumor source transplant.In order to get insights of the biological effects of implanting ~(125)I seed to treat bladder cancer,the development and the pathological modality of tumor tissues were observed through the experiment of implanting ~(125) I seed.The experiment was to provide theoretical gist and practical guidance for clinical(utilization) of ~(125)I seed for brachytherapy of bladder cancer. Methods: 16 T_(739) inbred line mice were divided into two groups(A:experimental group,n=8;B:control group,n=8) in random;bladder cancer model was established via tumor source transplant.When the diameter of the transplanting tumor was about 0.7 cm,~(125)I seeds were implanted at the tumor of experimental group while the empty seed was penetrated into the control group at the same position as the experimental group and the experimental mice were executed after 24 days.The longest vertical diameters(a) and(b) of tumor body were observed every 3 days and the volume of tumor body(V=ab~(2)/2) was calculated.Combining with the weight of mouse(W) and the weight of tumor body after execution of mice(w),the growth curve of tumor was protracted and the colony redouble time and tumor suppression rate were calculated.After fixation of formaldehyde,the pathological modality of tumor tissues were observed with HE staining. Results: ① The transplantation was 100% successful and there was no natural fade-away phenomenon.②After implantation of seed,the weight of group A mice increased by(4.93?0.60)g,and group B increased by(13.60?1.15)g(P
4.Effects of electroacupuncture at Zusanli points on gut permeability in rats with intestinal ischemia/reperfusion injury

Objective To investigate the protective effects of elctroacupuncture(EA)at Zusanli(ST36) points on intestinal villas damage and mucosal permeability induced by small intestine pro-inflammatory factors in rats with intestinal ischemia/reperfusion(I/R). Methods 30 Sprague-Dawley(SD)rats were randomly divided into three groups(each,n=10):intestinal I/R group(model group),intestinal I/R+EA ST36 group(EA group)and intestinal I/R+sham EA group(SEA group). Rats were subjected to superior mesenteric artery(SMA)clamping at its root part to occlude the vessel for 30 minutes,followed by reperfusion for 60 minutes to form intestinal I/R models. Rats in EA group received EA at the bilateral ST36 points(2-3 mA,2-100 Hz)for 30 minutes immediately after ischemia,those in SEA group received EA at bilateral sham points(the point was located at 0.5 cm away from ST36 point in its lateral side)with the same frequency and intensity of stimulation as EA group for 30 minutes,and those in model group received no treatment. Animals were sacrificed 60 minutes after reperfusion and segments of distal part of ileum were harvested,then the levels of tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)in intestinal tissue were measured. Histopathologic changes were viewed and graded via light microscopy. A solution of fluorescein isothiocyanate(FITC)-dextran was injected into the lumen of the segment of intestine 30 minutes after reperfusion,systemic blood was drawn via abdominal aorta puncture at 60 minutes after reperfusion,and then the level of FITC-dextran in blood was measured to determine the changes in intestinal permeability. Results Compared to the model group and SEA group,EA ST36 significantly attenuated intestine TNF-α(pg/mg:3.01±0.50 vs. 8.65±1.02,8.42±1.41,both P<0.05)and IL-6 levels(pg/mg:2.51±0.15 vs. 6.34±0.86,6.13±1.12,both P<0.05),successfully maintained low gut injury scores(1.50±0.33 vs. 3.18±0.39,3.04±0.37,both P<0.05), and significantly reduced permeability of the distal ileum and the content of FITC-dextran(μg/L:282.42±73.92 vs. 856.22±229.47,844.22±239.47,both P<0.05). However,there were no significant differences in all above variables between SEA and model group(all P>0.05). Sections of distal ileum from animals in the model group and SEA group showed no obvious difference histologically,and the pathological manifestations were villous tip necrosis, blunt-shaped and collapse. Compared to the model group and SEA group,the intestinal villous injury in animals of EA group was much milder. Conclusion In rats with intestinal I/R injury,EA ST36 points has protective effect on the gut that is possibly due to the fact it may obviously lower the levels of the pro-inflammatory factors of small intestinal tissue,alleviate mucosal insult of gut and reduce the mucosal permeability.
5.Detection of NPM1 gene mutation in acute myeloid leukemia

