1.COMPONENT ANALYSIS OF ENZYMATICALLY DIGESTED HUMAN AMNIOTIC BASEMENT MEMBRANE WITH REFERENCE TO ITS ROLE IN SUPPORTING GROWTH OF CHICK EMBRYO FOREBRAIN NEURONS

Cell culture and SDS-polyacrylamide gel electrophoresis (PAGE) methods were employed to analyse the role of human amniotic basement membrane (HABM) in supporting the growth of forebrain neurons and the components of HABM after enzymatic digestion. It was found that HABM treated with heparinase Ⅱ supported the growth of forebrain neurons 150% of that of the untreated HABM control. The results of SDS-PAGE analysis showed that HABM treated with heparinase Ⅱ had collagen-like polypeptides 2?_1 (260kD) and 2?_2 (225kD) reduced and lamininlike peptides (212kD and 200kD) increased; and that HABM treated with heparinase I or collagenase I had laminin reduced. This indicated that laminin is the major component in HABM to promote the growth of forebrain neurons.
2.EFFECT OF INTERACTION BETWEEN p53 AND HBV ON CELL APOPTOSIS AND p21 PROMOTER IN HEPATOMA CELL LINE 7721

To observe whether there is an interaction between hepatitis virus B (HBV) and tumor suppressor p53, plasmid PCMVp53 was transfected or cotransfected with pCMVHBV a (wild type HBV) or PCMVHBV b (mutation type HBV) into the hepatoma cell line 7721 by phosphate calcium precipitation. Apoptosis cells were labeled by annexin Ⅴ FITC and detected by flow cytometry. Another experiment was performed by cotransfecting the cells with reporter plasmid p21 luc in each group mentioned above, then the luciferase activity was measured. The results showed that the cells transfected by pCMVp53 alone exhibited high luciferase activity and high apoptosis rate. Meanwhile, the luciferase activity and apoptosis rate were further higher in cells cotransfected by pCMVp53 and PCMVHBV a , but remained unchanged in cells cotransfected by PCMVp53 and PCMVHBV b. The results indicated that P53 could induce of 7721 cell apoptosis by activating p21 transcription, and such effect could be enhanced by HBV.
3.MRI scanning technology for ankle joint and its clinical application

Objective To discuss magnetic resonance imaging(MRI)scanning technology for ankle joint and its clinical value.Methods 42 cases of ankle joint trauma were examined with MRI using GPFLX coil.It was performed through sagittal T2WI,T2WI F/S,T1WI;coronal T2WI;axial T2WI.FOV was big enough for the whole trauma ankle joint.To make image clear,some soft materials should be placed between the trauma ankle joint and the GPFLX coil.The injured ankle joint was not allowed to move.The technology of No Phase Wrap is also necessary.Results The high quality rate was up to 90% for the ankle joint.Among 42 cases,6 had traumatic arthritis,14 had ankle joint contusion,9 had Achilles tendon rupture,6 had other structural injuries,4 had late degenerative change,and 3 had normal images.Conclusion MRI is a sensitive technique for injured ankle joint.It can accurately show the degree and location of ankle joint trauma.It has an important clinical significance in finding bone contusion and other structural injuries,and it is very helpful for treatment.
4.Clinical study of milrinone injectionon acute heart failure with poor effect of digitalis and diuretics