Objective To analyze the frequency of NPM1 mutation in de novo acute myeloid leukemia (AML) patients and the relationship between NPM1 mutation and chromosome alterations,as well as FAB subgroups,and to analyze the mutation type.MethodsA total of 99 de novo AML patients from 2004 to 2010 in China-Japan Friendship Hospital were studied.Genomic DNA was amplified by polymerase chain reaction (PCR),denaturing polyacrylamide gel electrophoresis (PAGE) and capillary electrophoresis were used to detect the mutation of NPM1 gene in 99 AML patients,and karyotyping was performed in 72 AML patients by G banding techniques.DNA sequences analysis of NPM1 mutation was performed on 10 patients.Chi-square test was used to compare the frequencies of NPM1 mutation among the different subgroups,and McNemar's test was used to compare the different rates between denaturing PAGE and capillary electrophoresis.ResultsThe frequencies of NPM1 mutations were detected in 15% (15/99) of AML patients with capillary electrophoresis and 11% (11/99 ) with denaturing PAGE(x2 =2.25,P >0.05 ).The NPM1 was at different rates in M2(27%,8/30),M5(32%,6/19),M6( 13%,1/8),respectively (x2 =1.06,P > 0.05 ),and not detected in the other subgroups.NPM1 mutation in patients with normal karyotype(26% ) was more prevalent than patients with abnormal karyotype (4%) (x2 =5.61,P < 0.05)All of the 10 patients were of A type ( c.860_863dupTCTG).The C-terminal portion of the NPM protein by replacing the last seven amino acids(WQWRKSL) with 11 residues (CLAVEEVSLRK).Two intronic deletions were novel,one case was IVS10-18_-15delCTTT,the other was IVS10-17_-15delTTT.Conclusions NPM1 mutations represents a common genetic abnormality in AML patients,and NPM1 mutation in patients with normal karyotype is higher than patients with abnormal karyotype.Two new intronic deletion mutations are identified.

Objective: To develop ~(99)Tc~m labeled dopamine transporter(DAT) imaging agent ~(99)Tc~m-(2?-[N,N~(,)-bis(2-mercaptoethyl)ethylenediamino]methyl,3?-(4-chlorophenyl)tropane(TRODAT-1) for evaluating changes of DAT in patients with Parkinson disease(PD). Methods: The SD rats were divided into control group(n=5),PD models group(n=22)and generalized cerebral infarction models group(n=5).~() Unilateral smashing and injecting autothrombo into carotid artery of SD rats were used.~(99)Tc~m-TRODAT-1 distributing in normal rat striatum was observed.The uptakes in sound side and smashed side of PD rats striatum and in two sides of multiple infarction rats striatum were compared. Results:~(99)Tc~m-TRODAT-1 distribution in normal rats striatum exhibited a obvious uptake in striatum.And PD rats results exhibited that the uptake was less in normal striatum than in smashed striatum obviously.The result of multiple infarction rats is same as normals rats. Conclusion: ~(99)Tc~m-TRODAT-1 might betaked specificly.Its imaging can provide a beneficial evidence for PD disease early diagnose.
7.The effect of neuromuscular electrical stimulation on swallowing function after stroke

Objective To observe the clinical therapeutic effects of neuromuscular electric stimulation (NMES) on patients with swallowing disorders after stroke and to assess the value of surface electromyography (sEMG).Methods Forty stroke survivors with swallowing disorders were divided into a treatment group (20 cases) and a control group (20 cases) using a random number table.All of the patients were given routine medication and conventional swallowing training lasting 2 weeks,on the basis of which the patients in the treatment group were also given electrical stimulation.The seriousness of their swallowing disorders was evaluated and surface electromyographs were recorded for both groups before and after the 2 weeks of treatment.Results After treatment,the average dysphagia rating in the treatment group was significantly higher than before treatment,and significantly higher than that of the control group.sEMG of the suprahyoid muscles showed that the swallowing duration of both groups had significantly improved,but the values of the treatment group were significantly better than those of the control group.Conclusions NMES plus conventional swallowing training can significantly improve swallowing function for patients with swallowing disorders after stroke.sEMG can be regarded as an effective method for assessing swallowing disorders.
8.Study on the relationship between the grouping criterion of SWOG/ECOG and the prognosis of acute myeloid leukemia

Objective To investigate the incidence and distribution of aberrational karyotype in acute myeloid leukemia (AML), and study the significance of the grouping by Southwest Oncology Group/Eastern Cooperative Oncology Group (SWOG/ECOG) in the prognosis of AML Methods The chromosome was prepared with brief culture of bone marrow, and the karyotype was analysed by G banding technique. All the patients were grouped according to the criterion of SWOG/ECOG, and the survival function of different groups was observed by the Kaplan-Meier method.Results 56 (67.47 %) out of 83 patients had clonal chromosome aberrations. Among those 56 patients, AML with translocation (15;17) and with translocation (8;21) presented in 30 patients(53.57 %), and the other kinds of aberrational karyotypes shared the left proportion. Among the 74 followed-up patients, 42 patients were dead. Among three groups with favorable, intermediate and adverse prognosis respectively, there is a significant difference (P <0.001). The complete remission rate of favorable group is higher than that of both intermediate and adverse (P <0.05). There is no difference between intermediate and adverse groups(P>0.05). Conclusion Cytogenetic aberration is one of the important factors affecting the effect on prognosis. The criterion of SWOG/ECOG can predict prognosis objectively.
9.The repair of bulky tissue defect of forearm with skin flaps.

OBJECTIVETo evaluate the repairing methods of bulky tissue defect of forearm with flaps.
METHODSTwenty-one burned patients with wounds in the forearm were enrolled in this study. The injury causes were high-voltage electricity, hot press or crush injury. After local debridement, the forearm defects were repaired with pedicled complex flaps, latissimus dorsi musculocutaneous island flaps or large thoraco-abdominal flaps immediately.
RESULTSAll the flaps survived very well with satisfactory results except for 1 patient in whom local ischemic necrosis and sub-flap infection at the distal end of the flap occurred.
CONCLUSIONEarly debridement followed by skin flaps with pedicles or musculocutaneous flaps transfer could be simple, safe and reliable treatment strategies in the management of bulky tissue defects of the forearm due to burn, electric injury, or other devastating injuries.

10.Efficacy and drug resistance profiles of nucleosides retreatment in nucleoside experienced chronic hepatitis B patients

Objective To evaluate the efficacy and drug resistance profiles of nucleosides (NA) retreatment in NA experienced chronic hepatitis B (CHB) patients. Methods Totally 104 NA experienced CHB subjects were enrolled in this study.All these subjects had received at least 3 months NA monotherapy and stopped the treatment,and then received NA retreatment for at least one year.The subjects were divided into three groups according to the following criteria:reached the therapy endpoint of China guideline when they stopped NA-naive treatment (group A,n =39); did not reach the therapy endpoint when they stopped NA-naive treatment but hepatitis B virus (HBV) DNA<1.0× 103 copy/mL (group B,n=33); and with HBV DNA>1.0× 103 copy/mL (group C,n=32).The efficacy and drug resistance profiles of retreatment were compared among three groups. The effects of baseline alanine aminotransferase (ALT) levels,HBV DNA levels and HBeAg titers on the retreatment efficacies were analyzed. The mutations of HBV P gene were detected by nested polymerase chain reaction (PCR) and direct sequencing.The data were analyzd by Wilcoxon test and x2 test.Results The time to ALT normalization in patients with baseline ALT< 1.3 × upper limit normal (ULN) was shorter than that in patients with ALT≥1.3×ULN (2 months vs 4 months; Z=2.281,P=0.023).The time to virological response in patients with baseline HBV DNA<5 lg copy/mL was shorter than that in patients with HBV DNA≥5 lg copy/mL (1 month vs 2 months; Z=2.054,P =0.040). The time to virological response and ALT normalization in baseline HBeAg negative were both shorter than those in patients with baseline HBeAg positive patents ( 1 month vs 3 months and 2 months vs 4.5 months,respectively; Z=2.580 and 2.304,respectively; both P<0.05). The subjects in group A achieved virological response and HBeAg seroconversion after retreatment earlier compared to previous NA-naive therapy ([1.61 ± 1.76] months vs [3.48±4.066]months and [3.38 ± 3.34] months vs [9.92-11.22] months,respectively; Z=-2.854 and-1.094,respectively; both P<0.05).The cumulative HBeAg seroconversion rate in group A was higher compared to those in group B and group C (80.0% vs 36.8% and 37.5%,respectively; x2 =4.368 and 5.100,respectively; both P<0.05).Thirteen patients developed clinical resistance and four of them developed genotypic resistance proved by PCR direct sequencing.Among the patients retreated with the same regimen as previous in the C group,the cumulative resistance rate was highest compared to group A and B (44% vs 9% and 0,respectively; x2 =5.019 and 6.588,respectively;both P<0.05).No resistance was detected in the 14 patients retreated with combined NA treatment without cross resistance.Conclusions For NA experienced CHB patients who fulfill the indication of antiviral therapy,the retreatment should be started as earlier as possible. Retreatment with NA combination without cross resistance can prevent (reduce) the risk of developing drug resistance.