Objective To discussion the clinical study of milrinone injectionon on acute heart failure with poor effect of digitalis and diuretics.Metheds 90 cases of acute heart failure with poor effect of digitalis and diuretics patients from September 2014 to May 2016 in our hospital were selected,according to the random number table method is divided into control group and study group,45 cases in each groups,the control group was treated with conventional heart failure intervention program,the study group given milrinone injectionon on the basis of control group,two groups were treated for seven days.Serum neurohormonal factors,inflammatory factors and blood pressure were measured before and after treatment,and the clinical efficacy and complications were compared between two groups.Results Compared with before treatment,serum NT-proBNP,NE and ET-1 levels were decreased in two groups,NO content increased,the levels of serum TNF-α,IL-6 and Hs-CRP were decreased,the levels of LVEF and CI were increased,the levels of LVEDD and CTR were decreased,the differences were statistically significant(P<0.05).Compared with the control group,the levels of serum NT-proBNP,NE and ET-1 in the study group were lower,NO were higher,the levels of TNF-α,IL-6 and Hs-CRP in serum were lower,LVEF and CI were higher,LVEDD and CTR were lower,the differences were statistically significant(P<0.05).The effective rate of the study group was 86.67%,which was significantly higher than that of the control group(66.67%,P<0.05).All the patients were followed up and no cases were reported.There was no significant difference in the incidence of adverse reactions between the two groups.Conclusion Milrinone injectionon has a significant effect on acute heart failure with poor effect of digitalis and diuretics,can significantly reduce the neurotrophic factor and inflammatory factors,improve heart function,promote prognosis.
5.Study on influence of carvedilol combined with diuretics on BNP and cardiac function of patients with mild to moderate cardiac insufficiency

Objective To explore the influence of carvedilol combined with diuretics on BNP and cardiac function of patients with mild to moderate cardiac insufficiency.Methods87 cases of patients with mild to moderate cardiac insufficiency treated in Zhejiang Greentown Cardiovascular Hospital from June 2014 to June 2016 were divided into the groups randomly.The control group of 44 cases were given carvedilol treatment, and the study group were combined with spironolactone treatment on the basis of the control group, with the total course for a month.Clinical effects were contrasted after a month of treatment in the two groups, in order to explore the influence of carvedilol combined with diuretics on BNP and cardiac function of patients with mild to moderate cardiac insufficiency.ResultsBefore treatment, there was no significant difference in serum BNP level between the control group and the study group.After treatment, serum BNP level significantly decreased when compared with those before treatment in the two groups (P< 0.05), which of the study group was significantly lower than that in the control group(P< 0.05).Before treatment, there were no significant differences in the levels of cardiac function indexes, and after treatment, heart rate, EF (%), FS (%), LVEDD (mm) the four indicators significantly improved when compared with those before treatment, and heart rate, LVEDD (mm) levels were significantly lower than those in the control group(P<0.05), and EF (%), FS (%) levels were significantly higher thatn those in the control group(P<0.05).After treatment, the significant effective rate (73.91%) and total effective rate (95.35%) in the study group were significantly higher than that of (56.82%) and(86.36%) in the control group (P<0.05).After treatment, body function, life satisfaction, mental health, social adjustment the four parts of quality of life index scores were significantly higher than those in the control group(P<0.05).ConclusionIt has obvious improvement effect of carvedilol combined with diuretics on BNP and cardiac function of patients with mild to moderate cardiac insufficiency.It is worthy of clinical application.

7.A case of Langerhans cell histiocytosis and Sj(o)gren's syndrome

The patient was a 38-year woman.Six years prior to presentation,she developed erythema and papules with occasional pruritus in both labium majora,which gradually confluenced into plaques with the formation of superficial erosion and ulcer;extension into the vulva and crissum occurred later.Half a year prior to presentation,a subcutaneous firm nodule measuring about 1.5cm in diameter with intact epidermis,developed on the right submaxilla,associated with mild swelling in the area;the nodule enlarged gradually.A subcutaneous induration measuring 0.5 cm in diameter was observed in the left chin 2 months later.She also reported a 20-year history of dry eye and dry mouth.The patient tested postive for antinuclear antibody cally,there was a focal infiltration of lymphocytes in tissue of minor salivary glands.The pathology of lesions on the right labium majus showed a dense dermal infiltration with S100-and CD1a-positive,irregulady-shaped histiocyte-like cells with abundant eosinophilic cytoplasm and few mitotic figures.Needle biopsy of the right submaxilla area showed tumor cells.A diagnosis of Langerhans cell histiocytosis and Sj(o)gren syndrome was made based on clinical manifestation,laboratory findings and histopathological features.Combination chemotherapy with cyclophosphamide,vincristine,corticosteroids and etoposide resulted in clinical improvement.
8.Application of ISO9001 Quality Management Standards in the Construction of Clinical Drug Trial Institution

9.Observations on the Efficacy of Nazi Method of Midnight-midday Ebb Flow plus Warm Needling Moxibustion in Treating Transient Ischemic Attack

Objective To investigate the clinical efficacy of Nazi method of midnight-midday ebb flow plus warm needling moxibustion in treating transient ischemic attack (TIA). Methods Sixty patients with transient ischemic attack were randomly allocated to treatment and control groups, 30 cases each. The control group received routine medication and the treatment group, Nazi method of midnight-midday ebb flow plus warm needling moxibustion in addition. The main clinical symptoms were observed by TCD monitoring of microembolic signals in the two groups before and after treatment. The clinical therapeutic effects were compared between the two groups. Results After treatment, the positive rate of microembolus monitoring was 3.3%in the treatment group and 20.0%in the control group;there was a statistically significant difference between the two groups (P<0.05). There was a statistically significant pre-/post-treatment difference in the traditional Chinese medical syndrome score in the two groups (P<0.05). There was a statistically significant post-treatment difference in the traditional Chinese medical syndrome score between the treatment and control groups (P<0.05). The total efficacy rate was 96.7% in the treatment group and 80.0% in the control group;there was a statistically significant difference between the two groups (P<0.05). At six months after treatment, the rate of disappearance of TIA, the rate of recurrence of TIA and the rate of occurrence of cerebral infarction were 90.0%, 6.7%and 3.3%, respectively, in the treatment group and 60.0%, 20.0%and 20.0%, respectively, in the control group;there were statistically significant differences between the two groups (P<0.05). Conclusions Nazi method of midnight-midday ebb flow plus warm needling moxibustion can decrease the positive rate of microemboli and shorten transient ischemic attack to prevent cerebral infarction in patients with transient ischemic attack.
10.Analysis of prognostic factors for patients with hepatitis B viurs-related liver failure and construction of a prognostic assessment model

Objective The aim of this study was to analyze risk factors which may affect prognosis of patients with hepatits B virus (HBV)-related liver failure,and to construct a model for prognostic evaluation and further assess its predictive ability.Methods In this retrospective cohort study,569 hospitalized patients who were diagnosed with HBV-related liver failure from January 2007 to December 2010 were enrolled.All the patients were followed up and survival analysis was performed using the Kaplan-Meier method.Univariate and multivariate COX proportional hazards regression analyses were applied to variables such as age,sex,complications,biochemical markers,coagulation markers,and HBV DNA levels to construct a model for prognostic evaluation,and 79 independent cases of HBV-related liver failure were used to confirm the model's predictive ability.Accuracy of the constructed model and model for end-stage liver disease (MELD) was evaluated by receiver operating characteristic (ROC) curves.Results The median survival time for all the patients was 59 days.The survival rates at 1,3,6 months were 58.9%,46.2% and 45.5%,respectively;and survival rates at 1 and 3 years were 44.9% and 44.5%,respectively.Hepatic encephalopathy,pulmonary infection,upper gastrointestinal bleeding (UGIB),albumin (Alb),aspartate aminotransferase (AST),creatinine (Cr),international normalized ratio (INR) were determined to be independent risk factors (all P<0.01) which may affect survival of patients with HBV related liver failure.Accordingly,the prognostic index (PI) of the constructed model for prognostic evaluation 4.98 × assignment of hepatic encephalopathy + 4.57 × assignment of pulmonary infection + 4.41 ×assignment of UGIB-9.69 ×lm[Alb (g/L)]+2.46 ×ln[AST (U/L)]+5.18×ln[Cr (mmol/L)]+3.35×ln (INR) 15.36.The area under receiver operating characteristic curve was 0.838 for the constructed model assessing 90-d survival of the patients,and was 0.751 for model for end-stage liver disease,with no significant difference between the two models (Z=1.085,P =0.278).Conclusions Prognosis of patients with HBV-related liver failure can be accurately predicted by the constructed prognostic assessment model,which is consisted of hepatic encephalopathy,pulmonary infection,UGIB,Alb,AST,Cr,and INR as independent risk factors,and is able to predict the 90 d survival.
